Abuse and neglect

My father was locked in his room in the dark. Personal care was not being done. We moved him out to keep him safe because the BROKE their end locking him in room. Will not refund money because no 30 day notice. Wrong they should owe us for not doing the service we paid for. Do NOT put your love ones there. Also had items stolen. Not worth the High cost and poor care

Hostile staff

We removed our loved one from this facility after an awful blonde nurse whose name starts with [name removed] and her sidekick bullied my boy. Things started off badly when the key we were given by the facility didn't work. Instead of apologizing for the mistake, the nurse doubled down on scolding us because she had to get up from her chair to let us in. Another disabled resident discretely indicated he was not happy with staff either, and this nurse especially. My boy turned down some food, so the nurse labelled him "eating-disordered" and wanted him out. Think twice about putting your special people here with these lowlifes.

Kruse Village experience

On [date removed] my family and I moved my Dad to Kruse Village, hoping he would get more care as they promised. I was wrong, from the first day nothing was what I was told. First I was informed that my Dad's room would be ready on [date removed] and he could be moved on that day. My sister and I walked into his room with hands full and saw that the room wasn't ready because the commode was sitting in the shower and no running water. Since hospice had been assigned to my Dad a bed was already set up and his two recliners were already in the room. My sister and I should have taken my Dad back to where he was staying or to his home, that was a mistake leaving him there. So, from [date removed] till [name removed] my Dad was not taken care of properly. Examples, I had asked if Kruse personnel would help my Dad during meal time feeding him. I was informed by employees that they were not allowed to feed the residents, even the hospice nurse assigned to my Dad requested them to do the same, she was told the same. I asked employees that were contracted to Kruse and they told me to contact their boss and they would gladly do it. My Dad was left a lot alone in his room, of which, he slept a lot. On days I would visit many times other residents would walk in his room and look in his closet, etc. I also saw a resident leaving his room as I was walking down the hall to my Dad's room and when I came in they had been in the bathroom using his toilet. Not happy about these things and asked why residents were allowed to walk into any other resident's room and I was told it was okay. I did not agree and looked up in the residents' rights, of which I signed for my Dad, and read that a resident must be invited to another resident's room. I did not pursue the matter any further until one day, in September, I walked into my Dad's room to visit and smelled a foul odor coming down the hall. I saw my Dad laying in his bed asleep, and I thought someone had come in and used his restroom, as I turned to examine the room further, I saw someone had used my Dad's recliner as a toilet and a huge pile of human feces was in his recliner. It took me about a week to realize that it was not going to be good for my Dad to stay in this environment any longer and contacted the hospice nurse assigned to my Dad to get him out of Kruse Village and in two days, [date removed] my Dad was brought home to his residence in Brenham. The day he came home he had the biggest smile on his face and was so grateful I brought him home. My Dad lived 13 more days and died on [date removed]. I will not recommend this facility to anyone, especially the memory care department.

Disappointment

The old ED did an amazing job, all of a sudden she was just gone. Overheard staff that management fired her for no apparent reason. Shady company as she did a fantastic job and always updated us on our loved one. She took the time to get to know them and knew each of them by name and personal likes and dislikes. Was extremely patient with each resident and treated them like her own loved one. Can’t say the same for the new one that took over, her food is always closed and not as personal. The caregivers are amazing and do everything they can but not as supported like they used to be. Sad this place has so much potential but go through directors like crazy.

What has happened to the staff at Stoney River? The nurse is gone, several of the staff have reportedly resigned. The staff are being mandated to work hours of overtime because of the lack of staff. Many agency staff, some wonderful, some horrible. The Executive Director is short, and rather sharp tongued. She seemingly doesn't have too many good things to say about any of the staff. We toured and loved what we saw, but when we found out that there were so many issues with staffing, we decided to move on. Ran into an LPN that worked there, she cried as she told us how she was abused by the ED. It seems as though she needs a bit of VIVA herself. They pride themselves on VIVA, but the team members aren't aware. They have nothing good to report on the leadership team still working. Perhaps a visit from the upper management is in order. Change in the team might be a great idea.

