There are basic needs are met here. The family will need to provide most of the care for your loved one. The needs that are met are medications, locked community, and meals. There is minimal staff interaction with the family. The community isn't bathing my loved one, and my loved one is left to care for themselves. There are times that residents that wander, will wonder more unattended. It seems to be short staffed, which makes it hard to find someone to assist you when needed. My loved one seems to enjoy the meals and they are balanced.

Extremely slow internet. If you want to use the internet you need to gets separate plan for higher speeds. The more people who are on the slower it gets. Food is pretty bad but edible however they pay such low wages to the staff in the dining room (9 dollars an hour I asked one young lady) that they have high turnover and are always understaffed. Thus we have extremely slow service especially during busy hours. People are friendly but all of us residents know not to complain or they avoid us.

This place wasn't a locked facility and people could just wander. And the lady who did the tour was so not caring and nonchalant about me having to sign papers saying that they weren't responsible if my loved one got out of the facility. It was unappealing, worn out looking and smelled bad. Not a good fit for my parents.

When my wife was in the Legacy, I was not comfortable with the level of care that she was receiving. They were very short staffed and they were not trained properly on how to handle situations. They would not approach residents in a calm way. During meal time, the staff would walk around and rush the residents.

The staff did not know that we were there and they thought that we were a resident of the community.y We had to search for someone to assist us and they did not know we were there for information on the community. We were not comfortable going into rooms where there were residents... We wanted to see an empty room. This did not make us comfortable.going into a room with someone not knowing. Families might call for a tour for the care given here.

My mom was a resident in the Alzheimer's/dementia care unit called the cottage for 7 mos. I was extremely dissapointed & dissatisfied with the care she received. The staff were all extremely young CNA's I NEVER ONCE SAW OR SPOKE WITH A REGISTERED NURSE and in my opinion werent trained well enough for the specialized care required for Alzheimer's and or dementia patients. There were countless times I'd visit my mom while the staff were all sitting together laughing & talking with each other not interacting with the residents, there were several times I went into my moms room & found feces all over the outside of the toilet, found my mom wandering back & forth in front of the refrigerator while the staff were oblivious, she was obviously thirsty but couldn't communicate so I'd get her a drink. During the weekdays mom thru Fri they have a person come in to play games or music with the residents for an hr a day but sat & sun the residents are left to wander around, stare out the windows or just sit in the front room getting agitated while the staff ignored them. When someone would get agitated (which unfortunately happens with this disease) the staff wouldn't interact with them or give them something to do, they just write a report stating so and so was agitated then the next thing was to medicate or sedate them! I had many meetings with the director of legacy [Removed] to voice my concerns, I got lots of promises but nothing changed. I don't think the residents receive the level of care they need. If your expecting your loved one to be taken care of in a nurturing caring environment this isn't it. They couldn't even provide the basic care promised in the contract such as helping my mom brush her teeth when she first arrived. When my moms illness got worse to where she could no longer swallow pills (her medication) the cna's were oblivious & would put them in her hand anyway then my mom would drop them on the floor, not one of them had the training to understand she just COULDNT SWALLOW PILLS ANYMORE. When my mom could no longer feed herself during breakfast lunch or dinner not one of the CNA's noticed, I did, I fed her and had to speak with the director AGAIN to ask him to please tell the staff. Thank god I able to visit my mom almost daily to witness how ill equipped the staff was in taking care of her as the illness progressed & her needs increased. I absolutely WOULD NOT recommend sending your loved on here. Until better and more frequent training is provided the level of care won't improve. I wish I could provide a place where people would get the proper care they so deserve

They have large windows that make the community feel welcoming. The staff is very nice and listens to my concerns. They have a lot of turnover and are short staffed. When the call button is pushed it can take more than thirty minutes before help arrives and I don't believe they check on my dad as often as they should. The food has gotten worse and sometimes the food is served cold.

