I was a resident at this facility and my experience was terrible. My first night here, I went to find the tech and was told that he was in the Memory Care unit and that he could not be reached. I was extremely upset because part of my meds was pain management. I asked another Aide and was told that was not true; she called him and he was there in 5 minutes. Apparently, the first aide was reported to the supervisor. The food was substandard and very badly cooked. I lost 10 pounds will I was here. The portions were so small you always walked away hungry. There were always plans for outings that never came to fruition. As long as you walked through the door with money, you were accepted. Some of the residents should have been in a full blown nursing home. Some of the aides were so nasty it was unbelievable, but nothing was ever done about it as far as I know. I would not recommend this facility at all!

I LEFT COMMONWEALTH SENIOR LIVING 8 WEEKS AGO. THE FACILITY I AM AT NOW IS FIRST CLASS. THE NURSES, PATIENT CARE WORKERS AND MANAGEMENT HAVE GONE OUT OF THEIR WAY TO ROVIDE ME THE BEST CARE. I AM FINALLY GAINIG MY STRENGTH BACK AFTER WHOLESOME MEALS. MY GNAT BITES I HAD AT COMMONWEALTH HAVE HEALED BUT HAVE LEFT SCARS. THE NURSES HERE CHECK MY SKIN WEEKLY TO MONITOR THE HEALING AND MAKE SURE I AM NOT SEPTIC AS A RESULT OF THE BITES.THEY ARE DILIGENT IN LEAVIG MY DOOR LOCKED SO THAT NO ONE FROM MEMORY CARE CAN GET I N BED WITH ME AT NIGHT. I HAVE MADE FRIENDS AND FOUND SUPPORT FROM THEM AND MY THERAPIST TO OVERCOME PTSD BUT NIGHTMARES CONTINUE. I HAVE REPORTED A COMMONWEALTH NURSE TO THR TENNESSEE BOAD OF NURSING FOR DERELICTION OF DUTY. I WILL BE FILING MORE RPORTS WITH THE STATE OF TENNESSEE. I AM NOW HAPPY, SAFE AND WELL CARED FOR . THIS FORUM SAYS NOT TO BE ABUSIVE BUT READ BETWEEN THE LINES AND YOU WILL SEE THE TRUE COMMONWEALTH. OAK RIDGE. AVOID THEM!!

My uncle was here for a about a week and died. He never had any health issues, the only reason he was placed here was because the family was not able to give him the 24/7 care. They claimed he had a heart attack but I became aware that even though he had a doctors order that his food was to be cut up and fed to him, they left him and he choked on chicken and that is how he passed away. I don't understand if there are staff there 24/7 why wasn't he helped? I had friends who also had family members that went there and they died within a week or two even though there was nothing medically wrong expect that they could not fend for themselves, there should be no reason why 4 people died in one week. They ran out of gloves and was touching people, their food, their medicine, etc with their bare hands. I have not seen anyone wash their hands. This guy named [name removed] that works there gave me the worst attitude when I confronted him about the use of gloves. Plenty of others shared the same story.

We brought our mom their based on a good recommendation. The sales department was very compassionate and delivered a message of the facility being a close knit family that everyone keeps an eye out on our love ones. We moved her in as respite. Realizing she wouldn’t be able to return home. In less than 30 days it was recommended to move her to memory care. Sad to say memory care seems to be no man’s land. She turned for the worse with no ability to reach out when needing assistance. We had a few family meetings with the facility and promises we’re made but the never did anything change. Unfortunately my mom passed. No one from the staff was there when we came to pick her up her remains, not one word of condolences. A week has passed and still nothing. It hurts to think we placed her there for care and now she is gone. Management doesnt understand the needs of their residents. I can’t figure out what She was paying for as a high level of care! Upset with ER trip.

My mother went into this facility able to be self reliant and after 2 years and three falls of which we we were unaware of along with terrible care my mother’s overall condition went from good to bad. She was moved from one room to another for no apparent reason which caused her to have another fall while trying to get to the bathroom in the middle of the night . This fall put her into the hospital. At this point we moved her to another facility where she receives much better care and oversight. Do not listen to the company A Place for Mom if they did their research they would never have recommended this place. Do your own research for your loved ones.

My brother has been at Commonwealth Oak Ridge for 4 years. Recently they called EMS to transport him to Park west for evaluation. When they arrived he was soiled and extremely wet. They asked staff to change him before transport..they refused. They brought him back because Park West said the bad behavior they claimed was not present. Commonwealth called EMS back to transport him to a different hospital. Again he was soiled and wet and again the staff refused to clean him up. The EMS drivers cleaned him up and had no problem with his behavior. The EMS driver called me to report the severe neglect. They said he was being mistreated. I pay over 5000.00 a month for his care and they have not cared for him as they should. No one should be treated this way.

staff is warm and inviting when you are placing a loved one. after my wife was placed there the staff that runs the place became difficult to deal with. they did not return phone calls in a timely manner if at all. they seem to have a don't call us we'll call you attitude. in my opinion my requests and concerns regarding the health and care of my wife were not taken seriously . access to her was constantly restricted due to covid outbreaks among staff and residents. she passed away after only five weeks at their facility. no one at any level of the commonwealth community reached out to my family from that point on.

