Terrible facility

My husband has been there for almost three weeks for rehab. The staff barely interacts with him. The food is terrible and usually comes cold. Then when they reheat it, it comes back so hard he can’t even chew it. I could go on. They have him fill out a weekly menu and half the time don’t follow it. Their menu is very, very limited. One day he found excrement on his pillow case that he had been sleeping on. Also his room hadn’t been cleaned for almost two weeks. The place does not live up to promises or expectations which is too bad because it’s a beautiful facility. Looks can be deceiving. I have heard the same from other people who have had loved ones there. Wish I had known before choosing this place.

I would not recommend Oak Park Place to others. They kicked my loved one out on the first day. The nurse demanded that a family member be present at all times. They're also having staffing issues.

Would not recommend at all.

We were not pleased with Oak Park. My loved one was not bathed as often as she should have been. They were often critical of her - her requests and needs and activity level. Little or no respect. Also, instead of using a gate belt, I observed an aide yanking her up by the back of her pants. And since aides didn't follow safety rules, she had several falls. For the most part, my loved one liked the food. She also enjoyed interacting with her table mates. But I was not pleased with the staff / care. To be honest, I'm sure there were times she was not easy to be around. But even so, she still deserved to be treated with respect and proper care. We moved her to full care nursing at another community, and she was so much happier there.

Oak Park Place Madison review

It's a shame the bulk of the staff aren't more motivated to help the residents as the facilities are quite lovely. But the services that are advertised are not consistently available as promised. My mother loves her room and full refrigerator but we need to employ outside help to get her daily needs met

Many problems

This is for the memory care unit only I heard the rehab and senior living is better but don't have experience with those areas They were way understaffed, staff not trained appropriately, nurses not available, poor patient care due to lack of staff. My dad sat in wet pants more so than dry. Showers weren't given. We worked with administration to try to get improved care for over a month, they said the right things, but very little to no improvement with patient care. Food was good, but if you don't eat in the dining room, would get it hours late if at all. Very little staff/resident interaction. Only stayed a couple months only because we wanted to give them a chance. If you go here be sure family is available to visit throughout the day every day to be sure your loved one is being cared for.

Hello, I am writing this review at the request of a personal e mail from the sales and marketing manager, [name removed]. My experience with my parents at oak park place in BRIEF: The tour was magnificent and we were so excited to place our parents there! There were apparently so many wonderful amenities including a barber shop and rathskeller. So- we decided the VERY HIGH price point would be worth it for our parents who were not safe to live alone. However after that EVERYTHING has gone way downhill: their sheets are soiled, wet and smelly; some of the staff are very nice- but then they leave OPP quickly. Other staff has been aloof, on their phones and sometimes rude. The care that is prescribed for my parents is often not followed. They are rarely showered. The activities are minimal and they are not always brought to the activity; they do not have the capacity to go on their own. I feel trapped. Every move is a cognitive hit at this age and it is hard to move. OPP makes me cry.

Horrible

My mother moved into Oak Park Place/Oak Creek March of 2020. At first, it seemed like a nice place. As time went on, I realized my mother was not getting the care we were promised and were paying for. Housekeeping was never done; laundry was not done unless I asked staff 2 or 3 times; her medication was not being managed correctly (staff were supposed to watch her take all her meds, they did not. We found pills under her chair, in the bathroom, kitchen counter, etc.) We were told there would be a full-time nurse & there was for a while, until she quit. Then just part-time nurses came in sporadically. During the week between Christmas and New Years 2021, my mother was very sick and also fell out of her chair and had a bump on her head. No one from OPP called to tell me. I was told there was no Director of Housing on site that week and that they were short-staffed. After I called an ambulance, she ended up going to the hospital w/3 infections where she died a week later.

There was disrepsect from the staff. They were always short staffed. They did not follow safety rules. Mangement were not really helpful. They were not timely in responding to my loved ones needs. There was lack of respect, for my loved one and their safety.

Neglect

Don't bring your family to Oak Park Place, at least to the memory care unit. We've been having a terrible problem with them not providing baths once a week like they said they would. It's happening only about every three weeks. I just had a meeting with the director, and they promised him a shower. He still hasn't had one three days later. The caregivers are very nice, but there's just not enough of them. They're horribly understaffed. There was an incident where an aide came in and dropped off a tray for breakfast instead of taking him to the dining room. He hadn't been up or dressed yet. She said she would be right back, and she did not come back for three hours and did not answer his call button. He got very frustrated and tried to get himself out of bad, which he can't do. So he got stuck trying to get out of bed. This was at noon. So he had his breakfast sitting across from him for three hours with no one to help him eat, and he hadn't been dressed all morning or cleaned. That's happened a few times. He's literally stuck in bed for hours and no one helps him out. I see no activities going on, and nothing listed on calendars, nothing organized. I saw one art activity in the whole time that he's been there. They keep the common areas of the facility clean. But I've seen feces on the shower seat that didn't get cleaned up.

