When residents press the call button pendant, the caregivers come in and clear the button to show they responded, say they will be right back, but they never return. The director of nursing is an LPN not an RN. On my mother's last day at the community ,my mother asked for help to use the bathroom, , no one responded, she waited 5 hours, and after that they said" you know your mother is not the only resident here". During the height of the COVID pandemic, their screening process for visitors was inconsistent and unsafe. When I would bring concerns to the director of nursing, she asked her staff and they denied the accusation. Any awful experience with inadequate care on every level.

Our mom was at Pacifica Peoria for 9 months, during that time she had 4 ambulance calls, 2 were due to head injuries requiring stitches. We also had 3 other calls for non ambulance injuries. The staff could not explain how any of the injuries happened and no staff ever admitted to seeing the accident. We met with the ED and nursing director many times to voice our concerns with mom's care but no changes were noticed. The turnover of caregivers in the cottage mom was in was constant, which is counter productive in memory care. Often we would not see any caregivers in the cottage when we came to visit mom during the day. Hydration is not a priority to staff and mom would only have water when we came to visit and got her water. I would not recommend this facility.

Did not like the set up of the "dorm" style housing. Four separate bedrooms with a small shared living room and kitchen space does not offer much privacy. Feel resident will spend time alone in small bedroom.

We removed my mom due to lack of care, medicine mismanagement, after pointing out lack of care the staff would not even say hello. They ordered new medicine from their pharmacy without looking and seeing we had plenty and lost meds we provided.. Her room was 82 degrees with no air movement and caused stagnet bad smelling air. After pointing out the problem several times a portable unit was installed in her room and nextdoor only to be removed after less than a week, problem was not corrected. Evening staff was unable to keep other residents from sleeping in her bed which they had skin problems and staff didnt pay attention or replace the sheets before my mom went to sleep. Staff was only required to make rounds every 2 hours and many times we had to go get mom something to drink. It smelled bad and furniture was old and dirty needing replacement. At 6k a month parent company should correct these issues. After removing her one staff member said she was happy for us that we found a better place.

DO NOT PUT your loved one in this facility. The care provided in the main building isn't to bad but the MEMORY CARE unit care for the elderly is terrible. I understand they are severely understaffed and on some shifts don't provide the necessary/required care you're paying for. As your loved one's health declines the staffing is not adequate to properly care for them. My family member continuously had bruises and arm scrapes existing from someone not paying attention to the condition of the resident.

My mother lasted a month in this place because of their lack of care she ended up in the hospital then a rehab center She fell at least 4 times that we knew of Her room was always dirty It could be labels as abuse she was left alone for hours in her room A place for mom was of no help and did not find a place for my mom and her needs just sent me a bunch of places that fit in my budget

Very small apartments only. Not interested. However, am now waiting to hear from someone there, as info I just received from this facility refers to cottages, with garages; but floor plans do not show cottages??

The Atria has many positive programs for Independent living and assisted living. Recently my mother was asked to leave because she was on able to get out of her bed on her own. I found out after the fact they had only one CNA per floor. I have paid extra to have mom checked three times a night and if she needed it have her diaper changed. I found out recently they only check and do nothing if she needs changing. I have two excellent people taking care of mom from 10 AM to 10 PM Monday through Friday. They do most of the work including laundry housekeeping showers and all of mom‘s personal needs. The reason I’m giving the atria only one star is that I just heard there was only one staff member per floor and they were unable to care for my mother. Two months ago they did a rent reduction and now they’re asking her to move. My mother is 96 really doesn’t have many more years to go and those of you that have elderly parents they do not do well when they’re moved. Fortunately I have arranged for her to move into a private room in the small facility and basically will paint the walls in the same colors at the atria and furnish it with all the furniture that is in her bedroom and more. I have battled the atria for about five years concerning their patient care. At one time I had to put in a video system to provide me with information about when the CNA came to look in on mom, go in to check her and change her if necessary. In closing: the Atria Meridian is a clean independent living facility that I can recommend. I cannot recommend assisted-living basically because they are incredibly understaffed and poorly managed from 11 PM to 7 AM. Staff turnover is high and often in need of training. I like [name removed] the manager. She is personable and excellent when directly Confronted with a problem and quick to find a solution. The problem is having so many issues such as replacing carpets, and fixing air-conditioners as 2 examples. We have personally had the carpet cleaned several times and finally we got new carpet. We are moving out in about a week wish the best for Atria and hope they can provide better services particularly for assisted living. If your loved one is healthy and independence the atria is a good choice. When looking for a facility for your loved ones were parents be sure to check The fine print ask lots of questions take notes and be realistic about your loved one. Ask yourself are they able to care for themselves. If not what services are provided by the facility for assisted-living. Lastly when will they ask your loved one to move because they are no longer able to care for or him. Do your homework there are many facilities and I’m sure you can find one that meets your needs. Stay on top of The care your loved one is getting and make sure that the facility is living up to their agreement. For those caregivers good luck.

