Bonaventure

The staff truly was friendly. Unfortunately it didn't work for us, and we wanted it to. I'd encourage families to review the state website for any reports and/or complaints about any community they are considering.

There has been something gross on the hallway walls for a while. There have been cards that have been put in a "comment box" several weeks ago and there has not had anything done. The carpet does not ever get vacuumed in the hallway and the furniture in the hallway is dusty. They have one cleaning person for Independent Living to clean all of the apartments on a weekly basis. I have not had my apartment cleaned by staff for two weeks. The bathrooms are dirty. The servers are nice. I feel like the managers, [name removed] and [name removed] are only friendly to certain people. The driver is nice. One concern for the cost is other residents have made me aware that after one year the price has been raised $[removed] or more per month.

It's ok for the basics but for the value/cost I'd continue to look elsewhere.

Food is bad. Staff turnover causes food servers and housekeeping to miss often. I hope other places are better but I'm got nothing but stories to compare to. Not sure I can really expect any to be better.

When my sister and I first walked in, we were met by I very rude and abrupt elderly woman at the front desk. From there we were shown around by a heavy set women that never smiled and it seemed very rehearsed. The facility itself looks rundown on the outside. It has dead plants everywhere and boxes in the front that were maybe a good idea at one point, but someone forgot about them. Our feeling was if they can’t even take care of plants, how are they going to take care of our mom. The prices are very high and overall, I didn’t leave us with a good feeling.

They are amazing when they are recruiting then it all changes. The nurses are very nice. They have a care plan in addition to the rent that goes up all of the time without warning. The nurses are wonderful. The directors make promises but do not follow through. We were told memory care gets the same activities but doesn't seem to be true.

My sister died in [date removed] . She was never happy there but was also depressed about leaving her home. I visited her once this year in March. Some of the rates are my impressions.

Overall, I did not like getting run around to be able to speak to manager on site. Felt like I was being avoided. The cleaning was ok. [name removed] didn't let them in much. Staff was friendly enough. Quality of care is questionable- nurse (name removed) wasn't able to tell difference between anxiety and my mom not being able to breath because she had pneumonia and avian flu after one week there. I had her taken to hospital. She never came back obviously for those reasons. I would say meals were ok, never heard otherwise from her husband. No participation with the activities. Overall I am glad that someone could look after her husband but some other problems seen indicate that the site has other problems as well. Would not recommend the place

Bonnaventure has many facilities in US The Maple Valley Wa. Location is the newest in the Northwest, its a beautiful building that offers Independent, Assisted Living and Memory care living. They offer many amenities for active senior citizens. BEWARE if you want to transfer your loved ones to a Bonnaventure facility closer to you for care. As it appears they aren’t consistent throughout the company Maple Valley seems to want only a certain type of clientele and will deny a transfer for your loved ones from other Bonnaventure facilities. When you want to speak with the Regional Medical Director that makes the decisions, as to why your loved one can’t be at Maple Valley Bonnaventure but can be at another Bonnaventure, with the exact same medical issues, they will just completely blow you off and ignore your emails and calls

Overall I feel the IRIS is a nice place. It was recently renovated and under new mgmt. When you put a loved one in the hands of strangers you don't know what to expect. The fear that they won't care for them like a family member can. Well our 1.5 month experience was short. In that short period there was a big change in personell where there was a new director, new care staff etc and when dealing with elderly with alzheimers you would think that constancy is very important. If you can't have consistency it really affects the patients. There was lack of communication from old director to new director where information was not passed along. We still unresolved issues waiting for docs to be sent to our LTC policy so we can get reimbursed. We got reports from the director that my mother in law was doing well but reports from staff of high BP, crying fits etc. We moved my MIL closer so that we can keep a closer eye on her and check in on her more frequently.

A few things to note. Staff at front desk, RN and not around after 7pm. If anything is needed you have to go find a caretaker which not always can help you. They are not 24/7. Make sure you get an assessment that is thorough. They say they can handle but then call you and say they can't.

