Avalon Memory Care

Dad ended up as a single occupant in a semi private room, so the room size was perfect for him. Overall, having him here was a good experience. When he went on hospice care, we were able to use our provider of choice, and the staff worked well with the hospice staff. The activities were limited at best. Not at all as advertised. My sister and I made frequent visits to spend time with him, play cards, and celebrate occasions. He was able to receive visitors easily, and with whatever frequency he desired. His bedding was not always his, but it was adequate and he never went without. Warm foods were often cold by the time it was served to him. They were also good at streamlining his dietary needs as required. Generally, the caregivers were respectful, loving and I witnessed them to be good at meeting residents where they were. At the end, Dad had a wound on his ankle, and someone tried to put a compression sock over his big gaping wound. Of all the times for them to keep him in compression socks!!!! Really? I was not pleased that a good number of his care staff did NOT speak English. There were enough that did to get by, but I'm sure Dad was not able to talk with the ones with limited English skills. When he needed to go to the hospital, those transitions were smooth and good. We had to use a transportation service to get him to and from dr. appointments, and that was always smooth. We were able to pay one of the caregivers to go to appointments with him (along with me) which was very helpful.

I would give AVALON on a scale of one to 5 being the best -just below a four- One of the reasons being is, I am having to wash his clothing and bedding to ensure things get back to his room as a set of his sheets were lost for several weeks and his bed seems to be left for weeks without being washed Also, there has been several times when a different residence has been in the common area only in a pull-up when I came to pick up my father and several hours later when I brought him back, they were still in their pull-up with no covering, and it seemed he was left in that position with no modesty. What I do like about AVALON is they seem to be very attentive to his medication's -they seem to serve good meals and are very friendly. Also, his room does not seem to be cleaned on a regular basis, but if I directly approach them, they do get his room cleaned

They have a great staff. I feel he is in the place he needs to be.. I would recommend to others.

The staff who work here have their hearts in the right place!

My cousin[Name Removed] died on [Date Removed]. On the evening of [Date Removed], He choked on his dinner and began to aspirate. There was a A Do-Not-Resuscitate (DNR) order on file for [Name Removed]. Without contacting me, the staff called 9-1-1, performed CPR twice and inserted a breathing tube.[Name Removed] was on his way to the ER before I was called. I understand that this was a stressful situation and a hard call to make. I wish I had been advised.

The people are so kind, and dad has been really happy there. Staying engaged with your person seems to help the personalized attention received. The promised 'schedule' of things is somewhat unreliable, but overall, we're still very happy with our choice. Because residents spend most of their time in the community area, they are not in their rooms much to take phone calls. However, if you call the facility phone, they will take the phone to your loved one so you can connect.

The staff is extremely friendly and helpful but I did notice that when we first moved in many could not speak English I know my dad is a little introverted and possibly no matter what is offered He may not accept but I am not sure about the activities, but they seem to be geared towards those that enjoy coloring and my dad does not enjoy coloring, and possibly playing games and not being able to see well or hear is difficult to join in but I have not got a list of the activities I was told at the beginning that there was a form of pet therapy and that does not seem to be happening at all, which is disappointing The size of his room is a little small, but appropriate I have had to inform the staff several times that his toilet and sink are not being cleaned- and they seem to get the toilet clean, but not his sink- I do know that they have washed my dad's windows several times and I'm very grateful for that, but I do feel that the window facing the field needs to be replaced because the problem seems to be inside of the double pain and is extremely distorted and if someone is going to live there and knowing they spend so much time in their room, they shouldn't have to look out of such distorted windows but be able to enjoy the few things they have, which is looking out into a peaceful field I cannot even imagine trying to keep a memory care facility clean, but I do know that often when I walk the hallways, it does smell of urine I do know that at Brookdale they did use air purifiers Which did seem to help I have had to ask several times about my dad's sheets getting washed - I do think it is getting done now When I have alerted staff to the situations, it seems like they intend to take care of the need Many times it seems like the breaker pops and my dad's lights do not come on in his bathroom- that has happened four or five times since he has moved in -the good thing is he does have a light right outside the bathroom that seems to work, but that should be taken care of They are very attentive when you are needing to enter and exit which I appreciate and they greet myself and my father almost every time ??

The absolute best things are the manager [name removed] and caregiver [name removed]. They are angels on earth. The most amazing, loving, kind and patient caregivers. The facility is also in lovely residential location. They could use more staff like [name removed] and [name removed] - but I know it's hard to find. They could also use a full time cook with a nutritional background to plan meals and handle that on a full time basis.

I liked it was a family owned facility that was smaller sized. They seemed committed to getting the residents up for the day and meals and back to bed in the evening. Liked that they focused on behavior modification to correct undesired behavior. We were concerned that they seemed short of staff

The staff is genuinely interested in the wellbeing of the whole person of their residents. They are helpful, kind and attentive. There are a few people on the care team that have trouble understanding English without some form of special attention. That is concerning.

I think I would like for people to know how caring the caretakers were. They were very friendly and very attentive to the residents. We were very pleased with my brother's care. Hospice came in and took care of his medical issues.

The reason we choose Avalon was the low staff turn over then about 6 months in they started having a hug turnover Loosing your memory makes it really hard for the residents to adjust I hope they have fixed this. I would recommend them if they were able to correct the overturning staff.

Avalon has a very caring staff and is a great community for those needing memory care, but not as well suited for those with low mobility but normal memory.

Overall, we were happy with Avalon's care and would recommend it to others.

I was pleased with the facility/ staff at Avalon. The staff are very caring and try their best to accommodate the patients and family members. The only draw back is the food. Food seems to be made for teenagers and it is not diverse enough or appropriate for the elderly.

A father has been a resident for over a year now. He is very well taken care of by the staff. Our family feels the staff is loving and kind. We wish the facility could have an on site chat for meals so the care takers can focus on care on care only. The meadow Ranch site is on a beautiful serche setting with a tree line view. Feels like home.

The facility is a converted older, one level home on 3 acres so very nice setting. The 2 caregivers per shift work hard to keep residents clean and fed and individualize the care as much as possible. They (the 2 caregivers) are also responsible for all of laundry, cooking all meals, showers for residents, cleaning their rooms as well as rest of house giving and tracking medications. It's too much for 2 people to do.

I chose them because it wasn't too big, my mother could smoke outside. Seemed like a good fit. First time living away from home. I would say the staff is friendly, helps if I need someone to go turn on her TV. I would recommend it. But I don't care for the décor, doesn't give you a homey feeling. Would like to see a table where residents could work on a puzzle or something.

The thing I like most is that the caregivers are all extremely sweet and treat my mom very respectfully. I would like to see better communication between staff, but I will say this has started to improve over the past couple of months.

Staff was friendly and respectful.Shortage of staff and cleanliness could use improvement. Response time to call button was about 30 minutes.
