They had caring staff in the memory care unit. They seemed to handle his outbursts well on the initial time he was there. When he ended up in the hospital we received no follow up from the facility. It quickly became clear that he was not going to be able to return to this facility. Initially a newer manager of the facility said they would take him back but I am not sure he checked anything medically with the hospital staff before giving his ok for him to return. As family members, we figured that since he was needing the assist of two to be moved that he was no longer eligible for the memory unit. We were not call d for any updates. We finally told them that he was going to hospice care at a skilled nursing home. Dad died [date removed] at that facility. They had a deal that if we paid for a full six months up front that we would receive a free month. After he left Fields it took six months to be reimbursed. Luckily we had the funds to pay this. They were nice people but better follow through on their part was expected.

I would say it's fair - my sisters and I are always having to ask questions about his housekeeping and his physical therapy sessions, we were told he would be encouraged by staff and other residents to join activities but we have not witnessed that or been informed of any such things, the food is very good and his apartment is nice but it has had some mechanical issues that have been resolved

There are some caring staff members while others need a lot more guidance and supervision. I feel like I needed to check daily to see if care had actually been given when certain people were there. The head nurse manager appeared very non-personal and seemed offended if you asked questions. The day shift nurses weren't very helpful either. They seemed to want to be anywhere except at work taking care of the residents like they were burnt out. The evening nurses were very helpful and more compassionate in my dad's last days. While I am grateful we were able to place my dad I am glad he was only there for a short stay. I do feel with a bit more supervision and guidance there could be great improvement. Over all it has the potential to be such an asset to the community with better leadership.

My loved one says the quality of the food has gone downhill significantly since she moved in, She also said they seem short staffed a lot of the time. She is not a happy person and is hard to please but I think the quality of service has gone down.

They have done better than most other facilities I have dealt with in the recent past, however, their mgmt of medications seems not consistent and informing concerns to family/guardians can be lacking at times.

It is a newer and nice facility, and there are a lot of activities. Most of the staff are very caring, but medication management could be improved on

How often the rate changes. In 18 months it went from [fee removed] to [fee removed]. Way too much. I had to move my Aunt out of there. Her apartment was very dirty, although we paid for housekeeping. She was disheveled and hadn't showered, although we paid for personal care. I think it's fine if you're truly independent.

My mom was only there for 5 weeks before she passed away. We chose Carriage Hill for their cost, it was affordable. I would recommend them only if cost matters.

Yes, they need to do a better job of onboarding, making new tenants aware of the facilities and what they have to offer.

Expect to pay a lot for basics. We would only recommend the community to others if they don’t care about paying $[fee removed] for basic care. Everything else above that cos

Facilities are okay. Staff is nice but very lean in numbers. Food was not good.

My mom ended up in the hospital the very first week she was there because I didn't have her medication correct. And then my mom informed me yesterday that they don't turn the grill on for them to have meals at night.

At the beginning very helpful if you have questions but Once you move in You can ask questions and they tell you they will get back with you But they never do ( they forget) They need to have you make an appointment if they are busy when you ask a question or if they don't know the answer right then Such as how will your love one get their medicine Or help with the billing or what does something mean on the billing They could have the front desk write down what you need and then make an appointment for you

We were in the memory care side. The staff was friendly It was clean in the common areas. Our room was cleaned once a week and laundry done. Ours had carpet and when you have someone that has a lot of accidents, I feel they should clean carpets once a week also at least. The tv was always on in the big room. They did activities or had someone come in to play music or they did trivia. The food was really good. I just think they needed more staff for the 35 or so rooms. There were only ever 3 on duty per shift. Other than that it was good.

Cleanliness is very good . Food is good. Willingness to adapt to the resident was was inadequate.

The staff formed a great relationship with my mom while she was there. Unfortunately there was a lot of turnover. The staff was very unhappy with the amount of work they had to do, in comparison with their low wages. Most of them were disgruntled, and shared this with my mom often. The biggest turnover was cooks. The food was terrible according to my mom. They were not prepared to provide meals for anyone with dietary concerns. Because of staffing shortages they could not keep up with my mom's needs. Thankfully we had hospice care that was able to provide care for her.

They are short staffed ... by design. Only 1 nurse and two RA's for 36-39 residents. Per 12 hour shift. Those same RA's are also responsible for setting up and cleaning after meals. So the residents are getting very little individual attention. So there is a lot of turnover... so not really getting to know the residents and who needs to be fed vs who can feed themselves. They have stepped up activities for those that can walk and participate. The others are mostly left to watch tv all day. The facilities are very nice and pretty. The rooms and bathrooms are large and quite roomy and kept clean. The meals are adequate... though there is always a shortage of forks... even with salads they are served with spoons many times ... not always enough tea so residents are left to drink lemonade or water.

I had a lot of complaints when I moved in such as old flooring that you couldn't get clean. When I brought it to the executives s attention they would lie and say they never heard of it.

They are very caring, but often shorthanded. The facility is better than most that we looked at.

Nice place to live. We need more help in all areas. Management lets everyone know how wonderful it is but its not they accepts people who [name removed] me here. They need a new cook and chef me haves the same thing every month to nice stains are manage doesn't keep up with things like the. Cannot use no license plate. He never carries this with anything doesn't like to talk to residents or family.
