Night mare

Level care facility. My mom needed the highest care so we paid for it extra [removed]a month. There was so many issues in the 5 weeks that she has lived there. Gave our 30 day notice [date removed] She is now in rehad center and will not be returning there to live. Most of all the rudest of some of the workers. The nurses were great with my mom it was the aides and dietary employees. I don't need to go into details except to say. I would not recommend this place unless your love one can be independent walking, pouring milk, has good mobility .

Be careful they will tell you what you want to hear to get you in there then go back on everything they said. Food is all served cold. They will send off a resident without even contacting poa involved. My aunt was there for a total of 10 days had to pay a full month. Then they wouldn't let her back.

Our Experience

My husband was a resident at Seasons at Southpoint for a very short time. Worst place ever! The nurses didn't undress him for a night sleep and didn't help him with the bathroom. I was told before I brought my husband to them that he would be allowed to use a commode to go to the bathroom, but the nurses told me that he was supposed to go to the bathroom himself. He couldn't even walk by himself and ended up soiling himself. The nurses also didn't wear masks or gloves when they distributed medication. One of them had very long nails and couldn't pick the medications up with her fingers. The food was so bad that my husband didn't eat it. My husband also didn't receive regular insulin shots and after a few days at this facility, he was taken to the emergency room with blood sugar of 460! I decided to bring him home after that, rather than sending him back to Seasons. We opted to use Homecare services instead. The bookkeeper of this place didn't submit the correct paperwork to the Long Term Insurance and the insurance didn't cover the amount of $[removed] for the month and $[removed] for the room. Altogether $[removed] that nobody returned to us.

In general, the staff has been pleasant and helpful. The departments my loved one has had an issue with are maintenance, food service and general management. The food is "absolutely abysmal" and nearly inedible. The waitstaff is short staffed, not knowledgeable, not helpful and rarely available. The food is not good quality, not well prepared, literally inedible at times (ie: meat too tough), and they run out of a lot of menu items. The silverware and dishes aren't clean. The Lafayette certainly knows how to sell itself and gives an excellent first impression. No red flags were raised. We understood there would be an adjustment period and not everything would be perfect. However, once my loved one moved in, it all went downhill. The move in process itself was easy though. We would not recommend this place at all and are actively looking for another facility. One positive note, the transportation department is amazing, helpful, on top of things, and very pleasant.

My loved one was previously a resident at The Phoenix at Dunwoody and our overall experience with this facility was terrible. I do not feel like the the salesperson that we worked with was truthful with us. In fact, I think the salesperson was intentionally hiding information. Again, not a good experience. I would not recommend The Phoenix at Dunwoody to others.

Do not go to Phoenix.

Overall experience was terrible. My wife died [date removed]. The Phoenix did not cause her death but surely contributed. The cleanliness was ok. The friendliness of the staff was awful. They lie and do not care for the residents. The quality of care was poor. Lack of training and motivation. Meals and dining service was ok. With the activities there was little participation so cannot judge. Nice brick and mortar but the care is terrible. We left after 6 weeks and would not recommend this facility to anyone.

Saddened and disappointed

As someone in the business industry for over 20 years, I am well aware that strong businesses are built on a firm foundation of culture, communication and partnerships. That should be even more so the case when the business is based on the care of a fragile population such as memory care patients. That is why I am shocked and disappointed by the poor leadership of the Management of the Phoenix of James Creek. We were forced to deal with a series of bad decisions, untruths, and excuses rather than ownership, accountability and two-way communication. Good leadership is hard; however, when it is for your loved one, it is required. For this reason, I cannot recommend this facility at this time. Hopeful it will change.

Haunted by the experience

I would give a negative value if I could. Traumatic and one of the worst experiences and memories of my life. Do not send anyone here. I feel like we were pressured into joining so we could get the rate my aunt could afford. She had signs of dementia but the facility tested her and said she could live on her own vs memory care. Her health quickly declined. She was stable and doing well the last 4 years. She moved in around [date removed] and passed away [date removed]. My mom visited every week but because of work hours could only visit late at night and on weekends. It could take an hour to get in and out of the facility which was very inconvenient. The day my aunt died my mom received a call from a lady saying to come up to Villa palazzo because my aunt wasn't breathing but the ambulance was there. My mom waited an hour outside the room. I met my mom. We waited outside until we were told we could go in. And when we walked in my aunt was laying dead on the floor. It was an awful last memory for my mother. We haven't even heard from the facility after this happened. We were rushed to find a funeral home to come pickup the body. It was absolutely horrible.

Beautiful facility, problematic care

When choosing a memory care location for my father I depended on reviews and was persuaded by them and the friendliness of the sales and front office staff. However, our experience once we moved in was a nightmare. We moved our father after two weeks and have photos of the drastic decline he had in two weeks there with falls he had not had before, medication errors, poor communication, unnecessary ER trips, lack of programming, etc. It it a beautiful place with large rooms, and some nice staff; however, with the large number of outside staff there is a lack of communication between shifts that damages care. We moved him to Vitality at Milton which offered much more personalized care, effective med control, and much better communication. I just wish I could have avoided the awful two weeks my father had and hope to spare another family.

