Bonaventure

My husband was only there for 4 months before he died but his decline was very rapid once he entered the memory care - only 10 residents but only a couple were really verbal. My husband was lonely because he had no one to talk to except the staff and they had other jobs to do / laundry, cleaning. I was there 2 to 3 times a day taking him to the assisted living side to participate in activities and social functions. I also had him in Home Hospice and the CNa came to assist showering 2 X week, nurse came every week or so to take vitals. He fell numerous times and went from using a cane to a walker to a wheelchair.

Medical staff including med techs and CNAs have high turnover rate and they present as inexperienced, condescending to the elderly residents, and careless when providing day to day medical care. I do not recommend this place to anyone with memory issues or requiring medication management. Dining experience is frustrating because the wait staff is poorly trained, there is high turnover, and residents wait 20 - 30 minutes to order then 20 - 30 minutes to get food and drinks.

My loved one is currently a resident at Woodland Retirement and Assisted Living and overall things have been good for her at this facility so far. The friendliness is great! The staff, cleanliness and activities have been good. The care services, food and value for cost are okay. Communication with the family is not good, there is a lack of communication with the family. However, overall I would recommend Woodland Retirement and Assisted Living to others.

More eyes on residents and help with residents. Some one to answer calls , check rooms for cleanliness, make sure there's toilet paper especially because some can get to bathroom but if no paper, issue with a mess. If people fall asleep sitting up keep an eye on them, falls can happen fast. Don't know how many people I've caught and brought attention to who might have fallen. Be aware of how many days they've been in same dirty clothes day and night, breaks my heart to see loved ones in same dirty clothes for days (underwear included).

My loved one is currently a resident at Bonaventure of Thornton. We felt that we were misled on multiple issues. The turnover rate is high, they have been through three directors in three months and the VP is gone. Communication with the family is terrible. The staff such as med techs, front desk, and restaurant workers are great.

I picked this community for my husband because he required a high level of assisted care and they had 2 RN's on staff to create a detailed plan of care and ability to assist with and monitor the care provided by staff. Despite staff shortages and turnover having the 2 RNs to ensure consistent care made all the difference. 'Included' Housekeeping for Assisted Living apartments was inconsistent and sometimes nonexistent. Dining room tables and carpet not always well cleaned between meals. Dining for Assisted Living was poorly serviced and the food grossly lacking in nutrition, preparation, taste, and variety. During the time my husband was there there was also a high turnover in the Executive Director position which undoubtedly impacted operations at all levels across all departments. That situation was poorly handled by Bonaventure. I was with my husband every day and not only witnessed a lot but heard comments and complaints from other residents. One of the positives was my husband's 1 bedroom apartment was nice with big windows and lots of light and space that accommodated his walker and wheelchair with an accessible bathroom and shower. I do have to add my husband passed away a year ago so I can only comment on our experience during that time. I can't speak to what it's like as of today. Maybe staffing and operational improvements have been made. Maybe not. If I had to look for a place today I would proceed with diligence in evaluating all aspects of Silver Creek.They didn't deliver on all they marketed but it was still charged for in the overall cost.

Based on the experience we have had so far with Silver Creek, they could improve on all areas. I went over to visit my brother and he had been sitting in crap for a couple of days. I almost turned them in to ombudsmen's office but my brother likes the place so I didn't

I liked location and layout of the unit. I am pretty satisfied even if a few things could be improved. Except for a few things I would recommend it. They need more housekeeping staff and better food.

The Exec. Director, [name removed] is too consumed with her personal life and chaos that we often are told different things in the same conversation. She sets up meetings, forgets times, cancels and shows up late. My Dad is more confused than ever and I feel like half the info I'm told is false, exaggerated, or I'm not told the correct info at all. They assured us they would be able to handle his confusion and anger/elopement risk and they did not. He is currently in the hospital and in communicating with ER staff, & case workers, I'm being told something from them and something completely different from Bonaventure. Bonaventure told hospital staff dad wasn't taking his meds in over 2 weeks. When they told me he just refuses a dose here or there. When it comes to the cleanliness it looks run down, but overall clean. Kitchen area always has a cart of dirty dishes when I have visited. Staff always seems nice to talk to. As long as the patient is calm and willing to just sit around I'm sure they do fine. But we were told they don't have enough staff to keep people occupied or entertained. I've only seen one meal and it looked like it was a microwave dinner. Not appealing at all, however I haven't seen or heard anything else to make a fair assessment. There was an activity person there and he seemed very involved and excited to work with everyone. But haven't seen him since the first day and activities are not usually happening when I visit so I don't think he even is still there. My dad never got consistent meds, I am the one that worked with his med team, had to go and distract him or calm him on several occasions and I'd he wasn't getting meds, meals, entertainment etc... What are we paying them for? A very expensive bed.

