Management and Nursing staff Very Unprofessional

The nursing staff is very unprofessional. Getting them to do anything above handing out meds was like pulling teeth. My mom kept losing weight because the food is terrible and they were not tracking how much fluids she was taking in despite her being very dehydrated. I was told by a nurse that she has 65 patients she is responsible for per shift. There is no way the nursing staff would have time to do anything beyond handing out meds.

Not even close to a 4 star facility

My father was at Bethany Pointe for about a month and they discharged him without being ready. My father has brain cancer and do to the changes in his brain, he can have behavioral issues and memory issues which we heard staff comment about when giving his medications. He waited at least 30 minutes multiple times for a response to his call light, he was not privided transportation or even prepared for an appointment that the facility assured me both would be taken care of, after no talk of a release date, he had a discharge date the day after I placed a complaint (even nursing supervisor stated he was not ready) he was unable to even transfer from bed to wheelchair upon discharge... and the worst part was he was sent home with SOMEONE ELSE'S MEDICATION and the other person sent with his. Now 3 weeks later due to the early discharge, my father is being admitted to a new facility due to getting pneumonia with the lack of strength and ability getting out of bed. Completely unacceptable for any human to be treated this way

Well, a week a go I may have recommended this faculty. It sure does look nice on the surface .... but we all know it’s what’s deeper that matters. Their treatment of my grandmother today is beyond reprehensible. It shows complete disregard for her and for her care. I’m appalled. This is not a facility to go to if you, or your loved one ACTUALLY NEEDS CARE. If you or, or your loved one, is fully independent then Sylvania Lakes is happy to take your money.

Horrible service!

If I could give this place a lower score, I would. The care is absolutely horrible. 5pm in the evening and patient has not received morning medications yet. Don’t put your love one there! Nurse is rude and horrible. Please don’t put your loved one there!

Avoid!

The directors in this facility are horrible. They do not follow up on the care and concerns presented to them. Their failures to follow up, address concerns and properly train their staff detrimentally affects their patients. I witnessed failures to follow basic hygenic practices with cleaning commodes, failure to obtain doctors orders for certain items and a lack of providing basic care for its patients—bathing, toileting, potable water for drinking, addressing medical issues. Avoid!!

I was not impressed with the Ridge Wood health campus in Lawrenceburg. We were early, no one was at the desk, we walked around with no one asking if we needed help or what we were doing there! Several empolyees walked right past us and smiled. I told the lady on the phone this was just for day care service...yet she explained all the benifits of living there, even showing us a room. My aunt is very nervous about staying in an assisted living place. This only made her more anxious. The activity room was residents that were sleeping or really out of it. This was during a music activity. I was so disappointed.

EXTREMELY POOR

My name is [name removed], & my mother was recently at the Oaks of Woodfield. First viewing the facility, I was impressed. HOWEVER, once my mother was admitted to their facility she received practically NO treatment!!! She had been there 2 weeks and they were beginning to push her out of the facility!! My sister & I fought for my mom & she received an additional 2 more weeks, which was a complete waste of time!!The therapist didn’t want to work with her (except [name removed). There was another other patients that had been there 2 weeks (according to the patient ). The poor woman just cried. She stated she had not achieved what she came here for, and broke down in tears! The staff was so anxious to get my mom out of there it was ridiculous!! They had her in a bed that wasn’t flat, it went up on both sides . It was not a normal bed & I’ve got pictures of it! This place is great if you love good food & a single room , but if you desire good treatment that is REAL & “HEARTFELT” ........, DONT COME TO THIS FACILITY !!!!!!!!!!

Beautiful facility food is better than most however, staff is not trained in memory care at least not to the highest level from [dates removed] they had 10 residents fall and 2 with fractures and 1 resident died. Lot of things are not seen from the surface beware the staff act nice but they are anything but nice.

The Oaks needs someone to come in and do things properly

I reviewed this facility from the view point of a Registered Nurse with many years of experience in different settings. Baths are to be given twice a week at The Oaks. My friend arrived on Thursday and finally, after asking several times, was given a shower by a male attendant the following Tuesday night. How humiliating. She'd been there 6 days. She did not get a shower on her scheduled Friday. The R.N. left the medication for another person on her bedside stand while she went to retrieve the correct eye drops. What caliber of nurse do they hire there.? You have to wonder. That is strictly forbidden. Apparently they don't read the orders or it doesn't make any difference to them what or when they give a medication or a treatment. Her inhalers were mixed up along with her other meds. The call lights were on extended periods of time with staff sitting and ignoring them. My friend finally had to struggle into her wheel chair and go to the bathroom then was chastised because she got out of bed. There was an attachment to the bottom of the oxygen tank that dragged on the floor and I tripped over it almost knocking my teeth out on the oxygen tank when I tripped. So much for safety. The aids walk around the halls in pairs or stand chatting. I tried to get out at night and walked the perimeter before finding two people doing nothing.The place was totally empty. Meetings are held with everyone crammed into the patient's room standing. Did They never hear of a professional meeting held in a conference room at a table where everyone sits down?? Incidentally the food was good. I strongly suspect that your good reviews are given by non medical people who have no idea what safe and adequate care looks like.

