My parents have been living here since the place first opened. When my wife and I first visited, we were extremely excited about the possibilities. It was unlike any facility we had visited or ones that my parents had stayed in before. We signed up and moved my parents here several months ago. Like with any new facility, there are growing pains. One of the main reasons we selected this facility was because of the food. Their kitchen is littered with state of the art appliances and the idea of a home cooked meal was music to our ears. We visited for lunch before moving in and they served a tenderloin that was out of this world. This was a great relief as my dads biggest complaint in the two facilities he was in prior was the food. Well, that was short lived. The chef either quit or got fired and the food service from that point on suffered greatly. My dad started to complain about the food and the lack of it. For dinner, for example, they were serving hot dogs, grilled cheese sandwiches, or a bowl of soup. My dad complained regularly. We talked with the management staff on many occasions, even spoke personally with the chef explaining that my dad does not like the meals he is being served. Although the staff acknowledged our complaint and assured us that my dad would no longer be served these types of meals, all fell on deaf ears. During one discussion, the management staff promised to keep a log of the food my dad was being served and track how many meals he was refusing to eat. Promises, promises. When asked to see the log, one was not being kept. Since he moved in, my dad has lost 14lbs. In summary, this place has a long way to go to measure up to the standards by which they market. At over 6,000 per month, one should expect a level of care commensurate with the cost. They are terribly understaffed and often do not meet the needs of the residents in a timely manner. We are now looking into moving my parents to another facility.

We were really disappointed because this one was one of the newest and they had lots of plumbing problems. We visited 2 or 3 times and they never got resolved. The food was really bad, like cafeteria food. I had a fish with a lemon sauce, the fish was definitely overcooked and it was very rubbery; they tried to hide it with a bunch of sauce and that didn't work because it was too lemony. My son had a hot dog and that was also overcooked- I don't understand how you do that- but when you grill hotdogs they have grill marks, these weren't grilled but they were burnt and rubbery... overall it was all burnt and it had no taste, except for my fish that was covered in sauce

This place should be shut down, because they draw people in with a promise of a free month and then charge the rent anyway. It's a classic bait and switch place, with med techs that have no liscense. Rooms never get cleaned and the food is horrible. Stay away!

Room size etc was fine Care not so much. Often laundry was piled up Many missing items. Most of staff sat around on cell phones while patients slept in chairs and ignored. You always could smell urine throughout facility. We often were told they would address our comments but----all talk no action Wish I could be positive but-----We took him out of there

My experience with Walnut Creek was extremely disappointing. While some caregivers were wonderful, the administration, billing practices, and lack of compassion created constant unnecessary stress. Issues we experienced: • Repeated billing and administrative disputes • Confusing tray-delivery charges and policies • Level-of-care charges that did not always reflect the care I observed—some caregiver visits lasted only seconds • After Mom left, they removed my camera and began renovating her apartment while still discussing additional rent • After Mom died, I was confronted with administrative issues before she was even buried, with little consideration for a grieving family Before moving here: • Get the Residency handbook BEFORE paying a deposit/community fee, ask what is refundable • Do NOT authorize automatic ACH withdrawals, manually pay each month • Get fees and promises in writing • Get in writing what is included in the level of care I would not choose Walnut Creek again!

We were told that the unit was freshly painted. It was not. There were nails still in walls from the last attendant in all rooms. The baseboard was two different colors of white. The dishwasher was not installed into wall correctly. It was falling out of the wall…it was not screwed into the top of the cabinet to hold it in the wall. The electric cord was hanging outside the dishwasher. Ready to fall on someone. I opened the clothes washer and smell of mildew like to have knocked me over. That would make anyone sick. The shower floor looked like it had been painted several times and the pint was peeling off the floor. It was very disgusting. The medicine cabinet had brown stuff all over the door. Look like something that was body waste. The carpet was very worn down. We loved [Name Removed]. He is a great guy. Very knowledgeable about the business. There was no way I would even [amount removed] a month to live in that unit. It was dangerous for someone of our age.

I moved my husband in after being very transparent regarding his behavior and his Alzheimer’s diagnosis. He was given TWO evaluations and I was assured that all of the behavior issues were no problem as all of their staff were dementia certified with training. They assured me that they had had many patients like him. Long story but he fell repeatedly, knocked out a tooth which nobody seemed to know how that happened until the week he left… he had two sunburns, was chronically dehydrated, dirty and I was told they just let him go and staff were told not to bother him! ONE STAFF member took great care of him while he was there but the others were petty, offended by dementia behavior and frankly just ignorant and rude. I don’t know about the AL side but please, DO NOT place your loved one here! They are NOT safe. My husband got out of the doors so many times I lost count. This SECURE memory care literally had a sign on the door telling the residents how to get the door open. Just a big no

My mom enjoyed the assisted living area and the staff. The food seemed to have very little variation however. They said we needed to move her to the memory care unit so she could get more hands on care. So, we moved her. She had less care. They left her and her room in filth, the staff did not engage with the residents except when they had to. They instead sat on their phones. It was awful. She had bruises from what I can only assume were falls. The management failed to properly submit her claims to the extent of almost [Amount Removed] in claims not paid in a reasonable time. There was a lot of turnover in management staff. I think at least 3 or 4 directors in 1.5 years. We moved her out at no notice due to the condition of her care in the memory unit.

