I had a pretty decent tour of this community. There was construction going on at the moment. So the community was not the best looking at the time, but everything else looked nice. The staff was kind to my family, and myself. It was a good experience.

Met the Director and she was very organized, easy to talk to. She provided lots of information, great tour. Although a more formal institution for alz patients, it seemed clean and well run. For this kind of mid-sized facility, I liked it.

I was visiting my loved one last week and I primarily come in the mornings when I do come, but I decided to come in the evening because I was attending an event that morning. Long story short I was searching for somebody for over an hour. I needed assistance helping my loved ones in the bed. I recently had surgery so I was unable to do it. I finally saw a little girl come from the one of the offices down where my loved one is so I asked her if she knew where everybody was at. She told me she would go get her mom. About 20 minutes later I was approached by a caregiver [name removed]. I told her I just needed help laying my loved one down and she told me that she will go get the caregiver that’s assigned to my loved one. It roughly took around 30 minutes when a young lady came to help. After I said my goodbye to my loved one I walked into the activity room to see if my loved ones jacket was in there because I was gonna take it home to wash…TO BE CONTINUED

Staff turnover is astonishing here, much worse than many other area facilities that I have investigated. There are rarely enough care staff to sufficiently cover all shifts. This means care staff is regularly overworked. They can't keep a housekeeper or maintenance person, leading to long periods without these roles staffed. So, the place does not get properly cleaned, let alone resident rooms. The staff turnover problem alone makes one wonder about the management here. Food is horrible. They buy all food from Gordon Food Service, so we're talking about prepackaged food that just gets warmed up. Deli meat sandwiches with potato chips are served for dinner 3-4 times per week. Everything else just looks tasteless and unappetizing. They claim "homemade meals" in their brochures and the PR person will even lead people to believe that, but it is completely false. My mother was in this facility pre Covid and we couldn't have been happier with her care. But since the pandemic ended the staff turnover has left the care unacceptable for our family. Our mother was left in a wet bed so often we had to change her mattress and when we moved her the sheets smelled of urine and there was a dirty diaper under her bed. There is no art or music therapy and the new staff try hard but are in need of training on working with those suffering from dementia. We tried to get a care plan for our mother for over a month with no success.

They sure can sell this place, but don’t let the beautiful website and smooth talking draw you in. I was excited to move my dad in only to find myself in a nightmare. Didn’t get an actual key to his room for several months, so he had to leave the door open because he could never find anyone to unlock his door for him to get back in. Other residents wandered in because there was not enough staff to monitor them or they were all out on a smoke break at the same time. When other residents wandered in his room many things came up missing and even caught others urinating on the carpet and it never even got cleaned up. We have this on camera. His room was filthy and soiled sheets left on the bed instead of being changed. Dad had an accident and caregiver saw his soiled pants and she just walked away. She couldn't be bothered to help him or left him that way for someone else. They told us he hallucinated seeing bugs and ants. We have pics of bugs. Staff tormented him for their amusement.Cruel

My aunt lived here. My wife and I came to take her to lunch -We complained because room was soiled/dirty/dusty. My sister and I are guardians of our aunt. My sister complained that day also - 8 hrs later and it was still soiled and dusty and it STILL wasn't cleaned 2 weeks later. Check her google review! [name removed]. They responded to her review but they took 2 months to email her back and that was only because the State Board of Health came out to investigate that day on her complaint. Stolen items aren't investigated- they claim "alleged rings" when we have documentation of our aunt having jewelry in the facility. They will retaliate against the patient if you complain/file a grievance. They told a visiting nurse -"Don't listen to ***, she's always drama." This nurse was doing an exam on our aunt because we were moving her -so this is the experience that the new facility received at Grand Brook. Not a great impression for other facilities-and their insults of their residents. RUN RUN RUN

Facility filthy and no one answers the phone. Laundry running over no staff in all departments. Smells bad ,missing door knows and stains on furniture. Low food choices. No activities. My family member was attacked by another resident no one knew nothing or seen it. No trained staff. Floors and carpet dirty. Residents in too little clothes or too big. I was disappointed visting.

Don’t let loved ones stay Grand Brook (GB) Look at my Google Review and pics! Beautiful facility horrible care. My aunt was here almost 4 years.The food is NOT home cooked. Frozen food. I never saw fresh salad or veg. Snacks aren’t healthy. We had Dept of Health help get us medical info-why our aunt fell 35 times in 3 years.Half with injuries. GB gave NO details of fall/injury. 3rd party told me of wound care order. [date removed], my family went to get our aunt & the bed soiled/dirty diapers on floor. They reported it & when they returned at 3pm, nothing cleaned. I went at 5:30 pm same day-nothing cleaned. Notified Dir Nurses and Exec. Dir. We wrote "Clean Me" in the dust. It was still there on ][date removed] 2 weeks later.I never saw physical exercise!They went to see Christmas Lights BUT they "randomly" chose residents-not all were allowed. When my uncle got sick/died my aunt had no advocate from [date removed] since GB never contacted her emergency contact designated by my uncle!RUN!

While the location looks great, pay attention to the staff and level of care. The director ignored residents asking for help, stating she would get someone to assist them but didn't. Residents were left alone in common areas with no staff to engage with. They were clearly bored even though the activity schedule stated they were suppose to be having an activity. Food was left out in the bistro area from breakfast or lunch even though it was past those meal times.

My mother was living at autumn leaves until April 2020. Yes, [name removed] was the Director at that time. During the lockdown I was told she had a hip fracture and needed a hospital bed and pain meds. 24 hours later she was pronounced dead - and ALL OF HER JEWELRY HAD BEEN STOLEN! Neither [name removed] NOR any of the "family" owners can say what happened - Even though only employees could enter! Three months later - NOTHING! What happened to the investigation you promised [name removed]?

I would never bring my father back to their facility. He were there for 1.5 years and I saw the decline in his health. [Name removed] is a horrible director. She tries to make herself sound as though she is educated, caring, responsible and has the resident best interest, but fails in every way. My father was over medicated to make their lives easier on many occasions. I have seen so many individuals there decline in health and a few pass away. When I removed my father from there, I had called 1 month in advance to give them notice. [Name removed] made it very difficult, and charged my father for the month that he left. When I called to make sure they were not going to charge me for another month, [name removed] said she was because I did not give them a 30 day notice. She is very hard to work with and tries to come across as a good person. She accused me of cursing at her when I used the word "[profanity removed]" . I told her she needs to get her [profanity removed] together due to the fact, that she asked me to submit a copy of my POA for the 3rd time. I guess her virgin ears were offended. Give me a break! There are good people working for them that due care, but the top person in charge is her and honestly in my opinion she is not qualified.

We filed a complaint with the state and they gave us our money back after just two weeks. They almost killed my mother and several times the thermostat would be on 62 or 59 degrees. They were ugly to my sister and demanding. She had severe diarrea within a week of getting there and this was during the time they asked us not to visit for at least 2 weeks so mom could get acclimated. If they hadn't called us on day 8 she would have been dead. We then got her in the hospital where she remained for 8 days and now in a nursing. She never has fully recovered. I would never recommend this place and I was not the only one filing a state complaint, 2 other people filed complaints that I know, also around the same time I did.
