If you have a choice (and I did not), I strongly suggest against choosing Chehalem Post Acute in Newberg. Communication is severely lacking. Attempts to call the facility result in lengthy wait times with unpleasant hold music. Calls to staff frequently go unanswered and are rarely returned. My father was a resident for three weeks before anyone gave us an update, and that conversation was partly to inform us he was being discharged the next day. Upon my father’s transfer to his assisted-living facility, he arrived wearing unrecognizable clothing. Every item of his personal attire and belongings, aside from his underwear and the shoes he was wearing, was stolen. My father now exhibits definite signs of trauma following his stay at Chehalem Post Acute in Newberg. ——— The following is a critique from the resident's son: ——— As I am certain many people find themselves in the same position each day, I will provide my, personal, experiential notes. The facility, Chehalem Post-Acute Rehabilitation, while providing a “post-acute” location to be transferred to, it is not a facility which I would wish to be transferred. Nor, did I want to leave my father there at all, however, I simply had no other choice. To be blunt, it is not a place of recovery and improvement. It is more a place of human storage. I do not want to make it sound as if the staff do not care, or are not professional. However, overall the facility and its operation is severely lacking appropriate organization (policy, standards, procedures, processes, and review). My personal experiences while at the location, and within the facility are ones of chaos, lacking orchestration, and flirting with the edge of outside limits of compliance. This facility would absolutely benefit an internal or state audit, providing the facility Director and leadership a detailed Gap analysis of current business operations, communications information tracking, patient care, and medical documentation/operations procedure guide. There should be ongoing Gap remediation and a plan of action in place currently to prove compliance upon request by regulatory authorities. (i.e.; HIPPA, Oregon State statutes and healthcare facilities).

1. Terrible 2. Less than needed. From the time of move in the cupboards had old food crumbs in all the cupboards 3. 20% friendly, 80% "not my job". 4. Very little help available or offered. 5. Distrusting of what they say and then see what they actually do. 6. Awful. Common foods were not identifiable. How does one destroy asparagus? 7. Didn't participate. 8. Complete waste of money.

Totally misrepresented initially. Low number of workers. Very rude and dishonest administrator. Things have leveled out finally but I was told I was going to be included in monthly reviews. Not. I was told her fees would be $[removed]+ for level 6+ care. Her bill was $[removed] plus. I put calls in to admin, marketing dept and finance dept. never returned my calls. Not one to this day. Very disappointed. But… have nowhere else to take her. [name removed] was really having a hard time w the transition and upset quite a bit initially - she just had to sell her home, car and had lost all of her savings. A real lack of professionalism and compassion for her . Initially she only got 2 showers in 2 1/2 weeks and is incontinent. They kept sending in a very young man to do her showers which she wasn’t comfortable with… and when she refused they just didn’t do one for her. Incredible experience. Things seem to be a bit better now. I have to make this work. It’s been a terrible experience however getting here.

They refused to pick my family member up from the emergency room, repeatedly. Bland food. There are much better places for less money. They raised the rent considerably so we got out of there. Don’t bother with this place unless you have No other options.

The executive director and RN are new to West Hills as of early 2023. While there are some very sweet, helpful patient care aides, the overall situation at West Hills is extremely dysfunctional. Both of my parents, 89 and 91, live here. They do not feel management cares or even sees them. A resident told me she feels like she is treated like an animal here. Another one, who is wheelchair bound, said that one needs to be able to take care of themself to live here. While advertised as "restaurant-style dining," the quality of the food and sanitation of the dining area fall way short of what anyone would accept at a restaurant. There has been no bus service for shopping and no exercise classes for months. The place is extremely understaffed. Call wait times for assistance may be anything from fairly immediate to 45 minutes--and sometimes there is no response at all. Residents are charged a fortune for terrible care. My siblings and I are supplementing my parents' care heavily.

We were not impressed with Prestige Senior Living. The building is very old and they were trying to do some remodels, but it still felt dark and dingy to us. The presentation was not good, it was a depressing environment.

Oh my gosh... where do I start? Patient was brought to facility, weighed, spoke to briefly, and then left in the room in a wheel chair for .several hours with instructions to not get out of the chair. She was unable to get anyone's attention for the bathroom, so eventually made her way by herself. The room she was placed in was bland and worn out - inexpensive bookcase/cabinet in the room had doors broken off. She called us, crying, and wanting to leave. Only when the staff heard we were going to remove her did they go in and talk with her....to have her fill out forms for admission. One individual was very compassionate to her, but when we came to get her, the other staff seemed cold and distant. Horrible experience.

I was appalled by the things I witnessed while my Mother was there! Residents left in dining area for at least half an hour before someone was notified and escorted back to their rooms. If they were lucky to get to meals, which many residents would have missed, was told it is assisted living. we are not responsible to get them to meals. Staff many times all on break together,including Office Manager, Med Techs, both on staff, and Caregivers. I witnessed this personally during one of my visits. I couldn't find a staff member and was told they were in the back smoking.

