Sunday we took my mom from memory care downstairs and all around the facility. Everyone we ran into knew her name and was pleasant so that was nice and shows they know and care about residents.

This is where my mother is now and I would not recommend this ALF to anyone. They don't have the man power to take care of your loved ones properly. Her room smells and always a mess. They hardly ever make her bed. They took the protective cover off the mattress. Which enabled her to ruin the mattress because of her incontinence problem. They are letting it dry and allow her to lay in it. They loose her clothes in the laundry all the time. She was wearing someone else's clothes during one visit. The list goes on and on. Don't let your loved ones stay there!!!!!!

Terrible!

My mother has been there just over a month and it has been one complaint after another. I asked if she would get assistance with her insulin. They said she would and they still make her do it herself. They said they would show her around the first couple of weeks and help her familiarize herself with the place and that didn't happen. She has a incontinence problem and I asked if her garbage could be taken out daily. If not the room starts to smell and that doesn't happen. They said the food was prepared by a professional chef and that is not the case. I can go on! But, you get the point. Not a good place to place any family member!

My review of The Grande

This community looked really good, but something just did not feel right. They did an initial assessment and stated that he needed assisted living my doctor stated that he just needed a little assistance and not assisted living. They wanted us to do full pay instead of waiting on medicaid and that would not work. They also gave us a double room as a single because of how small it was but then when we moved in they charged more and we had to move.

Our Tour Experience

This place was terrible. I wouldn’t put anybody there. It was not clean, and the rooms were very institutionalized. They converted a hospital into this facility, so it felt like more of a nursing home instead of an assisted living facility.

dont trust the sales pitch

The community is understaffed and they have had issues with food preparation and they have had weekends where there was no kitchen staff. There have been times I couldn't find staff members at times. They have made promises that they have not kept and it's very frustrating. I would not recommend this community in the current state it's in.

Horrible Assisted Living Facility

Please don't send your loved ones here. My loved one is alert and oriented and able to do most things by herself. She has some issues with speaking and getting her words out and short term memory from a stroke, but she understands everything. She was on seizure medications as a precautionary measure from her stroke. I went to see her and she had a seizure in front of me. Come to find out, they ran out of her seizure medication. They advertise that they check on your loved ones every 2 hours, which is a lie. I bought cameras to keep a closer eye on her after her first seizure.The only time someone came into her room was to give her medications twice a day and housekeeping to tidy up. Another time, I kept checking on her on the cameras because I was working overnight. I checked on her around 2 AM and she was on the floor. I called the front desk and nurses station and no one answered. I was about to call 911 myself but I had someone's cell number and they got through. They said someone must have been on the other line on a personal call and that's why they weren't picking up. They checked on her and literally debated whether to call 911. I had to come over the camera and tell them to call 911 now. They also say, they are able to track your loved one anywhere in the building if they are wearing their emergency necklace. We went to pick her up one day and couldn't find her. She wasn't in her room or in the dining room. We asked the desk and they said "Oh, wow it looks like she hit her emergency button an hour ago" Immediately, we are worried thinking she fell somewhere. We ask them where they have her located, and the person responds that she doesn't know how to use the tracking system. This started an hour long search for her through the entire 13 floors. They were about to call the police to file a missing persons report. Luckily, she was found and she hit it by accident. I would take her out for the day and ask for her medications in case we got back late. Half the time, they were out of her aspirin or cholesterol meds. They would say we have them ordered. I spoke with the head nurse multiple times to fix these issues. He said they are supposed to order the meds 2 weeks in advance which wasn't happening. She had another seizure because they ran out of her seizure medication. I watched on the camera after the face and she seized, fell into the dresser and hit her face. Her whole face is bloody and she crawls to the door for help. No one is to be found in the hallway. She then crawls and goes to sit in the chair. The housekeeping lady found her with her fall all bloody. Another time, her blood pressure had been running high and her dr. wanted it checked three times a week. I thought it would be no problem since they have a nurse on staff. I asked the nurse what her blood pressure was today and she told me 190's/80's. This is extremely high and she had already had a hemorrhagic stroke so I was concerned. I asked the RN to take her blood pressure again and she handed me the cuff and said you're a nurse, you can probably take it better than me. I took it and it was high again. I asked her if she can get an additional blood pressure med ordered because her Dr. didn't have any openings that day. She said "we don't call the on call doctors." I said I'm a nurse, and you are an assisted living...someone has to be on call. Sure enough, she called and got an additional dose of blood pressure meds. I called her on the phone on day, and she told me there were bugs in her room. I called maintenance and had them take a look and they said they didn't see anything. I go to investigate myself and her bed is covered in bed bugs. She most certainly did not come in with them. The maintenance guy said she got them from a lady down the hall that she would dine with occasionally. We had to throw out her mattress, couch, recliner and they still couldn't get rid of them. We basically had to throw out all of her items when we moved her out to get rid of them.