Extremely dissatisfied

My mom has been here 6 months and we are looking for a new facility. Not meeting her needs is an understatement. Never enough staff here to even meet basic needs. Keep looking beyond this place for your loved one. We try working with staff and management for 6 months now and never get any problems resolved. We are prepared to move my mom at the very first opportunity we can get.

Horrible place

HORRIBLE, HORRIBLE,HORRIBLE If you ever want to see your loved ones again do NOT put them here!! They are understaffed, What nurses they have treat people HORRIBLE!! They do NOT care about their patients!! Their staff runs around and does anything they feel like doung!! It is so bad they made one of their patients wait for hours just for water!! When you call there concerned about your loved ones all they do is say why are you hollering at me!! TOTALLY RIDICULOUS!!

Not quality memory care

Just a warning, do not put your loved ones here. They make a bunch of promises to you and have a nice facility to get you in the door, but the quality of care is so low. My grandpa was in memory care here and did not receive the care he was paying for. They fed him, but that's about it. Didn't help him with hearing aids, didn't clean his clothes ever or his bathroom, didn't try to get him out of his room at all. Very sad. Took him elsewhere and found a much more caring place with friendlier staff.

Horrible experience

Although this place was beautiful and the caregivers that were there were friendly and caring. Mom wasn't getting the care she needed or that she was paying for and what we were promised on the tour. They only had one nurse and she was on call not even there most of the time. Mom moved in on the 3rd and died on the 19th, a week before she died we pleaded with the manager to get better care in action before she dies. It was just so understaffed here nobody paid attention to mom's oxygen we found her numerous times without her oxygen on for whatever reason. And when she was found unresponsive her oxygen had came off in the middle of the night. Where someone should've been checking on her. I would never recommend anyone to this community.

Terrible!

That place is terrible. It was a joke. It's gorgeous when you look at the place. It looks like it would have anything you would want. It's a beautiful facility but they do nothing. They kind of just left him in his room. There was no one around, ever. Terrible! Ew! He was so isolated. They never really cleaned his room or anything. He fell there and hurt his leg and had to go to the hospital. When we came back to get his things to move him, they hadn't even emptied hi pee container and that was 3 days later! He had to have his leg amputate and I'm not saying that it's their fault but they were not good about watching him at all. The staff was not friendly or sociable.

Dissapointment, neglect, and lack of compassion

My biggest regret in life was choosing Oak Park. What a disgrace of a “memory care facility.” Not one person in the entire building is qualified to care for the special needs of an Alzheimer’s/Dementia patient. As we discovered they have plenty of loopholes to cover all their mistakes/false advertising. During the initial walkthrough they make the place seem like a dream come true, however it is a house of horrors. My family member passed away due to heedlessness and we are pursuing a case against them. Between the uneducated, uncompassionate, limited and constantly turning staff/management, the director of housing and regional nurse were among the most stressful part of our time with Oak Park. As leaders they failed exceedingly, they provided no training for the steadily turning staff and complete disregard for making any changes to better the residents. We brought several issues to their attention, as well as corporate, that never got solved. Their feeble responses lacked any compassion and came across as sarcastic and merciless. It felt as if we were annoying them with our concerns. A few of those matters that our family member suffered from include: dehydration, improper wound care, neglected hygiene, toileting was unsafe, unsanitary and not often enough resulting in open wounds/hospitalization, no housekeeping as promised and paid for, sudden weight loss, minimal interaction with activities (they just wheel them in a room with a TV and leave them there unattended), residents put in unsafe conditions because of lazy staff, bed falls are not being reported, hospitalization for pneumonia due to failure to follow treatment plans, and just downright lack of communication, knowledge and complete negligence. This is how our family member lived their last months. The response we got in return “It's the disease, Alzheimer’s, they are old it happens.” I will not rest until this place is no longer in operation and the residents still residing there get the love, care and attention they deserve.

They do not have a nurse 24/7. No visiting physical therapist. Definitely no visiting podiatrist! They are unprofessional and a lot of their 'caregivers' have no experience! Good luck in your search!
