Our loved one is not happy here, and neither is the family! It seems like they really don't have their stuff together here. They are not very organized at all. The food is horrible!! There was a man that threw his plate on the ground, it was so bad! Nothing is fresh - it either comes frozen or pre-made. There are no cakes or pies for dessert. There's no flavor and no freshness, and it's just terrible! The ladies that serve the food are not servers. Also, there are no activities here! You could hear a pin drop in the dining hall. There's no buzz, or social interaction. The nurse's station is down a long hallway. Sometimes, they come, sometimes they don't. They empty trash throughout the week, but not on weekends. They charge for every little bitty thing. The staff are all very friendly, but they're really not very helpful. They are sweet, but you really don't get much help from them. It really seems like they weren't ready to open when they did.

They have a memory care unit which is where my husband was. They said they knew what his condition was and that they could handle him. They were not able to care for him and they sent him to Oceans. I noticed when we would visit that the residents were all just sitting there, not walking or talking almost like they were drugged. I would not recommend the memory care unit to others looking.

Stay away from this facility. There is a problem keeping staff. My mother ended up with several bedsores. The CNA’s are made to be the cooks and believe me most can not! Awful food.

Don't be fooled by the nice look of this place or the salesperson trying to get you to sign up. You will regret putting your loved one here, and it will be heartbreaking. We thought this would be a safe place for our mother, it is not! They are short staffed and there is high turnover, so residents in Assisted Living have to wait a very long time for a caregiver to come help. The call system is useless and rarely works properly. There are constant medication errors at Clayton Oaks. They allow unqualified people to distribute medicine, because they have very few trained staff. In Memory Care, residents are either parked in front of a TV for hours, or left alone in bed with soiled clothes and missing meals because it's easier for the caregivers to just leave them there. The staff has no empathy for them. Many of the residents have fallen and broken bones. Family members install cameras in the rooms, and have watched helplessly as their parent was treated with neglect. Clayton Oaks had a major COVID outbreak and at least nine residents tested positive, and many staff members. I would not recommend this place to anyone. Your parent will quickly experience major decline if you admit them here. I hope my review can save at least one person.

Horrible place to live, if you fall into the category of needing any memory care assistance. Although a new facility, the extent of the mental illness is unhealthy for the unprotected, unknowing residents that also live there. It isn't safe to have your loved one left here and to assume they are being closely watched.

I strongly encourage you to look elsewhere if your parent is in need of any assistance. My mom was kicked out of Mapleshade while on hospice after living there for three years, at the same time we realized through hospice, not mapleashade, that she had a stage 4 bedsore on her sacrum(please google stage 4 bedsore). Hospice told us we needed to urgently move her, and that same day we were told she had to leave Mapleshade. We moved her to a residential care home urgently and my mother passed away three weeks later. The constant change in Executive directors here is horrible, the constant change in Nursing directors and their use of agency or temporary care givers is even worse. There were a few people that were great and cared for my mom, lovely people in the dining room, but now looking back Mapleshade should have contacted us 6 months or a year ago that my moms care needs were too great. We had a constant nonstop battle about cleaning her room, prescription drugs, doing her laundry. Our relative who visited my mom three times a week actually wound up taking my moms laundry home to clean because it was never done. The day my mom left, no one said goodbye to her. That day no one had changed her, given her any food or liquid and no pendants were found in the room. The day my mother arrived at her new residential care home - which was amazing- she had a urine and feces soaked diaper, a stage 4 bed sore and bedsores on her feet, skin breakdown on her neck, and arms. My mother had great care needs and we looked at this place to be honest with us and tell us when it was too much, but they did not. We never were even contacted by the Executive Director. Shame on them, shame on the management and the staff that let this happen.

Hello Spring Creek Village Management? Do you just send out canned responses by corporate or do you address the very serious issues raised in my post? Just curious...... I decided not to return my parents to this place after my father suffered a heart attack from all of the physical demands related to the bathing and dressing of my mom because Spring Creek Village failed repeatedly to deliver the “higher need” levels of service they promised. So he did it himself After providing a 30 day notice of move out, apparently Tab / management decided to have their attorney send me a demand letter for @$6500 with no supporting documentation ....nice!!!!! DO NOT expect to receive advanced levels of care as your parents transition from independently bathing, dressing to needing assistance. This is NOT an Assisted Living facility My parents repeatedly commented that they believed they were understaffed, under- equipped and inadequately trained to deliver what they claimed they could.... I recall my father falling in the bathroom and pressing his emergency call button and later telling me that he laid on the floor for TWO HOURS before anyone came to help!!! My dad told me the staff’s only comment was that they were very busy that morning.... The building is very pretty though!!!!!

