First off the community is pretty, but the charm ends there. The pretty faces are severely lacking on execution of what they sell you. Care is inconsistent to the point our father ended up in the hospital due to lack of oversight from the care staff. A new RN was brought in who was not brought up to speed on our dad's medical needs and he was hospitalized for something that was a known medical observation to watch for. They have handled COVID poorly, routinely shutting down due to staff members contracting COVID. They are rigid to the point of unhealthy in not allowing family to see residents. They also ask that you take your residents to the doctors appointments because their transportation can't handle it, but turn around and refuse family visits. They are losing their head RN in [date removed], which will make a bad situation worse. The Executive Director and Owners are unresponsive and run the community like a fortune 500 company not a caring home. Value doesn't match pricepoint.

I have been to this facility many times and have been in the field 20 years, Although this has always been a nice looking and feeling place, my most recent experiences here have been horrible. The staff yells all the time non stop at night and early morning. They act like they own the place and not like this is the residents homes. They are loud and disrespectful. They do not knock before entering rooms. They need some serious management and teaching. They need to actually care and they don’t care about anyone sleeping here. Worst part is families don’t know because they aren’t here late nights or early. I’m listening to them now like they are partying. Talk and yell less, and work more.

We moved our mother into Habersham House (HH) memory care unit. We had many problems as detailed below: When applying for admittance for our mother, in addition to her financial statements (she is self supporting), we had to provide our own personal financial details which we found intrusive. We also completed a detailed account of our mother's likes, dislikes, personal history, etc. We appreciated the thoroughness of these questions. Sadly, it became clear that the information on our mother was either never shared with the staff or if it was, they ignored it. When our mother celebrated her 91st birthday, nothing was done to celebrate it, nor was it even mentioned. When we inquired if anything was done to acknowledge this special day, the Director’s reply indicated the staff had no idea. There were regular activities and musical events in the assisted living area that our mother would have enjoyed (as indicated on her questionnaire) but she was never taken to participate. In fact, one such day, my sister was present when a staff member came to the memory unit to ask if there were any patients to transport for the performance. The aides present immediately said "no", with no consideration that a few residents might have been interested. The memory care unit literature and advertising emphasize the Montessori Method. It was never explained to us how this method was applied to people with memory deficits. Although a bulletin board listed several activities each day, we never once saw any activities despite visiting several times a week, at different times of the day. The aides in that unit appear unsmiling and unresponsive when greeted and certainly, whenever I was there, they were more interested in their cell phones than in the residents, who were left in their chairs to watch infomercials or other inappropriate content on the TV. The residents lack stimulation and seem to be treated poorly by the staff who do not seem to care since their patients cannot voice any complaints. Our mother was in the Memory Care unit at Habersham House for 6 weeks approximately and her hair was washed only 2 times, even after frequent requests! On one occasion, the coordinator of the unit said the staff did not wash hair and we had to have the hairdresser do it (was this a kickback scheme?!). What happens to residents who cannot afford to have a hairdresser on a weekly basis? Of course, when we complained to the director, this was denied but it did happen. Hair was supposed to be washed once a week, baths 3 times a week. On one occasion, my sister arrived to find my mother's door closed. She heard yelling behind the door and when she knocked, a staff member opened it slightly and said they were helping her in the bathroom. When my sister spoke with our mother afterward, she said the staff were very rough with her. At that point, my sister inspected mother’s skin and found red marks on her back. You can form your own conclusions here. Our mother was moved to Habersham House from another facility out-of-state where she had resided for 2 years. The Staff there were wonderful and we were very satisfied with her care. The new residence to which we moved our mother after Habersham House, is also wonderful. Residents there are respected and treated with dignity; the staff are visibly caring. There are many activities in the memory care unit and our mother actively engages in them. Her mental state has improved enormously! I mention this to indicate that we are not newbies to this, nor are we typically complainers. Of the three facilities which we have experienced, we were happy with all except HH.

My aunt was here for awhile in memory care. It was not a good place for her. There really wasn't enough activities in memory care. Seemed to sit around mostly. We ended up moving her elsewhere. She seems much happier now, which makes us much happier. I can't really provide information on the other section of the Oaks as we only were involved only in memory care.

