What we like the most about the community is their caring staff, the visual cleanliness and the sanitary smelling. Yes, we would recommend the Oak Leaf Manor North community to other families.

I was very impressed by the beauty and the friendly atmosphere at first. My loved one was there and, while many of the staff were very friendly and helpful, there is one aide-[name removed]-that is allergic to work. I would walk in to visit my mom and my mom would tell me that she had been waiting to get help to the bathroom over 45 minutes (time and time again)! That aide would sit at the desk or eating or hiding in the nurses station or no where in sight when she was working my mother's section. I complained to the person they told me to go to which was the care coordinator, but to no avail. One of the aides told me she was disgusted with them and was turning in her notice and she was one of the hardworking aides! My mother has passed, but I went in to visit another resident recently and was shocked to find out that those two were still there. I am very disappointed and truly wish that the upper management will eliminate these aides from the building. I regret not going to the higher ups as I feared retaliation. I feel that I have to make this right now and know that the needs of others in the community are being properly cared for.

I am over there a lot to help mom with personal things cause they don't have the help to do it. Always problems with her medicine. They have high turn over of help. Since mom has been there went through so many nurses and med techs. Some residents really need bathing between body odor, the urine smell is bad. It's really sad. Some of the help are not friendly and just are there for a paycheck. You can tell don't even talk to the residents it's like they have blinders on. I don't like being disrespected from there help.

I recently was at Autumn House West and seen baby mice. I over heard employee's talking about bed bugs and how 3 residents keeps getting them in their rooms. I over heard a resident say what nasty child food will they be serving today. The staff walks right pass you and don't speak.

I was very unhappy with the management at this facility. My mother gets not even the minimum care. Med techs are the only people I see when I stop after 6 pm. I have sent letters to both Resident care coordinated and also acting site supervisor since my mother arrived [date removed]. I do not feel there was any attempt to address the issues and have Never been contacted by either of these persons. We are leaving this facility at the end of Oct and would not refer to this facility at this time

If your loved one requires a high level of care and they can't advocate for themselves - 1; If your loved one requires a low to medium care level - 4 My mother developed an ulcer due to sitting in one place while at the Bradford. That ultimately led to her passing. It can be debated whether the ulcer was preventable but it was 100% predictable and they did nothing to prevent it.

My family member was there less than a month. The care was terrible. The aides rarely interacted with the residents. My family member was over medicated with inappropriate drugs. The facility is beautiful but not a good layout for memory care residents. I strongly do not recommend this Memory Care facility.

Our experience was not a good one. I took Harvey out in less than 48 hours. The friendliness of the staff gets a 2 except for the woman who gave the sales pitch, visited our home, and signed us up. The staff gets a 3, but the administrator did follow up quickly after my letter!

This place was not very good--she was only in there for a very short time but the time she was there they weren't very responsive. The ladies in the office were extremely nice though. It was a pretty old facility. We pulled her out in less than a month.

They are having a lot of problems keeping staff. They also do not follow through on requests-such as giving my Dad his Repatha meds for his heart. I'm taking my parents out at the end of the month. I have more insight than other families as I was a life care coordinator. One thing that's not my parents but upsets me is that a resident there clearly has dementia. She needs one on one care. But she's left to roam the halls alone. I watch CNA's ignore her. It makes me very upset. It could be a great place. It's just not.

Owners are not up to the task of providing safe and caring environment. Moved mom out to a better and safer place in summer 2024. This used to be a [community removed] but care went down in 2024 with new owners.

The Exec Director ("Exec Dir") left in [date removed], following many others who have left after private equity bought it in [date removed]. New management (the interim Exec Dir and now the new Exec Dir, have compromised safety and take no ownership when residents are hurt. Although there is some incredible staff still there, there aren't enough of them left to make this a safe place for mom.