I recently had a conversation with the terrible manager of this place. I was told my Mom’s apartment floors will not be mopped and the bathroom will not be cleaned. I was rudely told they only do light dusting by [name removed]. We pay thousands for my Mom to live here. If she could clean her own bathroom and mop her own floors she wouldn’t need assisted living. We are looking for a new place ASAP with nicer management and better housekeeping. I’m disgusted.

The management staff did nothing to satisfy my many concerns about the facility. I called the management company to complain several times and was told to work with the staff at the facility. I have been taking care of my parents and in laws for the past 6 years at several different assisted living facilities in town. I have never encountered the problems like I had at Clayton Oaks. This review could go on and on with problems I saw first hand every day. I agree the facility is pretty but it does not have a system in place that is reliable. My continuous suggestions were ignored. I highly recommend you plan on being at the facility on a daily basis to be sure your loved one is being taken care of. I would not recommend this facility EVER!

Our dad lived there over a year. We thought it was the best place at the time, and being a new facility it was clean & nice, but that changed. Memory Care can’t get the urine smell out of the carpet. After a few months the really good caregivers started leaving. Understaffed, too many unqualified caregivers being lazy or on their cell phones, caregivers stealing from residents, caregivers leaving residents unattended in the bathroom, inattentive to residents’ needs, an unsafe atmosphere on their memory care side. Dad got too many UTI’s from poor incontinent care.....and the list goes on. Do not put your loved ones there.

A tale of two homes...This is a beautiful facility with good options for food, activities, and social interaction. Beyond the facade is a world of poor management and broken promises of care. I’m mortified that I ever rested well while my Dad was in The Villages at The Heights. Despite paying for upgraded care levels, we moved my Dad to a new home recently with two weeks of dirty laundry, a full urinal that had not been emptied, dirty clothes, diaper rash, a soiled diaper, etc.. He had not been bathed in a week. Many hourly workers were wonderful, but I’m still waiting on a response to my email to facility management in December 2017.

If my review can save but one family from experiencing what we have endured here, it will have been worth my time and effort. We entrusted this facility with our beautiful mother. It was the biggest mistake we ever made. The lack of regard and empathy for Altzheimer patients here is appalling. There are multiple Medication errors on a daily basis. Our loved ones are left to sit in soiled clothing. This is not what our aging parents deserve. Please beware and do your research.

Clayton Oaks has no knowledge of how to deliver care to Altzheimers patients. The patients were walking around confused and seeking help . Many were visibly soiled and the caregivers could not be concerned. It was incredibly loud in this facility which seemed to agitate many. The clock on the wall was broken and they had the day and date listed wrong on the day calendar. One resident remarked about it saying “ as if we aren’t already confused enough.” Not a good fit for our beautiful mother.

This place has really gone DOWNHILL! It is under new ownership and No positive changes have been incorporated. Medication errors run rampant and the elderly sit unassisted in soiled diapers. The director does not provide the staff with the tools and resources to deliver adequate safe patient care. The focus is on creating an opulent facade and not on the preservation of the dignity of our loved ones that we entrust to them. Heartbreaking.

Big Mistake! We selected this community after coming to the painful realization that we could no longer keep our 91 year old mom at home any more. They will promise you the moon to get you in there. We didn’t need the moon, just a safe, dignified facility where mom would not be afraid. The medication errors run rampant at Clayton Oaks. The medication distribution itself is sketchy at best. The residents are treated with no empathy or dignity. They are left to sit in soiled diapers. This facility has no knowledge of the proper care of Altzheimer and dementia patients. They are more conserned with projecting an opulent impression in the assisted living areas instead of focusing on safe regulated patient care. Luckily we descovered this after 5 months and are moving mom. Unfortunately anytime you relocate an Altzheimers patient there is some degree of decline. A consequence that our mom and family should not have to bear. Heartbreaking.