If you,or a love one, are thinking about placement here, rethink your options. This place is "the pits". Reasons: 1. The small elevator, to accommodate over 150 residents, is out of service for a week. If you can, you must take the steps. 2. Never see anybody cleaning. 3. The have one (quasi) maintenance man. 4. The corridor rugs are stained and never vacuumed. 5. The food is terrible. 6. No activities-unless you like bingo 7. Roaches in room 8. Not worth half the outrageous cost of $2100.00/month.

Too Bad, So Sad

There was roaches everywhere kitchen, halls, bedroom. The aids would be outside smoking or in the med room with door closed singing and listening to headphones at night. They gave my sister a call necklace that was not programmed into their system. She fell and laid on the floor for 2 hours until an outside CNA came to visit and found her on the floor. The floors are vinyl and very slippery. It took over 3 weeks for support bars to be place in the bathroom for her to use. The bathroom sink cabinets are square corners and she tore her skin numerous times. She fell 4 times while she was there. After being there for 3 months, of which 6 weeks were in rehab for the falls, I moved her out and will never recommend this facility to anyone!

Bed bug infestation

Do not move your loved one here unless you don't mind bed bugs. This place is infested with them and when they treat for them they are back in 4 months. My Mom has only been here since [date removed] and we have dealt with bed bugs twice now. Bought new furniture after first time and now have them again. It has been over a week and they have 't moved her or treated for them. Stay away if you know what is good for you.

Sadly this was not the safe place we hoped it would be. We had many issues: very little communication, problems with medications being given safely,and rude treatment of us as concerned family members. Our loved one suffered 3 falls, one of which resulted in a broken clavicle and another -a concussion/brain bleed. We just cannot recommend Spring Haven.

Don’t leave your loved ones at the Oaks of Clearwater

The worst place to put your loved one. Memory care unit stinks of urine . Bruises on my mother numerous times in the year and a half she was there! They have staff over turning every month to three months. They lost / stolen her clothes three times. I mean 90% if her clothes! I have talked to administration numerous times. I was also yelled at by their nurse manny, that I was a liar and the bruises came from other residents not anyone that worked there!! Didn’t even look into it! They covered up abuse that their own nurses relayed to me that in the past aides did do that!

I will never take my loved one here again. This place was just awful and the staff was not nice and did not help with much of anything. The food was bad and not prepared properly. They just didn't seem to have all there stuff together. I would not recommend them to anyone ever.

