Caregivers are very nice to him and me. The head staff is always friendly and helpful. The setting is beautiful, clean and comfortable. The courtyard is as gorgeous. My favorite place to be with my loved one. The food for the most parties homemade and tasty. I’m glad this place was referred to us when I really needed help with him. They said they would take good care of him and they have. The music and played events are enjoyable too!

We moved mom to Azura from Arizona after dad passed. The family was nervous about moving her back to Wisconsin in November but she loved her new home. Staff made her feel like a queen and she enjoyed helping other residents. Azura memory care made mom’s last chapter one of the happiest of her life. We will be forever grateful.

Our mother has been here just over a year. Azura is clean, the staff very friendly and we couldn’t be happier.

This is a wonderful care facility! Residents are treated as special individual to whatever their needs are each day. If one morning my mother-in-law did not want to go to the dining room, a staff person would bring their meal and hers and they ate together. Staff encourages each resident to functions all their highest level taking the time to know which behavior/activities would encourage independence/higher cognitive function – and which might be upsetting.

Azura is a beautiful facility with very caring staff. My father has been here for a year and his care has been fantastic. There are lots of fun activities. The staff always goes out of their way to make the family members feel welcome and comfortable.

Am very happy with the care my mother receives. They handle her limitations and restrictions in a safe and conscientious manor. All her needs are take care of in a professional way.

An upscale community that in riches the lives of our patients, family and staff. An understanding that the ones being cared for were different people before having to be in memory care, Azura pulles all of its resources to insure comfort and quality of life. That’s why I love being a guest in this establishment.

My wife has been at this community since November 2016 she is only 66 but her illness has progressed quit quickly. The staff and caregivers at Azura are first class, they take great care of [Removed] with love and respect. I always feel very comfortable that all of [Removed] needs will be well taken care of and she will be treated with dignity. I would highly recommend Azura.

I love the fact that Azura memory care was a state of the art facility geared toward love of people – memory and neurological issues. Very clean facility. Resident at Azura are kept busy during the day with a multitude of activities.

The reason I chase Azura was because 1. I watched the building being built and was very impressed. 2. [Removed] was in the hospital and when released I knew I couldn’t bring him home. I had two choices – Azura or the view at Pine Ridge. My first was Azura after meeting [Removed] and talking with her there was no second choice. I immediately decided. I was so fortunate to get the last room available. The people are so very nice, concerned and caring. The facility is kept so clean – the food is good. Everybody goes by first names and patients and visitors know each other. Nobody is a stranger for long. In all honesty – I couldn’t have picked a better home for [Removed].

Azura does an amazing job creating a culture of home and family. The activities program is innovative and pt centered. Focus on the physical, mental and emotional wellbeing of the patient is a priority.

We like this community for good care. We like the staff who are caring and attentive. The menu is good with a lot of healthy choices. They offer actives for the participate We would recommend this community for good care and a nice environment for others.

Since we moved my loved one to Azura Memory Care of Oshkosh her health and well-being have improved. The staff is very friendly and welcoming when I come to visit. The staff have changed and regulate her medication so there is a significant difference in my loved one's quality of life. They give her more free reign and are working with her and her needs. She looks healthier now. I call regularly and they keep me up to date on how she is doing. They walked me through their patient care plan that is based on her needs. As her needs change they will update the plan and keep me in the loop of what is being incorporated. The atmosphere here is like a home setting, not like an institution. They do have activities for the residents and she is participating. I think Azura Memory Care of Oshkosh is a good fit for her and her well-being.

We toured the community for mom's care. The staff was nice and the community was clean and maintained. We like that the staff was very kind and patient. Meals are offered that are healthy. We would suggest a tour for other families for the value and the levels of care that offered.

This was a nice tour for care for dad. We like the staff and the environment would have been nice, but we needed a small community for the care that we needed. The menu and the activities were helpful to know would have been offered. We would recommend others to this community for them to see this community for care.

I recently toured Sylvan Crossings at Creekside Estates with my mother and over all we loved it! The director was very helpful and was willing to give help and advice that we could use while touring other communities. From the moment we entered the building it had a warm welcoming home like touch. The cook was making homemade cookies. I was able to speak to a couple different family members and they had nothing but positive things to say regarding the care of their family member. We were told that the Director and care team was the best. This will be moms new home. I was told if you want them cared for by a loving team, Sylvan Crossings is the place!

The facility is really awesome! The staff are accommodating and friendly and the facility is kept well maintained. Everything is going so well, and we don't really have any complaints so far!

One of our family members did the tour for us at Edencrest and they did like the community, it was clean and spacious, the staff was friendly and they felt Edencrest had a lot to offer their residents. Between the two we felt that Prairie Hills would be a better fit for my mother-in-law.

I like the care that we had from this community. The staff was nice and friendly. They needed more staff members to be able to care for the residents. The community was clean and maintained. The staff needed to make sure that he had his personal needs to be taken care of.

It looks like a good fit for my dad. He does not have any issue with the community or care. The staff's care is good and I am getting updates but we are there to see him too and I do not see issues. It is a clean community.
