Trilogy Health Services

This facility felt more like a traditional nursing home than a residence to me. The tour was informative but probably not what we are looking for.

Nice place, friendly people. Memory care unit kind of small for someone who can still walk very good. Private rooms seemed small.

I wish we had more RN's. I am very happy with my room.

I believe progress is being made to make this a good facility.

I think the residents need more attention to their personal hygiene. I think the therapy dept. does a wonderful job. The office does a great job too.

Very clean facility. Good food. However the staff is very unorganized. Staff should be informed of daily happenings, and should be able to answer general questions. Several discrepancies with billing, overcharged, extra charges, double charges. Phone calls never seem to be returned. Medication is given at convenience of staff, sometimes not when ordered. Lack of communication with family members. This is a good place for those with independent care.

The rooms are very small. There is not a lounge area in each wing of the facility so residents with mobility issues can gather and socialize. When my father would wonder out in the hall they would quickly redirect him back to his room. The dining service was very slow, often times my dad would have to wait 45 minutes for his food to be served. This could be fixed by letting the residents order their food one meal ahead of time, and by providing a choice of two entree not just having a main entree and al a cart. The quality of food and care did not match the $ 8,000 a month price tag.

Not the kind of environment that we were looking for. Corridors look like a hospital. Staff members were pleasant and welcoming. A lot of a la carte services that require a much more significant cost. For example, rides to doctors or shopping that are included in other communities are an additional cost in this community.

Tour director did not seem to know very much about services offered in Memory Care

I have started to settle in at The Oaks at Woodfield, I have a nice sized apt that I am very comfortable in. The staff are friendly and they have a nice sense of humor plus I like helping the other residents who are in wheel chairs as we get to know each other. There are a lot of great activies and good field trips offered but the atmosphere is more like a nursing home here, the residents are quiet don't mingle and it's kind of cold there's just something missing. As for the food well, sometimes the food is good other times it is awful they just can't cook vegables right at all. They do have backyard BBQ's from time to time. One of the reasons I choose The Oaks is because they do have 24/7 nursing staff on at all times which is required by my long term insurance a lot of places do not.

The staff at Wellbrooke gave a wonderful tour of the community. They were very friendly and knowledgeable. There were a lot of positive things about this communities the apartments were furnished, but my Uncle felt the atmosphere was a bit too cold and he did not feel welcome.

This place was little bit older when we looked at it. The people were nice and helpful and they answered all the questions that we had. There were residents that were out and doing things in the community. Mom just did not like it well enough to go here.

The staff was very helpful with answering any questions. I just feel that the place was interesting. The amenities that this community offered was not what I was looking for. The community was clean and kept up.

My dad has loved this place. But the new director is very bad! My dad needs to walk twice a day. But no one will walk him!! So very sad! Have emailed the director regarding this, ignores everything. Not happy!!! I think the new director is out of touch. So sad to say...great place going down hill because of new director.

Our loved one used this facility for rehabilitation care after a hospital visit. The care they provided in the rehab section was very good. The room was very lovely. Because of our loved one’s memory issues, it was a confusing environment for her, but the staff was understanding and tried to make the best of that situation.

We toured this location when we were looking into care options for our loved one. This was a nice enough looking community, and the staff were friendly, but the rooms were VERY small! I wish they had had room sizes listed on line, I could have saved myself a drive. They also wanted to have our loved one in an odd care set up, where he would spend the day in the assisted living section, and nights in the memory care section. This would have left him in a position where he could not return to his room until the evening.

My mom was there for rehab. She actually stayed there a month for rehab and then two months after that. We didn't like it at all. The communication was horrible. They told us one thing and never followed through with what they told us. They never could get the billing right and their dining room needed help. It was a beautiful, brand new facility. We thought that we would really like it there but since they are so new they didn't have their systems in place. After 3 months there it didn't seem like it was going to get any better and we weren't willing to let them keep experimenting with their processes while my mom was living there. They even problems with the rehab care. Seeing as she just came out of the hospital she was in pretty bad shape so we really wanted great care and it just wasn't happening. All her basic needs were met but it was like pulling teeth to get them to communicate with us. The good thing there was mom's doctor there. We loved the doctor. That was the only thing that we really liked there other than it being nice and new.

Before covid, I visited several times, after covid, there were no activities, and he was in his room a lot and there wasn't much to do. Unfortunately, my loved one got covid and passed shortly after. before covid, I would have recommended this community but right I don't think I can.

IT WOULD BE EASY TO GIVE THIS FACILITY A 5 STAR RATING IF WERE NOT FOR THEIR DIETARY DEPARTMENT. MY MEALS ARE RARELY CORRECT ON FIRST PASS. WRONG MEAL SELECTION DELIVERED, NOT ALL FOOD ITEMS DELIVERED (NO SALAD DRESSING FOR SALAD, FRESH FRUIT = CANNED MANDARIN ORANGES ETC) IF THIS WERE ONLY A SINGLE OCCURRENCE, IT MIGHT NOT STING SO BAD; HOWEVER, ON NUMEROUS OCCASIONS IT HAS OCCURRED WITH E-V-E-R-Y MEAL OF THE DAY. THE LACK OF QUALITY CONTROL IN THE DIETARY DEPARTMENT, IT IS ACCEPTED AS A NATURAL OCCURRENCE. OTHER STAFF REMARK THAT THE POOR DIETARY DEPARTMENT SERVICE IS NOT UNUSUAL. THERE ARE TOO MANY GOOD EMPLOYEES IN THIS FACILITY THAT CARE ABOUT THE RESIDENTS AND THE QUALITY OF CARE SERVICE THEY PROVIDE THE RESIDENTS. IT IS MY OPINION (AND EXPERIENCE) THE DIETARY DEPARTMENT DOES NOT DEMONSTRATE A QUALITY-MINDED ATTITUDE.
