I almost moved my mom here. It's a nice community staffing seemed nice the location was the same as where I put my mom but something just told me not to place her here.

We toured this community for care. We did like the staff who did answer the questions that we had on the care that is offered. the community seems clean and the variety of the menu was appealing. The room was not what we were wanting and she did not want to have a small size room. We would suggest that other families tour this community for the care of a loved one.

My tour of Hickory Park did not go well at all. When I arrived, the lady at the front desk was not there. She came back after a while and said that she also was in charge of bingo. They were very short staffed, so it was not an option for me.

When you first walk into the Gracewood, the memory care unit is the first thing you see. I feel like it gives off an institutional feel that way. The rest of the community felt homelike and it was nice, but I could not get over the vibe that I got when I first walked in.

The aides I met seem to be very good at what they do. Its clean, friendly and I think for the most part take care of my dad very good. But like ALL homes even the best, the staff ratio is poor. Today almost all of the residents are sick with the flu and my dad has not has his diaper changed in assisted living until 4:30. They put it on at 7am. The staff is working with fevers the health dept. should know about his as well as the owner. There is no nurse there on weekends which is a sin and not appropriate. So whoever owns this needs to spend more money and have more help available on weekends and the whole home is in a crisis and sick. [Removed] social worker

My mom stayed here for a short time, she wanted to be home. The staff did their best to get her comfortable and accommodated her needs as much as possible. There were a little bit of activities that she tried out. The care and safeness was provided during the time.

I had a tour of this community that did go well. I got to see a room that was available. I noticed that is was close to a door that leads to outside and she had a balcony that was outside too and because it was on the lower level I felt that my mom could wander off. I also felt there was not enough staff for the residents and I did not feel right moving my mom into this community. The meals are planned out with no alternatives.

Mom did not like it here, they gave grilled cheese sandwich and soup all the time, and dessert was either canned peaches or another canned fruit. Staff was friendly and the facility was well kept. More staff and a better menu needs to be improved.

Not a bad place at all. Overall they seem to try hard to provide quality care, most of the staff is friendly and seem to be fairly patient. I believe that if HC offered better salaries and more of an organized structure with each shift having supervisors or managers present things would run much better. This would allow this HC to attract more qualified and dedicated staff members. The activities area really needs help. Finding a variety of appropriate activities would be nice. The field trips in the warmer months were great but during the winter, there was nothing much happening. Overall it's okay.

Hickory Park has done an outstanding job in resolving a challenging situation with my mother. They were able to quickly assess and accept her at their facility after a very harrowing experience with another senior living facility.

We don't use any care services, except for housekeeping. All of the residents have buttons they can press if they need anything, and from our experience, the response time is extremely slow. Based on our experience, she would have to wait a considerable amount of time for someone to come and escort her to the dining room, or other things of that nature. There are activities, such as BINGO, etc. Once a week, they cut out coupons to send to the troops. The activities are social, but they seem a bit mindless. However, they do offer trips to Wal-Mart, and other destinations in the community. They are able to adapt to the level of care that we were looking for, and with that, the price is extremely reasonable. The other places in the area charged much more for services we didn't need, so the price made the most sense for our situation.

I visited the Hickory Park community and found it to be very nice for what I was looking for in living. Everything about it from the staff to the care needs was of comfort to me. The only downside to the community was the price i was willing to pay, I just felt it to be out of my price range.

They never went out of their way. She fell a lot while she was there. Not a great experience.

I am not happy with them. They are not truly a memory care facility for Alzheimer's dementia. She kept getting out the door and they couldn't handle her angry outbursts. They called us constantly requesting us to come and get her. The courtyard is insisting we still pay for the month of January because we have to give a 30 day notice even though she is no longer there. I don't feel we should have to pay due to them not being able to handle her.

They were very accommodating regarding getting my husband in on short notice. They were not equipped to handle his night wanderings or his aggressive behavior. In the six weeks he was there he was sent out to the ER three times only to be released back to them after some testing. I had to hire someone to be with him overnight which was very expensive. Not the right place for my husband who needs constant supervision.

Staff are the best. Everyone goes out of their way to help you. Activities are really good too. There is a large variety of things to do. The food is yuck. One hour for each meal is NOT enough.

Very good staffs, but there are not enough of them during night time to monitor the residents, especially the high risk elders.

The facility staff were not always very helpful, but I get they were busy. Admin staff were mediocre. Mom's room and bathroom was not clean. Toilet was disgusting and they said they were changing the carpet, but they did not even vacuum. I complained, but no one did anything about it until Monday following the weekend we moved her there. The facility was clean other than that. There were plenty of activities which was good. Now I'm waiting for a refund for mom's short stay in July. Apparently, it takes 45 days for that which is utterly ridiculous. The cost was comparable to other homes. The facility was nice.

Residents must be independent. Very little support. Especially at night. Night staff sub par. No nurse support and food bad. The staff knew about all of our concerns.

The Courtyard of Centerville review: Nice rooms Nursing staff is very helpful and seem competent. Food is average but may be better than others. My input is based on a couple of years observations. I do realize labor issues of the current environment which makes it even more important to have documented policies. I question the top management experience and the lack of policies and procedures for employees to follow. The lack of defined policies and procedures assures one thing. INCONSISTENCY. I have seen it! Due to the lack of polices and procedures the staff appear to do what they want with apparent little accountability. I am not referring to the nursing professionals. I have examples but will not elaborate here. Focus is on the lack of policies and procedures and the inconsistency due to this. An audit would validate facility operating compliance to policy and procedures.
