A Place for Mom has scored 12 Oaks Senior Living with 8.7 out of 10 using our proprietary review score based on 207 reviews in the last 2 years. Over all time, 12 Oaks Senior Living has 1,378 reviews with an average "overall experience" of 4.4 out of 5.
We toured this facility recently (it was known as Emeritus at Clear Lake at that point) and it was absolutely awful. The memory care area was downright frightening. It was dirty, not very full, and many of the doors to the empty rooms were just standing open, and you could see that blinds were all twisted and torn up. When we visited most of the residents were just parked in front of the TV, and they all looked disheveled and in need of bathing. I would not recommend this place to anyone, based on what I saw.

I would not recommend this place to anyone, we had a terrible experience here. My loved one ended up getting very sick here and we decided not to bring her back. There were numerous issues here that indicated to me that it was not somewhere she would be cared for properly. More than once when I came to visit she had not been cleaned up after having an accident. There was also an issue with gnats in the dining room.

Did not care for the fact that independent/assisted living apartments were in the front of the facility, and more critical care residents were relegated to a small self-contained building in the back.

If you love your loved ones, please don't place them there. There is a lack of care there for the residents, that is from management and on down the line of staff. When the residents press the call button , no one responds. My loved one contracted COVID-19 there. She was negative upon arrival of moving in there.

My grandfather was a resident at Colonial Oaks at Pearland & he wore a pendant around his neck to call for staff when he needed assistance. It always took at least over 2 hours or more before anyone would respond & they seemed more concerned in turning the alarm off on the pendant when they came in the room, than finding out what my grandfather needed. The food portions were quite small and if he wanted seconds, it was denied as he was always told there was no more food left. If he wanted food after meals, he was required to keep snacks to eat from what family brought in or what he could buy if they ever went to the grocery store. The trips outside the facility were frequently canceled that were suppose to allow residents to go shopping for groceries or personal items or even go to a restaurant but the one van they had available was broken so often that inevitably the trip would not occur. The main occupation was Bingo, & it had no real outside grounds to walk around. The little central outdoor courtyard they have, did not have any lounge chairs set out or any outdoor activities scheduled there. (Mealtime Service at Colonial Oaks in Pearland: Unreasonably Long Wait For Service) It was a common experience for my grandfather to go to his table and be forgotten. If you're lucky, a busser might pour you some water, but your server is busy elsewhere and the manager is either dealing with some other issue or simply out to lunch (even if it's dinnertime). There you sit with no drinks, no acknowledgement of your presence. Sure, things get very busy. But five or ten minutes of sitting and talking amongst yourselves is more than enough. It was not uncommon for residents to have to wait 30 minutes to an hour before being served when they had shown up on time in the dining room. It was much too long of a wait for my grandfather. The cable provided was quite basic with very limited selection, and my grandfather had no ability to fast forward past commercials, or to record any programs as it was not standard at Colonial Oaks at Pearland. If you wanted that kind of service for your TV, you were expected to not only pay for the exorbitant Assisted Living fee but also pay for a separate TV subscription out of you own pocket (essentially paying twice for the cable service)!! The cost charged at Colonial Oaks at Pearland is quite pricey for a place that seems altogether understaffed or at least with a staff that does not seem highly motivated. I would recommend looking elsewhere for your loved ones if you seek attentive care.

My first intial tour was amazing however, when I came to tour the facility on a Saturday it was terrible. I noticed they were understaffed . It was an extremely chaotic environment. There were no activities going on and the place smelled of urine. The workers were pushing 2 wheelchairs at once and I can't imagine how there backs felt. I believe I came at lunch time because the residents were in the dining rooms. The ladies in the back dining room were moving from person to person feeding them. They would give one resident a spoon of food and then move to the next. I would not dare place my mother here

The majority of my rating is toward the Director, most of the problems stem from her and her management style. In general, most of the caregivers were friendly and cared for my mother; however, with constant turnover and staff shortages, I began to fear for mother's safety and well-being. Two caregivers on the floor for 30+ residents is NOT adequate! We dealt with several med cart errors that we know about. In an environment where transparency is crucial, the Director did not like the staff talking to family, which made one wonder what she was trying to hide. I sent my concerns regarding the Director to her boss and up the next management level only to be ignored for months and when I finally got a response, it was basically a "the Director is doing her job and we don't care" response. Apparently the bottom line is more important than resident care for Enlivant or at least for this facility. My mother lived here for 1 1/2 years. About six months in this director was hired and thus began a downward spiral. The website says: "a full-time registered nurse is available 24 hours a day to provide clinical oversight and coordination of care"; PLEASE note that this is a regional nurse, NOT on site, who may or may not answer her phone when needed. The maintenance man left and it took 9 months to hire a new one. The Head Chef was fired 2 months ago and to my knowledge, there is not currently a Head Chef. There was never a staff member available (due to staff shortages) to drive the wheelchair van if needed for a doctor appointment. Housekeeping was virtually nonexistent although the bathroom appeared to be cleaned somewhat regularly. There was no weekly housekeeping as the contract stated and my mother's room was filthy and the carpet stained when we recently moved her from the facility.

I would never recommend this place to anyone my brother was recently here and it went horrible for him. The administration was awful and had zero compassion on how they ran things in that place. It was always very messy. I always got such attitude whenever I try to fix something and try to get my brother better care. Overall it was a poor experience.

