I feel that the care at Oak Hills Terrace is adequate but they need more skilled nursing staff to help the residents there. The staff does try to get my mom involved in the activities there and they do offer a resident doctor program along with at home care to help the residents. There is a shuttle that does take the residents to doctor appts, field trips and shopping for the ones that want to go. The meals are fixed for their diet and there are some other options as well. If need be I would recommend Oak Hills Terrace to my family and friends.

I had recently taken a tour of this community for my loved one and I was not as impressed with this tour as much as I was the others. The lady who was initially supposed to give us the tour ended up getting busy with another situation and we ended up having someone else. That person was just as knowledgeable and did not have all the answers to our questions.

Based on my tour they were fine. However I just didn't getting a good feeling for a match. It looked clean. The staff were all nice and good at what they were doing. Everybody was happy when I walked through. The number of people, the layout, and the dining experience was not a good match for my mom.

They are trying very hard to do what they can for my wife. She's having a hard time adjusting. However, I feel they could be proactive on feeding her and the meals could be better tasting. The staff call me with updates about how she is doing. It is clean from what I can tell but the furniture could be cleaned or replaced. I piked the community because they seemed more like a memory care.

When I have been in Autumn Leaves several times. I am not impressed on what I have seen. The people are sitting in a circle and looked drugged out placed in front of a t.v. Seems that there is no life in that facility and is DEPRESSING.

My tour of this community was a good one, everything went well. The staff was nice to my family, and myself. The community was clean, and well maintained. It was a good experience, but there were a few draw backs. This is a nice community, but it wasn't the one for my mother, and her needs.

My friend moved here recently and is having a difficult time transitioning. They could improve by offering more gender specific activities. I realize they can't have a blow torch or wood shop and they have to appeal to everyone but variety would be appreciated. They could also improve by providing more fresher food options. Less processed, canned and high in sodium, sugar foods would be preferred. I ate with him yesterday and dessert was a giant piece of cake with a heaping amount of icing on top. We chose this community because because my friend liked it the most out of the three options we toured.

The staff seemed friendly and informed. We liked the layout, with hallways in a circle rather than dead-ended. Our challenge was the size of the rooms and the fact that the bathrooms were shared. Our mother would not accept a room-mate.

I can only comment on the memory care unit my husband stayed in for a short time. They did not do a good job of cleaning the restroom he shared with a roommate. They did not help take care of his personnel needs. When I asked about this it seemed like they were passing the buck. The food is fine and the common areas seemed to be clean. On the weekends, the nurse is not on duty but is on call.

My mom is trying to get use to the community. Most of the residents are not that aware and she is not that bad. The staff are trying their best and the girls are nice to her. During the the week they have a lot of activities for the residents. The bathroom could be cleaner and they can improve on the cleanliness and be aware of the hygiene of the residents. I saw someone walking around with greasy hair.

I have a friend that goes here and I have seen the community. I have not gotten to talk to anyone around here, but the community is nice and seems like they have a lot of activities for the residents to participate in.

Mom has been her for a couple of years. She is happy here, but I'm disappointed. The care they say they give is different than the care that is actually given. Mom's room is supposed to be cleaned weekly, bed made daily. It's hit or miss. The bathroom is supposed to be checked and cleaned, if needed, every shift—three times per day. It might happen once per day. Things are always missing from her room, even if they consider it trash, she or I should be informed that it's taken. They have quarterly service reviews, but rarely anything I say in those reviews is acted upon. The food—well, it's been ok, but on my last visit it was terrible. It's very inconsistent. Most of the food is bland, but then some is way too salty, vegetables are mushy and overcooked, chicken was very dry. The menu has two options, but only offers one dessert. Though, desserts seem to be good. The activities used to be great, but now they are so-so. Mom plays more bingo than does anything else. I'm sad, because I wanted this to be her final home, but it looks like I'll be looking elsewhere.

I had to move my mother back home with me because this community was not working out well. They were messing up the dosage of her medication. The tech was really trying her best to fix the problem. They were very under staff as well. We also have billed issues and all they could tell us was sorry. I would just say into anyone looking to community's just make sure you know the price for the level of care for your loved one.

The caregivers there are excellent. The staff is very nice. I am not too thrilled with the office staff. She's been there for a month, my mother looks so much better since has been there. She seems happy and she has gained weight since she's been there.

Emeritus Estates kicked us out with no warning. She went to emergency room due to a fall and we found out she had a bladder infection. They are saying they cant take care of her now. They didn't even have enough guts to tell us to our face. They told the social workers at the hospital to tell us. They accepted her in and gave us no warning that they were kicking her out. I am not happy with this facility.

Take a tour and talk to love ones of residents before you decide.

I'm disappointed in the way they are maintaining my father's medicine. They are suppose to call me ahead of time if they decide to make a change in his medicine routine. I would also like to see more activities for my father. I think communication could be better internally. The location of the Foundtainview Estates is good. The inside of the facility is nice and looks good. (Activities is not applicable)

Friendly staff and happy residents ! It's almost like an all inclusive resort community for our beloved elders

My loved one has been here for over 18 months. A lot has changed at Autumn Leaves due to the house cleaning of management and staff over the least 6 - 7 months. The new staff is fine, no real complaints, but the homey feeling and family atmosphere is gone. The focus is on profit now, on bringing in more residents, or at least that is how it seems to family members that have been here a while. The extra bit of warmth and attention that initially drew us to Autumn Leaves is no longer there. Caregiver and nurse turnover is greater than it was even a year ago, and that is not beneficial to the residents. I don't know what is causing the turnover, but the connections my loved one made with the previous caregivers is long gone. If you are new to Autumn Leaves, you will probably think it is a fine place (and it is, compared to some of the other places out there), though rather pricey for what your loved one is receiving. For my family, genuine family atmosphere (the former director's own children would come in and spend time with the residents, too) that drew us to Autumn Leaves of Oswego initially, and for which I was willing to pay the extra money for, is no longer present. Do know, the price will go up at least 10% every year but your loved one will not see a benefit from the price increase. Profit is everything now. I do hope this changes, and loving care and genuine warmth returns, since that is what drew us here in the beginning. It is a fine place, but it could return to being a wonderful place again.

My loved one was here for a brief respite stay, and our experience was somewhat mixed. The facility itself was a nice looking place, and all of the staff were very friendly, professional, and hardworking. While I was away they were very good about keeping in touch and updating me on her status. However, there were a few issues we noticed that could have been improved. The mattress that was on her bed was a bit worn in and broken down, and I noticed that her sheets had some obvious damage (a cigarette burn). I also noticed the same burned sheets on her bed when I returned from my trip, so I don't know that they were changed while I was gone. My loved one also attempted to attend the religious service that they offered on Sunday, however it had apparently been cancelled without any notification, which certainly could have been handled better. Overall it seemed like a nice place with hardworking staff, but there were quite a few little things that added up to it not being as pleasant as it could have been.