We purchased furniture for my father in law--- bed, mattresses, chest of drawers, linens, and a chair. He spent about 4 days there. They will not give us back the furniture. My husband has gone in person, emailed, called, and contacted the corporate office. His clothes were stolen when he was there also.

Depressing, staff was friendly enough but could not even get unoccupied rooms opened up with the keys they had. That is concerning. Kitchen where food is prepared was run down, dirty and unacceptable in my opinion. Staff member let me know they were changing over and would not be called Greenfield anymore. I did not inquire as to why but hopefully they will remodel kitchen and fix doors.

My mom lived there for a year in 2016. They were supposed to dispense her meds, but they got them mixed up so many times it was frightening. At one point they kept trying to administer another woman's meds (eye drops) and we knew this because my mom kept telling them it was the wrong eye; around the same time Mom got very confused and I had to send an advocate to assess how they were dispensing meds. Immediately the confusion disappeared! A few months later, Mom went through a period where her doctor was trying to find the right meds/dosage for her issue and I discovered that they *never* documented the dispensing correctly. As her symptoms changed I kept asking for the dispensing records and learned they were making it up as they went along. At one point (in ongoing rounds of trying to get the "correct" records) they provided me documentation that they had dispensed a med days before the doctor had even prescribed it! I was communicating with a "medical director" at their Falls Church headquarters trying to get the correct records--this went back and forth for a long time, while Mom endured many changes in meds; her doctor sometimes made changes thinking his script was off without realizing that Greenfield had not yet implemented the latest script. In the end they sent a registered letter that my mom had 30 days to vacate!! Anyone reading this knows (1) how much you pay up front just as a move-in charge, and (2) how long it takes to find a suitable location with a vacancy. But this is the way they do business. In practice, there are some good people who work there and some who are unethical and incompetent; unfortunately their headquarters contains leadership from the latter category.

Please be aware that there have been changes in the corporate management since summer 2017. This is NOT the same place that produced the former good reviews. Budget cuts, staffing cuts, [Removed] of maintenance and personnel problems = bad atmosphere, bad care, and a bad experience. Look for something else!

This place is unacceptable! My father was found dirty several times and had a period of insomnia that was never reported to us. Also, their pharmacy and psychologist dropped them. We were told that a new psychologist was on staff but after an incident we were told that he didn't receive the evaluation he was supposed to have. High staff turnover and treated rudely by management after raising some concerns. Daily staff was nice when they weren't quitting.

Not impressed with level of care provided to residents. One resident was asleep on a bench in the hallway, one was slumped over in a wheel chair with no attention being paid to them.

Please DO NOT entrust your loved one to this facilities care if they need mediciation management or they are not basically totally independent! My mother lived here for a little over two months. In that timeframe, they messed up her medication NUMEROUS times, they did not return phone calls, they left fecal matter on my mother (as seen by hospital ER departmemt) and robbed her of her dignity: just to name a few. All they care about, from the very top, (management: case manager, head of nursing, and their executive director) is MONEY. They are ruthless, for-profit, unwilling to work with you, refuse to hold themselves accountable for their errors, try to intimidate, pass the blame, do the bare minimum, and then try to stick you for rent and services after you've moved your loved one out (after a legal and necessary medical evaluation required by NY State proved they were unable to meet my mother's medical needs)! HORRIBLE HORRIBLE place. The medicine passers don't know what they are dispensing to residents, medication came up missing, was never given to my mom or they gave her too much. I tried to get answers numerous times to no avail. I do not understand how anyone in management at Greenpoint can sleep comfortably at night. I literally pray for the residents: especially the ones who do not have an advocate.

-Most “care managers” are paid minimum wage and have no more training to care for people than a fast food restaurant cashier. (There’s a reason they are called “care managers” instead of CNAs, and it’s because they are not certified.) -Understaffing is “hidden” by counting one person as two positions...if a care manager takes a weekend class to be a “medical technician”, then the claim can be made that there are is a “caregiver and a med tech” in the building, but it’s really one person doing two jobs, which means less care and attention for people. -Most people are doing two jobs; As of October 2018, the Designating Nurse is also the Health Care Coordinator, The Executive Director is also the Resident Care Coordinator, the Sales Director is also the Program Coordinator. There is no maintenance person and only one part-time housekeeper, so....with most people doing two jobs, does that mean most jobs are only half-done?

Ask to observe or participate in the “fitness classes” advertised. Ask the residents if they are allowed in the “kitchenette” to cook or bake. Ask to see the “beauty/barbershop”. Ask to meet the “management staff” listed in the current newsletter.
