The staff seem to care. But their hands are tied. If there's an incident, they don't have a nurse there that decides whether or not to send your loved one to the ER or hospital. They would put it on me to come in and make that decision. And it wasn't at all what I expected.

Our loved one has now been at CP over 2.5 years. We used to think we had found the perfect place, but all that changed about 6-8 months ago. There were changes to the top levels of management, and when the "new regime" took over, CP was a very different place. Many of the compassionate, talented professionals who weathered the worst of the pandemic storm, began leaving when the "new regime" came in. Those caregivers who were the most-loved by both residents and family members, were leaving in droves. Then families started removing their loved ones. There was never enough staff on site, and hose that did show up were so-overworked trying to do the jobs of those that didn't show up, they soon became disillusioned and they also left. This is a fairly expensive, private pay facility, and there are many other facilities to consider for similar amounts.

I was not happy with the care that was given to my mom when she was here. They did not have enough help here. They did not have the staff who were trained to help as promised. We had friends that had gone to see my mom and she was still in bed. She had not been taken care of. correctly. I did not like the area that she was in. I do not feel like the staff were trained. I would suggest families consider other options.

This community is not highly recommended. The food is does not seem appealing to my mom. They do not offer her activities to keep active. The community is clean, but she is not happy here at this time.

From the outside it all looks good. Clean and friendly staff but once your family member is in, the communication stops. They make changes to medications and don't keep family informed and doctors don't return phone calls. When mysterious bruises show up they have several different answers of what happened.

My loved one is at this facility. They do NOT like to communicate with family members. Their current DON does not like talking to family and letting them know how their loved one is doing. Their administrator does NOT answer emails, texts or phone messages from family members. There are always lack of care issues on the weekends even though they will tell you these are resolved. Please don't place your family member here. This was not my choice...

Staff does not ensure residents needs are being met as far as hygiene is concerned. This is a memory care facility, residents have dementia and dont know or remember to brush teeth, shave, and shower. I live out of town and everytime I go visit my relative his hygiene is deplorable.

Autumn leaves is a new place and very nice inside. The care is not up to par. They are short on the weekends and at night don't have many caregivers on staff. Also there is no nurse or doctor on staff if an emergency happens. People who do laundry loses it and does not find it. IF you text or place a call you do not get a response. But you can judge for yourself.

I was not impressed with this facility at all. When we went to go tour the rooms didn’t have carpet, the only had overhead lights, and the rooms just felt cold and empty. It is a smaller facility with only 30 people. The woman who gave us our tour was very nice and she really liked to talk. Unfortunately, when we asked questions, she wouldn’t answer clearly and was vague about everything. She just seemed too chatty. The other thing that bothered me was I mentioned to her twice that my Mother had a hard time walking and that she needed a wheelchair. She never once acknowledged it or offered anything. I would not recommend this facility unless things change.

On the tour we were assured of safety for our loved one, 2 snacks a day, daily baths, a doctor &/or PA who visited weekly, total supervision of medicines and dispersement of meds in a timely fashion. What happened in reality, the place was understaffed. Residents were "encouraged" to stay in the common area for ease of control. Rather than the quiet, calm scheduled environs we expected, our parent stayed exhausted from being in the noise, chaos, activity area all day. I saw snacks come out on one occasion, lemonade was the standard beverage. Fluids were not pushed as needed. Urinary Track Infection and dehydration were problems during the stay of our loved one. As for personal hygiene, my parent once went five days without a bath or shampoo. Clothing, linens were lost in the laundry process and never found. Finally, it was three weeks before my parent was "accessed" by the PA. We never met the doctor. The floor nurses were to dispense meds. Usually this occurred as directed, but there were instances when they were given "as needed" rather than as prescribed. One med was not given at all. I was happy overall with the efforts of the staff, however, the administrators were invisible especially director of nursing. Had there been more coordination, supervision, & communication perhaps our experience would have been more positive. The facility is beautiful. The salon, spa, etc are first rate. However, I would not recommend Clarity Pointe, Fayetteville for the reasons stated above.

