They have had challenges with staffing. The food is really bad, and the chef left. They need to improve on communication, and letting the residents know when something is not working. Management is not always available. The food needs to improve, and they need to add more protein, and offer better options.

For perspective to my answers, I would like to elaborate. I was advised that my loved one would be good in assisted living instead of being placed in a memory care unit as they are not likely to walk out and they are still quite social. Most residents in assisted living aren't dealing with memory loss and the behaviors developed to compensate as the disease is progressing. My loved one does a good job of hiding their deficiencies. I personally do not believe there were good schedules or process with a daily accounting of what each resident needed assistance with. So, when my loved one began putting on the same clothes after their shower no one caught the error. My loved one wore a set of clothes for a week on more than one occasion, but still looked clean and tidy. It took me reaching out to the executive director and director of nursing a few times and honestly is still a work in process. When I reach out the staff is always very accommodating and helpful, but I feel the staff should be more proactive of the needs and assistance of a resident. I do feel that the staff are positively working with me to see that we stay on top of the changing challenges my loved one is facing. Again, my issues have been because I do not believe the assisted living community was prepared for the memory deficiencies my loved one has. I do not believe residents with fewer memory deficiencies are experiencing these difficulties.

They are overpriced and understaffed to the extreme. They misrepresented who they would have available on staff. When we first brought my friend in, we were told there would be a 24/7 nurse on staff. Later on when he broke his hip, they had no nurse on staff or any other medical personnel in the building. Regardless of credentials, there was no one around for me to call out to for help, so I had to hunt somebody down, and they called the fire department. The room he was in was not clean. His toilet and sink were unclean to the point that I felt the need to clean it. There was no timeliness to housekeeping services. I had previously experienced a different facility with a family member of mine, and that facility was great - far superior to what we experienced here at Canopy.

I was very disappointed with our experience at The Colonnade at Northdale. We have had tons of problems with accounting. We were not happy with the lack of services that were provided and the treatment was poor. My loved one had covid and there was no sign or anything on the door. They said hand sanitizer was on the outside of the room but that was not the case. The cable did not work either. We were disappointed with the whole experience.

My father was on the memory care unit and we were just not impressed. I don't think my his room was ever cleaned. I wasn't there everyday, but did visit often and it was never clean when I went. He never looked well groomed/shaved or taken care of as far as hygiene. After he passed and we went to get his belongings, his room looked untouched, other than the bed being stripped. The TV was still on, it smelled, there were soiled diapers still in the room (which we weren't aware they were even using), and it was just awful. They also assured us that his rate would be pro rated since we'd paid the full month and that we would be refunded the difference. After several calls and emails, I finally got a response that the check was in the mail, but I still haven't received it. We asked that they not give him chicken because he didn't like it, but witnessed them serve him chicken several times. My son works in the food industry and hated the food. He visited often and would comment every time on the poor quality. They seemed to be understaffed and the quality of the staff they did have was not great. The assisted living side seemed to be much better from what I witnessed. I actually have a friend with a loved one on that side and they are happy with it. I would recommend possibly the assisted living side, but not memory care.

Landscaping is really bad. Some of the management has been rude. On weekends the front desk does not answer calls or return calls. My freezer went out on a Saturday and I lost all of my food. Dog park in NOT fenced in. One of the retention ponds does not have a fountain. The front gate does not work. There are NO peep holds in the front door. We do not have 24 hours security service as advertised. Was told that Seagrass offered a driving service within a certain millage distance every day except weekends, but now only offered twice a week at an appointed time. I have mold and lichen all over back yard up to screened in porch. Weeds all alongside of house where landscaping beds should be. Large Armadillo holes everywhere. Serious drainage issue in back yard from rain water run off. Backs up into my yard and sits. County told me it was designed that way??? There are no motion sensor lights in back of house, just woods and very dark.

We decided to look at Seagrass Village because we had seen the high ratings that it has and my family who works in the industry said they had never had any lawsuits against them. The facility offers multiple levels of care and has multiple floors. My loved one would have been on the second floor which would not have worked. I asked what the evacuation plan would be if there were to be a fire since she cannot walk and they said residents on higher floors would stay in place until the fire department got there and I did not like that answer. The director also seemed to be preoccupied when I was asking questions.

Observations included neglect and verbal abuse. Neglect from nursing assistant staff, observed through responses to residents that indicate indifference. Eating utensils were unavailable for residents to utilize during meals, when a resident requested a utensil they were told that "We don't have any", Napkins were often unavailable during meals, when a resident would request a napkin they were simply told "We don't have any". Delayed or no response to resident requests. There are residents living here who would benefit from assistance with meals, that was not offered. I was once taught that Staff perform in a manner that reflect how they are valued. Nsg was observed to shout at residents in an aggressive manner in response to resident behaviors (due to dementia). Perhaps these nurses are overworked, management should be monitoring. There are some residents that would benefit from Skilled Nursing, as they were no longer appropriate for an Assistant Living Facility.

If your loved one is indepent than this is a good fit. My mom did fine since she was able to bathe herself and walk around. Management changed twice in the 8 months she was there. Keep a close eye on your loved one if they need any assistance. Some of the staff is more caring than others.

The most important thing to my friend is religion and I was assured that they would have Sunday programs and weekday religion study groups. This has not occurred and has depressed my friend and other residents very much. But this is not the reason I give a bad review. It is because of the medical staff there. [Name removed] is the medical director and she is incompetent, uncaring, unfeeling and a liar. I have found very poor medical care there which is what we requested and needed for the resident. They do not answer questions. They do not check on residents. And they do not have proper equipment there. I have even had our healthcare representative try to contact them and no one follows up or responds to her.I would not advise anyone to take up residence here.

I've had to ask the community to change the sheets of my loved one several times. I've had to change the sheets for my loved one multiple times. There was a pile of clothes of my loved one at the bottom of the stairs. I ended up having to do the laundry for my loved one. I'm very upset over this. Creekside doesn't have enough help. There is only one person working there. The community isn't giving my loved one a shower or keeping clean clothes on them. We are planning on looking for another place. One time I went in to see my loved one and the residents refused to eat the food.; as the food isn't good. I've complained that there is no fruit around.

When putting my clients wife into seagrass it was the best thing we've ever done. At first it started off great a lot of care but then it went downhill after that. [name removed] uses the bathroom by herself but the many many times that I've been out to the facility she has been pure wet and it's sad that I have to be the one to tell them to go change her. I believe a urinary tract infection had triggered another stroke and unfortunately we had to place her into the hospital and died 10 days later. I believe with a lot more care than just leaving her in the bed all day long and checking on her they could have done more for this family
