Nice view

The caregivers here do a great job and always help us when we have questions, activities are great. But the management part could be a lot better every time I call the manager is never available to take my call or when I come in to talk he has left the building for the day, seems like I just get the run around and the assistant manager seems very nice but I over heard her talking to a couple other staff members and she is pretty rude to talk about some of the other residents! But the facility is beautiful and the food is great and they have a great view!!

We toured Azura North while looking for a place for my grandfather. We liked the feel of the community. It had a different feel than the others we’d looked at, and felt much more family-like; like you’re living in the house of a family member. The north facility is very nice but does not take state funding. So if a person is no longer able to self-pay, the patient would have to move to a different location at that point.

I toured Sylvan Crossing at Creekside Estates. It was a nice and clean community. It was a smaller community. There was only one room available and we weren't quite ready to move our loved one in.

We are very new to this whole experience of dealing with dementia. Stoney River Assisted Living & Memory Care was not helpful and lacking in letting us know what we might expect. It started with us approaching mom and letting her know that she needed more help and we wanted to get her the best care for that we could. We weren't aware that if she didn't go in voluntarily Stoney River couldn't take her. She was combative about losing her independence. They suggested to give her a sedative so she would come in voluntarily. That didn't work she only lasted five days. They said they couldn't take her back. They didn't communicate with us about what was going to happen. They didn't return our numerous phone calls and it made it very difficult to know what was the next step. We now have her in another community and she seems to be fitting in well.

wish they did not charge for every little thing!!

My mother has recently made the move into this community and so far things have been going okay for her. The staff in the memory care unit seem to be helpful with her everyday needs and helped with the transition into her new living environment. She has had no complaints about the meals and is enjoying participating in some of the activities that she can. I will say that I am not happy that they continue to add on to my bill every month on anything extra she needs help with.

We toured this community and did not like the feel from the staff. They did not make you feel welcomed. We did not like the neighbor hood in that area of town. We would suggest that others see this community for the care of a loved one. We were not empressed with the menu and they could use some improvements. A suggestion for families to tour for making a decision on the care of a loved one.

Some Concerns with This Community

My mother is being provided with good therapy at this community. They are doing a pretty good taking care of her, and her needs. Her concerns have been that there aren't really any organized activities. They offer minimal activities overall. The breakfast meals are great, but there are room for improvement with the other meals. This wouldn't be the first community I would recommend, but I would tell someone to consider it.

More Patience for mom

This is a nice community. The staff needs more understanding with my mom's needs. They do not have a full time nurse as I was informed. I would informed that I should not come see my mom everyday. I told them that my mom did not like certain food and they did not understand how to communicate with her. The staff needing more understand on my mom's care. I would recommend this community, but it needs a little help in a few areas.

Great Care Staff, But Administration Needs Improvement

My mother was recently switched from the memory care side to the assisted living portion of the community. She seems to have adjusted well to the assisted living side. The community look great, they do a good job with the cleaning. She is trying some of the activities they offer. The care staff is good, they are caring for her very well. The administration needs some improvements. The meals are fair. Overall she is in good hands and I would recommend it.

a few things to improve

One thing I do like about them is that they do treat my dad with respect, which is very important. That said, there are definitely some things they can improve on. My dad's sheets have not been changed since he has been there (two months now). Mom has eaten dinner up there with him a couple times and the food has been cold. There is not much activity - just listening to music or doing puzzles. Dad says there is nothing for guys there.

Pros: ? Home like atmosphere ? Residents can use the kitchen to cook or bake. ? Residents can use the laundry ? Nice Private Dining Area for Family Visits ? Nice common areas. ? Smaller building did not have a ?nursing home? feel. Grandma would not be just a number. ? We liked the assistance call necklace or watch and their response policy. They seemed to indicate they could tell if a resident had fell based on the necklace. ? They gave the impression the assistance necklace could be used for room service type requests, ?Can I have a cup of coffee and a PB&J sandwich?? ? Nice large bathroom with roll in shower. Cons: ? Small Rooms ? No price difference between Big Room and Small Room, so all Big rooms taken first. ? No locks on the resident?s doors even though the marketing brochure claimed there were locks. ? Price Structure seemed to take large jumps for levels of care. 3995, 4995, 5995 ? Level of Care criteria did not seem well defined. ? There is a slight concern about the level of staffing. The state mandates 1 per 8 residents, but only 2 staff at night for 20 rooms. We only saw one staff member besides the ?administrator? during our visit. ? There is a slight concern about the experience of the owners and sustained profitability.

We expected more

We had higher expectations of the level of care that our loved one would be receiving based on the cost. One of the biggest issues we have had is our loved one has fallen quite a few times since being here. More than half of the clothing items have disappeared and we have to keep buying new things. In the last week I have called the nurse and the director and they have not called back. In the beginning they were super responsive, but now they are lacking with communication. They do seem concerned about his well being but overall it has not been the best experience so far.

We toured Azura South while looking for a place for my grandfather. We had looked at the Azura North which we liked, but if we would eventually need financial assistance, we would have to move to Azura South at that time. This facility is a little more outdated and unkempt, but still felt comfortable.

This community was not the right place for dad!!

My dad did not do well when he stayed at this community. When they evaluated him they said he was more advanced than he was. He was not able to be as active as he liked and since leaving he has declined some. The caregivers were nice they just did not seem to engage him in things he liked to do. He sat around most days.

My grandfather resides here and has had bad experiences that are not just an inconveience, but [removed]. They mess up his medicines constantly, the nurse is exceptionally rude at EVERYONE in my family, the staff literally laughs at my grandfather when he is having problems. (We have seen it firsthand). A Vetran does not deserve the type of treatment he receives. (NO one does). Multiple complaints have been made about the nurse, but it has not changed. In fact, they are promoting her to manager soon.

Care and quality on a decline

At first was a fantastic place. Last few years decline in quality of care, staff are no longer friendly they complain and are annoyed by residents. The meals are not appealing or nutritious. The pictures of the house are deceiving, looks beautiful but is a dump. Rooms are not cleaned. Including the ceiling in an office that literally looks like will fall down any minute.
