Heron Place is a really nice looking facility. We were pleased at first then after several months the level of care and food went downhill. Snacks were no longer offered to the residents. Vegetables were undercooked and hard to eat for those with dentures. The last straw was when the med tech made a serious mistake in the medication that was given which could have been fatal. We were lucky it was discovered in time and our loved one was hospitalized. There was no RN onsite daily at the time the medication error occurred. This all reflects on the facility administrator and corporate management.

Staff not friendly. Nurse very rude & arguing with staff & residents. Not professional, no confindentuality.

We have not been very happy with our experience here. There were some things that they said they would do, that haven't been done. The girls are just run ragged. The bus has been broken down since before we moved in, they keep saying it is being fixed but it hasn't happened in over 6 months. The food is just horrible, what is on the menu is just not what you get.

I was charged for care that I did not ask for did not need and never received but I was still charged $[removed] simply because that would've been the first level of care it was $[removed] a month for me with that additional money the meals were shameful. A dinner could consist of a half of a grilled cheese sandwich with a few potato chips and a pickle if you got a salad, it was in a tiny tiny little cup very little salad. They were very disorganized as to when they came to your room to get your laundry there was no communication I don't believe between staff and administration. I think it was poorly handled by the director. I was there for about three or four months when I realized I absolutely could not stay there Many times in the middle of the night I would hear somebody hollering for help help help and one particular night my daughter was visiting me and spending the night she got dressed, got out of bed got dressed, went to the room. The person was on the floor. She searched the building and found one staff there for the night who was looking at her phone and [name removed] told them there is a person on the floor in their room, screaming for help, and the staff person looked annoyed and followed her to the room, so there was very little staffing in the middle of the night.

I am extremely disappointed with my experience with Avista Sun City West Memory Care. The way my family and I were treated was very unprofessional and upsetting, especially during such an emotional and difficult time. Before moving my dad in, we were told certain things that gave us confidence in choosing this facility, but after he was there, many of those promises were not followed through with. Changes were made without proper communication or notice, and it felt like our concerns as a family were not being respected or heard. One of the biggest concerns was the pressure we felt to switch my dad to a hospice company that the facility preferred, even though we already had a hospice provider we trusted and wanted to continue with. During that process, we feel they took advantage of our family’s emotional state and the difficult situation we were facing to push for a decision that aligned with what they wanted rather than what our family believed was best for my dad.

Staffing could be improved. Watch for separation of men and women

I did not enjoy or understand what was going on with [name removed]. Nobody told me anything because I was only a friend to him. Overall, I was left with a very bad feeling. Things were taken from his room without anyone's permission. I pray I never end up in an assisted living facility.

If all you need is for someone to cook and do laundry then both places are ok. But if you need personal care they came up D___ short. In home care for the money rates a 5 or higher.

Very disorganized. Can't seem to find time to help with showers or other needs. Food and food service very lacking. Took them a few weeks to get him on a laundry schedule and several weeks for showers. He's had to ask many times for things.

My mom was in memory care here for 3 years. The first thing I have to say is we didn’t move her because her neurological dr said the change would kill her. They do okay with people that can do everything for themselves. Showers, toothbrushing, medication and food are all bad. My mom went months without medication. We didn’t go through the facility pharmacy so we were able to track her medication. We didn’t catch the fact that her 30 day prescription lasted 6 months and her eye medication lasted a year for the first year. We were there and watched them get ready for the medication audit from the pharmacy. They had a med tech come over and fix the med cards by pushing out all the pills that should have been given but weren’t and sign off on them. Mom had to have surgery on her mouth to remove her dentures because they hadn’t been removed or brushed for many months. Then had to have the flap of skin removed. It looks pretty because that’s where they spend the money, not on staffing.

I caution anyone who is considering Summit to be very careful to keep a careful record of payments and charges. I paid approx $[removed]for my father and his wife to stay at Summit. Ultimately, my father never lived at Summit, and his wife moved 6 wks later to a different facility. I was due for a refund of almost $[removed]as of [date removed]. It's now [date removed], and I 've been trying to a refund for the past 7 weeks. I've called and emailed the Exec Dir and head of billing about this issue numerous times. I've been told that the refund is being processed, but there's always some reason for further delay. Summit never brought up a refund. After I brought up the issue, I was initially told that I was due less than half the actual amount. After I gave Summit a detailed account of my payments and incurred costs, Summit agreed that I was due almost $[removed]. I previously told Summit that I will file a consumer complaint, and it seems I will have to do so. I would not choose this facility ever again

I do not recommend this facility. Do not believe the website...all the pictures are staged. The memory care unit is dirty and the residents are ignored. I know this from personal experience. My mom died from things that happened in this facility. Don't trust these people, do not recommend.

