Do not put a loved one here!

This place is terrible! We had hoped it would be a good place for our mom, but the care she received in just two weeks was horrendous! The staff (even the head LPN) was caught in multiple lies about bruises and injuries we discovered. Staff prefers to medicate residents, so they are easy to handle. We moved her out after two weeks because we honestly feared for her safety. The food is disgusting and the staff doesn’t care If residents eat. Residents weren’t allowed to have butter knives and the food wasn’t even served in bite sizes, so how are they supposed to cut their food? We saw residents inappropriately groping other residents with staff no where to be found (even though there are cameras). The layout is a bit of a maze and saw multiple residents fall. We had to inform staff because no one was around. We were also guaranteed a refund because of the multiple incidents that occurred, and it’s been almost a year & continue to get the runaround. Please don’t put your relative here!

This place is terrible. We hoped this place would be wonderful for our mom, but we were horrified with the care. We moved her out after barely two weeks because her safety was at risk. The staff was caught in multiple lies about bruises and falls that occurred. The food was not edible. Staff did not care whether residents ate their food (no butter knives are provided, so residents can’t cut their food.) We witnessed residents inappropriately groping other residents. The layout is a maze and residents get confused. We saw many residents fall and we had to inform staff even though they are supposed to be watching cameras. We were guaranteed a refund due to the horrendous care & it’s been impossible to actually receive it. We continue to get the runaround. Do not put a loved one here! There are much better communities in Tulsa for a better cost.

While the location looks great, pay attention to the staff and level of care. The director ignored residents asking for help, stating she would get someone to assist them but didn't. Residents were left alone in common areas with no staff to engage with. They were clearly bored even though the activity schedule stated they were suppose to be having an activity. Food was left out in the bistro area from breakfast or lunch even though it was past those meal times.

appalled with management and handling of patients

My husband stayed their and the very first week, they never helped him brush his teeth, never put deodorant on him, he got pneumonia and then put him in the hospital, when he was released from hospital I went to pick up his belongings and 5 tee shirts were missing and 2 pairs of brand new slacks, the. bed was made the exact same way as when I made it the week before ( I dont think he ever slept in his bed) and I talked to nurse,manager, head of nursing home and supervisor over laundry aides and they all told me they would look into the missing clothes issue, its been over a month and not one of these people have returned my calls, and I keep calling them back numerous times with still no calls back and they are checking into it. [removed]......dont bring your loved one to the mingo locaton in tulsa!!!!

The only thing great about this place is the limited staff. Ashley, Donna and Elizabeth. They are the only ones who keep the place going. The new "manager" Julie its totally under qualified. Three people who own it aren't even in healthcare they are in rental property its what the regional director stated. Also there is NOT a nurse on staff at all. I'm happy I removed my loved one from this facility.

Owners could care less about your loved one.

My problem was not with the Tulsa staff but with the Regional and Corporate people. I thought being family owned would be a benefit but it is not, they could care less about your loved one. The food is not good and they do not give the residents enough to eat. In less than 2 years they have had at least 4 executive directors, that should tell you a lot!

Poor Management

As the loved one to a former resident in this communtiy, I can tell you, you need to drive on by and not stop. They are short staffed and when staffed to adequte employee numbers, these employees are from an agency and not familiar with the residents. The Director is unwilling to hear complaints and IF she does hear them they are NOT addressed! I was extremely let down buy most all of the staff with the exception of few CNA's/LPN's. Not worth the cash I paid, and I wouldn't even place a dying animal in that environment!

Run from this place!

I would never bring my father back to their facility. He were there for 1.5 years and I saw the decline in his health. [Name removed] is a horrible director. She tries to make herself sound as though she is educated, caring, responsible and has the resident best interest, but fails in every way. My father was over medicated to make their lives easier on many occasions. I have seen so many individuals there decline in health and a few pass away. When I removed my father from there, I had called 1 month in advance to give them notice. [Name removed] made it very difficult, and charged my father for the month that he left. When I called to make sure they were not going to charge me for another month, [name removed] said she was because I did not give them a 30 day notice. She is very hard to work with and tries to come across as a good person. She accused me of cursing at her when I used the word "[profanity removed]" . I told her she needs to get her [profanity removed] together due to the fact, that she asked me to submit a copy of my POA for the 3rd time. I guess her virgin ears were offended. Give me a break! There are good people working for them that due care, but the top person in charge is her and honestly in my opinion she is not qualified.

