Near family's work. Every apt is asst'd living. Many levels of help as needed. Studio, one-bedroom, two bedroom. All-inclusive pricing--all personal care, meals, housekeeping weekly, [all]laundry, xport to dr, shopping, outings, personal care svcs (med mgmt, bathing, grooming, acts of daily living), escorts as needed. On-site therapy, as ordered. Med Director on site. Studio $2400 w/sm kitchenette, One-bedroom 3000, two-bedroom 3400. Built in 1987 Drees. Private Owners One bedroom avbl now--VA OK.

I can't thank you enough for all your help with my mother's move to apartment 4309. I'm happy to say that the move out is complete. [name removed], her caregiver has been especially kind to Mom through this process. This was a tough transition for Mom after Dad's death, but she is so happy in her new place and credits it all to you and your gentle guidance. To say that she is happy at Mallard Cove is an understatement, I will travel back to NJ content that my mother is in good hands. The maintenance staff was wonderful and with the exception of a minor repair, everything has been taken care of - hats off to [name removed] and his very professional crew! Kudos also to [name removed] at the front desk for all her assistance with conveying important information to Mom, and coordinating/disinfecting all the online purchases which arrived. She does a great job of making sure all the residents here are safe. Please take good care and on behalf of my whole family - thank you so much!"

People that really care about their residents

I moved my wife in Autumn Leaves 8 months ago. Since that time I have seen my wife decline and need more and more assistance. Before COVID I visited my wife every day and we had lunch together, anytime I asked a nurse for assistance someone was there to help. The staff is very attentive and eager to please. The facility is clean and always smells fresh. I would recommend Autumn Leaves in Tulsa to anyone needing to move a love one into a memory care.

The Best

I love Autumn Leaves Tulsa and the care they give my AUNT [name removed]! From the minute she was placed there it has been comforting. She has been treated kind and with respect. The workers have done a great job and the facility is clean, calm and always having some kind of activity with the patients. Its a safe place for the people who need help. I can't say enough good about Tulsa Autumn Leaves staff. Way to go! The [name removed] family!

Fully Recommend Mallard Cove

My Dad and his wife have lived in the facility for 3 months now. The staff has taken great care of my father on a day to day basis, and when special assistance is needed. The staff are always very friendly and accommodating. If the staff identify a personal issue with either my Dad or his wife, they share the information with the spouse so they can work together to resolve the issue. The cafeteria staff are friendly and consientious of their meal needs. Both residents have special needs for their meals. The staff work to provide options that will allow them to have a healthy diet while not feeling like they are missing out on the foods they really enjoy. One other aspect that I've truly appreciated is that the facility works to meet the needs for my dad, which are part of the monthly rent, not an additional ale carte charge. One last comment, the staff really want to make sure the residents are engaged. They actively include them in events that are scheduled, make sure they are aware. They are also open to new activity suggestions. My dad and his wife have really acclimated well to this new environment and have become active members of the community. Mallard Cove, thanks for taking such great care of my dad!

Autumn leaves was a great environment for a loved one every facility has turnover and keep in mind these are Alzheimer's and dementia patients some things get broken and every resident has the right to refuse care... I'm not taking up for the facility but you do have to look at things from a bigger picture and really things that need to be dry cleaned like real leather shouldn't be there but to call caregivers stupid is uncalled for.... I as a past family member think some things can be better but the love and caring you cAnt get in a lot of places keep up the hard work ladies and gentlemen....

Top notch memory care!

Autumn Leaves is great community if you are looking for memory care for a loved one. My father has been here for almost a year now. The office staff is friendly and will address any concerns you have. There is some turnover in caregivers as to be expected in this industry but the important thing is that they bring in Agency staffing so that they are not short handed. Top Notch community when looking for memory care!

Needing a 2nd home for a loved one

Chapters Living of Tulsa is a smaller facility than most, only 40+ residents. The present admin & mkt person are working to improve the location and esthetics with repairs, painting, new furniture, etc. since taking over the building a relatively short time ago. The floor staff is great and caring. They are actively looking for an activities director. The building is clean and no hint of "odors".

Well, this is my first experience, nothing to compare. I have been impressed so far. All the ladies are very attentive to the patients. Usually very clean. She is in a shared room and has plenty of space. I don't know about activities.

Friendly Staff

I think the staff is wonderful very friendly. The food could be warmer although people who have been here longer than me say it is much improved. I enjoy many of the activities but might be to early to judge. Their staff is definitely their strong suit, they really care about their residents from top boss to housekeeping and kitchen help everyone is focused on the residents experience

There were good discounts offered. They had the services I needed available, some at an extra cost. The facility was tidy and adequate, but not fully renovated. The one-bedroom apartment was smaller than expected, only a little over 300 square feet. This was the first place I have toured; therefore, at this time, I don't have others to compare it with.

Review for Chapters Living of Alpharetta

Addington Place is now Chapters Senior Living. My sister is very happy there although she is not very thrilled with the food . She enjoys most of the activities. She has never been one to do crafts so she only does a few of the craft activities but she likes the games and physical activities and she enjoys the other residents.

Exceptional staff

At first you might think [name removed] is overselling the place, but they really do have an excellent facility and staff. All of the staff I have met have been very friendly and helpful. The residents are clean, neatly dressed, and for the most part seem happy with their surroundings. Generally speaking the level of care has been great, including their resident PT observing Mom in passing over the first month and acting to order her some beneficial OT. Cleanliness and condition of the facility is way above what I was expecting, especially for a building that is nearly 40 years old. The fact that they were licensed for 120+ beds in the upcoming skilled nursing addition and they decided to cap it at 76 beds speaks to their level of concern for quality care. I am looking forward to warmer months so that Mom and I can explore their outdoor spaces on the grounds.

Experience with the 2 Women Directors

My Aunt was a resident at the Chapters of memory Care Facility for four years. The facility was very clean very nice and all of the caregivers at the facility where most caring and empathetic to my aunt's situation. There were several instances that her belongings would not be there or would be in or on another resident even though her name was clearly written on everything we brought her . I am very disappointed in the actions of the 2 women that run the facility. My Aunt passed away very recently and I would have thought as we gathered her things that an empathetic word or two may have sputtered out of their mouths. As to my dismay, there was nothing and I mean not one word spoke to my family , my uncle especially . One women was more interested in her phone and ignored the entire situation. It is very upsetting to see that these women have become complacent in their jobs. Not even a "Sorry for your loss" was uttered from their mouths.
