Nice facility but they are having a hard time keeping employees so very hard to get questions answered once you move in. Also the independent living side only has transportation 3 days a week so don’t depend on them to take your love ones to doctor appointments.

My father recently moved in [date removed]. After several attempts to meet with their so-called primary care doctor, my father had to go to an urgent care instead which they found cancer growing in his throat. Once he was finished at the hospital and recovered from surgery he went to move back. Now there's no director of nursing apparently and the nursing staff threatened to not give my father his medication. Bottom line is this place is unsafe and should be investigated.

On 3 occasions, my mom’s room stunk because her bathroom trash can was overflowing with dirty Depends. I alerted staff each time. After this happened a third time, I took a picture of the trash can and included it in my hotline report to Senior Services. The staff key that I had to use to enter my mom’s room would not unlock my mom’s door. This was a serious safety issue, so I reported this to the Executive Director. On two occasions in November, I told the Head Nurse and the Executive Director that my mom no longer had a Life Alert. I had my mom admitted to a different facility on [date removed] when I saw her feet for the first time in years. I took a picture and included this picture in my hot line report. The investigator found that there was no documentation that my mom received two showers a week and that some of the residents received even fewer showers. Some of my mom’s protected health information was shared with other visitors not related to my mom.

My Father passed away [date removed] from a fall. They kept wanting to medicate him. I said no. Whether they did is unknown to me, but I can say the care he had wasn't good. They started to bring in hospice care patients who needed a lot of care. They weren't hiring more people which meant that the dementia patients were not getting the attention they needed. I liked the place at first but saw it going down hill towards the end of 2024. I would not recommend this place for a dementia patient. Maybe for assisted living when they still are aware of what's going on.

The staff was very lazy. My dad sat in BM for a long time. His clothes always got lost. I had to keep spending money on buying him things because they would lose them. Wipes would end up missing. As well as briefs. He would lay in his BM. Lights would be on in middle of night. Sheets were never washed. BM on floor. Other residents coming in his room and laying in his bed. Please don't recommend Vitalia to anyone. When he passed, Vitalia called the wrong number. I was not able to go there and see him because I didn't know. They continued to give him food he didn't like. We were paying [removed] dollars a month...and the care was horrible.

They won't show this. The care is very poor. Assisted living technology doesn't work and response time for calls is 20-40 minutes. Administrators do nothing but say what they think you want to hear. The food is poor, care is poor and corporate is cutting corners to save money. Very poor place.

This site is deceiving. Poor food, poor care. Not at all what they advertise. The food is poor, the Bistro is empty--especially if on assisted living side. If you need help or fall, respond time is 20-40 minutes. Administration gives "lip service" but does nothing to improve the poor services and typically ignores calls. Be prepared to get "Nickel and Dimed" for services you rarely or never see. Corporate cuts corners in care. Understaffed daily. Very disappointed in the poor quality for big money. Keep looking.....

I was deceived, lied to and will not live with people like don't trust. They have not returned the deposit I paid for a studio apartment that I really liked. The previous resident moved out earlier than expected, but shortly after, they told me the unit had been double-booked and was no longer available—even though I had already signed a contract. They later offered me a one-bedroom for the same price, but never followed up. My son later visited the apartment they claimed was available, and it was completely different from what I had agreed to. It had carpet I didn’t want, and the kitchen was missing basic appliances. It felt like they were trying to pressure me into signing for a unit I did not choose, and I never signed another contract. I also sent another payment when I believed I was still getting the one-bedroom, and they have still not returned that money either. Their food reviews are not good, and the entire experience felt very shady. I would never live there. They have held onto my payments far too long despite being repeatedly told to return them.

Horrible. I had to clean my father's bathroom . When he passed he was lying on the floor covered by a sheet . Limited staff care . Go ahead try and call me out i have video footage I kept a camera in my dad's room.

