This facility needs to be shut down. Bad mold issues are being covered up and unaddressed by their ownership and management group. The company should review the new Executive Director, as rude and uncaring towards staff, family, and residents. Unprofessional and unapproachable to address any issues that families are trying to address. As a family visiting new Executive Director was yelling at an outside vendor in the hallway. We are looking to place my mom someplace else. Many family members are leaving due to unaddressed issues. I would not recommend this place under any circumstances.

Because they are not licensed as most assisted livings are they do not have to comply with any minimum standards. Confirmed significant mold issue that the management ignores- may be the result of multiple leaks. Disappointing place with serious management issues.

I was very disappointed with the entire facility. I moved my mom after 1 month there. The entire building is in need of repair and updating. The food was terrible, short staffed, the best thing about this facility were the greeters that sat at the front desk.

The facility is beautiful. The OT, PT and social services went above and beyond. The nursing care is TERRIBLE. They left my dad on the commode several times for 45 minutes after he called for them. After we reported it, it didn’t get much better. It caused him to not feel safe in their care. They scheduled a release day and he fell the day before, no one came for SEVERAL minutes once he called for help. He had to yell to finally get someone. The head nurse practitioner is awful. She REFUSED to sign off on my dad’s motorized wheelchair when he was under their care. She had several weeks to do so but kept making excuses. Her last one was he wasn’t in their care anymore. He finally, after a couple months got his doctor to sign off. Meanwhile he is trying to navigate on a regular wheelchair with permanent nerve damage in his hands. He has since had to go back to rehab because he got very sick at home. We chose [company name removed] and he feels safe and well cared for.

We had our loved one in this facility for almost 3 years. We were there when it was called [name removed]. And we were also there when it was bought out and became Cortland of Howell. In the three years we were there, there was 12 different managers. 13 different health and wellness coordinators. Three different nurses. I can’t even count how many aids/med tech. Well over 50 in the three years we were there. Most of the time this facility ran with no nurse in the building or anybody with any type of medical training. - Consistent medication errors - Overmedicating/under medicating - Staff is not properly trained - management is not properly trained - most of the time you can’t find staff to help you They do not care about your loved one. Do not go to this facility. Do your homework. Research other facilities in the area. Look up the reviews and things that have previously happened in this facility. It might’ve happened under the name [name removed].

My mom was there for Rehab from [date removed]. She went in walking and came out frail and malnourished. When her money ran out they said she has to go or make other financial arrangements. The director of nursing [name removed] and social worker [name removed] actions verbally were very insincere. And made no effort to try to assist our family in finding a new location for my mom nor were willing to work with us. They called us on Friday before the Labor Day weekend to give off notice. Absolutely unacceptable I would think long and hard before putting your loved one there.

My mom was sent to Oaks for rehab after surgery. She was only there for 5 days before I moved her to another facility. She would push her call button and wait HOURS for someone to check on her. At one point, she pressed her call button to help her use the bathroom. Help didn’t come for about an hour and by that time, it was too late. The nurse was upset that she had to change her bed sheets and was got rougher than usual while moving her to her wheelchair. She was scared to be there. After I decided to move her to another facility, the staff would not cooperate with the process of moving her. After 5 days, she never got a shower, brushed her teeth, had her room cleaned, there was a random pill on her floor, and she never did any actual rehab while she was there. If I could give this place less than 1 star, I would because this place is HORRIBLE. Do yourself a favor and take your loved one somewhere else!!!

Keep looking. My mom broke her hip and was going to complete her rehab at The Villages at Historic Silvercrest. The photos and verbiage on the website looked like we were making a solid choice. What we were told: "When you arrive, you will meet with all of the department heads to make sure we are all on the same page regarding your mom's care." What we experienced: We arrived on Friday at 4:30. After a very kind woman transported my mom's belongings up to her room via a fancy hotel style transport cart, my mom's care ended. We were left alone in a dank cramped room. The bed was archaic, uncomfortable and crammed into a small space. It was so loud when we used the controls to put the head of the bed up, it was seriously questionable if it would support a human. The heater cooler was less than 12" from the bed and blowing right on my mom. The bathroom had nowhere to put anything, not even toothbrush and toothpaste. A brand new hire came to the room and tried to fill my request for

Beware of this place! My mom had an aid here who was coming to visit on her on the aid's days off she lived 30 minutes away one way and bringing her husband and staying for 3 hours visiting my mom. The other aids found it strange when they came into my mom's room that they would stop talking. Apparently they want to buy land. I told the Director she just acted like that was normal behavior. She said well your mom is close to her. I told my mom I felt it was strange and my mom said that this aid is her best friend and she started crying saying the aid will be so sad that she can't visit her like that now. I call my mom everyday and I go and see her 3 times per week. Imagine what they are doing to people who's children are not involved.

