My 90 y/o sister moved in the week before Thanksgiving, so she hasn't been there long. She seems to be doing much better than she was before moving in to Heritage. They make sure she takes her meds properly, and the meals are good and balanced--something she wasn't getting before. She likes the staff & they are so kind to her. The interaction with others has been good for her. It may not be as fancy or roomy as soon of the other places, but many of the frills seen in other assisted living homes would not be of use to her. So far so good. The family is relived to have her in a safe, sound environment.

When I first walked into Autumn Leaves, I did not like what I saw. I had been taking care of my mother for years and I had no experience with memory care units. When I first walked in, there were residents right as you walk through the door. There was someone tapping me, and some going up to my husband. I just feel that they should make it where there is a wall or something so you don't just walk into where all of the residents are.

My mother has complaints, but I haven't seen the things she is complaining about. They have been very good about a lot of things. They were really helpful getting us moved in. I have seen little things, there was a light out in a hallway. I haven't seen any major problems. They are probably a little better than average.

My father lives in the memory unit. The facility is very clean, has lots of natural light, and is attractive. His room has a sliding glass door (which of course can only be opened a few inches) that looks out to the pond and willow tree. There is a walking path around the pond and outdoor courtyards (all secure.) There are many activities throughout the day, including guest musicians and fun exercise classes. The only negative is the food but the assistant director is currently working with the kitchen to improve the situation. I would recommend that the family try to be as involved as possible, especially in the first two months of transition to a memory care unit. Try to be your loved one's advocate, learn the routines, and let the staff know their likes, dislikes, and any concerns.

Overall this is a pretty nice place. It's clean, looks nice, and the staff are all very friendly. They always seem to be a bit understaffed, however, and I feel like my loved one needs a bit more attention than she gets normally. They're always very responsive and happy to help if I point out anything I'd like done, though. I also like the pricing structure, most of the services they provide are simply included in the base price, so you don't have to worry about the price increasing drastically from a la carte services.

I wasn't impressed with the cleanliness of the room my dad was in. My dad struggled with the size of his room. They also seemed understaffed frequently

We've had a few challenges but worked through the issues. Mainly with medical care of my mom due to the pain issues with my mom's leg. The activities are great & mom enjoys participating; food is good and the staff is attentive. Overall we're still happy with our choice. I feel I have to more attentive with what's going on there with mom. I would love for the community to send out a monthly overview of how things are going with Memory Care Residents . . . would be helpful I think for all!

The administrator and her admin are very hateful not only to residents, they are hateful to the family as well. They say they do each person's laundry and never wash it with other residents. That is a lie. My mom is missing clothes and has other people's clothes. My mom complains about the food being awful. They do not do any kind of activity to keep the residents minds active. One finds themselves in a catch 22 with these types of facilities.

New management and ownership. Want to do what is best for tenants.

Family members need to be on attentive to care because staff don't seem to be concerned about it. My mother wasn't eating and the only reason our family knew was because we asked. Staff apparently wasn't worried and never mentioned it to us.

I can only tell you how my mom feels about it. She is in assisted living there and has helpers come in to aid in getting her to the bathroom and back. She calls me complaining many times a week to say that she waits long periods of time before anybody comes. She thinks the food there is pretty good. She never participates in the activities there, so I can't comment on them.

Everyone was friendly. They could have better food.

No matter where we placed my mother the experience of watching her pass away from anesthesia dementia was a nightmare. Some of my ratings will reflect her difficult situation. I felt that my mother was very secure in the memory care unit. There was a lot of staff turnover which I was told was common in that industry. I think that is detrimental to the patients.

It was ok.... some of the staff was great. The man that took over was never involved... night time aids needed more training.

The turnover rate is very high. You can't seem to build a relationship. The payment portal has been out of commission for about 7 months and it made it easier to process a payment. I asked for a yearly statement for tax purposes and I was told they didn't provide one.

We received reasonable care for the price given and the nursing staff were gracious.

Most of the staff has a great disposition and are friendly. But often requests aren’t answered or addressed as quickly as needed. The cottage units are really big, have great screened in porches and some are done up very nicely for but [amount removed] per month. But the food is the biggest detriment. It is often a casserole of some kind or a small sandwich. if you don’t live in the main building and live in the cottages, it is brought down to the clubhouse and thermal coolers which probably adds to it being lackluster.

I don't feel medical care nor medical referrals are adequate. The deep-pile carpet in rooms is an awful idea for reasons on cleanliness. The people are kind.

The location is great. Staff is seriously undertrained. Lack of knowledge about meds they give. We were told things that are just not true. They said nobody ever moves out of this facility once they move in. There were a few moved within the first few weeks due to issues with the staff. There is no accountability for staff members.
