It worked with the budget but now that we moved my mother in law in, we likely would not recommend. They don't have a director and the staff is not attentive.

Do not recommend this facility

This place had good reviews (under previous management) and claimed to be smoking friendly. It is not smoking friendly and they don't have the staff to take residents outside to smoke. There were two changes to management in 6 months, little to no communication between the staff and very poor overall quality of care. On top of the care provided we had one on one visiting angels come most of the time. Without that we would have had to find other arrangements.

I would give the staff category a lower rating if I could. The place was pretty clean. Other than that, I don't have much good to say about the place. Definitely would NOT recommend it! My parents definitely didn't like it

A nice community with room for improvement!

The communication is horrible with the manager. Food is all frozen crap, meat is often tough my dad can't chew it. One night I saw what they brought to his room and it was a small dinner roll, small canned fruit cup and about 1 cup of some type of casserole. My dad is 270 lbs. That wouldn't be enough for him. His room isn't the cleanest. Brought back his laundry all in a heap to someone who is in assisted living.

Mixed Experience at Heather Hills

My father resided at Heather Hills for approximately two years until his passing on November 24. While he found comfort in the meals and safety measures, our family encountered distressing lapses in the promised level of care, leading to significant health consequences. Regrettably, my father's well-being was compromised when he was found lying in bed, surrounded by his own waste, more than once. This neglectful situation likely contributed to a severe UTI and subsequent infection, ultimately leading to his untimely death. We express our deep concern about the lapses in care, and we hope this feedback prompts essential improvements within the facility. While we paid my father's final bill, we cannot endorse Heather Hills due to the grave consequences our family experienced. It is our sincere wish that sharing our experience leads to necessary changes and prevents others from facing similar hardships in the future.

The management is very difficult to deal with. They never get back to you! They always have a turnover in staff. I wish we had taken him to other places to visit but sadly we didn't . Food not great at all. Very institutionalized. He has been enjoying some of the activities. They do try to plug him in.

Has potential

1. Overall experience was a 2. They were constantly understaffed and my mother was ignored most of the day. There was poor communication between the shifts. the cleanliness was good. One staff member (RN) was very friendly. Others didn't speak to me at all. They also didn't engage any residents at all unless it was to deliver food from the kitchen to the table. Quality of care was not what I had hoped for. I was desperate for a short term placement for my mother so I had no choice but I wouldn't leave her there again for long term care. Meals were fine. Weekly events were fine but daily activities were non existent. My mother just sat there with nothing to do. Value for the cost was not acceptable. The cost of dementia care is high for a reason. It's exhausting work. However, for the money that was paid, I expected more attention would be paid to the residents. I was grateful for the last minute ability to place her there but greatly disappointed in the service provided. I am extremely grateful for A Place for Mom for helping me find a residence at Oak Leaf Manor for my mother in such an emergency situation.

Be careful lack of leadership

The exec director was full of excuses and blaming our Gov for his inability to attract and keep good staff. He thinks covid is a hoax spewing delusional rhetoric in our newsletter. I hope the new owners care about us and leave their political agenda behind. I don’t like being used as a pawn for marketing. I would rather have my privacy. If I can’t smoke the exec director shouldn’t be able to either!! Nice places right around the corner from here.

Dozens moved out, decline in quality, poor management

Poor management. High staff turnover. Unable to properly staff. Ask why there are 25 vacant units. There is new entrance and dining area but the halls are dark, with stained carpets & dry wall buckets for roof leaks. Unit carpet is worn out and cheap. Don’t do laundry weekly. Washers & dryers are old and dirty. Mice and ants are rampant. Carports are collapsing with gutters hanging, full of bird nests and poo. Elevators don’t work. The food is bland, not fresh, cold, overcooked, like rubber. Village Care doctor, Nurse Director and assistant are constantly leaving. Meds aren’t given on time and lost. Staff doesn’t respond to call buttons or answer phone. On the weekends & evenings no management is around; it’s obvious there’s nobody in charge. During covid management & staff didn’t wear masks yet required it of visitors. The staff were often exposed residents to COVID. We tried to get these issues addressed by the Director, but were repeatedly lied to and given excuses.

