It's a big place, with a lot of residents. My loved one was only there for 17 nights. Overall the few times I was allowed to go in, mostly just the move-in day. The building itself seemed clean, but the entryway in the dead of winter was disgusting. There were cigarette butts where everyone would congregate to smoke, so you had to walk through smokers to get into the building ,which was a turn-off to me. They were supposed to clean the rooms every week, but I don't think they cleaned it the whole time she was there. They did do her laundry maybe once. The sales lady was super nice, and the manager seems to be upfront and friendly. I didn't get a really good feeling with maybe a couple of the nurses. They were just kind of in a hurry, like maybe they were stressed with a lot going on. My loved one ended up being a fall risk, and I feel like she really shouldn't have been there. She needed more care than anyone realized. They did an assessment of her while she was at rehab, and I feel like they should have known. My loved one had a fall behind closed doors and ended up in the hospital. The day she went into the hospital , I asked to speak to the nurse on the phone and there ewas no registered nurse in for t he day. That was on a Saturday. It was all just a mess and not a good experience. It was too bad, because I was so excited goign ink, and then Physical, mental, monetary stress involved getting her moved out. The only positive was my loved told me she loved the food, the few meals that she had there. It didn't look like they were activities at all due to Covid.

Very good place for a self-sufficient senior.

We had some cleanliness issues in my loved ones' room. Housekeeping only showed up occasionally to clean, and things tended to pile up. The common spaces in the facility were fine, but my in-laws were kind of on their own in their living space. The people there were nice, in my limited experience. Most of my interaction with them was over the phone. But mostly the nurses were great. They were friendly and tried to get things done. There was one real stinker there, but the director addressed that for us. The director was very sweet and very nice, but we felt she was overworked. She just had a big job and not enough help. There was a high turnover, especially with the nurses. The director wanted to keep my loved ones there after they were hospitalized, and we agreed. But in the end, she could not provide everything they needed care-wise. I think it was just overwhelming for her. But she was very good. The lady at the front desk was very friendly. They did a lot to keep Covid out. There were some good things, and the people were great, but overall it was a disappointing experience. It was a very high cost out of pocket, and they couldn't provide the assistance that we wanted or needed. I think it would be a great place if you have plenty of money (although they do transition to Medicaid), and if you don't need a lot of care and are pretty self-sufficient. They brought meals to my 90-yr-old loved one in a styrofoam container and expected him to be able to serve it to his wife. They'll deliver your pills and make sure you have those on time, but its a lot to pay for that. They were very slow in responding to requests, including responding to the call buttons the residents wore around their necks. I think it's because they've overworked there. It's a hard job, I know. But still, we expected more. My loved ones were not really impressed with the food there, and there weren't really activities due to the pandemic. I hesitate to give them a bad rap if your loved one just needs a little help with medication and is otherwise active and able. I just don't think this was the right place for my loved ones from the beginning. At first Heritage Woods was hesitant to take them, and eventually they said okay we'll try. I just wish things had worked out better. It's not a bad place, so I'd recommend it for self-sufficient seniors. It's a beautiful place, inside and outside. It was very appealing to the eye. I just wish things had turned out differently.

Please Power of Attorney Onfile

My comments are meant to be a help for family members that have a loved ones living at Montclare-Lawndale. My brother was a resident there from [dates removed]. When he completed his original application and all required paperwork he did not have a Power of Attorney on file, he was in pretty good condition health wise. As a family member we had no knowledge of him moving to this facility. All was going well. However, he became ill, was transported to to a local hospital, from there he was referred and transported to a Rehabilitation Facility. During that time Montclare destroyed/threw out all of his belongings without any notification to him or a family member. We were told because there wasn’t a Power of Attorney on file the facility followed the rules in destroying all of his belongings. I was told by a State of Illinois Agency, the rule is a 30 day notification is required. That never happened, I strongly suggest Family Members of Montclare Supportive Living review the loved one file and make sure there is a Power of Attorney on FILE. I have been unable to properly resolve this issue. Thank you very much.

Not entirely happy

The place seems a bit run down. Difficult to reach patient as phones not always answered. Follow through is not always kept as promised. Example is we purchased an immediate response fob that was found to be defective. The replacement did not work. We were told it was most likely due to the system needing replacement. It would seem that this would be a priority as the patients with fobs would need immediate help. So far after a month it has not been updated.

My mother has lived at Gull Creek since 2017. We chose Gull Creek because of the spacious apartments, welcoming staff and excellent dining experience. There have been several management changes during this time as well as a status change from non-profit to for profit. Under current management, staff turnovers have impacted time to complete work orders for apartment repairs as well as continuity in facility operation. Upper management slow to respond. Food is poor quality. Snacks offer little variety and include single serve packaged crackers and fruit. Dining staff is extremely pleasant, friendly and kind. Floor staff are caring and patient.

Nice facility but lacking

Nice clean place but the services and maintenance have gone down hill the last year or so. It's sad and I wonder how far down things will go. Staff turn over has been great over the last year, therefore continuity of care and maintenance/housekeeping is suffering. The employees are very nice though. Maybe the turnover tied to search for better pay? We are thinking about moving my parent.

