Oak Park Place Madison West Nakoma review

Oak Park Place is a beautiful community, its brand new and super clean. However they are extremely understaffed and the staff they do have working depending on their shift were extremely inexperienced. There were a few good apples there within the nursing team, but the overall tension with the nurses was bad. When we would ask them for assistance with our loved one they would be on their phones or say it wasn't their job and we would have to find someone else. Our loved one was never bathed or the laundry was never done. They have level 1, level 2 and level 3 care - we were paying for level 3 care, but in reality our loved one was receiving maybe close to level 1 care. The food quality was so bad we would not feed it to our dog. Our loved one was never able to cut the meat, and the amount of salt was concerning - it seemed to us that they did not pay attention to those who have dietary restrictions because that amount of salt would make someone ill. If there are able to hire new staffing that are very well trained and knowledgeable this community has potential, but until then we would not recommend this community to anyone.

Need to improve customer relations

Caregivers are quite friendly and caring. The building / decor is comfortable. Facility is understaffed at times. Administration and Corporate administration was not responsive to the concerns that were brought to them by our family.

Bad Experience!!

Unfortunately, this place didn't really work for the family. The staff and the community gave us a call that they could not care for my loved one. Within a week of her move to the community we had to be there 24/7 making sure that she had care until we found another place. The staff didn't really try to be attentive with her or make sure that her care needs were being met.

Oak park is not a facility I would ever recommend. It's assisted living and skilled nursing are chronically understaffed, solely due to an unwillingness by the owner to spend more money. The cheapness bleeds into everything, including quality of meals, and underpaid staff don't perform their 100%, either. Oak park does not treat employees right or well, and this also bleeds into everything. It is plain to see in an hour in the assisted living section or skilled nursing section.

Our Experience

My loved one states that the food isn't that good. I don't think they clean like they should especially the bathroom of all places. They are suppose to change her bed weekly, I don't believe they do that. I believe they are probably no different than any other facility.

NOT a Place for Mom

Place may look good in the halls, but care is not acceptable. One star is for workers that show some care and respect and try to do what they can. Unfortunately the turnover rate is high (workers dissatisfied) and good staff are lost. One star for maintenance who are helpful and always trying to keep up. Rooms not kept clean, laundry mix-ups, medication errors and poor organization, no help to follow through with any PT or OT recommended, some activities planned but only for those who can remember to get there on their own, food choices not great (esp. for those needing special diets), short on staff doesn't help but should not be an excuse for everything, staff are not trained well and do not know residents' needs therefore making mistakes (sometimes critical), nurse not always available, lack of communication every which way! Thank goodness the residents watch out for one another and help each other when possible.

One or Two Good Staff People - Do Not Recommend

My loved one has been at the Oak Park Place location on Nakoma Road for about four weeks. We toured twice and thought it was lovely. Even though there were very few people (neither staff nor residents)around when we toured (not a good sign), my loved one was happy with the apartment. We thought it would be fine. The rooms are very nice and there are a couple great staff members who are qualified and able to deliver good care. But in general, they are extremely short-staffed in many departments, the staff on the floor is inexperienced, and they don't seem to be able to deliver the care that is necessary for my loved one. Considering the price, it is extremely disappointing. I know it's extremely difficult times right now for hiring and retaining staff at care facilities, and residents can be challenging to care for. Even so, I would not recommend Oak Park Place Nakoma.

I was really impressed by the facility when I went to tour it. But as my loved one move down and started receiving care, things went downhill. I found her on the floor at one time unable to get up, she was on the second floor for rehab and had been doing some vomiting. I had to ask for a bowl for her to throw up in. Finally after several days of this they called me with concern. She came home from the hospital and they put her back on the second floor and then moved her back to her room on the third floor without letting me know. That was the day that I found her on the floor. Even though it is a assisted living place there should be rounds made by the staff on a regular basis. There obviously is a reason why these people are there. They need help and doing things and can become confused and not push the button for help. For the amount of money that I paid for the facility she should have received better care. I got the impression that they were only out for the money and patient care was not a priority. I get such a sick feeling in my stomach every time I go by the place.

Our experience so far

One of the improvements needed by this facility is the personal attention to the residents in the morning. Such as checking if they have clean clothes on, depends, shoes, if they have their walkers etc. When I call I do not get calls back, so I think improvement on communication is also something that is needed.

They could work on overall communication and transparency

Our main issues was within the first 60 days of our loved one being there. There were three main decision that were made for our mom between the care staff and the doctors without every consulting the family first. It started off rocky and from there it is hard to build the level of trust

Tour of Oak Park Place

I did not get a good feeling from the beginning when I walked in for the tour. When we were walking around, the tour gal did not know any residents by name and they did not seem to know her well either. We also went down to see an activity that was supposed to be going on but something wasn't working with the internet. The residents said something like "this happens all the time". It was just not the right fit.

Did not meet expetations.

