Oak Park Place

When I toured the Oak Park Place, I wanted to like it. It's close by to my home. When I got there, the person that was supposed to give me the tour was not there, so another person took me and it was not their job. I was not comfortable with choosing this place for my mother.

I took a tour of Oak Park Place Menasha. The tour guide showed me the bedrooms, dinning room, and other parts of the community. When we went on our tour they weren't prepared my mother was no able to walk the whole tour she needed assistance and they didn't have a chair available for her to move around in.

If there's a professional chef here, I'll eat my hat. My mother-in-law has been here 2 years and whenever we're there the meals are not that good and don't even LOOK appetizing. Mom eats hardly anything. For the prices she's paying she should be served gourmet food.

When we moved our father to Oak Park Place, we were promised several things prior to signing an agreement that never materialized and truly altered the mood of the move. There was some minor finger-pointing at who should have said what, who should have known what, etc. But that's all in the past. There has been a great turn-over of employees but I find that it with all facilities. The pay doesn't make it worthwhile for employees to come to work 100% of the time. There were a lot of problems with the cooks / chefs for a while and that seems to have settled down some. For the services that our father receives, it doesn't warrant the outrageous monthly fee considering he is able to feed himself, toilet himself, walk himself, etc. In the less than 3 years he's been there, the monthly fee has gone up at least 4 times. It's easier to pay it than to look for something less expensive. This is the norm as my mother-in-law is in another facility about 20 minutes away and they have the same staffing, chef, turn-over, and price issues that I just mentioned. There was a director of the facility that left several months ago. Again...lots of talk and no action. I heard good things when the new director arrived and now I'm hearing the same stuff about her. Lots of lip service and nothing to back it up. If you're looking for a place that's nice, relatively new, clean, and safe, then this is the place.

Assisted living unit is very nice, good closet space and generous bathroom. The unit had not been cleaned as promised. Baseboards were covered in dust, drawers in kitchen and bath were not clean. The leasing agent was very nice and informative, but there was very little discussion of the billing process, payment dates, etc. and I had to email to get accurate information. Overall it is a nice residence with lots of common space and friendly residents and staff.

Very clean, well decorated, friendly staff- food areas need improving. Deserts are left uncovered prior to serving for hours, better cold breakfast items are hoarded by a few of the residents, leaving the later ones without. Spices could be made available on each table, as the food tends to be bland. THey receive their food from an adjoining building, and sometimes there is not enough of something to go around. Suggestions are mostly ignored. Activities are held in several places, but there could be more available without having to walk to the next building. Pub times are well received.

While the staff was professional and the facility appeared well run, we should not have been referred here. Our financial constraints were known in advance, and this community was not within reach. Ending the visit early due to cost created unnecessary discouragement and emotional distress during an already difficult process. Exposure to options that are clearly unattainable is not helpful and can feel defeating. I recommend stronger financial screening before referrals so families are not put in this position. That being said, the director who helped us was wonderful and what we did see of the facility and people seemed very positive.

Food was very good. Spacious room. Bright. Nice bathroom and shower. I would recommend it to others, depending on their needs.

Sales person was great and very helpful in the application and approval process. Onboarding at/after move-in was non-existent. It was very difficult to understand the daily routines, meds, etc. My experience with the facility was unique in that my loved one was in and out of a rehab or hospital facility for much of the time. Given a less stressful transition it might have been better. Other residents seemed to enjoy being there.

The facility is great. I like all the common areas to have gatherings with family/friends. The staff is well trained and compassionate but sometimes waiting for assistance is long. Staffing ratio seemed lower on weekends/holidays.

Initially we were fairly satisfied pleased. Everyone was pleasant and setting was more pleasant than previous facility. Over the past week however, care has not been provided adequately with resulting worsening of two physical issues with my loved one. That time span has been only 6 weeks. . .

My dad's dementia is such that he was placed on the memory care floor. A locked ward, so it removed any worry of him getting lost and creating a Silver alert if he wandered off. He needs reminders to do certain things, such as putting his hearing aids in the charger at night. Brushing his teeth, and taking his partial out. There have been several days where he was missing a hearing aid when we came to visit, and usually found in our under his bed. Which meant he slept with them in, and not in the charger. He also was wearing the same clothes for several days often. We've done his laundry at least 4 times when they haven't done it, even though it's suppose to be done every Tuesday. He has issues with his shower, some days nothing but cold water, other days nothing but hot. The staff is very nice, friendly. He gets fed well and the food is good. There are not enough activities for him. Luckily, we're close by and me and my daughters take him out weekly, often spending the weekend.

They don't have enough staff, but that is true of all assisted living facilities in the area. Other than that, it has turned out to be a nice place for her to live. They need more variety on the meu. Also, they ask residents what they want from a list, but sometimes they get something different.

I was not happy with oak park place. [name removed] evaluated my family member and I would have moved him there if it wasn't for the non refundable charge. There too much staff change and most aren't friendly. No I wouldn't t recommend this place.

The common areas of the community are very clean but our loved ones individual room would get a 1. When we first moved my loved one into the facility, it was agreed that the on staff nurse at Oak Park Place would treat my loved ones wound once a week and a grace staff would treat it twice a week because my loved ones doctor ordered it and required it to be treated three times a week for their ongoing health. After repeatedly missing the weekly appointments to treat the wound. I inquired to find out why. The nurse stated that she didn't have time. My suggestion for improvement would be to employ a different nurse who can actually follow through with commitments.

Good staff clean nice shared spaces.

Our loved ones room receives the 1 out of 5 for cleanliness, but the best part about the activities is the music. My Dad was there for 3 years and we only had one formal review of his status and care. My understanding is we were to have them every 6 months. Staff changed so often we never got to know any of them.

The facility did not seem very well-managed and was chronically short-staffed, in my opinion I was never sure about this, due mostly to the understaffing issue. We never had a care plan meeting until I had already decided to move her out. Meals seemed disorganized and there were altercations among staff in the dining room on a couple of occasions. This is a pretty small place and it was the pandemic period, so there was little in the way of activities and amenities. Overall, the place was extremely quiet with little sense of community. The management did not seem very professional, the staffing levels were low and staff were not always knowledgeable, and my mom was not very happy here, and all this for what I thought was a pretty premium price

Within the first two weeks of moving in, money disappeared from my mom's purse. I know that she had too much in her purse, but still. Overall my mom likes it there. They do a good job assimilating residents into memory care. The staff is overall great and kind to my mom. However, they regularly miss laundry days, and cleaning her apartment. They have a communication problem. For example, my mother fell twice and I was not notified. Her toilet handle was broken for days, and my husband had to fix it temporarily to get her through the weekend. Some food is served cold or barely warm and is not as satisfying as it could be.

They need to hire more staff.