Great transportation. Good location. The building and grounds are well kept. The lobby and downstairs is pretty nice. However the rooms and the halls are dated and old with dirt colored colors. It makes for not a very peaceful environment. They are repainting and updating some of the rooms as the residents move out. Hopefully they will do the halls soon. The food is not good. Service to get the food is very slow. They do not have high speed internet...if many people use it it is so slow you might as well give up. The activities are good and they have a variety. Really nice people live here. We have had a bedbug problem they are trying to fix and finally we smell gas a lot from the kitchen. When I ask about it they always tell me not to worry about it.

They five years ago was outstanding. I was once an applicant as I was leaving [Removed] a caregiver was impolite to a resident who asked for help. I thought maybe A better fit is a smaller community. I wonder if the aides know these people pay good money to live here it probably would flip her lid. Disrespectful

Incompetent: 1. not having or showing the necessary skills to do something successfully. Inept: 1. awkward or clumsy, inappropriate, unsuitable, out of place, absurd or foolish, 2. without skill or aptitude for a particular task or assignment. Legacy Village Management and the majority of the Legacy Village staff (see above) Legacy Village Management and Staff are incapable/unable to provide for the fundamental needs of their residents, such as dignity and privacy. The Staff and Management are unable to provide for basic services, such as food preparation, meal service and standard dietary provisions. Additionally, Legacy Village does not have the vocational capability to provide for a variety of critical, clinical needs such as medication administration and management. At Legacy Village those who require the most care, get the absolute least. If you read their missions statement, they fail at every level. There is nothing ‘premier or elevated’ about their level(s) of care.

No I wouldn't recommend Legacy house. The staff is incompetent, my loved was left on the edge floor after falling for almost two hours. My complaints weren't addressed and they lost/stole and damaged almost $[removed] worth of stuff.

This was our second time Touring this place . First time the man that gave us the tour was not welcoming . OR DID HE MAKE US FEEL Welcome left there with no good Vibes. Second time toured again with his Assistant .. One again same thing Touring was my mom sister & husband & myself once again bad experience. Bottom line we turned 2 more facilities. And she my mom decide to go with one of them.. We wanted this place cause it was close to our home. We would never recommend it .. You lost a Potional Resident Your 2 staff that do the tours Need more caring welcoming Charm. And be more people persons. Your Greeters were the same

I'm the legally appointed personal representative of my mother's estate. She was put in this facility by her estranged offspring in 2018 after being taken from her Maryland care facility. I placed a welfare check call to the local police department after my mother called me crying, scared, & not sure where she was. That report reflects law enforcement contact was stopped by an employee at the facility. An employee dissuaded law enforcement from speaking with my mother, the victim, under a valid welfare check call. I subpoenaed facility records for the probate matter. Prescribed meds weren't dispensed due to machine error, my mom was found drinking from a moldy coffee cup, she fell multiple times breaking her femur & then she was found stuck between her bed and nightstand, & staff witnessed she "had a seizure". I'm filing formal complaints against Legacy House of Logan based on the official records I've received. Signed, [name removed] Personal Representative & Loving Daughter

I've been here for 6 months. The Admin are great, they do try their very best, but the med techs and care givers, take forever to come to you and really can't be bothered. The food is very iffy. Very little variety or flavor. Most of the kitchen staff is nice but one. And she acts like a spoiled 3 year old and should have been terminated ages ago. I wish they would truly listen to the residences. When you are paying out this much money, you should be treated kindly, and with respect. Not being yelled at or put down. I'm not seeing how this is the way to spend your last days.