We hope that when it comes time to put our loved ones in a care facility they will be well taken care of by people who care as much as family. Especially for the price they have to pay. We recently had a situation where my mother had a major surgery scheduled. She is the primary caregiver for my step-father and needed to get some respite care for him. She looked at several facilities and proceeded with Prestige Senior Living High Desert. She was well organized for her husband’s drop off. She had all the items he would need for the time he would be there. She had written down his schedule that included what he liked to do, bed time procedure and medications. She reviewed with the Prestige team the week prior with the head of the facility and again with the intake person. You would think that would be enough. Well she dropped him off and although the room was clean the one thing that was missing was a T.V. Her husband loves to watch TV. It was in the agreement, but they failed to have it hooked up and couldn’t get anyone to do it until Monday. After that, things continued to go downhill. She went back to visit the next day. Her husband was still in the same clothes he was dropped off in, hadn’t been taken to the restroom or bathed. They also hadn’t given him all of his medication. My mother was extremely upset and placed several calls into the manager of the facility. Later that day, I went to see my step-father. I stopped at the front desk and waited for a few minutes before somebody came out to help me. When I asked where my step-father was, they had to text all the workers in the facility to find out what room he was in. My heart sank. Really, this is how these facilities are treating our elders? We put them in places to help us, hoping they love their jobs and will love our family members, but instead I feel many assisted living facilities just to gain high profits of their resident’s money, barely providing basic care. The employees that are hired are just bodies that don’t want to be there. It’s truly heart breaking. My mother pulled him from Prestige and cancelled a major surgery in order to take care of herself to continue to care for her husband. I do not recommend Prestige Senior Living High Desert because of this experience. If they are doing this with respite care members what are they doing with the full time resident?

My loved one leaves and is gone days at a time and he needs to stay there. My loved one is not getting his meds. It's dirty around the bed. It’s not mopped. He smears food on his bed. Allow him to hoard food.

Everyone I’ve talked to is nice. The way rooms are set up and the food is not good. The rooms small and shared. They have never contacted me.

My mom was a resident at Prestige Orchard Heights 2 1/2 months. She became Ill and required hospitalization and, after that, further care in a nursing facility. She was scheduled to discharge back to Orchard Heights but on the day of her discharge, the director there informed us there was no longer room for her. In other words, we were misled and my mother was being discharged with no where to live. This left me scrambling to figure out what to do. The director NEVER contacted me or let me know that this outcome was a possibility. Thankfully, the staff at the nursing facility offered to help with making other arrangements. In my opinion, Orchard Heights is being run by very unprofessional, uncaring people. What other explanation is there when a frail, 90 year old woman is turned away without any notification. My mom is going to be ok-no thanks to the staff at Orchard Heights. Stay away from this place. Their main goal is getting as much money as they can. The residents mean NOTHING to them.

Prestige West Hills had consistently poor staffing. Medication errors were made repeatedly. Housekeeping service was frequently not provided. The care expectations of the service plan were not met. Services were charged but not provided. Generally misleading, inadequate, unsafe, substandard care.

There were concerns. They overestimated their abilities, to take care of memory care. They were just training their people while we were there, to be able to care for memory care. We would find her not changed, and a bowl of oatmeal in front her. They never once cleaned her dentures, even after repeatedly asking them to do so. Eventually a family member found her on the floor with a broken hip. I would not recommend anyone to go here.

Had a confirmed appointment. When I showed up for the tour Beth was not there. The receptionist phoned Beth and she was to call me back. Didn't happen. Poor communication no follow-up. Wouldn't have my mother there.

When mom lived there she never participated in anything. Her room back at the end of the hall and only came out for meals. The rest of the time she was sitting in bed watching TV. My mom lived there for years. She moved in when my dad was still alive and he passed away and so she was there by herself. There seemed to be an extreme amount of turnover in the staff there. When she first went in there I thought it was good but I didn't really have anything to compare it to. It just got to where her room was never clean and she wasn't clean so it was not good at all. I really felt like when I would call about things that they weren't being truthful with me. I really can't say anything good about them.

My mom was at Monticello park in Longview for a very short time less than a month and passed away the end of July a beautiful place but not very happy with her care while she was there

Please do not place your loved one here. My mother was ignored to death. She died due to dehydration. The staff complained about her diarrhea as if she were a naughty child. Instead of trying to resolve this. My mother did not feel thirst so she did not remember to drink. She also was nearly blind and could not tell the difference between water or lotion bottles. The supervising nurse would not let her have more than one soda a day. A soda she would drink for pleasure so my family provided her favorite diet soda for her. We understand soda is not the best but it is better than nothing. Because my mom liked Dr. Pepper the supervising nurse Mary said it was causing her diarrhea because it is just prune juice. There is no prune or any other juice in Dr. Pepper but this woman in my opinion let my mother die because she didn't want to change her ... For a lack of a better term ... Adult diaper. And little activities. I tried to get pet theory for the whole site and they didn't even try to follow up.

Horrible, charge for services that are not provided. We first moved our love one in and everything seemed fine the major staff changes (upper management) brought the roof down. Poor food senior staff that were untruthful and let me tell you we were charged for services not provided. No toilet paper in the bathroom shower curtain broken and not fixed for a month until we notified staff of the problem. You are paying for sub standard living. Rental Equipment lost and you are stuck with the bill $$$$. Sub Standard food (canned even when in season) filthy dining room (food on table and floors hours after service). We were told that the room was monitored if the door opened well we opened and closed the door for an hour and guess what no one showed up this was done on several occasions with the same result.

At first we felt this was a great place. Light open dining and suites. It didn't take long for everything to change. Our parent was charged for services that were not given, the place had negative vibes and even basics like cleaning etc. went down hill. Management was/is not honest and there was no regard for our parent. They lost her wheelchair, took over a month to fix her broken shower rod holder and I could go on and on. The damage done to our elderly parent because of OH can never be undone. It is SO SAD that people, who are in the business of caring for elderly people in need, DO NOT CARE. This is largely a review of those in charge, not necessarily the one on one care givers. Don't be taken advantage of because your family is in a desperate situation. Your loved one deserves better. And this place is the worst. I'm sorry, but it's true.