My mother was there for 9 days. Not a good experience I found her sitting in a room w window open and 50 degrees. Shirt soaked through w slobbery. They didnt give her Parkinson's meds for the first 7 days even though they asked us to bring them in. Biggest thing she fell on day two if the month suffered 2 neck fractures. Not one person called to see how she made out. They wanted their money for the month though. They made out on that deal. Would not recommend.

If we’re paying you to care for our elderly, then why did we have to come in 3 times a week and bathe our grandma whose basic hygiene wasn’t being watched by the staff? This was a horrible experience and I will make sure that everyone within York County is aware. Moldy food left to set around her, inconsistent room check ups. Things that should never happen at that price point (or ever) lol.

The biggest concern that we had while our loved one was at Sunrise of Rochester, was that they were giving our loved one 5 times the amount of medication dosage for months and months. Our loved one had lost multiple items and clothing that they were never able to find. When our loved one had covid they kept her sedated to keep her in her room. The staff were amazing but the upper management was just atrocious .

Post-Covid they did not have enough help. The people who were there were good, but there were not enough of them by far. When my mother would go to the doctor, I would meet her there. The day she had her last stroke, the man in charge of transportation at Autumn House called me and said he didn't think they should send her to her appointment because she was sick and throwing up. So I went straight to Autumn House and found her in her wheelchair in her room, with vomit all around her, with no one around. She had had a second stroke, and they just left her be. Her necklace never worked from the day she went in to the day she died. Anytime I would push the call button for her, nobody came. They wanted their money, but they did not have enough staff. We had a very bad experience, and I believe my mother could have lived longer had they provided more help.

We moved my loved one in a few months ago and the day we moved him in we had lunch and the lunch was fabulous. Within 2 days of him being here the chef was not here anymore and the food quickly went downhill. He was getting cold meals delivered to him with no condiments. One of the reason we moved him in was because he needed assistance with personal hygiene. We asked for 2 showers a week and for the first 2 weeks they did not even come by. In 2 months he only had 4 showers total. All of the things that they promised in the beginning have not been carried through with. It is a poorly run facility and it has not been a good experience for us.

I moved my mom into the memory care unit in this facility. I had a number of requests that were never resolved about dietary needs. She ended up in the hospital due to what Dr thought was probably just dehydration. That was one of the dietary concerns I had. They failed to get a requested walker after a mild stroke until Hospice did it for me. When covid 19 hit , Gables quickly issued an email letter stating that if a patient tested positive they would be removed from the facility and would be our responsibility until covid free for two weeks. I immediately contacted the state ombudsman and that practice was removed. When I moved her to a new facility i am not an expert but I was amazed at the improved level of care she got there and how quickly her health improved. The hospice care people were very pleased with the care and practices(and privately shared with me how good the new facility care and practices were). She improved enough to be removed from hospice.

What they say you're going to get at the beginning is totally different than what you do get once you move in. My loved one is not mobile, and they were supposed to put her into bed every night. One night they left her up all night, and she then ended up going to the ER because her blood pressure dropped so much, and she became unconscious. The phone is usually right by her, so she called me three times to tell me that they still hadn't put her to bed. So I called and I called and I called, and kept going to voicemail. Finally someone answered and said she would put her to bed right now. Instead of transferring my loved one to the bed, she left her in her wheelchair and laid a pillow on the bed for her to lay her head on, and she moved the phone so my loved one couldn't call me. The paramedics came and got her, took her to the hospital, and they found open skin, like a diaper rash. So Azpira Place was not changing her on the routine they said they were and that they were supposed to be. From the hospital, she went to a rehab center, and now she's home with me with home health care. Because of our experience at Azpira Place, I'm having a hard time trusting putting her anywhere else. Their lack of concern from the director and VP was disappointing. They promised they would call me and they still never have. My initial impression of the facility was that it looked fine. I could smell urine when I would come in sometimes, but things looked clean enough. Certain staff were awesome and would call me on a regular basis. But when the incident happened, it all shifted. And now no one will speak to me, and nobody has called me back.