After having Mom there for 2 years and most of the time being sent to the hospital, due to medication issues and continual UTI's. Decided to take her out. She's been doing much better, no hospital visits like she was having, meds controlled and no more UTI's. They touted activities, but most of the time was closed down, every now and then something would go on. Food was terrible at the end. There was floaties in the drinks like they weren't cleaning properly. The director stays all day in her office and never returns calls or emails. The under staff had to deal with all the complaints. Only good thing was an employee named [name removed], you could contact him 24-7 and always responded. They are understaffed and too expensive for the service provided. Sad for such a nice place to have unfriendly management.

From an Assisted Living standpoint, they did an intake assessment the evening that my dad came in and they did not follow through with the services that were requested during the intake that included help with showering and getting dressed. Housekeeping was a disaster. They often came later than the day that they were supposed to come in. One week, they did not come at all. They only did his laundry two times during the month that he was there after asking them multiple times to please do his sheets and towels. One of the times they did it, they put it in the machine and then forgot about it and we found his stuff sitting wet, on top of the machine in the laundry room. The food was over cooked under seasoned and the nightly specials were often something like pancakes or a cold sandwich. The front desk was very friendly, but that's about the only good thing I can say.

She got off to a very bad start and they didn't seem to like her. She was left alone all the time. I don't think they cared if she ate. I was there almost every day to help her. We have her Ina much better place now. [community name removed].

My experience is based on the memory care side. They give a very good tour, I will say that. The tour made me think we'd found the right place. Somehow they even managed to make the place not stink like pee when we were touring. Until we moved in and I saw what it was actually like. They claim to be fully staffed but that has not been my experience. Maybe they have enough staff that are on payroll, but that does not mean they're appropriately staffed on the floor for every shift. It takes forever for them to respond to the call buttons, which when I had my tour I asked about that and they told me they had a 2.5 minute response time. I have to help them change/dress my family member every time it's needed because she needs 2 people to do that and they never have 2 people available. But honestly I prefer to be the one to help because the staff seems to have very little training before being left on their own. We're paying a lot for all of the "extras" they say they do but I'm not seeing any of that. They're supposed to be feeding her because she's not able to feed herself but most of the caregivers don't. She's been there almost 3 months and I don't think they've ever maintained her finger nails/toe nails. Stuff goes missing out of her room because other residents take things and I never get them back. I have brought my concerns up to management and have not seen any of the improvements they promised.

We toured North Creek during our search for a Memory Care facility for our loved one. It was disorganized. The staff didn't know the residents. It didn't seem super clean. Our tour guide's appearance wasn't a great impression for an older age group of family members to be working with. I told them multiple times that it wasn't the right fit, but kept getting texts and calls from them anyway.

My father lived at Bonaventure Salmon Creek for 19 months until we had to move him to another facility because they were not honoring the care plan agreed upon. He was in assisted living and was supposed to be notified of meals and made sure he ate. He lost 12 pounds in about 9 weeks. He also fell and broke 2 ribs and had been complaing to caregivers about the pain and the family was never notified. He also cut himself enough shaving that we found the bathroom trash full of bloody tissue and again we were not notifed. We reported this to the Washington Department of Human Services and subsequently they werecited.

While better than some places in the Rogue Valley, need to have family or outside caregiver do additional personal hygiene support and monitor care. Some staff have experience and are trying to care for residents, but staff shortages mean care is inconsistent. This is specifically for the Memory Care unit, but I would suspect applies to Assisted Living as well.

Our experience has been awful since day 1 when our room was not ready when we were told it would be due to it needing new carpet. The temporary room they gave us had not been cleaned and no one showed up to clean it, so I had to do it myself so I could get my loved one moved it. When we moved into her actual room there were bugs on the wall, could not get Wi-Fi to work, could not get TVs to work with their cable, they did not provide an alert bracelet until we asked constantly for a week, the food is awful. They quarantined all residents to their rooms due to a Covid outbreak right after we moved in and they did not offer my loved one food for 24 hours due to them not being prepared for a quarantine and not paying attention to who did and did not get food. I had to take her food and talk to several people to get them to feed her. I could go on all day. Been there for a few months, all the above happened in the first 10 days.

We didn't care for the community due to the tour guide. In addition, there was a foul odor in the memory care unit.