My mom was there for little over a month. Lots of issues with staff. Cleanliness was not good in her room and bathroom most days. Came in one day and "poop" was ALL over the toilet seat. After dinner each night, the staff either put some to bed or placed in front of TV. Pretty sad when an Alzheimer's resident complains about how bad they are treating another resident. These things were mainly with the second shift staff. Constantly yelling at some to sit or not to do something. Staff sat around and talked on their breaks and didn't clean their messes up. I know because I was there almost EVERY night until my mom went bed. Director heard from me a lot. He was trying to make some changes but with out supervision, it wasn't changing. Need camera's in all open areas. We left when my mom had several episodes of throwing up blood and they did not call the doctor like I had authorized. Took her to the ER and was admitted. She had ulcers and was anemic. She had a MRSA UTI and pneumonia. She was in the hospital for 6 days and we never went back to The Haven. What we were sold on did not happen in the month she was there. I do hope it has improved

It was overall clean, but there was a gentleman who was not well supervised, who was urinating in the corner, and it got progressively worse. They were very friendly. We had to remove my loved one for safety reasons. Despite having visited several times, we just didn't see what it really was. It was very disturbing. I don't feel like any of the clients were well supervised, all of the staff seemed to be preoccupied with housekeeping and medications, and the occasional activities, which when they did have them were really good. There was a really poor attentiveness to the residents. They were left to wander unsupervised for hours at the time. Safety was my biggest concern, when the staff was with the residents they were good. I don't think the activities were to the level of the functioning of the residents. I don't think there was enough individual attention. I chose it for the friendliness of the staff, I liked the resident to staff ratio, and we visited during activity times, and that seemed good. I didn't feel there was enough encouragement of the residents to go to bed. They didn't seem to be proactive with emergencies.

She suffered more than she needed to because of the errors and lack of descent personnel. It was a nightmare to see her suffer so much. They have lost so many staff and residents over the last six months. So many staff left because the new owners didn't seem to care about the residents. Even after my mom they still didn't get there [removed] together. My step father is been getting some terrible care so he is moving out real soon. I just pray that another family will not have to go thru what we did. Please please keep an eye on your family if there are there.

My mother recently left this facility and we did not have a good experience. There is not enough staff for the amount of residents that are there. The food is not that great and the place is filthy. The carpets are stained and i have seen bugs in the dinning room before. The staff always seemed to be changing as well. They tell you a lot of things to get you in the door and get your money, but they don't deliver. I would not recommend this place to anyone.

My grandmother was there for two years. She always needs her walker to walk. She had a bed alarm that would sound when she got out of it. We thought something was wrong so we placed a camera in her room. We have it on video where after getting out of bed for the bathroom, she fell backwards. After a few minutes of alarm going off, two staff members come in to find her on the floor. They just place her back into bed without checking her or reporting the fall. Another time, she again gets up for the bathroom, a nurse comes in and detaches the alarm and leaves out not even turning on the lights for her. She is allowed to strip her bottoms off that are wet by herself. She then is allowed to go back to a urine soaked bed naked to sleep until the morning shift comes in. I take the videos to the Director, Catrina Smith. She said she would look into it. A couple of days later, two different night staff go into grandmother's room and turns the camera around so you can not see what is going on in the room. At that point in time, Ms. Smith was the only one at the Haven who knew where the camera was. I can not prove it but truly feel she told them about the camera. Since she has been there and Five Star bought the place, it has declined. The living area is always dirty. The food is just nasty. They never have enough staff taking care of these people who can not speak for themselves. We moved grandmother out of there. I would not move another loved one there.

I would never recommend this community to ANYBODY with memory care issues, especially those who have Lewy Body Dementia. The facility is not equipped to handle that type of care and they almost killed my loved one. We had a TERRIBLE experience.

The community didn't send pictures of my loved one when asked for this. The management changed and the community didn't inform us of this change. The community wanted me to sit with my loved one 24 hours a day for 14 days due to them contracting COVID. They gave me another option to hire someone to do this.

Be very aware of their physical therapy services. Their staff will tell you they have had your PT approved and your copay total will be a "nominal amount " and give you a price which is not even close to the ridiculous bill they send. Then they try to tell you your insurance is out of network. ( The same one they say was pre-approved.) I would advise residents to get PT outside of this facility.
