They were not prepared for us. Did not know there was a scheduled appointment. Did not meet with director. Does not matter. Not a good fit for us. She said it looked like a funeral home. I say, nice place but I would not want to live there. Even a person w/o short term memory problems could get lost.

Close to family. 3 levels. Neighborhood would need Aid & assist before long.

Actually, after speaking with [name removed] (director) and [name removed] (medical) my rating is now 2-3. My mother has been paying for additional services (almost the most that can be paid) and she has fallen 3 times in less than a 30 day period. She moved in [date removed]. We are now seeking other alternatives for my mom.

Overall, [name removed] the gal who helped us with the whole move in process was very very good. She really worked with us and went above and beyond to make the stay a good one. Unfortunately, one person cannot make the whole facility great. I was not very pleased with the medical staff, [name removed] was great but he did not work every shift. We had some issues with the other staff, they just did not seem to care as much as they should have.

My loved one was able to participate in more activities they enjoyed. My loved one fell during their stay at Ashbrook Village. Ashbrook ended up having to send my loved one to the hospital. The community charged us for two months, even though my loved one wasn't there for that entire period. Ashbrook Village had good food, and nursing aides.

We know our loved one can be difficult at times. Suffering w/dementia, stubbornness, & almost a century in age, makes for a handful of management. Our loved one entered The Haven @ The Summit w/assistance of a walker (due to the long walk). When we picked him up after 17 days in respite, he had to be rolled out in a wheelchair. He could not walk nor stand on his own. The charge nurse did not give a report on his condition upon discharge. She handed us his bag of meds & said goodbye. A custodial worker (w/double-duty responsibilities) found a wheelchair while we packed his belongings. The custodial worker also helped get him into the car. Once home, a male neighbor had to lift him out-of-the-car into his wheelchair which he’d never used. He was bed bound until doctor prescribed weeks of home physical therapy. We also noticed weight loss. After being home for nine-10 weeks, he now walks throughout the house w/out the use of his wheelchair or walker. He regained his weight. Before being admitted to The Haven, we decided to let him go back to stay after this respite period ended. That did not happen! We were not pleased w/his care. The facility (building & grounds) seemed clean, but the memory care was disappointing. They seemed short staffed. Initial meetings & touring were great sale jobs.

The location is near my home, about 5.2 miles away. This review is in regards to the memory care section. The outside looks very good but I was not able to go back and try out a meal prior to placing her in this location. I understand why after seeing the meals. I have been in a couple medicaid only facilities and the meals are better there. I had a major issue getting the medication she is to take straightened out. After taking in a heating pad initially, it was taken away as there was not an order. I did get an order and they have not found the heating pad yet. I will probable take in another one. There is a lack of discipline among the workers as they play their music and they congregate together and not checking on others who are in their room. The place is clean. There is an activities chart on the wall but all the times I have been in there the only activity is music and trying to get the residences to also sing along. The TV is going all the time.

really disappointed in the service provided. Cost was far more than initially quoted.

My husband was moved into Brookdale on Wednesday March 1 2017. On my visit today Saturday I noticed that none of the staff was around on the floor when I was ready to leave. Finally a young lady came out of one of the rooms and let me out and people waiting to get in. There was no supervision on the floor where most of the people were sitting. I wanted to ask a few questions but no one was around after lunch was over.

I have lived at the Village of Southlake for over 5 years. It used to be a wonderful place but it has changed in the last 2 yrs since new ownership. Many of the food servers quit & that leaves only 1 server per meal for over 70 residents & that means meals can take over 2 hours. They shortened their workweek to below 40 hours & that means no one gets benefits. The morale at this place is very low among all of the residents & staff. It has caused a lot of stress & anger due to them changing the rules of what we can/cannot do & we have no say. The food quality has gone way down because the new company gets its food from out of state, frozen & it’s not local like they advertise. They have taken away many things which were included initially in our contracts & now charge extra/double for things that used to be included. Living here for everyone has become very disheartening & stressful. At our age we should be able to live in a peaceful environment & this isn’t it.

It's all smoke and mirrors. I do not recommend The Colonnade at Northdale.

There has never been a hair salon nor a gym appropriate for seniors. Transportation is limited and hard to get. But the quality of the food made up for all the shortcomings. But that changed. It no longer feels like we're living in a fine restaurant. With a change in chefs, things are very different. The dining is not available all day. It's open for three meals, but there are long periods where it's closed. Good luck finding anything healthy. The menu was drastically reduced so it is difficult to find a healthy choice if you are sick of pasta and rice. The wait for your food can be up to an hour and a half. Sometimes we just walk out and leave the food behind. It's usually not worth waiting for anyway. The quality is just awful. The presentation is no longer beautiful. Our celebrations used to be spectacular buffets with many choices of entrées and salads and desserts. The last few celebrations were just a plate of food. The last one we didn't even have a choice of entrées. Just pasta . If you live here, it would be a good idea to supplement your meals with some more protein. Be prepared to live on fried food and pasta. The salads have very little protein and we never get real lettuce - what can only be called lawn, clippings, or tiny trees. At this time, I cannot recommend this place. Check back in a few months. Things can change. We still have perfect location, beautiful grounds, a beautiful building, good maintenance and wonderful residents. The physical therapy team adds an energy and love that can't be beat and frankly keeps a lot of us from giving up on this place.