Very disappointed! The building smelled and the staff smelled of cigarette smoke. I’m okay with tattoos, but this generation they serve is not. Manager seems not up to date on nursing issues and an RN only comes 3 times a week. Food also looked horrible

Not a good place for a loved one.

My loved one was a resident at Azura. While they were there they received very poor care sitting for extended periods in their own urine received no grooming care. fell numerous times receiving facial injuries due to inadequate supervision. Staff observed sitting and talking and playing on their phones ignoring resident's needs. They admitted to not having a housekeeper to clean rooms resulting in dirty rooms. When staff was asked for a report on resident they had no knowledge of what had occurred on previous shift

Keep your loved one at home

[Removed]. I would not recommend for anyone to place their loved one at Autumn Leaves as I feel the care is not good. The caregivers do not have the knowledge or compassion toward the residents that they deserve. There is not proper oversite of residents. Wish I had kept my Mother in her own home. Biggest mistake I've ever made.

OAKRIDGE offers great services and a beautiful facility and setting. So many wonderful opportunities for the elderly exist. BUT, my mother is seldom, if never, out of her room--even for meals lately. My mother and her room smell of permeating urine, her physical appearance is dehumanizing. NOT ENOUGH HELP! But the charges for all the special services continue. My mother's money is soon gone. Gone to services that were never provided!

Would not recommend for memory care

When she moved in they were supposed to get all her medications. They did not get her insulin for a week and they never told us. In the memory care unit, usually facilities will do memory enhancing activities - they do not do any of that here. They would wash her sheets and it would take days to get them back. The garbage bag in her bathroom would be completely full. Every time I came to visit I would have to tell staff to empty it. I had multiple talks with the director. Many times when I went to visit they did not have the proper staff to resident ratio. I would try to find people and it would take 15 minutes. Our loved one fell out of bed and the nurse seemed irritated with her - she got to the ER and we found out she had broke her back. They never took her back and never called to explain why they would not let her back. The director finally called us back a few days ago and was telling us we need to come get her things. This has been the worst experience.

Beware and do you research

Due to the covid restrictions we were not able to see our loved ones room. We were not impressed with the care services, the medicine was given at irregular times, and we felt the staff members were not adequate enough. The chef quick, so the maintenance workers and care takers were having to make their meals.

Terrible care

This place did not take care of my father. He fell two nights in a row. He was found hours later sleeping on the floor with a gash in his head. He was suffering with severe and painful constipation and they did nothing to help him. My mom had to go out and get medication for him. My mom took him from the place 5 days after he got there and they didn't want to give her a refund. In the end, she only received a small portion of what she had paid back.

Oh my, where do I start. My mother and father are in this facility. My father needed the memory unit (assisted living)and the care he received there was first class until the head nurse was replaced. He then needed more care and was moved to LTC facility. He walked to the LTC facility and within 4 days he was in a fetal position in the emergency room semi comatose. My mom is in assisted living and they chart that they check on her hourly, but don't. Very poor care. We are looking at moving them. Poor Long Term Care unit. This place has really gone downhill since it has been taken over by the new owners. My great aunt was there 5 years ago and it was excellent. New owners, terrible care.

Sylvan Crossing is not the place for your loved one. The activities are nearly non-existent and the meals don't look very appealing. When we toured, the place didn't seem very clean and the staff were busy gossiping.

Poor care re: medications

I am a healthcare provider in the Green Bay area and have had several patients who reside at this facility. I am very dissatisfied with the care provided to residents at this facility especially regarding medications. Unfortunately, I have seen a lot of medication errors made at the facility, sometimes serious enough to cause problems such as kidney failure and hospitalization. Meds are often not given as prescribed, med lists/records are often not accurate, and orders/recommendations are sometimes missed or not followed. This is a serious problem. It poses significant patient safety concerns, and as noted above, some of my patients residing at this facility have suffered serious consequences as a result. My office has notified nursing staff and management at Age Well of our concerns and problems on multiple occasions in attempt to improve the care they deliver and prevent future problems, but unfortunately, the same issues continue to occur. I would not recommend this facility to any of my friends, family members, or patients.
