Bonaventure

I was not impressed with this community. The assisted living and independent living areas are in separate buildings. Each building has it's own dining room. I would not recommend it if you have both parents that need different care options.

My mother was here for 4 months and I have no complaints about the staff or the facilities... but the management not only lacks follow-thru, but at times short-staffs the residents... leaving one person to get 200 residents ready for a meal or bed. The management also fails to follow-thru with financially critical information if you are using Long-Term-Care insurance to help cover the cost of your stay. I finally had to rent my mother an apartment and hire a 24/7 caregiver because she was not getting the advertised level of care.

Cost more than expected, assisted living service less than expected. Mother only stayed one month.

I moved my brother here in March. Everything was going good until a new manager came to the facility June 1st. He was taken to the hospital emergency room in May which they thought was a stroke, but was a UTI. He was in the hospital/rehab for 20 days. After returning to Oswego Place the new manager decided that his care cost needed to be raised. She called me to say he would need a bed bath and that would be a little more. When I received the July bill, the cost had gone up $2133 per month. This amount was never discussed with me. This should not be allowed without prior notice. The contract I have says that 30 days notice will be given for increases. We have given 30 days notice that he will be moving.

Lots of lots of stink bugs. Had to kill them all the time in the unit. No activities really for Christian community. Lots of alcoholic events. Not a lot of activities for really elderly people.

The manager lied to me about my mom using the f word (my mom has never used that word in her life), she would put on her call button and staff would take a half hour to respond (they were understaffed) there is no one to call after hours (the front door locks at 8:00 pm and is not staffed) the manager was defensive and rude (we filed formal complaints).

Bonaventure has a few major problems. The majority of them stem from the fact that their staff is on a revolving door. Everyone is always new and still training, so care tends to be hit or miss. Staff rarely wear name tags, and they don't bother to introduce themselves, so you never really know who you're talking to. The food quality isn't very good, and the portions are usually very small. The tables and arms of the chairs in the dining room frequently have dried food stuck to them. After a recent snowfall, they waited until the next day to shovel the sidewalks, so residents who own dogs and the ones who smoke had to slog through several inches of snow. There is one gentleman who clearly needs assistance eating. They leave him unattended in the dining room for hours after meals. The new nursing director decided that my dad needed help transferring (he doesn't). She was going to raise his care plan charge $[removed]/month. This wasn't so he could call them anytime he needed help. It was so that IF he needed help during one of the times they happened to be there to check on him, they would. They have a memory care unit. (Imagine how disorienting it is for the residents there to have the staff changing continually.) Because of this, all of the exit doors have locks and alarms. The residents have RFID fobs to open the doors without setting the alarms off. The alarms still get set off regularly. When those alarms are set off, what is supposed to happen is that a staff member promptly goes to check to make sure no one from memory care has managed to get out the door. Those alarms will go off for anywhere from five minutes to over an hour before they get turned off. When they do get turned off, staff usually don't bother to look outside to see if there is someone out there. There is a sign on the door to the nurse's station that says that the door needs to be closed and locked when people leave. It's usually left ajar, and I have seen residents rummaging around in there on more than one occasion. My dad was unable to take a shower for three weeks because the end piece of the water dam had come off, so the bathroom floor would flood if he took a shower. After waiting over two weeks, I finally put the piece that needed to be reglued on the front desk with a note asking, again, for it to be fixed. It was finally done the next day. My dad enjoyed their trips to Cripple Creek. The first one he went on, the van broke down, so this group of elderly people was stuck on the side of the road for hours. The next month, they didn't give my dad a calendar of events, so the trip was over before he knew it was happening. He did go on the third one, but after that, they stopped having them. All of the activity rooms are upstairs. There are two elevators, one in each wing. The one for the assisted living wing was out of order for several weeks. So these people with mobility issues had to either hike to the other end of the building, or try to use the stairs. My dad only lived there for five months. In that time, my overall impression was incompetence and indifference. They're expensive for no good reason. there was no housekeeper for the assisted living wing for a couple of months, so rooms weren't being cleaned unless the family did it. Supposedly, the caregivers were supposed to be helping, but they didn't.

The food is not good. We have gone thru at least 5 chefs since last July when 7 staff all walked out on the same day it often takes 45 minutes to get your food most time deserts is only 3 choices of ice cream this place needs help we have gone though 3 or 4 head extractive in the last 5 months also. Programs they had when we signed up stopped when the staff left in July we miss them we run a lot of activity's our self no staff involved.

