It's a nice place, it's new, it's clean. But the personal help is not too personable. I do go there several times a week, so I do see what's going on. They take good care of my loved one. I just wish there was more one-on-one care, but they're understaffed.

$4050/mo for studio; $5750/mo for 1 bdrm; includes meals, 1 1/2 hr care (meds, etc); RN assessment, care plan, transportation to and from medical and shopping); WAITING LIST with $500 dep; 5 people waiting 30 - 90 days; one-time community fee $3500; Verizon and Charter; weekly housekeeping, laundry (towels and linens)

This community is outstanding. The staff was nice and they were able to answer a lot of our questions, but we were misinformed on a few things for her care. This community appeared to be clean and maintained. Activities are offered to the residents. We would recommend that families tour this community for the care of a loved one

This review concerns mostly the Memory Care unit. Communication with nursing and care staff was poor. Staff told me what they thought I wanted to hear rather than what was actually going on. Screening before entry was haphazard. Options regarding medication management were not explained adequately. Finally, my husband proved to be a danger to staff and possibly other residents, and was removed to a local hospital emergency department. No one told me his lease had been terminated until a social worker at the hospital gave me the phone number of a low-level staff member at Keystone Place on the night shift who said he wasn't "allowed" to reveal his surname. A call to the Executive Director was never returned. The Director of Health and Wellness did call almost a week later but was impossible to find on site or to connect with during the two weeks of my husband's residence.

Spoke to a nice marketing person. Will visit next time in area .

Actually it was not our first choice they had an available room. Hind sight being 20/20 I should have waited. They are understaffed.

I toured this community and I did not get a good impression. Cleanliness was definitely lacking. Not a good choice for my loved one.

I was dissatisfied with our experience at Keystone Place. I felt that they did not respond to our requests made and did not have good solutions. They do not offer 24 hour nursing care. I think for the average person, the facility would be just fine but they were not able to accommodate our needs. The facility is beautiful and brand new, it just was not the right fit for us.

In the past my expérience and opinion of Meridian were all positive. Weve had several family members and friends in the Rehab unit and have a friend that lives in the Vintage Gardens Facility. Unfortunatley thinngs have really seemed to go downhill. The Vintage Gardens area is filthy, the staff is awol, cant find a Tech they are nowhere to be found, except for a few of the older staff you are on your own. The dining area is dirty, no cups for coffee,no sugar or condiments on table. Took us twenty minutes to find the stsff in kitchento get some creamer. Apparently they're not in the unit fromaroubd 10:30 to lubch time. So since my friend eats later she gets no fresh hot food or anyone in the kitchen to talk to. This is a daily thing so it must be how things are now. Also in the years past on the Rehab Unit, we were always so pleasantly pleased with the staff, the cleanliness, the greeting and smiling faces of the Ladies in tbe kitchen . Let me tell you thst is no more. Its filthy, staff unfriendly, young people dressed in street clothes so you dont know actually who is the kitchen staff. There was a young very unfriendly man there in jeans, hat on backwards, wearing ear plugs with rap music blaring. In my opinionthey must be desperate for staff or just do not care anymore. Id in the past gave Meriden five plus stars, but not anymore. Its dirty unproffessional and very disappointing and I myself will not refer anyone else there. Very saddened by how things have changed as Meridian had everything in one building to suit all aging needs. Just sad and disappointing.

My husband feel and hit his head on [date removed] in Memory Care and was sent to the hospital .The incompetence of the staff of being able to take care of a catheter that my husband did not need but that he got at the hospital my husband died on the [date removed] in hospice. Do not under any circumstances allow Mobile Physicians to assume responsibility for your family member and their medical needs. Do not allow Thrive at Beachwalk to administer any medical care to your loved one they cannot do it even though they state they can take care of diabetics and oxygen they cannot. [removed]

Keep in mind...Meridian is a BUSINESS! Their goal is to make lots of $$$, by providing meals and a bed, while sitting your loved one in front of a tv all day or keeping them in bed. Warning...If your loved one needs additional care and attention, you will be required to hire an attendant at $21+/hour. Do the math...that is thousands of $$ a month! Look elsewhere for your loved one if Meridian will not provide the care they need, which they "should do" for the high fee they already charge per month. They do not have enough staff to go the extra mile and really "care" for your loved one!