I would not want to send anyone I cared about to this place. It is severely lacking in all ways. Couldn't wait to get my husband out of here.

Staff- Told everything was All-Inclusive but it wasn't. Different costs were given to my sisters than were told to me. Activities- All that I ever saw was them coloring. The rest of the time they were sitting in the area where the television was. They weren't allowed back in their rooms until bedtime. Food- 2, not at all the way that we were told. One night they had some kind of patty that I had no idea what it was and [removed name] didn't even eat it or know what it was. One night 2 of my sisters were there and she had a desert compote dish of chili and a package of 2 crackers and a piece of cake. Room-2 because the size was alright but when my Mom flushed the commode water ran all over the floor AND human waste came out in her shower. It took two days for it to be fixed. Her new clothes were washed with towels and ruined them. Overall, things were not as we were told. The tours were totally STAGED to make the sell. My sisters were told to get our Mom out of there as fast as we could by two different people, not staff.I know that we may not be your run of the mill family. That's because we were raised by wonderful loving parents and we expect the best for our Mom. That place is just not for our Mom. I'm sure that this is not what you wanted to hear but it truly was not a good experience. Two more things I need to add- 1- The day that my sister and niece went to get our Mom. They asked for Mom's medicine and she told them to wait for her to go check on it. They waited for 45 minutes and she didn't come back. She was giving another tour. My niece did act up after they found out that Mom had been out of her medicine for 3-4 days and it hadn't even been ordered. 2- we were told up front about the Community Fee and paid it. My sister has now received a bill with a Move-In Fee on it for [removed rate] and we Were Not told anything about that!!!

The staff are very nice but quite honestly there are not enough people employed to properly take care of 30 memory care patients. Our loved one was not getting baths, hair washed, laundry done each week. We are visiting regularly and know from day to day what is taken care of. She went from daily showers at home to one in 8 days the 1st week. She was not refusing them either. We finally went down to nurses station after many days of requests and a very nice tech came down and took her in the bathroom, showered and washed her hair, and changed her linens and took her laundry to be done. She felt so much better and it immediately changed her mood. It's been distressing to say the least to worry about her basic needs being met. They all say she is very sweet. And I really like the staff but it is unfair to have the workload they have daily for 2 or 3 people. The food is okay, no complaints. There was an incident where our loved one became very afraid of another resident who pushed her ?? this resident is known for bullying patients and the staff but not sure what can legally be done about that. We are just going every day possible and will continue to address these things and hope and pray they get more staff. At this point we could not recommend Westhill, but hopefully that will change.

The sales director seemed very uninterested in my mother living in the community. She was very rude and seemed like she couldn't wait until we left. The other staff were very kind. The person who makes a first impression should not be so unfriendly and put off if she's trying to sale for the facility.

I spoke to my mother and lunch was wonderful as was dinner and the live music at their happy hour! Communication is my only real complaint. The independent villas are lovely, well appointed and very clean. Yard maintenance during construction on Hwy 9 has declined.

The facility is new so presents well but it is not managed well. We have had issues with meds running out and med mistakes. My family member has to ask for a shower instead of being on a regular schedule. Community activities are regularly cancelled. Admin does not respond to requests for meetings. I would recommend staying away from this facility until there are changes made. There is no accountability and lots of excuses made. We are sorry that we chose this facility. They make promises they do not keep. I feel so guilty that I chose this facility.

I like the décor and the friendly residents. I don't like the harshness of the staff and there has been occasions where they run out of medication that is vital, missed dodes should not happen but they do. They also leave incontinent residents wet for long periods or leave them on the toilet for very long periods of time. I would not recommend this place. It's way too expensive for the care that's received.

My grandparents moved here a few months ago and things couldn't have seemed better. The tour and initial meetings went fantastic and everyone was so friendly and understanding of their needs. Unfortunately, it was a completely different story barely a week into them living there. Aids weren't giving them their medicine, giving bathing assistance, or even being around when an emergency button was pressed. The former head of the aids assured us that things were changing, only for her to be fired a few weeks later. Our family is currently looking into rehousing them because we were told by new management "they shouldn't have been accepted here based on the amount of care they need." We were deceived the entire onboarding process as to what kind of care they would be receiving. Major red flag for anyone looking to move a loved one here.
