Met with Kara Leek. Since no independent living offered, residents likely to need more care than at some places. Upside MD and nurses on staff a plus. Apts and storage were spacious. Common areas light and pleasant.

This was a wonderful facility. The facility was very clean and bright. I was really impressed with the memory care area.

Extremely impressive place.

Visited with director. Pleased with what I saw. This is probably the place.

POSSIBLITY LIKED CLEANLINESS, WOULD PREFER LARGER ROOMS

When searching for a place for my father, we toured Bethany Village Assisted Living. I do not remember much details about it, but based on, my father's situation, it would not have worked out for us. It was a nice place, though.

We are staying temporarily at the Rosegate and so far it has been good. The place is comfortable and all of the staff are very nice. The place is clean, for the most part. It is an older complex so there are certain things that need to be replaced. They have a lot of activities and outings available.

This community did not have a room available in their long term care unit. The assisted living area did not have the staffing to care for mom properly while she was staying there. The food is a little expensive and did not taste that good. They started eliminating the choices of food options after mom moved in. They do offer a cafe for the staff and I thought the food from there was better than what mom was receiving. We did not always have good communication with this community.

My mother is very independent and sometimes the staff doesn't give her some of the space that she needs. She administers her own medications and doesn't eat breakfast there. The community wants to have her engage more in their programs. Her apartment is great and the staff is nice.

Felt mislead by sales. When we first went to rosegate we were told mother would have a room up front. Two weeks later (and day before mom had to move in) we were given keys to a room in very back of building. There was no way mom could get to meals or activities, however since we were in at her 100 days rehab we had no choice but to accept the room. We were forced to go spend $1600.00 on a motorized scooter so she would not be a prisoner in her room. We were told at that time by Mark that he would move her in two weeks. When two weeks came and nothing I went to him and he said it would now be a month and room would only be half way (still to far for mom to walk). Two times I was mislead by Mark so I am very disaapointed with him. Activities, Nursing and Kitchen are fantastic. I would reccomend this place just beware of being promised things just to get you to sign.

There are not a lot of tenants in the facility so not a lot of people in dining area etc. It's hard to get in the facility on weekends or after hours because the codes don't always work

If memory care is needed be sure the needs and the care available are compatible.

Communication with the family is lacking greatly and when you cannot rely on what your family member says, you rely on the staff to keep you informed. At the moment, we would probably not recommend this community to other families

The selling part of the facility is great! Getting things set up like PT and OT is a struggle! Some staff are not around as much as probably needed! The dining room hours are a bit odd but getting better!

I would tell them to not send loved ones to the assisted living building. It is horrible, needs a total revamp of leadership and staff.

I can now confirm what many of my neighbors in the garden homes expressed to me as I got to know them many who have been here from 3-15 years . Many advised me that the executive director made them feel like garden home residents and their concerns were not important, and I must admit I had the same feeling after 2-3 garden homes town hall meetings, however that executive director has been replaced. The new executive director just arrived 2 months ago so again we will have to address issues and concerns that were previously presented to the former executive director and wait for feedback. Now on to your survey, all comments are based on my experiences as a Garden home resident at Beech Grove Meadows. The staff is very friendly and helpful for the most part. As a frequent visitor to people/ friends that I have come to know in the area including the Assistant Living area the cleanliness of the area has improved somewhat within the last 2-3 months . The quality of care for the assistant living area has been a big issue especially during the weekend hours which has been presented to the new executive director by several that live there and yes even by me who visits there daily. The food and menu's leave a lot to be desired at times you are not served the food that appears on the menu and yes I have had several meals in the dining room with my friends from the Assistant living area. Now on to the activities when I first moved in 9 months ago the activities person for the Garden Homes was off ill and there were no "activities " for Garden Homes residents except a bingo game every now and and then no outing's at all , after 4 months into my lease the activities person returned and I was so excited however very little has changed we only have one outing a month and that is for lunch only which is a big disappointment to me because per information given me on my on site visits to Beech Grove Meadows prior to my move in I am sorry to say has not been as I was told, that was my reason for moving here. I will keep a positive attitude however since we have new executive director and an activity person again for Garden Home residents. Would recommend a nice community with room for improvements. I remain optimistic and looking forward to seeing improvements with the new Executive Director who just came on board.

The facility just changed ownership and it has not been a smooth transition. They appear to be short staffed. This may be a temporary problem. I have had to prompt the staff to empty soiled incontinence products that leave the apartment very smelly. The food appears to be good. The staff is friendly and they appear to be kind to my loved one.

The staff is rebuilding, but is still short of ideal at this time. This especially effects housekeeping and maintenance. The nurses and CNAs are excellent - very skilled and caring. Each one of our regular caregivers takes the time to be friendly and helpful. Most go above and beyond the call of duty. Food service has several new employees, so it is improving as they become more accustomed to the job.