DO NOT PUT YOUR FAMILY HERE, particularly if they have Alzheimer’s or dementia. My brother and I put both of our parents at Legacy House of Southern Hills because we liked that it has assisted living and memory care to accommodate our parents’ unique needs (Mom is in assisted living and Dad is in memory care). Fast forward a few months and formal reports have been filed with the Department of Health & Human Services and Adult Protective Services with the support of the long term care ombudsman’s office. The problem begins and ends with the director, [name removed], who is grossly incompetent and uncompassionate. Everything I share next has been documented and is intended to make the public aware of the real danger here, while respecting my parents’ privacy. Dad started on the assisted living side and was settling in well, socializing with other residents, attending happy hour and going for walks. However, he wandered outside one day and we made the collective decision to move him to the memory care side. That’s when the care abruptly changed. There is little to no activity or social programming to speak of for memory care patients, so the stimulation and interaction ended for my dad. That’s when he started exhibiting understandable frustration and agitation. As a result, Dad had an “incident” and we got 3 different explanations of what happened. As with all incidents there, you’re lucky to be notified, let alone receive a clear story, and you’ll never see any documentation. At least that time they sent him to the hospital, but upon his return, there were two more undocumented incidents, one of which was a fall that resulted in a limp, but no one sent him to the ER to ensure he was OK. On the assisted living side, my mom also suffered a bad fall that resulted in injury, but we weren’t so much as notified, and they didn’t have transportation to bring her back from the hospital. My mother has also been dealing with a dangerous resident who lives across from her who threatened to kill my mom before trying to attack another resident. I asked [name removed] when this would be addressed, but there’s still no plan. Speaking of plans, we tried meeting with [name removed] to come up with a care plan for my dad to ease his restlessness, but she wasn’t interested in identifying activities and therapies that could benefit him. Instead, her answer to us, and a senior health advocate who joined the meeting, was for him to leave if we weren’t happy with the standard of care. Her idea of helping your loved one is to send them away if they’re not complacent drooling in front of a TV. I asked her for copies of their memory care activity calendar and received one for next month. When I asked for previous months’ copies, I received some scrappy Word docs dating back to [date removed], which strengthened my suspicions that she’s creating these retroactively to cover herself. As we try to wait out the COVID pandemic, my dad has been left heavily medicated and neglected. My mother sent me a picture of him from one of her recent visits. (We had to fight for our mom’s right to see our dad, as a Legacy caregiver told my brother and I that [name removed] had forbidden them from seeing each other.) The photo showed my once chipper father, slumped over in bed, shirtless, unshaven, in wrinkled and dirty sheets. Mom said he was nearly unresponsive and couldn’t even sit up to talk to me on the phone. He's basically naked because no one washes his clothes, even though that's included. Prior to moving my dad to Legacy, we had a private caregiver who was so wonderful that Legacy hired her. Needless to say, she’s been transferred from memory care, likely so she doesn’t see my dad’s drastic weight loss and rapid decline in cognitive functioning. My parents are everything to me. My dad was a 747/MD-11 airline check captain that could tell the best jokes in the world. Now he’s a shell of a man who cries when he hears your voice on the phone. I cry constantly about my parents being at that house of horrors and will stop at nothing to expose [name removed] for the fraud she is.

Mom laid in bed for a week, after falling & breaking bones in her room at Legacy. She had no ability to move w/o pain. Legacy CNAs helped her up when she fell and put her back in bed. Unbelievably, NO Ambulance was called!! X-rays taken days later at Legacy showed mom had broken bones. Yet Legacy still would not call 911!! The US is not a third world country! Mom had no access to an MD’s evaluation for over a week. Assisted living is No hospital! No rehab. No nursing home! It’s NO place to lay in bed w/ broken bones for over a week w/ few pain meds & no hospital or MD’s medical attention!

The place looks nice. The food is like high school cafeteria fare with fancy names. They don't pay the poor employees enough and have high turnover and lack of good service. You can wait a half hour or more to be served. The internet is OK sometimes and other times it is awful. If you complain they tell you you can pay more and upgrade your plan. MY TV is sometimes good and other times there is no picture. (Very Random) I am old and all I can really enjoy is eating, watching TV, and surfing the web. I can't walk, I can't participate in most activities. They should spend more money on the food, and internet than the fancy furniture, decorations and image in the halls and lobbies.
