I like this community very much. This is a community with excellent staff and are caring to all of the residents. this is a clean community and I like knowing that they take care of the grounds. I like knowing that she is offered to do activites with the other residents. I am happy to know that they work with the residents on being active. I would highly recommend this community for outstanding care for families.

My mother in law likes it here a lot! The facility is very clean and it feels very homey and warm. We also got a chance to share a meal with her and the food was great! Overall, Bellevue stood out to us because it felt like home!

I cannot say enough wonderful things about Autumn Leaves of Crystal Lake. My mother has resided at the memory care facility for the past three years and I am grateful everyday for their exceptional care. The staff is very caring and attentive to every resident, assisting whenever needed. Activities are provided on a regular basis. I feel welcome whenever I visit my mom. As my mother’s dementia progresses, the Autumn Leaves staff and the nurses are always informing me of what needs to be done to continue her treatment. Their communication with family is wonderful, helpful and always knowledgeable. The facility is beautiful and makes residents and their family feel like they are home.

I am very happy with the care that my mom is getting from this great community. The staff is wonderful. We like knowing that the community is clean and maintained.The meals are outstanding and the atmosphere of the dining is great. We like knowing she is active with the other residents. We would recommend this community for the care of a loved one.

Loved this place as much as bell gardens and it's a little closer to our home. Even tho it is completely private also they do have an opening available

So far so good for my mom. she seems happy and we are pleased with the move. They are giving updates about my mom, making good meals, and experienced with dementia. I like that they specialize in dementia residents and from what I compared, they provide the most activities specifically for dementia Everything is designed with the idea of a dementia resident living here. I would recommend them to family or friends.

We arranged for our sister-in-law to move in. All of the people we've worked with there have been very helpful and friendly through the entire process. They seem to have quite a few activities to try and keep the residents stimulated. The facility seems quite clean although a little worn. Our sister-in-law's family were her animals. She was able to keep one cat (so far anyway).

We found Fox point through A Place for Mom. They were a tremendous help and offered so many resources and support to my brother and I. I truly appreciated all of their help! We are so happy with Fox Point. The atmosphere here is so friendly and it truly feels like a community. People are always going downstairs to gather at the fireplace and socialize. It was everything we were looking for as far as proximity to family, cleaning services, and overall atmosphere.

The staff is very welcoming and knowledgeable. They were also very caring and understand based on our difficult circumstance. It turned out to be a very smooth transition for us and our family member that moved to the facility. Over the first few weeks I received many updates on our family member. It was wonderful to know how great she was doing. [Removed] are truly wonderful at what they do!

Very caring staff and clean surroundings. I just wished I moved my mom here sooner while she was still able to enjoy all that they offer in activities. The staff is top notch and trained well. Would recommend to anyone wishing to place a loved one with memory problems.

* Bellevue is everthing Mom & I hoped it would be. They even welcome pets! * There are a number of floor plans to choose from, including 2 bedroom-2 bath, 2 bedroom-1 bath, 1 bedroom-1 bath, & studio. * Residents who transfer from independent to assisted status may stay in the same unit. * Bellevue assigns volunteer resident "ambassadors" to new residents, who take them to meals & activities & introduce them to others. * The monthly fee is quite high, but it includes everything, although that might not be ideal for folks who still want to do their own cooking. * Guests may dine for $6 per meal, which is added to the monthly bill. * It is a large community. Mom has to walk quite a ways down hallways to get to elevator, dining room, mail box, front desk, laundry, storage unit. In her case the walking is good for her. * Activities include 30-minute daily chair exercises which are excellent.

We are happy with the care or my dad. The community is clean and the staff is great. He is please with the menu and the variety. He is offered to do allot of activities that are offered to him. We would recommend the care and value of this community to other families.

We had a wonderful tour, they went through all the details and this is a beautiful facility.

Mom stayed here while she was recovering and needed some extra time before returning to her community. The staff were excellent. Mom did not like the food, however since she was in the memory care they did not offer a large variety of options. They offered a lot of activities and encouraged the residents to socialize. They have a central courtyard in this lockdown unit for the residents to get out and enjoy.

Our family is stilled amazed that we were able to find such an fabulous home for our parents. We have 5 siblings in 5 different states and with the help of "A Place for Mom" were were able to assess our options in Green Bay, WI from various states. Our parents are very happy in their new home at Bellevue.

i really liked the community when I toured. Every individual staff here are very good, it was nice meeting them. This facility was my number 2 choice. The room I saw was suitable with a private shower. The staff are skilled and they had activities available for the resident to enjoy.

This community is outstanding in all ways. the food is good. The activities are wonderful and helpful. I would highly recommend this community to other families for great care for a loved one.

My husband likes the care given here. The staff is very nice and helpful. the community is nice, but needs a little more care. Activities are always offered. We would recommend this for other families.

There is parking available for the people that reside there. This was one of many amenities offered by the community. There are meal plans available, as well as various activities. The tour guide showed me all around the bedrooms, the dinning area, and activities room. I thought the guide was very informative and answered my questions.