I would not recommend this memory care facility to anyone. During our experience, the residents room was consistently not kept clean, despite ongoing concerns being raised. Basic housekeeping standards were not met, which is especially troubling in a memory care environment where residents depend entirely on staff for their well-being. The level of care provided was also very inadequate. Communication was poor, staff to residents ratio was well below what was needed and it often felt like residents were not receiving the attention and support they deserved. Families place enormous trust in memory care facilities, and unfortunately, this facility did not meet even reasonable expectations. Even staff members mentioned that I was doing the right thing by placing my family member somewhere else. Overall, our experience was very disappointing, and I would strongly encourage others to carefully consider other options before choosing this facility.

Since the departure of [name removed], [name removed]and [name removed], this place has turned to garbage. I loved this place for my mom before. She had exceptional care from so many and all management and staff knew her and me. Now you walk in and it’s like night and day. There are still some phenomenal staff who should not go unappreciated. These people will go above and beyond. As far as the others - it’s a paycheck and their behavior shows that. They don’t care about the residents or the family members. My Mom was on hospice and her final days there were miserable for her and me. I spent day and night to be with her. The lack of compassion and humanity is overwhelming. The lack of professionalism is mind blowing. “Attitude reflects leadership” and let me tell you from personal experience” I would never recommend this place to anyone and it’s heartbreaking because this place used to be a place of peace for me, knowing my Mom was so well taken care of. It changed. It showed. It’s over now.

Need better system to return resident's own laundry when it comes to the care services. The food is a beautiful presentation, very heavy in starch and sugar, low in protein and vegetables. Value for money-not qualified to rate

Out of the two weeks my husband was there there were only a few nights I didn't receive a call. Sent to the hospital at least four times. They lost him one time. Doctors and social workers at the hospital, advised us to move him. They said he apparently was not being taken care of. For memory care unit staff was not very attentive. Really disappointed in them. Also had to spend extra money for someone to stay with him at night..

When my husband was there the help were mostly untrained and didn't care for him. He was not bathed and bed sheets were dirty. I could go on. That goodness I got outside help until I could mot him. They didn't do anything they promised!!

Some of the girls are very caring but overall please don't send a family member there. Giving wrong or to much medicines. Losing clothes. Staff that should be able to answer your questions cannot. They have excuses for everything. Go through employees like crazy. Hide from you when you want to talk. Contacting corporate they just send complaints to [name removed] and [name removed] (they have NO idea about their job). Half the time have to complain that not getting showers. Smells like urine.

Don't be fooled by the fancy building. Lower level staff were on phones instead of caring for patients. Like a ghost town. I observed one staff member insisting that one resident apologize to another, as if they were trying to teach her a lesson. On another occasion, I asked about a posted activity for residents and the staff looked at me blankly. Food was terrible. Unable and/or unwilling to provide care for more aggressive residents despite advertising that they are. Uncommunicative and rude to family members when questioned.

The facility is newer therefore it may look appealing from the outside but don't be fooled. The main issue with this facility is the lack of quality leadership. [name removed] the director is nonresponsive to family concerns. Phone calls are not returned, requests for meetings go unanswered, any response and communications from her is by email. There is no follow through if she does promise to address concerns. [name removed] the nurse is incompetent. The staff that they do have are hard working and try their best, [name removed] is lazy and has no clue about simple medical issues. Services contractually obligated to provide are often overlooked. Be sure to pay close attention to your bill as they are often incorrect. Most of these facilities are currently understaffed but professional and competent leadership can make a difference, not at this place.

Don’t put your love one here. My mother has not had decent care. She is checked on very little. It is very under staffed. Employees do not stay for any length of time . No one seems to be trained to care for the ones with memory problems. The room itself has no pull cords or buttons of any kind to push incase a patient fell or had problems getting themselves up! The room is clean very seldom. You can ask the same thing for weeks and it still will not be taken care of. Obviously not much communication between staff. And as the previous reviewer said , they do not communicate with family members. If so , it’s only one. I was told they couldn’t contact the whole family of which I thought was very disrespectful of family members concerns of care. The food is absolutely horrible ! I don’t how they can call what’s served a balanced meal. My mother has lost weight while here and is terribly unhappy !