This facility is misrepresenting the housing. The elevators are often out of order. The windows look like they have never been cleaned. The food is lousy and repetitive. Many of the residents lack cognitive skills and many are just plain rude and are disabled. The management is always asking the residents to be patient with things not working. My rent went up $80.00 the first 6 months. Transportation is not provided as advertised. All that is offered is excuses.

BATHROOMS ARE EXTREMELY SMALL, LIMITED COUNTER SPACE NEXT TO SINK. ROOMS ARE TO SMALL. WHEN FURNITURE PUT IN, ALMOST IMPOSSIBLE TO NAVIGATE IN WHEELCHAIR. DISINFECTANT ODER IN CORRIDORS.

No interest the apartments do not have kitchens. More like pure assisted/skilled nursing home.

My mother slipped and fell after a new flooring was put down. She broke her pelvis and died .. I got no sympathy from any of the staff.rip mom

My husband did not do good. I had to take him out. He kept leaving, the doors had alarms on it. They tried to give him more medication because he kept wandering off. We didn't want him to be that medicated. Their tactics to get him back in were very bad. They would tell him I was there and he would spend the whole time looking for me and then walk out the community again. The director was wonderful but the lady in charge of the Memory Care she was awful. They would call me all hours of the night and she would have him call me too. He would say please pick him up. It was a nightmare. Luckily, they gave us back our community fee.

Priced too high and smelled.

I toured this community and did not like that it was out dated. I did not stay and talk with the staff. It was not the environment was not good. I would suggest that other families see this community for the care of a loved one here on there own

at this moment i am no happy with Brookdale Palm Beach at all-- I am having so much trouble trying to find out about my aunt- she has recently been diagnosed with dementia, and a questionable power of attorney has appeared on the scene from across the country. Brookdale has since denied my aunt telephone and visiting privileges with her only 2 friends who have been there to love and support her since her 'incarceration" I believe this to be horrible judgement, she is now saying she has nothing to live for, and I cant find out any information or get any support from the director of memory care or health and wellness services. Very disappointing! their supposed to be patient advocates!! I have filed a report [removed].

I was not impressed at all with this community. I had an appointment and when we arrived they were not there. the LPN came in and she did not know anything about the community. I just did not like the lack of professionalism at all. They also said that they had a garden and when I went to go look it was just a bunch of weeds.

The care team is great, but the management is awful. They don't care about our loved ones in their care.

There are so many good homes in Fort Myers, do not make the mistake of going to Pacifica, wasting your hard earned money just to have your family treated horribly. The staff is horrible especially [Removed]. Worst human being ever. Very disrespectful and disgusting. Please reconsider.

I took a tour of this place and found that it looks and feels like a prison. For the safety of residents door and entry ways are locked. The rooms are private but have to share a toilet and sink. I was showed a shower down the hall but it was still under construction. I preferred a private shower/bathroom for Mom.