I think they do a terrible job I'm not happy at all they don't even supply toliet paper they don't follow through with showers and cleaning her clothes. For what we pay I'm so disappointed

While Crestone presents as a well organized and clean community, it is anything but. The executive director is unprofessional, ignores obvious needs of residents, deflects and avoids common questions, and ultimately does little to better the experience of her residents. This lack of leadership trickles to the wellness staff, and the dining staff- each with large turnovers impacting care. Crestone has potential, but mostly they are concerned with image. I was disappointed in the care provided to my loved one. I will say there are kind people there, just not the ones that influence the other all experience.

The medical staff and others were very good for the most part, especially the Executive Director and the Wellness Director. The food and food service was mediocre at best. Maintenance was slow to respond and sometimes almost non-existent. Waiting weeks for television service to be restored, a microwave in an apartment to be replaced, electronic room keys to work, or the only bike in the fitness center (used by PT and OT) to be fixed is not acceptable.

The staff was terrible when my mom was there, they dropped her a couple of times, management changes, communication issues on when she was supposed to get showers and be ready for rehab, and just not friendly. She hated it there. They may not have been equipped to deal with the level of care she needed at the time.

HR and management of accounts and other managers need to Communicate clearly to each other. So they won't have this problem in the future. Not truthful. They said they could get [name removed] in within 3 days. Took a deposit check, then said she would have to wait two weeks. She was in a position where she needed a place asap.

This facility is: Unsafe Understaffed Poorly managed Has 11 current state citations Incapable a caring for those with real medical issues Do not be fooled by the chandeliers and the smiles at the door. The building is not a good fit for anyone that cannot care for themselves or has memory impairment.

Staff is great but the upper management is terrible and the billing system backwards. I know more people trying to find new facilities due to the poor upper management. They bill you but can't tell you what they are billing you far. Nothing is itemized and they can't give you an itemized bill after I asked for one several times. After my dad passed away they sent me a bill for thousands in extra fees. . We paid in full at the beginning of every month. Would not recommend them.

Not a lot of follow-through from staff… Lack of communication and understanding from staff Food was not warm multiple times… My father had fallen and no family member was contacted… Simple request were not addressed… No phone calls were returned so I could have my issues resolved. Would not put a family member there again Tried to see one of the facilities… After three weeks of not getting the paperwork to do so I had to take my loved one out of [facility removed] for 45 minute drive so he could see his doctor.. That was totally unnecessary for my dad to have to travel when they had a doctor right at the facility

I looked at RC as an option for my loved one after VA’s recommendation. Knowing there were other vets there, we set up a tour. Looked at a couple units, saw one he liked, paid the deposit to hold the room. We were told that if he signs a lease by [date removed], the "deposit" would go to the community budget (not off his rent for the first month or on hold like a normal deposit). If he DIDN'T sign a lease by [date removed], it would be refunded. Notified RC that we were going with a different property and requested the money be refunded on [date removed]. I received a response from [name removed] saying they will get it processed. It is now [date removed], I've emailed [name removed] 3 times (including the original request) asking it to be refunded and a status as to why it hasn't been done. No response. She emails me asking if the move went smooth for my loved one but will not give me an update on the refund. Emailed [name removed], the facility ED, STILL NO ANSWER. Make it right Rock Creek!

My husband got out of memory care twice and was out on the parking lot. I immediately took him out. Another gentlemen got out twice also. Administrator is horrible!!! I would not recommend this place to anybody!!

My family member was in the memory care unit. She was wearing the same clothes for the whole week. We paid them almost [amount removed] a month and she was not being bathed or watched after. We started going to bathe her in Wednesdays and Saturdays. She fell 4 times in one week and was sent to the hospital twice. The last time she was admitted and from there went to another rehab facility that took good care of her. When I talked to the nurse who cared for her the night of her last fall I found out that he was the only person on shift that night. He came on shift, found out he had no help and immediately handed over the keys that the nurse uses for medication administration and told them he would only play an aid role so that he didn't put his license at risk. I have hesitated giving a review online because I am not that kind of person but I'm comfortable telling you so that no other family members will be steered that way and have to go through what we went through.