Not the best, not the worst.

My loved one has lived there almost since it opened. It has been disaster after disaster. They start with one director -- she tried but seemed like she had no support. Then they brought in a new director. Simply amazing. She cared about us, listened to us, followed through on our requests, ect... Then I come to visit mom one day and she is gone. No warning, goodbye, anything. They said she was laid off to move another direction. Well, here we are again starting over. HUGE mistake on their part. As far as I know including my family there are 5 others that are currently looking to move. Since moving in in June we have had 3 administrators, 3 nursing directors and now 3 memory care directors. IN 10 MONTHS! The CNAs are good for the most part, however most do not stay. Nursing is okay -- not the best but also not the worst. All in all I would suggest finding a different place. One with more consistency and stronger roots.

That one seemed to be crowded. The dining room was the smallest of the four places we saw. There were a lot of people hanging around the hallways. I didn't take a look into the rooms because I didn't like the feel of it from the start.

There was never adjustment for special meal requirements- mom needed soft food and the liquid Food in general was just ok, there was not a cook they just reheated prepared meals Having nurses in charge of this facility should of made the facility better but they did not like to be questioned or corrected, which is what occurred between them and the hospice nurse. When we did a review of moms care they lied about moms ability which was a last straw but nothing to be done because I was aware that we were in moms last weeks. Mom was assaulted by another resident last fall, which was stopped by a staff member but it was not handled well. They then offered my mom a space in memory care which was ok. The memory care has a good reputation with hospice and the aides and med techs for the most part were excellent. They like all places had problems with staff shortages. I did hire sitters for mom and they covered every afternoon into evening for 4-5 hours. The facility told me they (the sitters) were not necessary but they were my eyes when I wasn't there so I'm sure they improved the care. If there would be any thing that I would say about future residents - they should not have problems and be able to move themselves (use wheelchair with out assistance).

My experience with Oasis at 56th Street was extremely unpleasant. I always went outside to smoke but due to having Covid, I was confined to my room. I lit one cigarette and a nurse walked in on me. I was evicted and they kept my rent refund. [name removed] on the 2nd floor and [name removed] on the 4th floor smoke openly every day and go unpunished. My watch was stolen and they took my laundry and it was never returned. The food leaves a lot to be desired and it's always cold. One day when I was sick, I ask a nurse if she would take my temperature and she sent me on a wild goose chase looking for a thermometer. None was found. BED BUGS throughout the community. They took every article of clothing I own and kept it for 2 weeks. I showered daily but had to wear the same dirty clothes. Privacy is out of the question - they freely use a pass key to walk in on you.

Review for Independence Village of Greenwood

I would say there are a some great caregivers who were. Good to my mom. Alot of turnover of staff and short on nights. Do not particularly care for upper management and [name removed]I feel I had to be very present and do many things for my mom myself. I would not necessarily recommend the facility.

Our Experience

My mother was approved to go there in Assisted Living. That surprised me and I told them that, but they said she had been evaluated and qualified. She was there for basically a week and was sent by them to the hospital and was not allowed to return because she could not stand and pivot on her own. The facility itself is wonderful.

Our loved one had spent several months at Carriage Court and we have to say it was a very poor experience. They have changed executive directors since then, but all of their system they have in play are not good. Everything from food, to individual service, to their follow throughs with personal care and just everything in general need to be worked on. We were paying more and receiving much less.

Definitely disappointed with the experience

Every time I come to visit my loved one is just sitting in her room. There is never anything going on for the residents. Whenever I go visit I have a hard time finding any staff to get any information. There has been some miscommunication with staff and they are very organized at all. When I looked at the facility at first, I really liked it because there is a great bit field in the back but my loved one has not been able to use it once and that was the main reason we sent her there. The staff try to tell me that my loved ones dog is using the bathroom inside but we know for a fact that that is not true. Overall it has not been a good experience.

Far from exceptional

Minimal help with ADLs. RN works 40 hr wk, not there nights/weekends. Family told all staff in-house but aides mostly agency = rotating strangers providing care. "Good" activities, going out somewhere are RARE. On Medicaid? [price removed] left after cable/phone, you can't go unless family pays. HUGE positive is Medicaid residents get private room. [price removed]+/mo private pay. Our big problem - LACK OF PROMISED CARE/ACTIVITIES. Treated like complainers by apathetic Administrator when we pointed out specific items in Dad's contract not being done. No 2/wk showers, aides spent under 10 mins/day interacting with him, they complained LOUDLY if he'd buzz for a little help standing up from toilet. Calendar activities a joke, shows many daily options but most are lame worksheets left on door to be done alone. Contract states care and activity plan individual to Dad's needs, never happened. Activity woman snippy when asked about activity plan. Average food. Waiting on partial refund 3 mos after move-out.

our experience at Gull Creek Senior Living Community

The community has had some staffing issues and the care that they have provided was sub-par at best. No communication until we told them we were leaving and I saw the director for the first time after 90+ days. The food was inconsistent and the staffing could be rude. The corporate office did listen and was able to help fix some of the issues.