I give higher marks for independent living than assisted living. In assisted living, Staff has a high turnover rate, and lack of staff makes it hard to take care of assisted living residents in a timely manner. I would recommend Bonaventure of Salem, but with the reservations that I have outlined.

What we really like is the the close proximity to child's house, that is number 1, but the areas they could improve on would be communication, safety, and the food. Sadly, I don't think we would recommend this community to other families. The previous management/sales group has failed to live up to promises it made for my mom prior to her moving in. My mom was also hurt while on property, slipped on ice when walking outside. No signs, no attempt to remove the ice, no apology, and no checking in to see how my mom is recovering.

Overall experience is good and the building and lobby and dining rooms are quite impressive but not meriting the exorbitant fees. The executive director is cheerful and wants to be involved in everything - which is unnecessary and sometimes unwanted. She tends to be a hugger which I usually do after I know someone for more than 30 seconds! Boundaries need work! All staff is friendly and willing to help. Cleanliness is good. Quality of care is fairly good although the depth of knowledge about diagnoses could be better and communication between staff more complete or comprehensive. Food is good. However, Wait staff need longer orientation than one day! Especially if they are expected to work alone. Activities are fair. Wine tasting and happy hour are enjoyed by many. There could probably be more hands on with newcomers for the first month including introducing new folks to people willing to do be "guides" for a few weeks - taking newbies to meals, activities, introducing them to other residents, etc. we have had too many phone calls and having to go in to problem solve... I also think sales for caregiving needs to be more transparent and not a "flim flam".

As with everywhere they are short staffed. It can be challenging when it comes to assisted living and not getting the care in a timely manner. Quite a bit of turnover. The residents are wonderful and the place is beautiful. They do a good job at trying to keep it neat and clean.

There have been issues with many different things that management did work with us on. The last one was a financial issue that took a month to correct and they were not responsive. The aides are wonderful and treated my loved one very well. I'm not sure I would recommend them unless the resident was fully able to care for themselves.

It is concerning to me this facility does not provide monitoring for falls for its independent residents. If you are in assisted living you can get it but if you are independent you cannot. Most rooms do not have landlines so alert systems are not an option. People have laid on the floor all night [name removed] was found deceased a day later. I would think it is required to have an option for all residents.

Bonaventure was definitely not a good fit for my mom. It was too big and too fancy.

They need better housekeeping services and better communication with medication administrations. We picked this community, because of better rooms and size and my loved ones decision to move there.

They are going through a lot of staff challenges, so the food is good when they have staff available. The people taking care of my loved one are very caring. The communication, with the administration needs to improve. [name removed]from A Place for Mom was just fabulous, and really helpful.

1. Food. The menus are planned by a central office for all Bonaventure facilities so there is no customization for local or seasonal food. Mushy canned or frozen veggies year round. The meals are carb loaded, and the corporate dietitians are unwilling to customize local meals for diabetic needs. For example, the "Breakfast for dinner" theme night was pancakes with no Low-sugar syrup available, and a brownie for dessert. 2. Staffing. The Salem facility cannot keep enough staff on hand. The new hires constantly quit. This resulted in insufficient care for the residents. For example, one Sunday morning in October there was no wait staff available to serve meals at the dining room so the residents just sat in the dining room. Medical team short staffed and sometimes nighttime medicine isn't delivered to sediment rooms until 11 pm. Also staff have to work extra shifts to cover the missing employees, and this results in hardships on the staff.

We picked this community because, It was an emergency, to get my loved one into memory care. Sometimes they do laundry, and don't bring it back on time. The personal care, and communication could use improvement. In addition, they could do better with following-up on things.