The director here promises the world and delivers NOTHING! The food is fair, however, a lot of times they run out of things and you have to have something else. The once a week maid service is just a joke. It is not a great place and they think since we are old we will just take what they dish out.

Difficulty in reaching facility director. Inexperienced caregivers and staff. Understaffed no activities director. Little family support. Have witnessed residents fall in the dining area even when staff was close by. Misplacing residents personal belongings such as hearing aids, glasses and clothing. Laundry service mixes up clothing.

The aides, medtechs, and [name removed], the nurse, were all great and we really appreciated their dedication and kindness. Our experience with directors and the rest of Spring Creek and Bonaventure management would be comical if it weren't such a serious situation. The level of incompetence across the board was staggering. They repeatedly failed to follow through on promises to fix things from accounting/billing, to maintenance, to properly following healthcare requirements. The food was absolutely terrible - at one point I asked the director if she ever ate the food there and she seemed surprised.

My Parents have been residents of Woodland for several months. They do not, and have never gotten the services such as housekeeping and transportation that they signed up for and pay for monthly. There is NO Housekeeping, they have not been able to keep a staff. The food could improve and they run out of basic items during meal times in the dining room daily. The facility is nice looking, but needs major cleaning.

New Director was rude & childish. Staff was not helpful about 75% of the time.

I don't think we knew the right questions to ask. If you can share some of these it could be helpful. No one is prepared to move their mother. How common is it to increase the monthly payment? How long has your staff been working for you? What is the turn over rate? How is your staff trained? Do you document the times you check on a person? On the positive, They have a lot of activities that people could participate in, but my mother didn't...she would get tired. She did enjoy the food. Overall I would not recommend them.

Many issues. I felt like I was doing most of the care and issues were not addressed. They had housekeeping issues as well. My mom was physically assaulted by another resident at Bonaventure and they didn't handle it as well as they could. A med tech was signing off meds that he never gave and eventually was fired after I was able to finally prove that my mom didn't receive them. I could go on and on. Just happy that I have found a safe place for my sweet mom.

The high turnover, and lack of communication between med techs regarding when changes were made to my mom's complex medication regiment and schedule. Also, delivering on promises made to my mom about coordinating and assigning a social worker and transportation needs. My mom has had to endure through potentially dangerous periods of going without medications because they were not refilled on time multiple times, for instance.

At this time I do NOT recommend silver creek Bonaventure It is not managed well, the food is horrible 90% of the time. They are on their 4th month without a shuttle driver. That is something that was to be provided for residents. I can't tell you how many activity directors they have had.

#1 - Many times, the staff would just come in to ask if I needed anything. One time it was at midnight, door was locked, no knock, just used their key. #2 - After our things were moved in by movers, I found a drawer in the bathroom with hair and some belongings as we as prescription drugs belonging to the prior person. Gave them the drugs, they said they would send someone up to clean, never did. #3 - Staff was always friendly. #4 - was only there a short time prior to being hospitalized for 57 days, then I moved to a different community. My husband was in memory care and came up with wearing women's clothing and losing some of his in the process. #5 - Administration did not return my calls or respond to emails. After messages left a second or third time, another member of staff called or emailed. #6 - food was okay, but they were usually out of something, i.e. coffee creamer, ham or bacon. #7 - I wasn't able to participate in activities as I was hospitalized almost the entire time we were renting there. #8 - It was not a horrible place, but if expecting whistles and bells it would never get there. All communities tell you to treat it like home, but you really can't. Each one is different, and I'm in my third community. So far, I truly liked [community name removed]. We left because I wanted to live with my husband in memory care and they don’t have the option. Husband passed two weeks ago so I'm independent living now.

Communication of expectations for family members, keeping memory care rooms clean and organized, staff not making assumptions without confirming. The memory care director was a nightmare to work with. She testified in court falsely.

So disappointed at how fast the turnover rate is for some employees here. They can’t keep an eye on their residents when needed and they don’t feed them delivery and healthy food. Over 1/2 the residents I have encountered have refused to eat, have to have a sandwich for almost every meal, or had to force themselves to eat the food that comes out of the kitchen. Residents are not getting what they pay for and are not being listened to when it comes to comments about such things. I am very disappointed in this
