The memory care facility is a waste of money! DO NOT PUT YOUR LOVED ON HERE! For over $6000/mo the care you receive is terrible. There are a few caring employees but in general the care is not what you would expect for the cost. The care is what you expect from a Medicaid facility not one that costs $6000. The cleanliness of the rooms was poor (dirty, not cleaned up after accidents or partially cleaned, mold in cabinets), The hygiene of those in their care was subpar (nail trimming, hand washing, teeth brushing were rare if ever) and had to requests to such services to be provided. There is no responsibility for lost items such as hearing aids, eye glasses or shoes. I understand clothing can get lost and things can wander but for them to be lost forever and no compensation for their replacement when they were lost on their watch? Staff spend more time on their phones than tending to those in their care. Residents are left unsupervised and spend most of their time sitting in the main room watching TV. Management of the unit is overwhelmed and not effective so conditions for the residents will not improve. New administrator of Evergreen is a snake and I don’t say that lightly. You will never get a straight answer or a response to your questions. The sales staff are there to sell you on the facility and say what you want to hear but they misrepresent and lie. We were told they cater to the residents tastes, and make you think they are going to be accommodating, but that is all smoke and mirrors. As always, get everything in writing that is stated to you. We were told that my parent would not have to leave as one of our main concerns was how he would handle having to move to a Medicaid facility once the money ran out and we were assured that would not be the case and that they would be able to stay there. That got us in the door and they got all the money only to have to do what we wanted to avoid and move them to a Medicaid facility. Now, that said, the independent living is quality as long as you can afford it. You will be better off going to a continuum care facility.

My uncle currently resides in memory care at Sheridan Tyler Creek. He has Alzheimer's disease and has been there for seventeen months. He came to the facility because he couldn't live alone anymore & had fallen. Corporate has decided to evict him because he doesn't sleep at night. They want us to hire a 1:1 caregiver & pay for it from 9pm till 8am.. HE ALWAYS HAS PAID HIS BILL ON TIME! Only 1 tech for 18 patients on a memory care unit at night! Be careful before choosing this facility!!

Wellsprings is a beautiful looking place. However, the staff including everyone from the Administrator down to the aides and the therapist are unprofessional caretakers and horrible communicators. Do NOT, I repeat Do Not put your loved one in this facility. There was one nurse who was very professional and kind. All of the aides acted like they hated their job and did not want to be there and the Therapist always acted like they were in a hurry. They preferred to keep the residents in bed because they did not want to be bothered. This place is a joke. An aide put my physically disabled mother on the toilet and told her she had to leave and someone else would come back and take care of her. No one ever came and she fell off of the toilet. She pulled the emergency button but no one ever came. She was on the floor for over 1/2 hour before someone heard her call for help. The aide was written up and made it a point to tell my mother about it. I have spoken to other people that had loved ones in this facility and everyone says the same thing...Wellsprings is not the place that you take your loved one to for rehab or to stay permanently. I have reported them to the State and I will continue to tell people how awful this place is.

This place is a piece of [profanity removed]. There are numerous negative things that I can say about this place but...let me just keep it simple enough to say that they have very very poor management and only care about the money. The ED that's there now ([name removed]) is a [profanity removed]. I overheard some things about staff that should be kept between management but the staff, residents, and families knew everything. They will discuss your finances and personal information with other staff, residents, and families. The Wellness Nurse ([name removed]) doesn't care about the residents, she acts as if she doesn't even want to touch them. She doesn't direct her staff so basically the doesn't get done. I've heard a family say that their loved one hasn't had a shower in n over a month and the room hasn't been cleaned good in a very long time. I never came back after this incident. And to think, I was gonna put my dad here. [Profanity removed], ruuuuuuun.

Since the previous director left, several staff and families have also moved out. Costs are too high and the new director doesn’t seem to be as engaged. The food used to be excellent, but has really been unsatisfactory lately. My loved one lives in the personal care neighborhood; however, the residents seem to know everything going on... including that a memory care resident was missing and the police got involved. He was 4 miles away. Residents really shouldn’t be privy to private hippa info. They also heard the director never came in. Too many great staff members have left due to the lack of leadership both locally and from the corporate office in Tampa. Too expensive for what we are receiving

I had a nightmare experience with this home care agency. I was not expressed with the quality of care givers they sent me. They did not wanna work. When I asked one of the care givers to tidy up the restroom a little bit, she rose to her feet, and called the agency. They then called me to inform me that she is not allowed to do that. Which I think is ridiculous. Some of my appliances were broken by the caregivers. I had a horrible experience with this home care agency.

