My grandma had a terrible experience at this hospital she had a heart condition yet they didn't give her anything in case of an emergency, she also fell a couple times yet they wouldn't help her. I would suggest not taking any of your older family members to come here

My loved one was not even there one month and she escaped 4 times. The ate was suppose to be closed and never was. We spent 2500 plus one months rent and they kicked her out. They are not very professional.

The activities director and I butted heads pretty badly. One day I was talking to her about how I was going to use a specific physical & occupational therapy company and she turned her nose up at them and it bothered me. A few days later told I was offended by her response, how I thought it was a unprofessional, and she should keep her opinion to herself. She wouldn't speak to me again after that and she made it very awkward. On a scale from 1-10, it was 200. She made it extremely awkward when I went to visit my mom. I tried to get past it, but she wouldn't move on. It was just the most horrible experience I had working with a community. I tried to tell the ADON, but she acted like she didn't know what was going on. She didn't try to address it, it was almost like she was scared of her herself. No one did anything about the way I was treated. It was very crappy to be treated like I was a second class citizen. She made it extremely obvious; she'd be extremely friendly to everyone else. I'd rather my mom live on the street than Canyon Creek. They didn't do basic things and my mom regressed. Every time I went, she smelled like urine and I didn't feel they took good care of her. I felt extremely uncomfortable talking to anyone at the front because of the way things went with that lady. We've moved her and we're very happy. I'm glad I'm able to speak up and she's happier. I'm treated with kindness, dignity and respect. No one should be treated the way I was treated. If she can't have thick enough skin to be confronted about what she said, she shouldn't be working there.

I would not recommend Rockbrook Memory Care to anybody. I felt like they made a lot of promises that were not kept. They did not return any of my phones calls that I made to them. Overall it was just not a pleasant experience.

This place was dirty. The staff was poorly trained and very disinterested in their patients. I honestly thought that I was going to die there! I couldn't wait to get out. They served junk food: no salads, no greens, only boiled/canned vegetables. My impression was that they were trying to spend as little as possible on the patients. I feel sorry for all of those that couldn't leave. There were 3 ladies that told me they begged for their children to take them out of this hole!

I am RN and had friend that was put in this facility. She there for less 2 weeks because there was such an outcry from her friends and family. The place was dirty. There were holes through the carpet right through to the subfloor. The fronts of kitchen drawers and cabinets were broken. There was no therapy dept., no activity dept. (the activity director came in to introduce herself and say that it was her last day and as far as she knew there was no replacement). The food was: pizza, hot dogs, spahetti-o type casserole, mac 'n cheese, basically garbage diet. The staff was insensitive and my impression they spent the bulk of their time trying to avoid the patients. The place deserves a MINUS -10! The only places that I've seen that are worse were in the southern part of the US.

This facility has been nothing but drama. They have allowed my parent to fall and lay in the floor All night with a broken arm with no one checking on her. They never change the bedding,family has to do that and clean the floors. She was given the wrong meds. She says she never sees a nurse practitioner, or doctor. She was a diabetic going in, and they could never get her meds adjusted. They do not give you a choice of foods. If you do not like what they bring, you do without. I would not recommend this facility to anyone.

I would tell others to not put your loved one in this facility. They withdrew money randomly (and still owe us), they are unresponsive, and the care here was completely inadequate. We did not have a good experience with them.

Older facility. They are slowly renovating some apartments but it is minimal. Visited on a Saturday. Only saw a couple of residents moving around. There are no activities on the weekend. Just gave a sad feeling overall.

