My mother and father in law moved to Ellen's Home South in 2021 and there have been issues from day one. The tour is great and the director promises many things but the place just doesn't deliver. The nurse/director supposedly trains the pca/cnas/and medication aides but when 4 aides didn't know how to use a hoyer lift, which the facility provides, that's an issue. Expect long call wait times: my loved one was on the floor after falling for over an hour before the light was answered. Infection control: what's that:? CDC c-diff guidelines are gown, glove: they just wear gloves. Activities: movies. The staff act like it's an inconvenience to ask them for anything: 4 staff and 2 residents were watching TV how dare I interrupt them to put my loved one on the toilet. Both loved ones got butt ulcers. The staff don't wear name tags or introduce themselves so when there is a problem t's hard to find out who was involved. LOOK ELSEWHERE!!! I could go on but am limited to 1000 words.

I would rate this place a -5 if I could. This is the worst Assisted Living Facility in Wisconsin. The residents there are treated VERY Horrible !!! If anyone is considering sending their loved one to this place, I would STRONGLY URGE You Not To. YOU WILL ABSOLUTELY REGRET YOUR DECISION!! Management is very disorganized unprofessional have 0 ounce of empathy and compassion for the residents and caregivers. There caregivers are extremely overworked and unappreciated. The residents are not getting the proper care they need at all. This facility need to be SHUT DOWN FOR GOOD!!! “ THEY ARE TRULY A DISGRACE!! ELLEN’S HOME is a “EMBARRASSMENT” they give Assisted Living a BAD REPUTATION!! There main focus is MONEY MONEY & MORE MONEY!!

I wish I could leave half a star.. Rude staff members, dirty facility, and expensive. The staff is very hateful. Especially any higher ups, like in HR. You pay so much for very little things to do. Don't waste your time, this is not a safe or kind place to have your loved ones trapped in!

My mother was here for several months, with many MANY scares occurring. One thing I observed more than once was her unsupervised, eating 2 and 3 slices of pie with her fingers even though she’s on a 1500 calorie a day diet. Also, almost every time I came to visit, my mother didn’t have her cane (ultimately falling a couple weeks ago) and they never called an ambulance or asked the doctor for an X-ray of any kind. The nurse on duty didn’t even know which one of my mothers knees was fake, and didn’t bother to inspect the injury before calling me. She simply stated Your mother fell and I think her knee is possibly out of place. I can tell through her jeans that it looks lop sided but I’m not sure. She then asked which knee had been replaced. Just wow. How do you not know? My mother LIVES THERE. Another tragedy. My mother has vascular dementia and MUST wear her Cpap at night because without it, the next day her mind is that of a small child that has outbursts. Her cpap averaged 3-4 hours a night which was PROOF that she wasn’t getting enough oxygen to her brain and PROOF that they weren’t placing it properly or checking on it regularly. Instead of fixing this very simple problem, the facility went behind the family’s back and asked the doctor for my mother to be given a sedative for her “behaviors” (medication that we as a family already declined a month prior to this). So rather than do their job and make sure she wears her mask properly at night, they wanted to drug her. Also, they lost her prosthetic breast more than once, several pairs of glasses, and countless amounts of clothes and jewelry. Yet every time we would ask about these items or ask why my mom didn’t have her cane, workers (particularly [name removed]) would blame my mother for losing or forgetting things, which is completely ludicrous since my mother can’t be held responsible for these items hence the reason YOU as a facility are getting paid $4000 a month to care for her. Another major issue is they contract their nurses for a few days at a time, so the nurses aren’t employed by life’s journey. For this reason the nurses never get to know the patients, and this came straight from a nurses mouth. Also, most of the staff here are not CNA’s, they are “resident specialists” who have NOT had the proper training at all (and again, this information came from a nurse). The day we moved my mother out last week, one of the cna’s actually apologized to me on behalf of life’s journey and on behalf of [name removed], who was more than rude every time she blamed my mother for something and showed no bedside manner. To top it all off I found out that their DON walked out as well because of the mess that is going on. The straw that broke the camels back for us was that last week my mother swallowed finger nail polish remover, and rather than call poison control they called her doctor (who was already on a log to call back for something else and was extremely busy). Well 45 minutes after I got the call that she swallowed the remover, while I was on my way there (because I live an hour away), I called to speak to [name removed] (director) to see if the doctor called back. He was very flat and almost irritated, and stated that they were still waiting on a call back. I said “Well in a situation like this you should call poison control”. He simply said he’d look into it, and got off of the phone. We moved my mom out of that place that night. During that time I heard 4 stories about what supposedly happened with the finger nail polish remover, but I do know she drank it because someone finally came clean and said they saw her drink it, and that my mom was saying that her throat burned. This is a long post but it is truly just a few of many MANY incidents that happened at Life’s Journey. Please, PLEASE do not bring your loved ones here. The good news is that my mother has not had ONE “behavior” since she left! My mother is one of the lucky ones that made it out. I feel sorry for anyone that is still stuck there and their family who mistakenly (just like us) trusted these people.

