Emerald Place

Don't be fooled by the fancy building. Lower level staff were on phones instead of caring for patients. Like a ghost town. I observed one staff member insisting that one resident apologize to another, as if they were trying to teach her a lesson. On another occasion, I asked about a posted activity for residents and the staff looked at me blankly. Food was terrible. Unable and/or unwilling to provide care for more aggressive residents despite advertising that they are. Uncommunicative and rude to family members when questioned.

Poor leadership, understaffed, breech of contract

The facility is newer therefore it may look appealing from the outside but don't be fooled. The main issue with this facility is the lack of quality leadership. [name removed] the director is nonresponsive to family concerns. Phone calls are not returned, requests for meetings go unanswered, any response and communications from her is by email. There is no follow through if she does promise to address concerns. [name removed] the nurse is incompetent. The staff that they do have are hard working and try their best, [name removed] is lazy and has no clue about simple medical issues. Services contractually obligated to provide are often overlooked. Be sure to pay close attention to your bill as they are often incorrect. Most of these facilities are currently understaffed but professional and competent leadership can make a difference, not at this place.

Don’t put your love one here. My mother has not had decent care. She is checked on very little. It is very under staffed. Employees do not stay for any length of time . No one seems to be trained to care for the ones with memory problems. The room itself has no pull cords or buttons of any kind to push incase a patient fell or had problems getting themselves up! The room is clean very seldom. You can ask the same thing for weeks and it still will not be taken care of. Obviously not much communication between staff. And as the previous reviewer said , they do not communicate with family members. If so , it’s only one. I was told they couldn’t contact the whole family of which I thought was very disrespectful of family members concerns of care. The food is absolutely horrible ! I don’t how they can call what’s served a balanced meal. My mother has lost weight while here and is terribly unhappy !

If I could give this place a zero I would.

My mom was at Grace Point for 5 days and did nothing but decline. By the 5th day I found her unresponsive and no one could tell me the last time anyone spoke to her. I left her at 8:00pm Thursday night to find her Friday morning at 8:00 am in the same position and non responsive. She then was taken to the hospital and did not return to Grace Point. We took her home the following Tuesday on hospice care. I would not recommend this facility. We thought we were doing a great thing for our mother but soon found out it was not what we were sold when visiting the facility. If you cannot visit your loved ones day and night please don’t bring them here. I would have to show up every morning and get my mom up and dressed for breakfast and return after work to get her to dinner and then to bed. She was taken care of better at home. We had a terrible experience.

They are very friendly and sound very caring and professional, but they do not follow through. They make lots of excuses. We fell for the friendly presentation, but found they do not have the skills to handle dementia patients and resort to extreme levels of medication to keep order. I found 2 pills of my loved ones medication on the floor under the bed along with huge dust bunnies the size of a woman's fist. This was about 6 weeks after move in. The medication is critical to be taken daily. They lost other medications and just stopped giving it to my loved one. I luckily asked and they at least shared honestly.

Please do not allow your loved ones to live here

We admitted both of our parents to Charter this summer. Soon after their admission, that Leadership team left and the facility spent the summer in limbo without clinical leadership or guidance. They relied heavily on agency nursing staff who were poorly and minimally trained on care of persons with dementia. There were so many problems and issues with care, including lack of care coordination, improper response to medical conditions and no communication. New leadership is lacking and these problems still exist. Caregivers are still poorly trained and only a few are truly dedicated to compassionate care. They have also just lost two key staff from Activities and Front Office who REALLY CARED. We have now transferred our parents to another facility where they are receiving the professional, kind and skilled care they deserve. We would not recommend this facility at this time or in the future. They have a very long way to go.

Clueless new leadership

Anywhere but here!

My cousin was a resident at this godforsaken facility. I attended a care plan meeting on her behalf with the Executive Director, Director of Nursing and Regional Nurse. One of our main concerns was engagement, safety and supervision of residents. Clearly Grace Point did nothing to fix the problem as my cousin is now in a hospital being moved via hoyer lift and totally unable to walk. This place is bad, bad, bad news. They lie and cover things up. They don’t do anything right including giving residents the medicine they are prescribed. You have to stay on top of them all day long or your loved one will truly suffer. One nurse even read confidential information from another residents chart to my cousin’s daughter! Please, find a better place for your loved one.

willingness and effort was there, but ability to deliver was lacking

Our experience was not very good at all and the main reason being the lack of staffing at the facility. We felt that they were very understaffed for what they were offering and therefore could not follow through with what they said they could provide. They required our mother-in-law to stay there for a minimum of 10 hours and would only get breaks when we would substitute. We were paying for something that we were not getting which to else just defeated the purpose of having our loved one at a community.

