The community is very good, [name removed] is awesome. It is disappointing the directors change so often. COVID sucks!!!

There are multiple buildings and types of apartments to stay in. The food is average in restaurant style dining. We chose this community because it is close to our family and they had good recommendations. When we toured they were on time for appointment and the director of the community a the tour and he answered every question that we had. I would absolutely recommend this community to other families.

I think it is a good place. I like the layout of the community. My mom is happy and I think she is being treated well. You need to watch the costs they tend to increase over time. They have a small center courtyard for outdoor space.

Nice clean place no beds

This facility has recently been purchased by new owners and many changes are in the works. They are moving to all dementia care. We have some familiarity with this facility since our father lived here.

I'm very happy with the care he receives just a few things that I feel need mentioning. Most of the time when I visit, which is weekly, I need to ask for toilet paper, and as it's gotten better the toilet needs cleaning! Also 0 nine care has drawn a red flag while trying to figure out Medicare plans! With all that being said his new home is very clean and staff is so pleasant and helpful!!

Overall Azura does a great job caring for my mom. They get Mom involved in various activities such as bingo and bean bag toss. They are good at staying on top of Mom's changing medical needs and the facility is warm and homey! Over the years, the staff has been great to communicate with and they keep me informed in what's happening at Azura.

The facility took good care of my loved one. The workers were friendly and professional. The food was good. The visiting physicians were really handy - most of the care could be in house. Over all we were very satisfied.

I am pleased with the quality of care my father receives and the dedication and compassion shown by the staff. The meals, however, needs to be revised. Although good, they are not what the residents like. These are people who grew up during the Depression and are used to basic meals, not Mexican and Chinese food which tends to be spicy and difficult to eat. Many of the residents have difficulty using silverware and cutting, for example, tacos is next to impossible. Consequently, most of the food on these days is untouched and thrown away. The meal planner needs to remember who the recipients are and plan accordingly.

We love the small family atmosphere and warm, friendly, caregivers. The only reason for 4 stars is that at times it seems a bit much for 1 person to be alone and the food is not always appropriate for elderly patients, i.e. spaghetti (long noodles) or anything messy.

Mom has been at Azura for over a year, and I never have to worry about her care when I can't be there to visit her. SHe has a lovely apt. that meets all her physical needs. There is a core group of caregivers that excel in their care and patience with Mom. They always have outside entertainment coming in. The backyard is a lovely spot for walks or sitting outside. The grounds are beautifully tailored and decorated throughout each season.

Overall a good experience. My mother has been at Azura Oconomowoc for a little over three years now. Well into her 90s and with advanced Alzheimer's, she needs a lot of help. Azura does a very good job, all things considered. They really seem to do their best to hire caring and capable staff, although high turnover means that her care takers don't know her personal needs very well and many are just learning the job. The mental and physical state of her fellow residents has degraded considerably since she first moved in, making it a bit of a depressing place to visit and spend time, despite the staff's warmth and caring. My mother almost never leaves her room anymore, even when family visits. I'm not sure how is due to her aging and how much she wants to avoid the other residents. The building overall is quite clean and the grounds well cared for. My mother's room is often messy and dirty when I visit. I'm not sure how much of this her habits (I won't get into them here) vs. how often her room is cleaned. I think this it is incredibly difficult to find a place to provide good care for a group of elderly dementia patients. We tried to care for my mother at home for a long time and evaluated many homes when we finally decided she was more than we can handle. Azura is probably as good or better than any of the others, even if it is not perfect. I would recommend it.

Having been in many communities, this is one of the best. Staff is very caring and knowledgable.

We really liked this one. Very personable. Concerned for the ala cart prices. Very homey atmosphere. Room was a bit on the small side.

I have been a guest from Hometown for almost a year and it has been good. I've made good friends and the residents are wonderful. It's a great place to be.

My mom has lived at this location for over 2 years. She has had many obstacles health wise, as many do when we are blessed to love long enough. The care and compassion for mom has been wonderful, even with these obstacles. She is happy - my family is happy and thankful we chose Azura Oconomowoc.

Overall very good. Having cared for several relatives and parents over the past 15 years in various nursing homes or assisted living facilities, I would say what is best about Azura is its small size. This environment allows for more personalized care and attention. Would like to see more phasis on stimulation throughout the day for all residents not just those who can participate in a game, puzzle, etc. More options for outdoor activities. How about caregivers who can take residents on a bus or van to a park, local concert, out to eat, etc? (for example). Memory care residents should not be isolated in the facility. Some do not have family to take them out for simulation.

The Director of Community relations at Azura Memory Care far exceed my expectations for assistance with learning about health care options available to our family. She diligently worked with us to ensure our mother would be able to reside at Azura during a time of family crisis. We will be forever grateful for her caring, understanding, compassion and cooperation!!

I live in FL and have only visited a few times. I was pleased with the facility, the staff and apparent care provided to my mother.
