The change in our mom since she has moved here is Wonderful! She is making friends and she is enjoying herself she is even becoming a real "Cheesehead" and wears her Greenbay Packers jersey. The staff is great, they are friendly, and they show that they care about the residents by their actions. They make sure mom takes part in the activities, special events and she enjoys coming to the dining room and is eating very well. Mom likes her room, the community is bright, open and clean. We're just so very happy with the change we see in her.

My dad is doing so much better, this was the right move and the perfect community for him. I am very thankful, he was suffering from dementia and he was acting out. There were very few places that take a dementia resident who acts out. They were the only one who would take him in. Their staff are trained specifically for dementia. The set up is very calming and excellent for dementia residents, it is less confusing than larger facilities. it is also peaceful and it has really, really made a difference for him. He can go outside when ever he wants. There is an enclosed garden area where they can sit or walk around. It in the shape of an octagon with flowers and shrubbery to enjoy. I am really grateful for A Place For Mom, I was at wits end on where to place him and their help was a huge relief that I appreciate.

I am comfortable and confident with my mom being here and she is doing well. Since she is in a wheelchair, it is very easy for her to get around. The set up is great and open all through the community. She is eating the meals and socializing in the dining room. She was barely eating at her last community.

We like this community but was not as nice as we wanted. The staff was nice and they have a lot to offer. The menu and the activities would have been offered. We would of like this community but it was more than what we could do at the time. We would recommend others to this community for the care of a loved one.

I’m a family member of a new resident, My search began in 2016 looking for a home for my wife. I visited many locations. My search ended the day I stepped into Autumn Leaves of Crystal Lake. I met [Removed] from sales that day. But that visit was not about sales. It was about the care for my wife that was explained to me on what they could provide. Then we toured the facility where I met [Removed]. Both of them took time to talk with me. Where other visits this did not happen. I had a service help me with my finding a home so I didn’t have to take notes. All I did was observe the employees and facilities. I was amazed at the care for the residents that was given out. The happy employees were everywhere. That day I went home and knew where I wanted to place my wife for her care. Today three months later and I’m amazed at the change in her. She is more talkative, active now, then what she was doing at home. The employees give her the time of day when asked. No door is closed. I know she is slowly slipping away from us. However I cannot ask for any more wonderful and caring place for her to be at. To [Removed] and the entire staff at Autumn Leaves Crystal Lake, my family wants to thank you for the love you give my wife. Your family is our family. We were very impressed with this community, the amenities that they offered their residents and the staff did a fantastic job on our tour, they took their time showing us around, and they answered all our questions.

Very nice. ..a bit far and pricey. Would recommend for someone else. [Removed] was a pleasure to speak with. The gift package at end of tour was a nice touch.

We are happy with the care from this community. The staff is helpful to the needs of my wife. I like knowing she likes the menu. We like the cleanliness of the community. The staff offers alot of activities.We would recommend this community to other families for the good care of a loved one.

When I walked in Hickory Park, I thought WOW, it's just like a hotel. I have been living here now for almost 2 Years and it feels just like home to me. I like how it feels safe with the 24 hour Caregivers. There are alot of activities to do and everyone is really nice!

This is a great community for care. The staff is wonderful and caring. I like knowing that she is in a clean community with a nice environment. The menu is very good with a lot of variety. They have great activities that she enjoys with the other residents. We would recommend this community for the care of a loved one.

After reviewing four facilities, Autumn Leaves in Bolingbrook, IL stood above the others hands down for our Mom's Care. Not only do you read about the Alzheimer's/ dementia patients care but you see it first hand. I knew this was the best residence for Mom. Everyone participates in the residents' care! There is a refreshing attitude among the staff, that is not found unfortunately in rehabs, hospitals & nursing homes. You will witness the administrative staff calling Bingo numbers, walking a resident to his/her room, or taking time to visit with the residents. Their focus is spelled RESIDENTS. I also am impressed by the caring & compassionate nursing & CNA staff. They are cheerful & uplifting as they earnestly take time with each resident. This residence is very clean both in the nicely appointed TV rooms, residents' rooms, protected patios, & dining rooms. It's common to see the continuous maintenance throughout the facility. It stays new because everyone cares about the building that is "home" to a wide variety of people with dementia/ Alzheimer's. I noticed one very important practice that puts a smile in my heart - the residents are engaged daily. They are not sitting in their rooms, or sitting around the nurse station in wheelchairs staring into space! There are activities designed to helping them be alive as best they can. Whether it's listening to a volunteer play on the gorgeous grand piano their favorite songs of the 30s, 40s or 50s, singing along with the music they know well, or tapping their feet to the rhythm of the music, these ladies & gentleman have a smile on their face because they remember those good times through music. That is a beautiful gift! The residents are well fed. One day, my Mom was asked by the Director of Sales if she slept well. She in turn answered yes & asked him if he was going to make her pancakes. Well, the very next morning the chef made Mom pancakes. Now, that is caring & teamwork among staff. I have to say, I look forward to my visits with Mom. It's truly time for us to sit out in the gorgeous patio & talk about what's happening in life. When I leave to go home, I'm happy that I visited Autumn Leaves & chose this as "home" for Mom.

This is a nice community. We like that she is happy and the community is clean and maintained at all time. We like knowing that she is very happy with the menu. They offer her a lot of activities that she is active with the other residents. We would recommend this community for the great care for a loved one.

Outstanding care facility. They understand, the needs and care of their clients. They have been doing upgrades to buildings infrastructure. We are fortunate to have our loved one being a guest at Harbor Chase. Their caregivers and support people are excellent. Pricing is normal for a memory care facility. Thank you Harbor Chase.

My brother and I were very impressed with Fox Point the first time we went there to check it out for our sister who has Alzheimers Disease. We moved her in on July 1st. 2017. She is doing well there so far and we are pleased with our decision to move her to Fox Point. I would recommend Fox Point to anyone who is looking for a nice place for their loved one.

I didn't like the community when I went to see it. I was not comfortable. This is not a place for my loved on to live in.

I like the care of this nice community. We like the staff and they always keep me informed on her progress or setbacks. I like the variety of the good meals with a nice variety. It makes me good knowing that she is in a home feeling. We highly recommend this care to other families wanting a nice community for the care of a loved one.

I like this community very much. This is a community with excellent staff and are caring to all of the residents. this is a clean community and I like knowing that they take care of the grounds. I like knowing that she is offered to do activites with the other residents. I am happy to know that they work with the residents on being active. I would highly recommend this community for outstanding care for families.

My mother in law likes it here a lot! The facility is very clean and it feels very homey and warm. We also got a chance to share a meal with her and the food was great! Overall, Bellevue stood out to us because it felt like home!

I cannot say enough wonderful things about Autumn Leaves of Crystal Lake. My mother has resided at the memory care facility for the past three years and I am grateful everyday for their exceptional care. The staff is very caring and attentive to every resident, assisting whenever needed. Activities are provided on a regular basis. I feel welcome whenever I visit my mom. As my mother’s dementia progresses, the Autumn Leaves staff and the nurses are always informing me of what needs to be done to continue her treatment. Their communication with family is wonderful, helpful and always knowledgeable. The facility is beautiful and makes residents and their family feel like they are home.