My mom has only been there 9 days and so far very dissatisfied. Activity calendar is still on October and was told waiting on new ones. I was told I have to collect the UTI sample. I see aids, house keeping and Activity Director cooking and serving food. Now I am told I can't have code to get in and out of door. My mom is paying good money and her daughter can't have access to door code. So I have to wait for someone to open door and let me out. That is not right. I never know who to ask for what. I ask for cup for UTI and one couldn't be found. I have ordered Physical and Occupational therapy and heard nothing. Seems very poor managed and understaffed. My mom has been moved from Florida to Texas and all this is a major transition for her as she has Dementia. I don't feel like aids are spending time with her and trying to help her or be understanding, except one or two. I feel like staff should be more caring and she complains that they are ruff with her. I can't be there 24 hours a day, but my mom is not happy.

I agree, Toni should be fired!!!!! Our mother could barely breath one day and I asked Toni to please come and help and she refused and said you will have to call paramedics. I thought when you take an oath to be a nurse, you are obligated to try to help until paramedics can get there. She is unfriendly, a terrible example to others and disrespectful! We loved the facility and other staff. She ruined our experience in the end.

DO NOT PUT YOUR LOVED ONE HERE!!!! Put my parent there after being misled that this was a warm community that "cared" for its residents. I was specifically looking for a place that offered music at least a few times a month. I was told they had a dance every month and live music once a week. No dances and I can count on one hand how many times they have had music. They did not even have music for the holidays - my parent has been in a couple of senior places and they always had local school or church groups come in for holiday caroling. They are all about the $$$ and their activity director is a joke. I spoke to several residents who were also fed up and bored out of their minds with her and the lack of quality activities. They have been remodeling for nine months and still not finished. Doors from the outside are left open all day even when it's cold. A few times there have been stray cats lingering around the halls. The final straw was when they fired the home health company that was helping me with my parent on a Friday without telling me and had the new home health company call me on Saturday to find out if I was interested in their service. What if I had been out of town or not able to get back with them for awhile? Would my parent be left without care - no meds, no shower??? The only saving grace this place has is one staff member named Michael who has a real HEART for this (don't know why he's there) and a hairdresser named Alicia who is sweetheart. We finally made the decision to come out of pocket and move my parent into a more professional, caring environment in order to have more peace of mind.

Its poor management. It seem like they are there to get a check. They cover alot of things up that happens to the residents. Everytime I come the manager is not there. And when I do see her and I have a compliant its always Im working on it. The residents are awesome and some of the staff but if I were new coming in I would reconsider because they always act like they care until you sign and they get your check. If they had a better person running it then maybe I would feel more secure.

Okay family members here is the real deal. I wouldn't even rate this place 1 star it deserve a NO STARS!!!! My father was only at this facility for 2 weeks. [removed] They charge extra for everything. Anytime your loved one has a meal sent to their room is $5 each meal ex. coffee and banana =$5, Next the medication management or reminder is $11 each time so if your family member requires 8 pills at 8 different times a day that $88 for the day. My family member had a incident and I was not notified of the incident. I was told by the dept head we don't do incident reports etc. At least notified family members that an incident occurred due to the facility is under constructions which is highly unsafe!!! My family member was hospitalized [removed] and after seeking treatment the MD recommended assisted living. Well once my loved one and I contacted the assisted living area this is were the problem ball was dropped. We arrived there at noon and didn't leave until after 6pm and my loved one was not even transitioned into the assisted living due to administration issues, clerical issues, lack of customer services, lack of confidentiality, degrading, lack of genuineness, lack of care!!!! [name removed] the health and wellness director was unprofessional, degraded myself and my loved one, lacked confidentially about my loved one's medical history. She had no moved at a slow pace and was all over the place. She had no clue what she was doing!!!!!!!!!!The error was on Brookdale's end!!!!! My loved one and l just got up and left. Neither did [removed] or [removed] attempted to check on our well being or apologize for the long process which wasn't even complete!!!! I had to find [removed] in order to see what information was lacking. Her response was short and abrupt and ended with "do you have my business card?" I removed my family member on the next day due to know one even bother to check on him/her after returning to this facility. I WILL NEVER RECOMMEND THIS PLACE [removed]

Both my parents lived at Colonial Oaks. It is an overall nice facility and they do a great job of selling you on their services. Then it is downhill from there. After moving my parents in and after multiple requests, I was unable to get a copy of the countersigned contract. They suggested using OmniCare out of Nacogdoches for medications and never gave them the Medicare and AARP information and we are still trying to work through exactly how much is owed and how much insurance has paid. The medications were ordered and or shipped wrong and they dispersed too much of a medication causing significant issues with my mom, then started charging me more each month to take care of her. Again, without providing me any sort of countersigned contract or documentation. We are not sure of the long term health impact of over medicating her. The director, Lisa Gilles-Petruna, berated my parents and would no longer let them go on any bus trips or eat in the dining room. The team did not clean their bathroom, room or kitchen area. I had to go in once a week and clean. I gave notice to Colonial Oaks and it took a few emails to "negotiate" the final amount due. When I moved them out, Lisa committed to providing the care plan, medications and final bill. All we received was a trash bag of medications. Colonial Oaks never sent me anything and after a month of not receiving any response, I received a call from their collection agency. After the initial collections call, we were able to finalize the final payment. I would not recommend or trust Colonial Oaks.