Very disappointed with our experience

I was not pleased with the lack of services provided at Porter Place. They have a bunch of amenities they show you when you go to tour and when she moved in I would go visit and she would be sitting there by herself. The last straw was she told me her feet were hurting, I took her shoes off and they were extremely swollen and she needed to go to the hospital. I took her out of here after [date removed] and they still charged me for the whole month.

Grace Point was very bright and had a warm inviting atmosphere, however during our loved ones stay they were very short staffed which resulted in a lot of turnover. Since there was a lot of turnover there were a lot of inexperience workers there, and the ratio from staff to residents was inadequate. They also said they could take all levels of dementia but that was not the case, and the communication was absolutely terrible.

Don't choose Charter Glen Ellyn

My loved one currently resides at Charter Glen Ellyn. Charter took over April 1st, 2019 and facility has gone downhill since then. Current executive director is very difficult to work with and many staff have quit (including some who came with her from previous employer). Majority of caregivers are agency staff and don't know residents' names or habits. I witnessed an agency nurse on her first day asking RESIDENTS their name in order to give medication. Residents are crammed in TV room during day even though there are many other areas for sitting. We are very dissatisfied with the current level of care and are actively looking for another facility.

This new company CSL took over in May 2019 and no improvements have been seen at this facility. The new staff pushed out all the people that cared for the residents. There is no one to trust with my mom. Need more help for care for the residents. I am very disappointed with the care here at this facility and considering moving my mom out of here. This company has other assisted living facilities and have never had a memory care facility. The learning curve is very slow.

They changed their name, but if you look under Clarity Pointe, Fayetteville, you'll get a much more balanced assessment of this facility. My mother was here, briefly - - and we could not get her away soon enough. It's clean, it's newish, but the management doesn't listen to residents - - and it's woefully understaffed. Lots of mistakes being made in medication, people left untended... You can find a better place. We did.

Porter Place

My mother who has since deceased, spent a total of two months (after paying for four) due to two major falls at this facility. One which caused a broken eye socket, then a subdural hematoma. This due to an obsession by the nursing supervisor of OSHA laws and political correctness in regards to patients having "the right to fall". I feel if a dementia patient is a high risk for falls, common sense should prevail and a facility should allow for common sense measures. The CNAs and nurses on call at this facility (with the exception of a few), seem very indifferent to the needs of the residents and are insensitive to them and are more concerned about socializing with each other. I spent afternoons there every day, with my father stepping in til bedtime because of the nonsense we would see and would often times have to assist patients about to fall while staff stood around discussing hairstyles or looking at cellphones. The only positive we saw were the two activities persons who were very kind, sensitive, and engaging with the patients. They were wonderful. We also felt the two months room and board my mother was away in hospital/rehab due to falls at the facilitys should have been refunded..regardless of what's in the contract to "hold the room" as there were many vacancies and obviously nobody lined up waiting to get in.

Lovely Facility, But Dreadful Care

Our dear, dear friend was in Clarity Pointe for several weeks, during which time we visited weekly, and met with the Executive Director, Murry [removed], on two occasions. We were dismayed by the quality of care, and by the attitude of Mr. [removed] and the staff. The facility is lovely, new, and clean - - but medications were handled carelessly, oversight of residents was lax and our friend actually deteriorated in his weeks at Clarity Pointe.

Undocumented Injuries

We just pulled my grandmother out due to undocumented injuries we found when we visited, including bruised nose and a fractured foot. No reports were made nor did we get contacted and notified. I observed hateful behavior when care takers did not know I was there watching and when I expressed my concern, lied straight to my face. When having a meeting with the head boss, he did not show any concern. Although the place is new and nice, I whole heartedly do not recommend this facility.