The staff is lazy and mean, I got calls from my sister daily about how she was treated. We ask for certain staff members not to be allowed in her room they were so rude, because of COVID we were not allowed in, after not reaching my sister for a couple of days, I kept trying to reach staff, no one would answer the phones. I finally went down there and banged on the door, one of the resident answered the door and asked me if I wanted to come in. Finally I was able to reach a staff member and ask her about how my sister was doing. The first staff member said that she could not get her to respond I had that time said should we call 911, then a second staff member went up to check on her came down and assured me that she was fine. This was on a Saturday, I was going to call first thing Monday morning and get more details but before I could call Sunday morning I received a call that she had passed away. I am heartbroken and angry. I never received one word of condolence or compassion from any of these people the only phone call I ever got was when are you going to come get her stuff. Please don’t make my mistake, keep your love ones away from this place.

We did not have a good experience with Lakeshore. My mom went 3 weeks without having her hair washed. Her toenails were so long that they were cutting her feet, even though I asked them multiple times to cut them for her. We had no impact at all with her care or anything like that. I would not recommend them to others looking.

I would not recommend this community you should go somewhere else.

Even before Covid, the communication from Admin was poor and the staff to resident ratio was not good. The caregivers were kind and efficient and our mom was relatively independent so we kept her on the assisted living side of the facility. With Covid, we were at the mercy of Lakeshore to ensure she was being cared for as we weren't allowed inside the building for almost an entire year. As our mother's memory declined, we called her daily to check because she was basically quarantined for months in a room totally alone. She would comment that multiple food trays were beside her bed and there were no towels hanging in the bathroom to indicate bathing was being done. Once I was allowed inside (they knew I was coming), I found her room and living conditions to be poor. Multiple food trays out, bathroom and sheets not clean, ants on her countertop and the smell was terrible due to old food. We moved her out as soon as we could. If your loved one is relatively independent, Lakeshore is fine. If your loved one needs assistance, you may be dissatisfied as we were.

For close to a year, we weren't allowed inside her room and visitation was very low (sometimes through a window and some outside on the patio). Once we were allowed inside, we quickly discovered her care was minimal at best based on the her physically and the conditions of her room - food trays left for multiple days, dirty sheets and the smell was terrible. If your loved one is relatively independent then Lakeshore would be ok. If your loved one requires any assistance, then they will not likely get what they need due to low staff to resident ratio.

My father has been at this facility for the past 2 1/2 years and it has gone thru 3 directors. They have no organization, no hired cleaning crew, no maintenance man, his toilet has leaked for 3 weeks and they keep promising they will fix it but they do not, I have now reached out to the state concerning the safety of this facility. One newer shower had not worked for 2 months. I might have to move my father if it continues to get worse. Not enough staff. My dad's sheets might not get changed for 3 weeks.

It’s taken me a couple months to be able to write this review. I try to keep things positive however I would feel guilty not expressing my concerns about the quality of care being provided. My mother lived at Park Place Assisted Living for a couple years and things were great in the beginning but once COVID-19 hit everything changed. Park Place became more like a prison and less like a happy home. My mother felt so isolated, lonely and depressed and was starting to give up on life. It was heartbreaking. After they cut the hours of the staff the quality of the food got so bad I was having to drop off groceries for her at the front gate. I wouldn’t recommend moving anyone into Park Place Assisted Living until they make changes and start caring about the residents. In the end they didn’t care about my mother. All they cared about was making money.

The community told us that they would have nutritious food for my loved one, and it were small meals. I was so surprised. The menu wasn't even what they were serving. I feel they didn't review my loved one's chart and served her the wrong food, since she didn't eat specific food. I ended up taking my loved out after two months.

Can’t review cleanliness etc since we could not enter (Covid). I have told two people looking for a place for their loved one to look elsewhere. Mine died from severe dehydration. I have to assume he got zero assistance in holding a cup etc. The staff is super nice which makes it hard to be brutally honest.

It was once good. Now very bad. Food not for people.

I was not impressed with my visit / tour of Windsor Court in Weatherford a few week ago. We were shown a couple of the Independent Apartments both still had personal belongings in them from residents, this made it a bit uncomfortable. I would not want my family member to be shown this type of respect of their personal property. As we had questions afterwards took several calls to the office staff to get answers.