BAD Review NB Memory Care

We filed a complaint with the state and they gave us our money back after just two weeks. They almost killed my mother and several times the thermostat would be on 62 or 59 degrees. They were ugly to my sister and demanding. She had severe diarrea within a week of getting there and this was during the time they asked us not to visit for at least 2 weeks so mom could get acclimated. If they hadn't called us on day 8 she would have been dead. We then got her in the hospital where she remained for 8 days and now in a nursing. She never has fully recovered. I would never recommend this place and I was not the only one filing a state complaint, 2 other people filed complaints that I know, also around the same time I did.

this was the worse place I have ever seen in my life I love that pic of the waitress over the couple that is false advertising. those 2 must go and stay at every nursing home their pics are in so many the staff well 90 % is a joke they give RESIDENTS peanut butter sandwiches to watch after other residents so they can do other things like nap and yes I mean nap. they do have great people to come in to perform but I wouldn't put my dog in there when I went on vacation. they had bed bugs in there because it was not kept up .

I get a kick out of the picture they show with woman leaning over the couple in suit and dress. there is not any that dress like that for meals. The meals are not elegant simply a meal you would see in a military kitchen such as grub, they do not watch after the residents but instead have them watch after each other . if you have pains it takes a while to get your pain meds or regulars. if you can be there 24 hrs and watch it would be ok but I would not leave anyone there if I couldn't. they do not take actins when medical needs is needed and house dr. is not a good dr.

My mom received HORRIBLE care at Autumn Leaves!!!

My mom received HORRIBLE care at Autumn Leaves!!!!! They did not bathe her!!!! They let her room get like a pig pen!!! They didn't do her laundry anything like we were told they would!!! She had several accidents in the room & as soon as we noticed this we ask the staff to remove her chair & to shampoo the carpet. 4 days still nothing had been done!!!! The sheets on her bed were the most dirty I have ever seen!!! They put the residents to bed no time after the evening meal & their door is locked so that they can't leave their room. If they need anything too bad for them. The string you pull for help never has worked since my mom was placed there. Every time we came to visit my mom she had dirty clothes on & no underwear!!!! We were promised that they would receive help in getting ready for bed & getting dressed in the morning. On a regular basis we would find my mom in the same clothes for several days!!! Also if you visit after 6:00 pm it is almost impossible to find a staff member to let you out the door!!! We have given a 30 day notice & wouldn't recommend anyone to put their loved one in this place!!!

Autumn Leaves - Not a place for Mom

My mother has lived at Autumn Leaves since they opened. At first, her care was good. Our issues of "The Wanderers" wandering into her room and freighting her were addressed though not quickly enough to suit me. Since then the battle has been all up hill. All schedules are set by management. Her laundry is not done on schedule. Her bed is not changed on schedule. Her bed is not changed when it is obviously urine stained. Soiled or wet Depends are left draped over a wastebasket, making the room reek of urine. Her are not properly cared for. Even though I have posted instructions for staff regarding her clothing, I consistently find her leather jackets in the dirty clothes. Her dry clean only suits have been washed - and since several have disappeared - shrunk. Items have been stolen (her emerald and diamond ring disappeared the first week). Things have been broken and either disappears or are left in her chair - broken glass in a chair is not my idea of responsibility. I too thought a family owned and operated business would provide the type of care my Mom needs and deserves. Staff is inadequate and either lazy or just down right stupid. Management has not listed to the litany of complaints I and my two daughters have expressed. The director would take notes and I truly believe trashed them as soon as I left the office. New management has come in. Seems they are doing a lot of back peddling to make amends for their lack of interest in the Tulsa Autumn Leaves. I feel this is way too little way way too late. If I could, I would move my Mother is a heart beat. Since she doesn't tolerate change, that isn't an option. We are paying $6500.00 a month for maybe $2000 worth of services.
