This community was okay. We were disappointed in our experience. My loved one had a short experience here. My ratings are based on experiences from other people, such as my loved one. I met [name removed] and I think she was a nice lady.

[Name removed] was very warm and understanding, she would be a fantastic care giver, but the facility itself fell way short. We got a overwhelming "Nursing Home" feel. Rooms were super small, no room for anything but a bed and a chair. Mom and Dad could not stay together in the same room (deal breaker!!!) and therefore cost was high, paying for 2 rooms. Facility smelled like disinfectant and the hallways were narrow and cluttered with dirty linen and housekeeping carts. What they referred to as a "homey" feel, felt more unrefined. Transport services were very limited, only for group outings.

This review is for the Chapel Crossings location of Sylvan Crossings. I was looking into moving my other into Sylvan. It would have been ideal for her to live here, as it is quite close by. However, I was far from impressed with them. We initially had them in for two home visits. On the first visit, one of the people in charge came. One the second, someone who was just a general employee came. After those two initial visits, we were never able to get any sort of follow up from Sylvan, no answers to our questions, and no further assistance. It was easy to make the decision to ultimately place my mother elsewhere.

Pretty expensive per month especially considering the condition of the facility and the actual resident room. Unkempt grounds. Everything felt outdated and not very cheerful. Residents seemed "on their own" and saw no staff engaging or interacting with the residents.

dreary place. The cook dumps your bathroom trash. staff seems friendly enough, and therapy is very good.

We toured this community when looking for my mother. The staff needs more training on awareness of each resident's ability levels and accommodating that. Offering an environment that recognizes every resident is different. The atmosphere should be more fluid.

A beautiful new place, but they apparently didn't write down we were coming and were not prepared. I did have a confirmation email however. They didn't seem to have things organized, and didn't have anything going on for the memory care residents, and weren't able to answer that there is much organized for them. We were not able to sample any food, but the menu was less than inspiring when I viewed it on the tables in the assisted care area. The receptionist seemed put out that we were there when we first arrived. Not at all welcoming. We didn't see much to sell it to us.

Our tour of the Sylvan Crossing Hunter Ridge made us reakize we would never put our mother there.The place was not clean and was very opressive, I was there with 2 of my sisters and we walked away thinking we couldnt put our mother there.

This facility does not allow residents to have a scooter and our father can only get around with one so we ruled with place out.

This community is a newer community and they have already had a high level of turnovers. Things are left out and they are sloppy. I have come in to find dirty diapers on the floor. My mom does not get her baths as often as she should. There have been some growing pains with the kitchen and the food. I have had nothing but issues with this community since she moved in. They are slow to open and slow to fill. It is a nice community as it is about 6 months old.

The facility may look beautiful but the care is not. Our loved one has been there for several months, as the residents level of care increases it seems that care is not provided. It appears if he resident is independent they will be ok but god forbid as your level of care increases the care will not be there. Family must advocate for their loved one. Even though the care plan includes a detailed plan it is not followed. The hourly or two hour checks are not done has per plan of care. Showers \/bathing not given as indicated on plan. We have been very disappointed with the cleanliness of the room and bathroom. Phone calls are returned and plans made but not followed through by all staff.

Rating of zero. Studio was $[removed]/ month and it was ghastly tiny. There were no activities to speak of because they put crime scene tape across the area that most places commonly use for activities which is the lunch room. After the meals it's taped off. People have to stay in rooms. The little activity room was just women's type stuff. There was no encouragement to wander about. I'm not sure about the food/ it was passable. But I really don't know. The cleanliness was ok except there was no housekeeping in the room. The wastebasket was not emptied by the door. The shower was kind of rundown. They tucked a blanket across the window by his chair. To stop the draft because I guess the window had an issue. They say him right by it anyway. The staff and management care were very concerning.

There are not enough staff for the number of residents. So the residents don't get the attention they require.

This facility was disappointing from the start. Poor directions to the entrance. Inadequate handicapped parking. During a week of visits and discussions, the staff tended to sidestep questions dealing with my assessment and kept erecting small obstacles during what eventually appeared to be an evaluation for acceptance to rhe facility. Finally, I was rejected as not being a "good fit" (whatever that meant) without any true assessment of my abilities. The staff didn't seem caring, and the facility itself seemed rather dead compared to another I visited later. My entire experience with them was very disappointing, to say the least. It turned into a blessing, however, because I later found a much better place!

My sister lived the last 6 months of her life there. She was terrified of living there to the point of crawling under the dash when my sister brought her back from an outing. She had Alzheimer’s and had little to no short term memory. She was strong and very agile physically to the point of climbing on the counter and retrieving items from the top shelf. [date removed], family was notified that she was sent to the hospital with 6 fractured ribs. The circumstances surrounding this remain deeply troubling to those of us who love her. My niece shared images of extensive bruising on several areas of her body- a heartbreaking testament to suffering no one should endure.

Avoid this facility. My experience was a nightmare due to systemic dysfunction and a total lack of empathy. After assuring me a long-term care bed was available for my mother, staff gave me less than one business day's notice for discharge on a Friday afternoon. Admissions staff was condescending, gaslit me regarding previous conversations, and hung up when I asked for help. I had to hand-deliver referral paperwork myself because they failed to fax it. I emailed the Administrator and the President of DePaul Hospital to document these failures; neither responded. Ironically, since moving to a different facility, my mother improved so much she is now moving to assisted living proving the "skilled" care here failed her. This administration is unresponsive and indifferent to families in crisis. If you want compassionate care and professional communication, look elsewhere.

A bunch of young kids trying to run a nursing home with no compassion no caring about the residence the food was horrible I mean horrible and the care was so bad that my mom had to hire a private nursing company to come out they say they do do their stuff and check make bed checks they don't they just mark off that they do stuff we couldn't get her out of there fast enough and the turnover of that place should stand as a witness that nobody should be putting their parents in there. My sister died of cancer in the facility and they barely checked on her they give you pendants that you push if you fall my mom laid on the floor for over an hour at night because her pendant was inoperable if you have to have your food delivered they only give you the leftover scraps if there's anything left and they don't even care if you got a special a diet that you tell him verbally everything has to go through the doctor in order to get special meals. Her neighbors were moving out as fast as they could and the day we moved my mom out of there there was a lady that said that she was so glad that they finally got her parents out of there they have a high turnover rate there and out we're still waiting to drop some reviews on Yelp and their website as well sorry this this is such a long thing but people need to know there are bad places like this

Do not trust this place they are out for the money and the money only One housekeeper all good CNAs ran off they too short staffed and the lady up front was a bold face lier don’t trust this place it’s crazy mad house can’t keep good staff and the food for guest is outrageous! [price removed] a plate! lol it’s not worth it

My moms husband is there. The care is terrible the management dont do their job judt sitting in the office. The caregives walk around on their phones they are lazy dont give showers dress the clients in clothes that arent their own and dont even fit. I would not let a loved one live at this place it should be shut down.