I arrived for tour and was told that the guide was out of the office and could I wait for "a while". My job is 25 minutes away from this facility and the tour was already going to bust my lunch hour so I left. No call, explanation or apology.

To be honest with you, our experience with [name removed] in ALL the assisted living facilities were awful! They are all understaffed and a lot of the people they hire don't want to work. I personally witnessed that on several occasions. [name removed] had pushed her button for help and to tell them she hadn't been given her pain meds. She pushed it three times with no response. So, I walked down to to nurses desk. I found everyone sitting around watching their phones! The RN was watching a video, not a training one either! I was not happy!!! That's when we moved her to a private home. I think the problem with this places is that they are no longer locally owned. Corporations own them. They are all about large profits and no longer about quality care. Plus, from our experience and others I've talked to here and in other states, a lot of the younger generation wants those paychecks but really don't want to work abd do their jobs with pride. The work ethic is very sad

Rented a cottage. Showed local management serious deficiencies making the cottage unlivable. They promised fixes which didn't happen. I then had to contact Brennity Corporate to get a refund.

My mom was here for over a year. They did not fill her necessary medications and we saw her anxiety increase and her cognitively decline. We addressed it with the administrator who said, “go ahead and call the state”. He also raised our rate heartlessly.

It has gone down hill dramatically since the previous director retired and they got rid of the wonderful Activities Director.

I recently toured Landon Ridge for my mom. From the exterior and interior, the community looks nice at first glance, but it is very poorly maintained. The overwhelming smell of urine upon entering made a strong negative impression. The tour was confusing, with staff seeming disorganized and unclear about details, and communication before and during the process was poor. Overall, despite appearances, I cannot recommend this facility based on our visit.

Food is inconsistent, tableware is not clean. Staff was not timely in responsiveness to issues. Medicines not dispensed properly. And the aides are not well trained. My mom fell twice while directly under their care, unacceptable in my opinion. Cannot recommend for assisted living at all. My parents just moved to Palms of Largo, 100% better so far.

Since Buffalo Creek has been purchased by Sagora quality has declined. They are having staffing issues. They have an activities calendar but it is not accurate. There are very little activities on the weekend. I noticed above states it has complimentary transportation but there is a fee for all transportation. Very Limited offsite activities

We have since moved after only 6 weeks very disappointed. Nothing was as sold as. Dining was not good, staff did not pay attention. No follow through with things that were asked for . Laundry, cleaning ect. After I gave notice they didn't cared or gave concern.

We moved my aunt in because we were told that it was the best place in town. It is also close to our new house. Within a month, my aunt had gained 20 lbs, she was swollen from head to toe and having trouble breathing. Because we were new in town (in fact we hadn't completely moved to San Angelo) I asked the charge nurse for a referral to a doctor and asked her to make the arrangements for her medical care. They put her (age 103, with poor hearing and vision) on the bus and took her to the dr. There was no one accompanying her, only the bus driver. I wasn't told of when her dr visits were or the results of the exam, either from the dr or Brayden Park. She was taken to the dr like this more than once in the first 2 months she was there. She tried to tell the nurse that she was having trouble breathing, but I was told later that the nurse thought she was just complaining. (That nurse was fired several months later). (My husband was very ill and he was admitted to the hospital before we completed our move to San Angelo). After the move was completed, I was able to take Aunt [name removed] to the dr, she was immediately sent to the ER, with a heart rate of 170, and congestive heart failure. After she was discharged from the hospital, she was returned to Brayden Park, where she within a month- six weeks she was back in the hospital again for the same thing. When she was discharged that time, a space at the Baptist Skilled Nursing facility was available, so she was moved there. She never returned to Brayden Park. I felt like the administrators talked a good game, but I felt that there was a lack of follow through. I felt that the aides were very caring and concerned about their patients. I tried to ask for a low salt diet for her, but was told "they don't cook like that." Otherwise, the food was fine, everyone got to order their meal from a menu, and the food was at least reasonably warm when it was served. We ate several meals there with her. The place was kept clean, but there were problems with the maintenance of the building; the elevator door didn't shout properly and shook in transit, the front automatic handicap doors were broken for several months, and it took them awhile to finish roofing part of the building. All of these problems were observed last fall and winter. I haven’t been over there since she died in January.

I am very sorry but I have not been treated well here and I do not think they should get any recommendation. Very poor service quality and concern here and so many lies

When I moved here, I considered it "a little corner of Heaven." Since Sagora purchased the facility, IT IS HELL (and I am not exaggerating). (1) The food is of extremely poor quality; (2) too few Staff members, and the "good" ones quit or were "let go." (3) Every time we turn around, there is a monetary increase. Sagora is only interested in the rent money; residents are treated like trash. (4) The description(s) in their written information is inaccurate. (5) The positive reviews prior to [date removed] were about features provided by the previous owner--and enjoyed by happy residents.