I was very disappointed with the care that was provided from such a highly rated facility network. The money you pay to provide the best care for your loved one. No one took the time to comb their hair or make sure that they had their shoes on the right feet. I still cleaned his room and did his laundry. I offered help day or night If had any issues. As for activities coloring book crayons in front of the TV and staff on their phones in the corner. We had things come up missing an stuff was ruined from the carelessness. He had an episode an they wanted to kick him out. The facility is responsible for their safety. Don't let them fool you. When talking to them about the concerns an losing my dad. Was told we are not perfect. When cleaning out his room we even wanted to donate his clothes..an hour later went back his clothing was already gone. They told us that the clothes had already been donated. When all said nd done 1 person gave their condolences. My father deserved better.

I have pleaded our case with [name removed], who is the general manager of Heron place, and [name removed], who is the account manager at Heron place. [name removed], specifically, told me that I would receive payment for 3 days of room usage as well as 3 days of medical care we were charged for even though our sister had passed. [name removed] said we would get everything [name removed] promised. Later, [name removed] made it clear that we would not be compensated for the room usage as our sister's belongings were still there. That seemed fair and correct - we agreed. For the remainder of the promised fees, [name removed] put us off time and again - for four months. I suppose she figured she would just wear us down. That is an absolutely terrible way to treat grieving family members who trusted this institution and has caused quite a bit of pain and suffering, especially for my wife who is still trying to get over the death of her sister. Again, in the grand scheme of things, we are not talking about a tremendous sum.

They lost her [fee removed]. Hearing aids as well as some clothes. After [name removed] had been there for a few days, I was called on a Friday afternoon to come and pick her up as she kept trying to leave the facility. They then required that she have anti anxiety drugs to return. I arranged for that and she returned to the community on Tuesday. I was then called Friday evening to come pick up [name removed] as she had gotten out of the facility and was all full of scratches from making it into the bushes. They had not given her any of the anti anxiety drug. They were very friendly, enthusiastic people. They have no business dealing with people who have Alzheimer's and are still very mobile.

This facility is struggling. They are seriously understaffed but not full, so they are in a Catch-22. There is no Chef, no Activities Director and I have seen no sign of the LPN who should be there daily. New hires are announced and introduced then whisked off to corporate for "training" and then don't always return. Absolutely everything, except for the grounds, could be improved at Avanti Senior Living at Peoria.

My parents were both in this community less than a month. My Dad passed away, we left the facility that night and never returned. This facility charged my Mom the single occupancy rate after my Dad passed away. She never set foot in that room again. In the planning of the funeral etc we as a family made great strides to clear everything from the room. My Mom was taken advantage of in being charged the higher rate that my Dad would have been paying if he lived. There was so much contention between the kitchen staff and the CNAs that many times my parents were not well cared for. I would not recommend this place to any of my family, friends or loved ones. We are glad that this is all behind us now and that my Mom and family can mourn our Dad and continue on in peace and honor his memory.

PLEASE READ BEFORE DOING BUSINESS WITH JASMINE PLACE/ENLIVANT: My mom complained about the horrible food, the lack of response when she called for help with her call button and how rude the staff was to her within a month of moving in. She would have me on speaker phone and I would hear how rudely the staff would talk to her. They are constantly understaffed because no employee will stay for any extended period of time, which says a lot about how Enlivant treats its employees. It came to light that they were overdosing my mom with anti-anxiety meds - one that should be given every 4-6 hours was given to her hourly. They do not reply to emails and rarely answer the phone. There are no activities, outings or anything fun for the residents. I rarely write reviews but I wish someone would have written an honest review before we moved my mom to Jasmine Place. They have no clue how to care for the elderly with compassion or basic human dignity. I would write more but ran out of space!

Quality process management is non existent. Quality of care is terrible. Activities were great.

Aurora Place had constant roof water leaks, couldn't keep AC units running, a med tech refused to take my loved one to the restroom during dinner hour resulting in soiling themselves. and then that med tech wielded our loved one to their room after dinner and put them back in her chair, still soiled. our loved one had diarrhea for 2 weeks, the resident RN and other staff did not assist in trying to figure out the cause. then when I removed my loved one from the facility, I was handed their medications and discovered they had not given our loved one 5 days of their blood pressure and stroke medications. Aurora place is extremely dangerous. if I could give them a zero rating I would. a rating of one is much too generous as far as I'm concerned.