My aunt has been at this facility for a week and hasn’t been bathed once! She went straight from the hospital to the facility and is still sitting in her own filth. She is so weak she can’t open a bottle of water for herself, so she needs help with everything. When she’s rang the buzzer, no one comes and she’s left to pee herself and sit in it. This place needs to be charged with neglect. They’re charging $200 a day to let their patients rot. Being understaffed is NO EXCUSE to let this happen. I PRAY their administrators, local law enforcement, local government and all local DOCTORS see this and get their patients the help they need!!!

I would not recommend Thornton Terrace to anybody, we had a terrible experience. If you are looking for a place to send your loved one to die this place would be appropriate. The care was absolutely terrible.

Unfortunately, we had a terrible experience at this facility with our family member. Wellbrook of Carmel is so incredibly understaffed it is utterly alarming. Our family member came in for short-term rehab to get stronger after a broken ankle bone. Aside from the leg, they came in healthy and in good spirits. In less than two weeks, our family member is currently in the PCU at the hospital with hyponatremia from dehydration. And, we are facing palliative care. I would absolutely NOT RECOMMEND this facility.

Cypress Pointe are not qualified to care for dementia residents and the challenges of dementia. I let the Director of Memory Care that my loved one had an active lifestyle prior to their dementia diagnosis. The Director left 3 weeks after my loved one moved in, and then there was no Memory Care director during my loved one's stay. The aides were the best part of the unit. The nurses seemed either overwhelmed or didn't care about the residents. They had no patience towards the residents. My loved one's room was always messy. They would only run the vacuum in parts and they didn't put my loved one's laundry away. I had high hopes for my loved one and the community, and it turned out to not be a good experience for us. The community decided to discharge my loved one unexpectedly with a 30 day notice. I'm very disappointed in this experience.

The care on the rehab unit was average. The waits for care were long, but he was assisted out of bed and to the restroom as necessary. When he was transferred to the Memory Care, it was horrible. Shortly after his transfer we were not allowed to visit for 2 weeks due to a COVID outbreak. After 2 weeks we returned, and found that they had not assisted him in moving or walking during the entire 2 weeks. He was left to sit in the wheelchair all day. All the progress he made in rehab was gone. He had deteriorated horribly. Often when I came to visit he was so wet that it went all the way through the sheet. They also put 2 depends on him so they would change him less. I had several care conferences with the staff with no improvement. I had to push for them to transfer him to the skilled nursing care unit, as he had deteriorated so much. I also contacted Hospice Care to get them involved. He passed away about a week later. I would not recommend their memory care for a loved one.

I been trying to contact River Terrace at all times of the day and evening. They never answer the phone even at the nurses station. Tried calling a patient and she never answers. You'd think a nurse or someone would hear the phone keep ringing and assist the patient. I would have never taken my mother to this facility.

The nursing staff is very unprofessional. Getting them to do anything above handing out meds was like pulling teeth. My mom kept losing weight because the food is terrible and they were not tracking how much fluids she was taking in despite her being very dehydrated. I was told by a nurse that she has 65 patients she is responsible for per shift. There is no way the nursing staff would have time to do anything beyond handing out meds.

My father was at Bethany Pointe for about a month and they discharged him without being ready. My father has brain cancer and do to the changes in his brain, he can have behavioral issues and memory issues which we heard staff comment about when giving his medications. He waited at least 30 minutes multiple times for a response to his call light, he was not privided transportation or even prepared for an appointment that the facility assured me both would be taken care of, after no talk of a release date, he had a discharge date the day after I placed a complaint (even nursing supervisor stated he was not ready) he was unable to even transfer from bed to wheelchair upon discharge... and the worst part was he was sent home with SOMEONE ELSE'S MEDICATION and the other person sent with his. Now 3 weeks later due to the early discharge, my father is being admitted to a new facility due to getting pneumonia with the lack of strength and ability getting out of bed. Completely unacceptable for any human to be treated this way

Well, a week a go I may have recommended this faculty. It sure does look nice on the surface .... but we all know it’s what’s deeper that matters. Their treatment of my grandmother today is beyond reprehensible. It shows complete disregard for her and for her care. I’m appalled. This is not a facility to go to if you, or your loved one ACTUALLY NEEDS CARE. If you or, or your loved one, is fully independent then Sylvania Lakes is happy to take your money.

If I could give this place a lower score, I would. The care is absolutely horrible. 5pm in the evening and patient has not received morning medications yet. Don’t put your love one there! Nurse is rude and horrible. Please don’t put your loved one there!
