I found a home in the community for my patient. It is always very clean. The staff have been providing the care he needs. I liked that it is around the corner from my house so it is easy to get to. After an incident which lead him to be in a wheelchair he will need more care. It was good here during his time.

I can't say enough about the staff. They were always so caring, supportive and helpful to mom. Mom was getting acclimated with the community. They respected there residents and had good food and a clean community.

I have been overly pleased with the great progress my mother has been making at this community. She loves the fact that she can go down to the dinning room and get her meals anytime of the day. They also offer a fantastic activity program at this facility including an exercise class three days a week that my mom also really enjoys to participate in. Overall she has made her self right at home and is very comfortable in her new living environment.

Our step-mom has been with Azura just a short time, but we have been pleased with her care. She can be a handful at times but the staff handles her with care and compassion. We feel we are very blessed to have found such a warm and caring place for someone who means so much to us.

My step-mom is doing good here. She has made friends with a group of residents. She loves the therapy they provide for her. Sometimes she likes the meals and some times she does not. Options are available for the meals. She has not gone on an outing yet but she has participated in the ice cream social and the Lutheran prayer service.

I am very pleased with this community, they have provided my loved one with excellent service and have helped her get involved with the other residents. The staff have also motivated her to get involved with the various activities and field trips they have going on throughout the week. Overall my mother is comfortable and feels safe in her new living environment and has no complaints.

This is a outstand community. The staff members are great. They have snacks available at all times. they offer great activities to stay active if possible. We would hight recommended this to excellent care.

The staff that offered the tour at Country Manor were very helpful and friendly. They were able to address all of my concerns. They had a beautiful courtyard! The bathrooms for the residents were in the hallway and that wasn't the right fit for my loved one.

My mom loves the meals that are served at this community. The employees are attentive to my mom's needs and I am able to communicate well with them. My mom has been participating in activities that is available. She is doing well here.

This really feels like a great place for a family member. We have not yet made a decision as to where we will place our father.

Azura Memory Care Center has been the best place for my father in law. They have been welcoming and for someone who never wanted to leave his home, he is very happy there. He thinks he is "in a first class hotel" and now never wants to leave.

The staff at this community is really good. There is good communication with the family. I did try the food and it was good. They offer daily activities, however he does not feel comfortable participating at this time. Azura is kept very clean and I would recommend this community.

The staff have bent over backwards to make sure we were happy. They answer all our questions. One of the nurses Dot is phenomenal. They are so caring and kind and make us feel like part of their family. The food is good and they offer lots of choices. They accommodate my mom's dietary needs. I would recommend this community with no hesitation.

We were very, very pleased with this community! It met all of our needs at the time we were using them. All the staff couldn't be more wonderful and when working with all of them they were amazing. Barb was just amazing and was great support til the end! I can't say enough good things about this place, our experience was over and beyond great!

I just spent a week with my mom at Age Well. Although she is still having a hard time being away from her home, the staff at Age Well are really working with her to realize this is home now. They are so very caring and helpful, especially with getting her involved with all the activities and there are so many. I was able to do many of those with her and was also made to feel so welcome to be there. I did also have a chance to go to one of their residents meetings and was impressed with the problem solving and managements ability to listen to the residents and come up with solutions. Was also there to experience the happy hour on Friday with entertainment that was excellent. I just want to thank Lori and Wendy and all the staff that are striving to help my mom and all of her family make this work for all of us.

My Dad is thoroughly enjoying his time living at Age Well. The staff and residents are friendly and attentive. The activities are varied. The restaurant style dining room is very much to his liking. He has only good things to say and I would agree.

This is a beautiful community and everything is working out very well for my father. This is the perfect place for him, we are very pleased with everything they have done for us. Everybody is very nice and friendly. They offer great care, so we know he is in good hands. The food is wonderful and he is trying some activities. He enjoys the bible study class and the exercise class. I know he is safe and they are taking good care of him. I would absolutely recommend it.

It is going very well and we are happy with Sylvan Crossing at Creekside Estates. The staff are very good and the community is very clean. There are activities but my father-in-law does not participate, He likes the food and I would recommend them because of the smooth transition, extra attention in the beginning and settle in nicely.

My dad has been living at Age/well for 3 years and his time has been very pleasant. His crew is outstanding and the food and activities are great! As my dad is aging they encourage him to stay involved to enrich his life. We are treated as family with a smile and kindness. Every individual that works there, truly cares about my dad. They go out of their way to comfort him when he is down, and laugh with him when he is up. It is a wonderful, pleasant living experience for my dad and he is taken care of very well. I have great admiration for why they do for all residence there.

This is my home. I live here. I can have bad days and be at home. I can good days and be at home. I can go to the hospital and be fought for to be at home. Thanks to my home!