Beware

I am a single woman with no children. I suffer with a disability that includes being nearly totally deaf. I have an aging mother. At the beginning of 2015 it became apparent that with my own health declining in certain areas and my mother’s declining health, I needed help to care for both myself and my mother. As the progression of things occurred my mother was placed at The Grande, 725 DeSoto Avenue in Brooksville, Florida 34501 within the Independent Care area. During my first visit to my mother there was an incident at 10:20 pm on July 21st of this year. I was just returning to my mother’s apartment when one of the Residents was stressfully calling me to come quick. He was pointing to someone in the Lobby where the elevators are located. There was a man on the floor at the elevators crumpled with his legs under him crooked. His hands were in the bottom railing in the door of the elevator. He was out of consciousness when I got to him. I spoke and asked if he was okay. He did not respond right away. I told him he needs to move his hands in case the door to the elevator opened. He was so weak he couldn’t move. I finally was able to get him a bit away from the elevator door and had him leaning on his walker seat. His name is [removed] who passed away on August 8, 2015. There was NO ONE at the front desk. NO ONE in case of emergency. NO emergency information located at the front desk. I called 911. The emergency response came from the Brooksville Fire Department located at 85 Veterans Ave. Phone number [removed]. They are 2 minutes away from The Grande. When they arrived they were confused and alarmed as to why The Grande has NO ONE posted at the front desk for such emergencies. They finally had to walk over to the Assisted Living area to find someone. Needless to say, these emergency response personnel were extremely concerned about the lack of staff at such a vulnerable time for our precious seniors. My statement was taken at The Grande in regard to this incident. I was informed that there is staff from 8:00 am to 10:00 pm. Unacceptable. I inquired the next day regarding the lack of staff. I also wanted the contact information for the owner of this facility. There was no information at the front desk. I was told that the information was removed some time ago. This is the owner of this facility: Holiday Retirement 5885 Meadows Road Suite 500 Lake Oswego, OR 97035 [removed] [removed] I wrote a detailed letter to them presenting them with issues that I observed while visiting my mother; issues including Security, Nutrition and Medical Emergencies. Their response ‘…While some things are easy to improve other areas may take longer…’ Yes, they have improved the front desk staffing from 8:00 am until 5:00 am. What about the lack of staffing between the hours of 5:00 am until 8:00 am? One resident’s family member writes: Just this morning, (October 25, 2015) my mom lost her balance and slipped to the floor. She now wears an Alert 1 button around her neck so she can call for help when needed. When Alert 1 people called, no one was at the front desk. It is not manned from 5:00 a.m. until either 8 or 9:00 a.m. on weekends. The folks who rushed to mom’s aid were three nurses from the assisted living side of the building. The big problem was that they were unable to find the master key to gain entry to my mom's apartment rendering her helpless lying on the floor for an unnecessary additional 15 minutes. I have befriended a few of the resident’s families. We talk, we share what our mother’s are going through and we cry with each other. We are working together in this journey to make The Grande all that they promise on their website and especially within their Mission Statement: We are in the business of providing security, comfort and value to seniors seeking an independent and fulfilling lifestyle. We provide the holiday touch. This is not rocket science. This is CARING for our disabled and aging Senior Citizens. Citizens of our great country who admire and respect our Seniors. But the sad truth is the fact that The Grande is in desperate need of help on its own. We held a meeting to discuss our Complaints/Concerns/Comments on October 15, 2015 in the Social Room of The Grande. Attendees included three Residents, four (including me) family members, the Executive Chef, the Senior Regional Chef and The Grande’s Executive Director. We left with hopeful optimism that our Complaints and Concerns would be handled in a swift manner. I am saddened to report that there are continuing issues. Let’s begin with their Sales tactics. One resident’s family member writes: My mom moved into The Grande nearly six months ago. Prior to signing on the dotted line, we were promised some things that just were not true. We were told mom would be transported to her doctor, even after we informed the salesperson that her doctor was in Pasco Cty. After moving in, we learned the transport only incorporates Hernando Cty. It cost us $150 for a private transport to get mom to her doctor. We were not made aware that there are doctors/nurses/podiatrists/phlebotomists who actually come to the facility until just yesterday, October 24, 2015. We were told that nightly bed checks were done on every resident. We recently found that was not the case unless you request it and then you are charged for each visit. A few weeks ago, my mom slipped onto the floor from her bed and was unable to get back up. This happened sometime after 11:30 p.m. If a bed check had been done, mom would not have spent 9 hours on the floor resulting in a two week hospital/rehab stay with severe cellulitis in her arms and legs. Let’s now look at their Nutrition. I’ve eaten their food. It’s mainly disgusting. On Saturday, October 17, 2015 my mother and I ordered the sloppy joe’s and peas from that day’s menu. The plate was cold and the food was slightly warm. My mother’s food was the same. We asked for hot food. Food was returned. My mother’s came back with a green bug crawling off of her plate. It was not known from where it came, according to the chef of the day. My mother is mainly eating hamburgers or hot dogs because she does not find the menu items appealing at times. Extremely poor nutrition. Recently I heard tales of the peanut butter and jelly sandwiches that some ordered had a small amount of peanut butter and a small amount of jelly between two pieces of bread. Unacceptable. One resident’s family member writes: It is ‘hit and miss’ with the meals. Another resident’s family member writes: The quality of the food is low. Meats are usually over cooked and too tough to cut with a knife. Vegetables are over cooked and mushy. Lettuce salad is never made available. When mom asks for one (as instructed by the head chef to do so) she is always told they have no lettuce. Menu choices are ignored or overlooked. Often an item that a resident chooses does not make it to their plate. Whoever fills each plate order is not paying attention to details of each resident's choices. More often than not, the food arrives cold. And a resident must wait at least 45-75 minutes to be served once a menu is picked up. On two separate occasions, my mother's dinner was completely forgotten. When asked where her meal was, she has been told she never handed in her menu and there is nothing left and is offered a piece of heated up pizza. Mom has type 2 diabetes and all that bread would put her blood sugar at dangerous levels AND her menu was filled out and picked up. I know because I was present. Kitchen staff misplaced it and tried to put the blame on mom. One evening, the only thing residents were served was a 'Waldorf' salad. It was void of any dressing. It was a Sunday night. More than a dozen residents walked out disgusted and hungry. My mother called me soon after with her complaint. From my cell phone, I was able to contact Amanda who was in charge that evening. She admitted to me that she had to "step it up", that her dinner that night was

Very bad experience

This place was a nightmare. The individual caregivers were friendly people, but this place had a real problem with communication, across the board. The staff did not communicate well with each other, when shifts would change tasks would be left half finished. Communicating with the management was impossible. I had multiple issues with billing, and I would leave messages trying to get my problem resolved and would never hear back, or just be passed around between phone operators. We were billed for extra months after we left that we never received word back about, or reimbursement for. At one point they switched medication providers mid month, and we were only provided with half of the medication we needed, and couldn't get information from anyone on how to get the medication for the second half of the month. This company was in the middle of transitioning to new owners, which may explain some of this, but it was still completely unacceptable behavior and I would not recommend anyone place a loved one here.
