The coming of a new year at Lewis Park Estates brings with it, new management and care teams. I am looking forward to getting to know them; as they diligently work to care for their many residents which, also includes my mother.

The staff are all very nice, the facility is clean and well-maintained, and there are a variety of activities to choose from. My only complaint is that it takes a while for requests for new services to get implemented.

So far I do not have any complaints on the Wellstead... I have found most of the staff is very pleasant, they usually have quite a few activities that go on in the afternoon or evening. They have a monthly calendar that comes out with the list of everything that will be going on in the area which I believe is very helpful for family or visitors in case they want to come and join them. The dining experience Was a little difficult for my friend at first because a lot of the people in memory care do not communicate very well and she was not expecting that. But the food seems like it would be very good and she is diabetic and gluten free. So at first it was a challenge but now they are giving her the right things to eat. I think the room she is in is fairly good size on bathroom shower, room for a twin bed, And then she also has a chair and love seat in the sitting area which I feel that could have been a little larger. But it's okay...

The staff make me feel like part of the group.Its easy to get help.i get informed on things . I get help with my exercises Staff makes different exercise to help stay involved .I would recommend to my friends

I enjoyed the tour. [name removed] was friendly and explained the materials in the packet. I noticed how she greeted staff and residents. I liked the views on both sides of the building. I like that the hallways are short, not what I expected for the size of the building. I know this is an older building and it was originally not intended for assisted living. Therefore, the doorways seem narrow for walkers and wheelchairs. The "cutout" step-in tub is interesting. It definitely makes the tub accessible. I'm a bit concerned about whether the walk-in opening and whether it would be a problem for me.

What we like the most about the community is the Adequate # of staff to meet all the medical needs. The nurses and their staff always accessible and have good response times when called. All the staff from reception to cooks are friendly and interactive. The activity schedule is second to none. Menu selection and food quality is good. My dad LOVES the pool table, games and weekly outings. So the Forum just went through an ownership change in the last couple weeks that new company keeps up the good work. There have been issues, but change is always hard and I'll extend a little grace

The overall experience was as good as could be expected. All staff were respectful and considerate and took pride in what they were doing.

He's a great activities coordinator. They do a lot of daily and monthly activities. Go out for field trips to go out for a luncheon once a month eve band is really good size and it's wheelchair accessible. The building I think itself is lovely. The dining room is really pretty. Unfortunately the food is not well liked by alot of the residence. It's provided by Sysco and I just think it's done for large groups and not a lot of local fresh vegetables or fruit. They have a number of staff who have English as a second language and for my mom who's a little hard of hearing that makes it very difficult for her. There is definitely a language barrier.

Rooms were very nice. Dining was satisfactory. Loved all the activities. The only complaint I have is the explanation of the refund after my mother past away. We were told by 2 different people that we would get a partial refund for the month if we could get all my mothers things our quickly. Then when it came time for the refund we were told that there was an additional 2 weeks added after her death.

I would rate the facility a 4 all the way around! The staff and communication. I feel like they listened to us, the family, about his likes and dislikes when we moved him in. The RN at the facility has been very good and professional when it came to his needs being addressed in his care plan and delivery of his meds.

Friendly helpful staff that interact personally with residents. I highly recommend Duluth Heights.

Mom is in the memory care wing. I looked at several and it was by far the nicest and most spacious, including both the rooms and the common areas. I especially like that they have a lovely outdoor patio that is both beautiful and secure. It is clean and quite pleasant with plenty of room to walk a nice "loop" around the unit. There are a couple of seating areas inside, one with a fireplace and the other set up to be a calming space. They have a dedicated activity room and a lovely dining area. Staff at all levels, including housekeeping, aides, nursing staff, front door staff, and administrative staff never fail to be friendly and helpful. Communication has been spot on. If a need or concern arises on their end, staff reach out to me via a text and/or a call to resolve the concern. When I have had questions, as this is a new experience for me and quite the learning curve, I have felt that whomever I have spoken with was responsive and helpful. Also, as Mom is still high functioning, it was important to me that there were other high functioning residents with whom she could form friendships. That has indeed been the case and Mom has adjusted well.

1. 4. Would be a 5 but our salesman resigned shortly after my brother moved in and we were not transitioned to a new liaison before he left. He was wonderful! 2. Very clean and inviting 3. Staff was very friendly. The wellness director is a no nonsense type of person, but she was very kind and helpful after my brother had to go to the ED 4. Nursing assistants are very good. Some hiccups getting him seen by the PA 5. Haven't had much contact with them. Still don't know how we are billed. Imagine we'll learn soon since we need to pay for November 6. My brother loves the food! His favorite part 7. Compared to other places we toured, I feel like this is the best value for the money. Now if we see a lot of extra expenses on this month's bill, I may have a different response

The staff members are extremely welcoming, and helpful. The facility is beautiful and well maintained. Categories that received a 4, I felt I could not rate fairly, since I am not living there yet . I know, a 4 is not that bad, but it's not a 5 either.

My mom chose this community because she wanted to be near her friends that could come visit her. They all live in the area. I would recommend to others.

Nobody is perfect and there was some fine tuning. They have great staff. The food is good, the room size is good, it's on the noisy side with the indoor playground attached.

The staff were great, Who wasn't great the people in charge of parameters on coming back from his operation. No one told me with a colostomy bag he wouldn't be allowed back. This should have been told from the beginning so I could look for a new place. So nurses and staff were always good and [name removed] seemed to enjoy himself.

The management is very helpful at all aspects of the journey. You really feel like they have an "open door/ability to contact" relationship with the family too. I would say the only drawback or remotely "negative" thing we have experienced is ---- it would be ideal if the room could be cleaned on a more predictable basis. We have repeatedly told housekeeping the best time is Fridays at 11or1130.....but they come on Wed Thursday etc AND I'd say the level of "clean" is 6/10. If that could be improved, I think residents and families would be appreciative.

Overall experience adequate. Additional driver for outside activities would be a plus. Food prep could be more consistent.