Horrible Experience, Do Not Place Your Loved One at This Sunrise Location

Do not place your loved one here, they will not be properly cared for here. There was a lot of medication issues including lack of the medication that my loved one was to receive, and my loved one didn't receive them. The community's source didn't have it and there were days, my loved one didn't receive the medication. There was lack of communication with the family to notify us of this challenge. There was a lack of transparency by the community as well. The laundry was not getting done regularly in a timely manner. Due to the laundry not getting done in a timely manner, my loved one sat in soiled pants for about a week. There was no linen changes for a month. These items were to be included in the monthly fee the family was paying. There was no follow up on medical care. These requests were made by the family through the family requests. It was a horrible experience for my loved one and the family. We would make the community aware, and they would promise to look into it or follow up on it. However, there was no follow up by the community to the family. There was a nurse change during my loved one being here. The new nurse stated they were made aware of the issues in the community. The new nurse attempted to provide course correction and give proper care, by the time this was being implemented the family decided to move our loved one out of the community.

The marketing director is young , so a tried to overlook some of her comments. even With an appointment for our tour she showed us a room with excrement in the toilet and a dirty old toilet that had been removed sitting in the middle of the room. Nope not the place for my Mom.

My sister has been in this facility for one year. The staff give excellent care. I rated the facility a 2 because of the awful food. The residents have complained for months to no avail. Food is cold, overcooked, chicken last week was burned on the outside & raw inside and baked potato cold. Soup is cold. It is a shame that this facility cannot hire a chef that knows how to cook. What a waste of food that is thrown away.

Little Too Late For Showing Improvement

When you feel the need to remove a loved one from a facility then you know things are bad. Despite the numerous discussions and concerns that have been brought forth things are slowly starting to change for how long hopefully permanently but who knows. There is poor communication between staff issues with medication. We no longer feel that it is in the best interest to keep our loved one there so we are taking them out.

This community is not making us happy at this time. We have had to talk to staff about a few small things, but they do seem to cancel often to bring things to their attention. My father is not pleased with the memu. My father was not able to see a nurse when he was ill. The staff are very nice, but have a hard time with follow up on issues that need to be brought to their attention. At this time I could only suggest that families take a tour of this community for the care of a family member

Lack of communication!

The Autumn House East staff do not communicate with the family when needed in cases such as a fall or any bumps or bruises. When we visit no one seems to go in and check on her. They do offer many activities. My mom has no complaints about the food and eats well.

Before we even moved my mom in they said they would have the room cleaned and cupboard moved, they did not do it so we had to move her in to a dirty room. My mom has been there for a little over a week now, her room still has not been cleaned. I was sitting there visiting one night and they forgot to bring her supper and when I went to them and asked where her supper was, all they did is apologize for not feeding her and never once asked what they can get her to eat. And there was one night she asked the caregiver for a pain pill and the nurse never brought it to her. Starting to get a little concerned about this place and it has only been a week.

I wouldn't have my mother here, the AC is in the hallways but not the rooms. The people that live there, ya, they do get cold but what about when the weather changes? The dining room is a nightmare, if you have any sense of style. Its the gaudiest red carpet you've ever seen in your life and it has an antique-y pattern , its a big big room with a whole bunch of these tables in it, they bring everybody in, no matter how your care needs are so you had a lot of people who needed a lot of help with people that didnt and it was just distracting for the peopel who didn't need help. They don't enjoy eating in that room and I don't blame them. Then in the halllways there are people sitting out there that don't really seem to know what's going on, which is fine, but they should have certain hallways for certain needs patients so that you can communicate with like people and communicate on the same level.