I came in unannounced. The administrator who an employee told me was new was the most ignorant, uncaring person I have ever seen. I heard her interact with employees as well as the loved ones of "somebody". She was rude, condescending, down right mean and disrespectful. I spoke with residents at the facility as well and what I heard was frightening. There have many outbreaks of BED BUGS...that resulted in bites on residents and lack of concern. I observed residents just sitting with no companionship. Most need to be in a nursing home where they are cared for. This is a place that will take your loved one and your money and then they are forgotten. The smell of urine reeked throughout the building. Very sad that whomever owns this place is allowed to as well as shame on the woman who is not in charge but in control. The smell of cigarette smoke was throughout the building. Residents smoke in their rooms, as I witnessed as I walked the building. Employees disgruntled. I also spoke with family members of loved ones and they are searching for other a new place for mom/dad. Tinley Court is a nursing home

Where do I begin... My family member lives at Cedarhurst in Centralia. And I am NOT very happy with anything in the facility. I can't wait to get my family member out of there. The staff IS NOT very friendly at all! They are so hateful and rude. Particularly the 'HR' ladies in the office. I won't use full names but I'll use initials of these people. [Name removed] (cant remember her last name) is very rude and snarky, she will pretend to be super nice to your face and then talk really bad about you or your family member behind your back. She's also very disrespectful to her staff. All the same goes with [name removed] she is very rude to your face. She is very disrespectful to her staff and on constantly on her high horse. As for the staff unfortunately all the nice ones left. And I see why. A few have stayed but, slowly more and more everyday I like them lesser. I dont like that 80% of the staff are fresh out of high school with very little to no experience in training with alzheimers/dementia patients. Cedarhurst takes pride in activities. One of my favorites unfortunately left but I can understand why. [Name removed] was a very kind and hard worker. But I can tell towards the end of it she was very mistreated. Same with the old DON, [name removed] she was very nice and helpful. And there was a good team for a short amount of time. I could name CNAs that was pretty good. [Names removed] Then great nurses like [name removed] and [name removed]. But honestly, that was it. I loved how they all worked together for a while. But now it's nothing but constant drama and a hateful environment. The cleanliness is okay, sometimes they clean. The place is infested with bugs and the workers constantly smell heavily with cigarettes. The food is pretty good, better than it used to. But the kitchen staff can be pretty hateful and unclean as well. The family staff meetings are always terrible, they're monthly. And if you complain (which I do, I try to be better with it but, can't help it) it goes one year and out the other. It's pointless going. The CNA's, especially the younger ones, dont truly watch the residents. My family member is incontinent and constantly gets bottom sores due to lack of care. They also get sick constantly because they go outside when weather is too chilly without a jacket or some sort of protection. Again, the activities, it used to be really good. But in the middle of August 2019 when of my favorite staff member leaves, it went downhill. The activities are not very mindful, fun, or helpful with the resident's condition. My family member constantly tells me that they do not enjoy activies. My family member says they're constantly pressured to do activities. I understand its important to keep the mind activie to prevent boredom and depression, butwgat about the anxiety they feel when pressured to do an activity that's not fun or exciting to anyone. The place is VERY expensive as well. Even with food and activities included, it's not worth it. Constant financial struggle when the place gets more and more expensive everytime I turn around. I'm so over this place...please I beg of you do not waste your time. And if someone you know lives there, do your best to get them out. They're getting mistreated one way or another. I can promise you that!

This place was not at all like they advertise it to be. Consider all other options before moving a loved one in here. I feel as though the staff is not there for the residents. It is not clean. Main meal is served at lunch,dinner is a sandwich. Difficult to get shower assistance if your loved one needs it. I am very disappointed.

Everything was nice while mom was there, but once she passed, that was another story. The administrator was rude, condescending & a liar. It was like pulling teeth to get our deposit back - had to call to get the money & was told things must have fallen thru the cracks - that is not acceptable to us.

just go there and walk into the building, the odder of human waste is what greets you. They have had "bed bugs" for over a year now and will not take steps to protect the people. A rear door was being held closed by a snow shovel. When I tried to talk to this so called administrator Debra Scinto was condescending and rude! not a place for my mother.

this is the most God awful onsite home health company. the building smells of urine, and the residents do not engage in anything..just sit in the dining area alone, with out any interaction with anyone. this is a nursing home
