The facility is a dive

The facility is very old and lacking in maintenance. Requested grab bar in bathroom has not been done in 7 months, resulting in a fall. Management and supervision of staff is lacking. I have observed Assisted Living standards being ignored by CNA staff who regularly bring their children to work. The staff is unhelpful and at times disrespectful to residents. Avoid the facility if you can do so.

Horrible care!!!

Care is horrible, staff sleeps in cars & on couches in common areas at night. Food is not good I’m told. Staff doesn’t speak up when talking with elderly. Staff on phones and social media all times of the day. They disappear behind closed doors and can’t be found when needed. Meds are not on time. Call lights not answered in timely manor especially at night. What else can I share…… Don’t put your loved ones in this facility!

Our aunt would be found wearing clothing with feces all over it. A week after she moved in, they reassessed my aunt and tried charging her over $[removed] more a month (my aunt didn't have those funds). They stated that my sister signed for the increase on the day of our Mother's funeral (untrue). Medications were administered but she was not watched while taking them. When we cleaned out her room upon moving, we found at least 20 various pills all over the floor. Housekeeping was not done on a weekly basis. Feces was found everywhere. It all depended on the staff. Some of the staff were great - kind and patient. Others seemed to like to aggravate residents to cause a ruckus. Knowing what we know now - I would never recommend Brookdale to anyone. Meals and dining were good. Out of all of the times that I visited, I never saw any residents involved in any activities. Once we started pointing out the inadequacies at Brookdale, shortly after - employees no longer worked there.

Sage Meadow is going through a lot of staff changes, from executive directors to nurses in charge of medications. I would not recommend Sage Meadow based on the following challenges: 1. They administered medications continuously when prescribed "as needed for sleep". 2. The carpeting was soiled and had burn marks in it-they didn't offer any options for replacement. 3. Several safety issues were not resolved as promised in writing before move in date. 4. When calling the phones just ring-nobody answers the phones 5. Executive team does not communicate well with the staff 6. Our Mother was transported by ambulance to the ER and they didn't know their resident had left. 7. Staff gave 1 opportunity per week to help with bathing-if she missed it or wasn't feeling great it was pushed to the following week. This was a poor experience all around for my mom and for us managing her affairs.

Our motivation for writing this review of the Cottages of Madison is to warn others — Don't go there. There were a number of problems. The patient was seen in the ER because of cognitive changes and was found to have bed sores and was found to be dehydrated. The patient reported to his family that the staff was mean to him. There are no call buttons . At our request to remedy this, we were told a baby monitor could be used but it was never set up. We were told that on two occasions in a row, on the same day that the patient was not given his medication. First we were told that the patient refused the meds. Upon further discussion, and a look at the records by the manager, she said he was too lethargic to take pills and that there was no order to crush meds. We later found there was an order. We eventually reached a point where we told the decision makers that the care was neglectful and abusive and that our family member had to be moved immediately.

Visited this facility in November, along with their Racine location. Both are more memory care level facilities and beyond what my mother needs, or would feel comfortable in, right now. They are both very small and felt confining. But that is perhaps because my mother is still active.

Move On Not In

Find some place else-as a physician I deal with said "No one wants to DIE here, find someplace better" My parents where both housed here for about three years During there stay I continued to look for some place else. At the time they moved in it was the only place available that could take them both and they could share a room. So It was ameans to an end. The director runs hot and cold. You never know what you will get. I absolutely Loved the activities director. She is wonderful and cares for the residents a lot. She tried her best to keep people and families engaged. The caregivers that actually dealt with the residents for the most part seemd to be caring. Ther were a couple who would belittle the residents and refer to them as stupid. This is in the memory care area where my parents where. They are not trained very well. My Dad's hospice nures and social worker offered to hold some classes to teach the staff better techniques when dealing with demntia patients it never happened. Unfortunately we were not able to find place that was more professional and treated everyone with the compassion and understanding that our loved ones deserved. The real corker was the evening of my dads death.One of the residents fell. I ran to her and tried to comfort her, then went to find the aide on duty. When I did she was very caring and called 911 after the resident complained about a lot of pain in her hip and back. Another aide came in looking for something and when told of the woman who had fallen went to her and her exact words where, " NAME, can you roll over and straighten out your legs I need to see if your legs are the same length." The resident could not due to the pain. The aide took the resident by the knees rolled her to her back and straightened out her legs. (Even I know this a complete NO-NO, and I have had nothing more than a simple first aid course years ago.) The EMT's arrived and very carefully moved her to a gurney and to the hospital. The resident suffered a fractured hip, needing much rehab and will not be coming back to Artisan. After witnessing and being there through this I accelerated my search to get my Mother out of there. It took 2 months but my Mom is now in a very nice facility with activities such as sit and stretch, walking outside, dancing and being with residents and caregivers that actually talk with her not at her. She is clean and her teeth are brushed her hair is combed. All which was not addressed very well at Artisan. Her whole disposition has changed. My Mom is HAPPY for the first time since she had to leave her home. I DO NOT recmmenend this place for anyone.

My loved one moved in this year. This is the most poorly run facility. The staff are never on the same page and don't care about their clients. Would NOT recommend Artisan!