Really couldn't believe my experience the first several minutes of arrival

Well let me start off with we chose here b/c of the awesome reviews we received. So why first evening I was let down no one wanted to assist me with getting my parent out the vehicle. One aide was doing nothing but going bk n forth about why they couldn't cuz they didn't know anything about my parent. Really you gonna deny care in front of me, then more went on but I'll just not say any more. I was truly upset though. Met my parents aid for most of evening and her name is [name removed] very nice kind and didn't mind helping. Once I left after hours the alarm went off the aid that saw me walk by her didn't bother to ask me if I knew there was a code to enter before leaving or try to assist me in as my way. When she had to move cuz alarm was ringing she was so rude, like I bothered her from sitting behind the desk which is where she was for a long time. Good help is hard to find and people like the 2 aides that were discourteous gives the oak a bad review and makes me concerned if my parent has to be so called card for by them. I admit I had only met a couple good nurses and 1 good aid [name removed]. For my first time ever being in contact here with anyone I wasnt impressed and disappointed and upset within my first 2 minutes here. I pray asking God to please watch over my Loved one and touch the nurses and aids with care and compassion when it comes to all the residents especially my parent. The Lord knows all and He sees all. So your staff that isn't doing their best I hope they know they reap what they sowe. And one day if blessed they'll be an elderly person who may need assistance also. I pray this place will live up to the good reviews I've heard. But I'll be there as much as I can to help my parent just in case. Place looked clean but haven't had a chance to really see the place overall besides hall and 1 room. Was not very friendly at least not evening shift. Care sucked until [name removed] came along. Have not experienced activities yet. Value remains to be seen.

My mother was in Forest Glen after her strokes and for respite care. They are a poor excuse for a nursing home. Our family had to constantly be on them to take care of her basic needs. She always smelled of urine and I always had to clean her up and change her when I was there which had to be daily or the care was horrible. Always had to request she had water beside her bed and then her tray table would be so far away from her bed that she couldn't reach her it. We had to request to have them bath her. She always came home from respite with bruises, bed sores or rashes when she was there. They dropped her on several occasions and never followed our instructions to always have two people to assist to Wheelchair and bathroom. They had to take her to emergency room once as they almost tore her finger off when transferring her in bathroom when only one person was transferring. We had several meetings with management and it would be a little better for a few days and then back to the same horrible care.. They were just really bad. My Aunt was also there for a short time before she passed and they were terrible with her as well. I would never suggest anyone to take their loved one there. It's heartbreaking and their administrator and employees should be ashamed how they treat our elderly. It breaks my heart.

I can say are walk there the nursing home seem clean and friendly staff, Until my Mother was placed in the Memory care , I have spoken to the Director over my issue with my Mom not getting her showers, I was gone on Business for five days,Husband and I went to see mom and she smelled her hair had not been washed since I washed it, which was almost week.MOM ended up getting crandle crap in her hair I had go every day just about and wash her hair, Then the other issue was late that evening after I got my Moms hair and shower, I put the covers down on her bed the sheets were soaked and the blankets were fifthy, I was appalled I went to the front desk, Same nurse is always rude never on the floor she spends most of her time outside smoking, and off floor,When aid came back to room, I ask why her bed wet and dirty, and her excuse was they short aid so my Mother would have slept in that all night if it had not been for me, We don't get any government help we pay big bucks for this awful nursing home, I am looking to move my mother out that place, I have talk to main Director until I'm blue and nothing gets done, There is one Nurse that works on that floor does good job But all lazy ones hurt there job need clean house get aid that want to work, There suppose be working there [Removed] nurses desk during nothing and either do they volunteer to help,

Beware - Don't Judge a Book by its Cover

We thought Orchard Grove was great at first...nice-looking, clean, newer facility...until we discovered that a lot of the information given to us upon admission was incorrect. There is a huge lack of communication. They tell you what you want to hear to get you in there, then are not able to follow through on their promises. We have witnessed staff segregating and discriminating against residents. There is a lack of staff training. We have seen a lot of staff and resident turn-over. They evict residents that have any issues at all. We pay over $6,000 per month and they arbitrarily raise that fee at any time saying your loved one is requiring more care than others, which they have done, to encourage our family member to move out because they don't want to provide care. They have tried to require us to retain private duty nursing staff in addition to their own staff, which tells me that they don't have enough staff to provide proper care. There are a few great staff members, but they are not there at all times and do not represent the entire staff body. I would never recommend this facility to anyone.