Overpriced and continues to go up every year due to what they stated as not a competitive price. Actually overpaid for services not received or received poorly. No fault of the employees. Just no housekeeping staff per se. Apartment floors and rooms extremely dusty-1/2 " thick dust on floor in bathroom and shower. Toilets not cleaned. Rugs need cleaning. Laundry gets mixed up with other residents. Mother got other people's clothing after laundry service. Food atrocious--unappetizing. Not only looks unappealing but tastes old, dry, and like mush. Mostly carbohydrates served--little protein. Not a good or balanced diet. Dietary department needs to be headed by someone qualified and who CARES. New director does not return phone calls and listens to complaints without caring through on results--having no solutions to the problems. Very disappointed in the facilities. Family and patient is at a disadvantage since resident is acclimated to the facility and would be uncomfortable in going to another place. That is what OPP Management counts on. This is only a money making scheme for them. This saddens me. Very disappointing. The new director is clueless and not qualified to accept the challenges and changes OPP needs to make to make it a first rate assisted care living facility. Housekeeping duties and dietary offerings are poor. Laundry done of resident's clothing and bedding gets mixed up with other resident's. The food is another subject. Choices are inappropriate for senior citizens and often unappetizing, cold, lacks any nutritional value and consists mainly of carbohydrates (cheap and easy to make). Seems like management's goal is solely PROFIT orientated. Activities are lacking and staff does not seem to encourage residents to attend. Cannot get staff to check Mother's mailbox. Have been trying for two years now. Bills and special occasion cards go unopened. Issues are not addressed by administration. Complaints are listened to at times but not addressed or resolved. Need to find an organization that will act as a resident's and family's advocate. I would not recommend this facility. There are better ones out there. Too bad we are kind of stuck. Administration knows this and uses it to their advantage.

Attractive Place With Poor Management

I was told cable and housecleaning was provided in monthly fee. Only basic channels, not even our local PBS is included. Then they give you a # to call to upgrade. There is no regular cleaning schedule. An aide came once after I complained. He dusted and vacuumed. The bath hasn't been cleaned since she moved in 3 weeks ago. My relative fell because the aides were standing around talking. When she asked for help, they ignored her. She's supposed to be getting a higher level of care and isn't getting it. They are forgetting to come and get her for meals. There is always a shortage of aides. One morning at breakfast, there was no milk for cereal on the floor. Another morning, no coffee because someone forgot to make it. Some of the aides are poorly trained or don't seem to have any training. My relative can't eat some of the food they serve. The manager said she would have the dietician meet with her. That was over 2 weeks ago and that hasn't happened. It's an attractive place. I never thought we would have all these problems. I don't know what to do. I hate to move my relative, but I may have to if things don't improve. I have to be there all the time to make sure my relative is okay. We are paying extra to help her get dressed, bed made and be escorted to meals. Today, no one came. She called and the aide said she was busy and would come as soon as she could. She never came. So my relative had to get herself dressed and go to the dining room by herself. She is at high risk for falling. That's why we pay for the higher level of care, which she is not getting. My relative is very unhappy and so am I.

Administration concerns

Highly concerned about administrative level. There are no systems in place. HUGE medication management issues that caused us the greatest concern. I understand the head of the organization [name removed] has finally come in the beginning of the month to, maybe, find out what is going on at this facility. I would like to know what steps are taking place, because although we have left, I care deeply for those remaining families who have no one to advocate. Thank god they hired [name removed], RN before we left, excellent care and respect for the residents, we did not find that from the other nurse, still there. Total disrespect and disregard. PCA's we found to be caring and open to suggestions, and no problem there! Additional concerns regarding process in place for shower assist, cleaning (in the room and facility!), meals and supervision at meal time, and building/resident safety. And yes, we were told to 'speak up'. We did and unanimously we were always told, "well that shouldn't be", but no change! This is why we just did it ourselves, because we didn't want to make to big noise so our family member wasn't retaliated against. This increased her frustration and anxiety as well. Knowing she is paying an astronomical amount of money, and her family still has to be doing all the work. Not a good place to be in. We'd like to let this go, but this brings us so much sadness. I want this to be the place the [name removed] family must have envisioned according to their website. Maybe was like that in the beginning, but something drastically changed.

Bad Neighborhood Nearby

We toured here, and thought that the community itself was nice. However, the neighborhood nearby is not good. The residents windows overlook the town dump and the police station, which just didn't seem very comfortable to us.

Be very careful

When we toured the memory care unit it was sparsely filled. We were told it was because of the pandemic, but later felt it was because of poor care. The Oak Creek locaton had problematic staff turn over both in care, sales and nursing. Often we couldn't find staff on the floor when we visited. They also had a theft ring preying on residents and their belongings. This was true for many residents. There was inconsistency in medication, laundry, showers and little to no housekeeping. Only the activity staff was consistent and connected to residents. Please be very careful in checking this location before placing a loved one there.

Horrible care

A family member went to Oak Park Place for rehab and the facility was horrible. On one occasion she went to the hospital for a drug overdose. On two occasions, she was given another residents medication. The rehab portion was fine but staff is overworked and on several occasions they were non existent. The social worker and management are too chicken to meet you in person with your concerns and prefer phone or email conversations. On one occasion they were hiding in their office when confronted. A very poor sign! I would never return or recommend this facility.

Would not recommend

The entire experience was overwhelmingly disheartening. My father in law did not get the care that was sold to us. While there were a few wonderful employees, none of them had the resources needed to do their job well. As I nurse, I was shocked by the quality of the care. I would not recommend OPP to anyone.