Traumatizing

Appearances can be deceiving. Don’t be deceived. Unclean residents, poor nutrition, your loved one’s clothing being worn by other residents, unclean rooms and especially bathrooms,, little assistance with feeding, soiled bedding left to be handled by family, horrible communication, no empathy, empty promises, high turnover at all levels, horrible resident-to-staff ratio. I could go on. My experience with a parent there was horrible and traumatizing. The lack of transparency and basic decency towards your fellow human is hard to grasp. Please keep your loved one at home as long as possible before putting them in any assisted living facility. My heart broke every time I visited seeing how my parent was treated.

Memory care manager is rude and sarcastic. Everytime I come to the facility I hear her talking about someone every day to other employees. She is rude to employees and family members, this is probably why they can’t keep staff. The managers up front seem to allow bare minimum behavior from workers, when you walk into the facility everyone is on their phones, just sitting. While family members are drenched in urine and waste. You voice complaints and they are never addressed. The meals are nothing for the amount you pay to keep your family here. The carpets are disgusting and smell of urine and waste. The house keepers don’t clean anything. Never toilet paper and paper towels I just wouldn’t recommend.

I have moved my father into his third assisted living and this facility was the absolute worst facility I have had the unpleasant opportunity to deal with. The food is disgusting and difficult to identify. We had to pay a large facility fee; however, there was no heat in my fathers apartment for around 10 days. I had to personally provide a adequate heater. The door lock did not work for 5 months. The management staff is allowed to be deceitful and will not keep you informed about your family member. When I addressed issues with management I was treated with hostility and unprofessionalism. I finally had the opportunity to meet with two directors and a corporate employee who was rude and crass. I was told i was not allowed to discuss “the past” which was 5-7 weeks later due to a death. I would truly not waste my time with this facility. Expensive and unprofessional.

Bad Bad and if I did not say it loud enough Bad

Please notice that all the good reviews of this place were given over 5 years ago when it was owned by a different group. Since then it has gone downhill drastically. We put my mother in Phoenix and she stayed for only one year. They did not take care of her and the staffing was if not illegal it was definitely complete negligence. There were times when we went looking for a staff member and the only person we could find in the building was on the front desk. Often my mother who needed assistance was not put to bed and sometimes not not taken down for meals. After a constant complaining we finally decided for my mother's safety sake to move her to a different place. Don't be fooled by the good looks of the facility and the people who greet you when you come in. They are not the ones who do the care and the ones who do won't be there long because of the constant turn over.

Terrible food service

My mom has to have meals delivered due to health issues breakfast comes at 10::00-10:30 lunch maybe by 2:00 recently they served her a piece of pie no bigger than my thumb shameful

Keep looking

My bad experiences with this place compound with each visit with my mom. I would keep looking. The front desk staff is very nice and accommodating. One person in the dining room seems to be the only person in the whole facility that actually cares about the residents and runs herself ragged serving them. If it was up to me my mom would be out of here in a New York minute. It is almost to the point that I don’t want to even visit her here.

Under-staffed, lacking adequate supervision, and failure to deliver even a minimum of care

'The untrained and disinterested staff will not even wear the name tags they were supplied with. There is staff supervision only part-time Monday-Friday. No meaningful night supervision from attendants asleep at the nursing stations. I witnessed a naked man with dementia lying on a hall floor for hours. Medication supposedly delivered by med techs, but often skipped - especially at night. Strangely enough the food is often adequate and of good quality. No regular nursing or nurse- practitioner visits.

Terrible

The ED does not return calls or emails. The memory care "manager" is very rude and certainly does not care about the residents needs. My mom is always soiled and her nails are filthy. You can never find an employee when you need one. if you can find someone they are always on their phones. I am currently waiting for a memory care bed to become available at another facility then will be moving her asap.

very lacking of compassion

the office staff that used to be there were great.New people took over and they treated my mom like a nobody. The cna's are lacking , meaning there needs to be more. Never have i had such an experience at such a beautiful place. Awful.

Name change, staff replacement is stalled, and we're on the precipace of exiting

New name is here but I'm not sure if their strategy to bring the establishment back is consistent with what used to make them great. We are well aware of the reasons for the name change without being told. This major reason in itself tells us that they have an obligation to addrress the staff shortage first and foremost. We are taking into consideration the "great resignation" we have witnessed in 2020 to present. We have to visit daily to fill in the gap (the law says there must be XX caregivers per resident). Great that they are upgrading electronic equipment, tv's, furniture, paint, flooring. However, there is a shortage. Stop focusing on electronic sign-in platforms and VET and hire. Pay shift differentials instead of trying to appease by making them lazier. We will be demanding an updated timeline of their strategy to turn this place around.