NIghtmare!!!

People, people, you'd better check this place out thoroughly. DO NOT be fooled by first appearances. I only wish they put more effort into their patient care, communication with family, cleanliness of their patients and their rooms, food, nurse to patient ratio and that is to name a few. My loved one has suffered and suffered, asked several times to be moved. We were in the process of making the move and illness struck again making a move impossible. Loved one has been in the hospital at least 3 times since being there, fallen out of bed and laid on the floor screaming for help. There are no bed alarms, hygiene is terrible....feces under fingernails and other skin rashes in private parts. On the weekend's patient care is nominal. Good Luck finding someone! There is soooo much more. Guess there is a reason why they can't keep a decent director. We have seen at least 3 in 3 years. This place is a nightmare.

Don't send your loved one to Forest Glenn

My dad was in the memory care unit at the Springfield Ohio Forest Glen for a little over a month. During this short time he developed several painful pressure sores all over his back, shoulders, feet, and sacrum, from where they would seat belt him into a wheelchair every day and leave him sit in the same spot all day. Some days they wouldn't even get him out of bed, we went there at 4:30 pm one day and he was naked, for anyone walking by to see, he was laying in a bed completely full of urine, and he had nothing to eat or drink all day. He also lost an alarming amount of weight during his short stay. Dr. [Removed] did not want to give him any pain medicine, and cut him off of the pain meds that he had taken for the past 15 years for his chronic hip pain. When it came to end of life care, this doctor refused to consult Hospice, so my dad could be kept comfortable, even though he met all of Hospice's requirements. Please don't send your loved ones to this horrendous place. Management knew about these issues, and still nothing ever got done. The last month of my dad's life was spent in agonizing pain from those pressure sores. Horrible place!!!!!!

The nursing/support staff acts as if it is a bother to help their clients. My mother has spoken to the administration about this, but it has not helped. The facility seems extremely understaffed to provide proper care.

Please Reconsider This Facility

Before even considering this facility, please do your research. Before COVID-19 hit, I was fairly pleased with the care my family member was receiving. The place was clean and it appeared that the care was good. Then the virus hit, and this facility became something that was not recognizable. They became very secretive, and the rumors starting swirling. It was then reported that there was 32 Covid deaths that the Lakes hid from the public, and the infections were running rampant thru the facility. This was also confirmed by a daughter, a nurse, who worked for the local health department. They started losing staff, and still the family members were relying on information from the media. In the 6 months since COVID-19 began, over 90% of patients and staff became infected, and the staffing levels are now ridiculously low. and patient care is non existent. One fact that is being kept from the public, all residents are confined to their rooms and not allowed to walk the halls, or even get a bit of fresh air. Please do not consider ever sending your loved one to the is facility. There is a reason that this facility was one of the worst during the pandemic.

DON'T PUT A LOVED ONE HERE

TERRIBLE PLACE. Dishonest, shady. My aunt looked at places here. The marketing director told her that the place she looked at was THE LAST UNIT left. He told her she'd better snatch it up or it would be gone. She committed, signed the contract and gave them $2,500 deposit. The next day... The marketing guy called and said he had made a mistake and wanted to change the unit that she signed for to a different unit. (What? He claimed she'd better hurry and sign cause there was only 1 unit left! Now there are more???) The unit she paid for was a model and had a blue colored wall. He wanted to switch her to a different room. He was pressuring her relative to get in touch with her while she was at an oncologist appointment, finding out how much longer she had to live! One of the head administrators and another guy became extremely angry when she wanted to stay in the place she put a deposit and signed contract on.... These people are only friendly and professional until they get your money and you sign the contract! It is all downhill after that...... I know that we all love our relatives, so I would tell you or recommend to you....under no circumstances should you put your loved one here if you want them to thrive! STEER CLEAR OF THIS PLACE

STEER CLEAR. Grandma was here. Not enough staff. Grandma waited 10-15 minutes for CNA after she pressed her button to go to the bathroom. I was there numerous times when this happened as I visited regularly. Don't dare complain. or social workers cop an attitude and we were treated very poorly after doing so. Terrible food as well.