What you are told during the sales process is not what we've experienced. The care is woefully insufficient given the needs of the residents, so much so we have hired additional help, driving up the monthly cost from $9,500 to more than $16k per month. My wife worries constantly about the care her mother is receiving. Who is responsible to ensure North Shore lives up to its promises?

Very dirty bedroom. Smell of human and dog urine and feces. No sheets on bed. Mattress covered in pee both dog and human. Furniture filthy. Staff very rude. No notifications when relative went to hospital until their DEATH. Not a very good place. Would not take my dog there.

We were shocked at how poor the care was for our mother. The facility is beautiful but there are severe management and oversight issues. The ED discussed our case in the open with family members of other residents. We were called names by the staff. The ratio of caregivers to patients is dangerously low. We saw residents sitting in a room alone with vomit on the floor while the aides stood around waiting for someone else to clean it up. Our mother was incontinent and multiple times we found her briefs so soaked she had urine running down her legs. The aides are surly unhappy and express it openly. Moms hands were never washed and she had three rounds of pneumonia back to back - but they still took her th the dining room even though she wS contagious. We ended up having to hire outside aides 24/7 at enormous expense to keep our mom from getting worse and to get the care she needed. It was a nightmare. I will talk to anyone in person if desired.

Is a negative score possible! The executive director is a foolish individual, he tried to act sincere but was pretty fake the entire time. Maybe he should try harder a little less and be natural. The line staff were friendly but I just got the creepy feeling from the Administration, but the sales people were excellent. I just have a sense for BS and I sensed it strongly during my tour when we spoke with the upper management. Plus they seemed waaaay over priced and they have no options after the resident runs out of money, other than going to the nursing home or moving out!! Just got the feeling that the almighty dollar was all that mattered.

My aunt lived here for six years. Her room was always very dirty and the bathroom was even worse. There is no housekeeping to speak of in the residents personal rooms.

This is not a good community for getting proper care. I have had many issue with the place on keeping my mom clean all the time. Things were always missing. They were always understaffed and the weekends are more bad, 1 staff for 30 residents. I had to re-buy basic toiletries. When I removed my mom all shoes were missing. The staff are not responsible at all.

The management has lied from day one about the level of MEMORY CARE and assistance Mom would receive. They sold me a bill of goods and have not delivered on anything. Five key management positions have turned-over, been dismissed, resigned or otherwise, since opening day - Nov 2015 (first 90 days). The staff and management have failed on all key metrics: dining variety and quality, assistance with meals, respect and adherence to food and dining preferences, personal hygiene, number of showers, baths, etc, toileting on schedule, laundry - putting soiled items in with Mom's clothing for laundry, bleaching or otherwise ruining Mom's clothing- or losing it, using Mom's towels and wash cloths to clean feces from her body, etc.... The food and dining assistance is appalling - it resembles a correctional institution more than a "memory care facility". This facility is managed by one of the largest manage-for-profit firms in the country - Life Care Services - Des Moines, IA - and the owners do not give a hoot what the management company does. They are a robotic, unresponsive and uncaring organization. I have never received a proper response - in writing - to any care concerns or issues from any staff member - local or management company. Do not bring your family member, or anyone else you know to this deplorable facility.

The food is terrible! In fact in borders neglect. How do they get away with serving nothing but cabbage all day? There's no fresh fruit. Let alone all four food groups. You don't want your loved ones living here. Trust me.

This is a very fast talking crowd - they promise a lot and deliver virtually nothing. When she first toured and met with the manager, she was promised a driver when she needed it, despite using a wheelchair. She was also promised daily made service and good food. Once she has moved in it was another story. The food and service have deteriorated tremendously since Arbor took over the facility. Maid service is once maybe twice a week and the dinners are worse than being back in college. She is repeatedly denied transportation because she is handicapped. My parent was on the assisted living floor and as others mentioned, it was as if she had been banished.

She was on the memory unit – with their ratio between residents to care givers – my mother was forgotten. They didn’t manage hydration which lead to serious health care concerns. There just are not enough staff to take proper care of the residents

This facility has poor management which is reflected on the care given by the staff. The residents are the ones who pay the consequences when the staff does not do their job properly due to the poor management. They work short staffed often and have no praise for the employees who do what is expected of them. The employees are overworked, underpaid, and underappreciated. I feel for the residents who live at this facility.

A tour may impress you BUT never let your loved one stay here. My mother was left to lie in urine daily and they often did not have her out of bed even by 3pm. They made bed with urine sheets under bedspread. Multiple complaints to highest levels went unresolved long term. Management constantly turned over-5 times in 2 years. Unless your family member is independent to bath themselves, do not consider this facility. Moved my mother and her care has been excellent for last 2 years. A suggestion- non profits treat their staff and residents better than for profit facilities.
