My loved one had been staying at this facility, but we decided to move her out once she started having hygiene issues. One of the services she was supposed to be receiving was bathing assistance, but if my loved one said she didn't want a bath they would not give her one, and she was not in a state to be making that decision. After we moved her we also discovered she had a few other issues that could have been easily treated that went undiagnosed here.

My husband was actually penalized for having a UTI that put him the hospital. His behavior was combative with this UTI & they decided that he was not allowed to return to Cortland. I have yet to get a refund for the remaining time that he did not reside there. I would not recommend Cortland to any of our friends that are looking for a place for a loved one.

My mom resided there for 33 months until her passing in Feb. 2024. It is NOT a state licensed facility, meaning they do not have to comply with state guidelines. PLC’s communication skills are lacking immensely. Weekly, they are short staffed, especially on 2nd/3rd shifts and weekends. The facility went through 15 general managers, and numerous department heads, in my mom’s stay. Meals are subpar, lacking much protein or fresh vegetables/ fruits. Activities are very limited and lacking creativity. Very little training is given new staff, which causes inconsistent care. Rates were increased bi-annually even thou they were not receiving the services they were paying for. We had high hopes for the new owners/ management company (PLC), and were sadly disappointed. I would highly recommend you look elsewhere!!

My loved one was previously a resident at The Willows at Springhurst and the experience was not good. I would not recommend this facility to others.

I am often angry about the care that my mother-in-law receives at Forest Springs Health Campus. Some of the more egregious offenses have included the inability for anyone on staff to ensure that she received the latest Covid booster shot. It took me four months of advocating for her in order for her to receive the latest booster, in spite of numerous Covid cases among employees and residents. Only when I told the administration that I had contacted the health department did they follow through with multiple requests from me, from my mother-in-law, and from her husband. She is supposed to be bathed twice weekly. This does not occur. She is sometimes only bathed once per week and only after multiple requests. I could go on.

Only if you are on Medicaid should you go there. Full pay price is outrageous and not announced going in considering the care. At nearly $10,000 a month her care should have been extensive...someone to make sure her food was always correct, someone to sit with her and talk, feed her, make sure her tv was working, briefly read to her....just a bit of that. One of us was there every day so we know care was lacking. If you have the money , get private care. We did! So much better.

As a family member of a current resident I have spoken with the DON and ED about unsafe and inadequate nursing care without resolution. Calling CMS and the state board of nursing to report unsafe, negligent resident care.

This is one of the most poorly managed facilities you’ll encounter. Errors in administrations amount to illegal financial mismanagement; errors in medication and unsafe set up in terms of no medic alert pendants; residents left alone in showering. Looks good and kept clean but you should take a pass and keep your loved one safe elsewhere.

My aunt was at this community for her rehab services. Unfortunately they did not provide the level of care I was comfortable with. I feel they were under staffed. There is room for improvement in this community overall.

The staff were nice and they did keep the community clean. We did not have a good experience with this community. My loved one started to wonder and they called us on a Friday afternoon and told us they had to be out by Monday. We did talk to the director and found out they are not allowed to do that as they are supposed to give a thirty day notice. They did not seem to be able to care for him once he started to wonder. I would recommend this community for a more active resident.

Nice facility but rather pricey with minimal move-in specials. Only had 1BR available.

My husband was in rehab there for 3 weeks. Its a beautiful facility but that beauty hides many many problems. Once he finally did get some therapy it was very good but also very minimal. Staffers informed me that its done that way in order to use up each patients allowed days of inpatient therapy according to that pts insurance. They claim to be a "skilled nursing" facility. My husband begain to have increased breathing issues. I asked a nurse to check his lungs. She checked by laying her head on his chest! These so called "skilled" nurses also had no knowledge in how to care for a pt with a feeding tube or a pt that requires oxygen. Ultimately 911 had to be called for my husband. BEWARE before using this facility! Don't be swayed by how nice things look there.

My 87 year old Mother had a 4 week stay following a hip fracture. PT, OT, Speech Tx very good. Attractive new facility, but the building does not provide patient care. Poor staffing, unfriendly, unhelpful, sometimes rude staff. I was told once to go search for help down the other halls myself. Another time, the nurse told me she had 14 patients to medicate before she could help; she did not offer to find alternative help. I'm an RN myself and these responses by nurses was appalling. I found my Mom left in the dining room by herself two days after the hip surgery, she was trying to move herself; no one ever returned for her. Immediately post op, I was there 2 hours with her and no one checked on her, no one brought ice for her hip, they did not check on her in her room or offer her liquids, she was like a sack of potatoes left on her bed. I left messages for upper management regarding these happenings, but no response. While I was told NL staff would help with paperwork, appointment, and transportation, I did it all myself. There were activities, though I'm not sure staff made sure residents were transported to participate. Food was decent. The NPs were helpful; we did not have contact with the MD. Nursing and staffing always are the weak link in the care facilities. There is a lot of room for improvement. I would've expected excellence. I agree with review above; OK for a short stay if your loved one has good cognition and can communicate their needs.

Found facility unsatisfactory, poor staffing did not help with adjustment to their environment and forced us to to seek different care. Staff seemed unwilling to properly evaluate the specific needs of our family member. Situation could have been handle in more professional manner.

Beautiul Facility, Legacy unit has alot of improvement needed staff treats memory loss patients like bad children. Obviously they are not trainned properly the unit had 10 plus falls from August to October 2 residents with fractures staff hides they don't do there job and sometimes there is no nurse on duty just an aide with a walkie talkie in case of emergency to connect to the main building. They won't post this review because they know its true...

The Meadows is a nice comfortable nursing home. They have some awesome aids and nurses who truly care about their patients. Unfortunately based on my own experience with this place I have to call out the director and executive director whom don't seem to have patients care as their #1 priority. They make some decisions about care with out checking with the nurses who are taking care of the loved ones nor talking to the family. They appear to make decisions on the fly and not care about the consequences to their patients. I would have expected more compassion from senior management as they should set the example. Not the case there at Kalida! So if you choose Meadows of Kalida, stay involved in your loved one's care and don't let senior mgmnt persuade you into making quick decisions but take the time to set up meetings, talk with the nurses and make them explain things. Periodically review the nurses notes and learn as much about what's happening. Speak up for the ones who can't speak up for themselves!

This is a beautiful facility, but the level of care leaves a lot to be desired. My father lived in this facility for almost two weeks and was admitted to the hospital this morning. He won't be back. Medications were rarely given on time, even though they were told specific times they were to be given. The meals did not follow any sort of recommendations from dad's cardiologist, kidney or diabetic doctors. The reason he is in the hospital is because of an issue with his medication -- something the staff should have noticed or at least appeared interested in. I wouldn't recommend this facility to anyone.

My mom is there now and you cant trust the staff, the PT department is all over the place and doesn't keep the family informed - all we've ever herd is how great Moms doing. Then all of a sudden when she needs them the most they want to discharge her saying she hasn't been making the goals. We believe it's because they don't want to pay for the radiation treatments and transportation, so they're booting her. We are seeking a lawyers advise to get Mom what she deserves - fair and honest expectations. Figures lie and liars figure. The staff is immature and incompetent, always pointing fingers at each other. Heads of PT and Nursing are very unkind and hurtful to my Mom and our family. It's all about the money - period.

There are so many things wrong here I won't even bother. They are too new. The right hand has no idea what the left hand is doing.