Beautiful facilities, but haphazard staffing. Services in the contract are not consistent. (No change of bedding in two weeks!!). Similar lapses are routine. Close to negligent

It was terrible! The CNA's, except for one, were rude, the Apt was cold because they wouldn't turn the heat on until sometime in October, the hall rugs were filthy. The food was good, though.

The apartments look amazing the staff that takes care of the residence are very friendly, but lack of the training to deal with assistant living residence. Keystone should really be considered an independent living place because they are not capable of taking care of residents with any Behavior issues they really need a social worker on staff. The administrator that's in charge of the building. Will promise you the world and it doesn't happen. There's no physical therapy on staff on site. They will tell you they do they do not and when you finally get someone in to do physical therapy there isn't the equipment that is needed for that physical therapy so you end up having to do it outpatient wise, which transportation is an issue. They will say they provide transportations which you will use but the resident would not gets their appointments on time so your appointment was canceled and you had to reschedule their appointment. Communication with family members on the resident care is lacking. We had an intake conference. The only Care Conference we had was at the point where we were going to get kicked out . And we were paying the high-level of care for my brother to be there. For what we were paying, we were not getting care we needed. My brother ended up having to be a hospitalized and he is now in for right now in a long term care center. I would not recommend Keystone.

Know your rights if you choose Keystone Place-Forevergreen. When our loved one began to require a higher level of care, KP began to pursue an involuntary transfer without justification or the required notice. In multiple instances they did not follow proper procedures as outlined by state regulations, presumably in hopes that we would either not question their actions, or that we would choose to leave voluntarily. What a disgrace to see these tactics used against such a vulnerable population. While individual care staff members were very attentive and kind, it unfortunately does little to compensate for our experience with management. In my opinion, Keystone Place-Forevergreen appears to be a corporation eager to collect high rents/profits and not one that is committed to elder care.

The management is horrible. The food is disgusting and the nursing is way understaffed.Did not like my time there, they kept saying things were gonna get better, but nothing ever did they went through three chefs. They’ve been through numerous amounts of nursing staff it’s just the revolving door of madness. So I had no choice but to leave and I am so glad I did because the place I’m at now is just wonderful.

We took a tour of Keystone Place at Terra Bella during our search for an Assisted Living facility for our loved one. This facility was not great. The facility is small and seemed overpriced to us. Also, out of all of the facilities that we toured this one was the least welcoming, they didn't even walk us out. We decided to continue on with our search.

Seaside at Fort Meyers had major flooding during the hurricane and lost one third of their roof. Prior to the hurricane, my family member had already been taken out of the facility and put into a nursing temporarily for other reasons. Her unit sat vacant while we waited to see what would happen. I never heard a word from Seaside regarding what happened to her belongings. They neve once contacted me to see how she was doing at the nursing home. Two weeks ago, they told me the facility is going to become a memory care only facility, but I won't be able to come in to see the state of her belongings for six weeks while they're under construction (no family allowed during that time). But then I got a call form the director telling me to come and get her stuff now. Prior to the hurricane, the facility was very nice. The rooms were very nice. The meals were great. But as far as how they handled the aftermath of Hurricane Ian, I am very dissatisfied.

Very unorganized. Admin can’t keep regular staff. Huge lack of communication between care givers and family members. Excuses are using outside staffing agencies. I complained and was asked to move my mom to another facility. They are hiring third cleaning agency in 4 months. Staff that is there don’t seem to care about residents. The surroundings are beautiful, the people working there are not. Doctors nurses and staff are over worked and do not take care of informing families of their loved ones conditions. I’m sure there are better places. [Community name removed] was very organized, it’s just not close by.

This has been on of the most unfriendly in knowledgeable facilities I’ve been this far. Staff is not properly trained to provide care to meet residents need. Nurse are not as friendly.. facility promises that they can me the needs of my transitioning loved one yet they was not able to. I’m very disappointed if the service we have received. If it make sense I will not put my dog let alone my fish here at this unfit filthy non caring facility
