Oak Park Place

My one suggestion would be to keep the resident's rooms clean. My loved one's was awful. Many times I asked for a vacuum to do it myself and was told there wasn't one. The bathroom was rarely cleaned....I did it myself. He is no longer a resident here.

Oak Park Place was a nice facility, but it was too large and we were scared our loved one would get lost in the shuffle. It was brand new when we toured and not a lot of people were around, so it was hard to get a good feel for what it would be like.

We virtually toured this community and it was nice. The staff was very friendly and answered all of our questions. The rooms were smaller than we were looking for.

My mom was in the memory care unit. You need to visit and check out the other residents in the unit, The room sizes are a little small but the layout of the community is good. The food is good. They have limited secured outdoor space.

It has a good sense of community and they bring in a lot of different programs. The people are very caring. The rooms are very nice. The care is good for the number of people that the aides have to care for.

Location was great. Facility was huge. Appearance like a hotel. Very large memory Care unit. Long hallways to community space.

Oak Park was not covered by my parents insurance and rooms were a little small when my dad stayed there last. The staff was nice. Occasionally my Dad stated the meals were cold as they were brought from another area of the facility.

I have recently taken a tour of this community and I really enjoyed my experience. The staff where on time to our appointment and made sure to answer any questions I had for them. i just thought the rooms seemed to small for my mother and it was too tight of space.

This community is nice and there have been some changes that have improved slowly. I do like the cleanliness. They need to have staff that is more caring and they need additional staff that is more available to care for the residents. The menu is ok but they need to make changes. They are offered to be involved with the activities with the other residents. They could improve on some of the cleanings. I would suggest that families tour this community for the care of a family member

Management staff was well informed and tried hard to meet our needs. However, the care staff did not appear to be as conscientious. We identified my wife as an extreme fall risk from the very beginning. During the short time she was there, she had five falls. In the case of the last fall, she was allowed by a care staff to wander into a vacant room while she was in an aggitated state and she fell, resulting in a laceration on the back of her head and a concussion, which resulted in a six day hospitalization and three weeks in a rehabilitation center.

I had a good tour of this community. I toured it for my mother, and her needs. I didn't really get to meet much of the staff, but the community looked great. I will say this is a beautiful community overall.

I toured this community for my parents, and their needs. All went well, everyone was very kind. The community looked nice, and well-kept from what I saw. The staff I came across was friendly, and nice. The experience was a nice one. My tour of this community went well.

We felt that the community was great. They had a beautiful fireplace in the front and the grounds were nice. We just felt that the community paid more attention to the look of the facility than to the residents. I just did not like that.

I have concerns that the staff to resident ratio is not high enough. There needs to be more people available to the residents. I am in the building 2 - 4 times a week and there are always people asking me for help as I walk to my mom's room. I am also concerned about them not having enough activities or a larger variety of activities for all residents. My mom is higher functioning than many of the others and she is bored with the things that are offered to do. She will skip them and sit in her room which is not good. The nursing staff is good at calling me if any problems came up during the day. The dining room should be warmer and more cozy. No music playing in the background. I don't think they clean up my mom's room often enough. Trash is always full.

When my loved one moved into Oak Park Place, I thought it was a wonderful place. Nine months later I have many concerns. The housing director left, the activities director was fired, the new doctor with the Bluestone physicians, was put on the wayside. No one is in charge. The nurse is, I suppose, but she is spread so thin. And why they have not replaced the director is of serious concern. Lots of new staff and patients. [removed] I would not recommend this place.

My loved one has only been here a short time, but in general Oak Park Place seems nice. We're still in the settling in period and things have been a little rocky. We've had some issues with the caregivers not following through on aspects of the care plan, which I hope is something that gets worked out eventually.

When we first toured we were pretty impressed with the community and the way it looked. The memory care area was ok, but not as nice as the other side of the community. I don't feel like he got a lot of individual attention, maybe they didn't have enough staff. They let him call home a lot, and that sort of agitated him. It wasn't bad, but it wasn't great either. They were just sort of average.

Met director, large facility, noisy near interstate, price good, no support for indepedent living, good transportation. Independent has real apartments w/granite, fireplace & balcony, 2 meals a day and all untilities

Oak Park Place is a beautiful community, its brand new and super clean. However they are extremely understaffed and the staff they do have working depending on their shift were extremely inexperienced. There were a few good apples there within the nursing team, but the overall tension with the nurses was bad. When we would ask them for assistance with our loved one they would be on their phones or say it wasn't their job and we would have to find someone else. Our loved one was never bathed or the laundry was never done. They have level 1, level 2 and level 3 care - we were paying for level 3 care, but in reality our loved one was receiving maybe close to level 1 care. The food quality was so bad we would not feed it to our dog. Our loved one was never able to cut the meat, and the amount of salt was concerning - it seemed to us that they did not pay attention to those who have dietary restrictions because that amount of salt would make someone ill. If there are able to hire new staffing that are very well trained and knowledgeable this community has potential, but until then we would not recommend this community to anyone.

Caregivers are quite friendly and caring. The building / decor is comfortable. Facility is understaffed at times. Administration and Corporate administration was not responsive to the concerns that were brought to them by our family.
