My MIL has been there a year & has never received physical therapy, no one has asked her or talked about her doing PT & my husband & I were sold to this place after the initial tour which included the PT area. The management & owner keeps cutting staff & hiring teen adults to work with little experience. She does the staff that takes care of her when they work but the others do not. She is forgotten sometimes. It's too late to get her PT since she can no longer walk & is now bed ridden. She's not even 80 yet. I would not recommend this place even if it is better than others in town.

Great facility and grounds...kudos to maintenance and housekeeping. Makes a great first impression and easy sale. Pool is sadly not used. Piano is rarely used. High staff turnover. No apparent program to improve staff service and training based on resident and family feedback. Seems to be a habit of mid-level management to kick the can down the road by saying, "I'll talk to them" regarding issues with staff. Any improvement is fleeting. Poor communication up and down staff levels. Good care or service comes on the back of the person who has the individual passion or professionalism to do so. Without that handful of people, the grade would be a 1 rather than 2. This may be one of those places owned as an investment rather than desire to serve the care needs of an older community of people. They just don't have managers who know how to monitor both resident and staff needs and concerns and adjust properly for appropriate resident care and staff coordination and support.

I moved my loved one into my home for better care and attention. Maintenance of the facility was poor. Water stains in the ceiling. Dead light bulbs. Closet doors falling off and left unrepaired for months after request. They confiscated a hot plate from my Mom and did not return to me upon request. Food is not fresh and all packaged goods thrown together. They actually served stale old factory made cinnamon rolls microwaved with canned cherry pie filling as dessert for for one meal I had. I had called to ask them to help my Mom charge her phone by plugging it into the wall but they simply refused. Things went downhill since the operations director, [name removed] left.

My mom was previously a resident at Almond Heights Senior Living. The cleanliness of the common areas was okay. However, they never had enough staff to clean the residents' rooms. If you could take care of yourself, Almond Heights would be a very good facility. I would not recommend it for anyone who needs more assistance than taking meds or getting help to meals. If I wasn't there, I never knew if my mom was being cared for. When I was there, I was the one taking care of her. The staff hardly ever came into her room. My mom passed away in March and only one caretaker contacted me. No management or other staff. Even while moving her belongings, only the other residents expressed condolences!

They are very inconsistent and will raise your rate. You have to be on top of them.They don't follow through.

Ways the Palisades at Broadmoor Park could improve on is to have patience with our loved one because it takes a lot. According to our loved one the food needs improving - they could maybe not have so many fancy meals and have more down home cooking. That is how most of them grew up.

They just do not have their crap together. The communication between shifts is horrible. Seems like the turnover is high. My loved one was not eating or drinking and getting very dehydrated and no one was doing anything about it until he was having to be hospitalized. He had bedsores. Just a not good experience for the level of care that would be expected in such a facility with such a high price tag.

be sure you know what you're paying for the monthly care level they've established for you and what it includes. Initially my loved one was paying an additional [ amount removed] per month for her care level. Nobody ever reviewed it with her and nobody ever showed up to provide the help or services described in the care level. We were told they never attempted to provide the services because they noticed my loved one had a private caregiver with them, Their private caregiver was there to handle the physical demands of moving, setting up, organizing, recycling, dumping etc. which they could not do because of their ill health. Although nobody at Northgate Plaza ever offered or provided the help/services outlined in her care plan, they would not refund any of the charges. We were told the care levels are reviewed monthly and adjusted accordingly. I would also warn my friend NOT to have meals brought to your apartment. Lunch delivery to the rooms is $10 (per person/meal) and dinner is $15 (per person/meal). I would also warn my friend NOT to leave anything valuable in his/her car. My loved ones car was broken into one night and ransacked.

The place is very nice looking but overall not a warm place. There is one nurse that is kind but I don't remember her name. They seem to be friendly when we toured, but of course that's how they get you in, right? They don't communicate on a timely manner when meds need refills.

Crystal Terrace of Klamath Falls has really gone downhill. While we were here they could not keep staff, there was a ton of turnover. Especially management, I think we had 3 directors while my loved one was here. The cleaning was HORRIBLE. I was having to come in weekly and clean her room. I would also find pills on the floor because they did not watch her take them. None of the complaints that I made were recognized. It is a beautiful facility and all, but it was a bad experience for us. This is not built for assisted living.

The Wellington director seems inexperienced. The Wellington offers degrees of care. When the director of Wellington said our mom needed more care, she Reccommended care that had a circle for her to walk in and another community. We had her evaluated and by two other communities and were told she could be on the assisted side with the more assisted package. The Wellington forced her to move out. They would not allow us to pay for that package that they offer too. The staff has talked about my mom, in uncaring ways. The receptionist has told me and my sister no one likes my mom. My mom has felt very uncomfortable coming out of her room. I hope this review will be helpful to other families who are trying to decide where there loved ones will receive the help, the love, and the acceptance, they need .

There are times the cleaning of the room is neglected by the memory care staff. If the staff know a family is visiting the staff will make sure that the resident's room is extremely clean. There are times that the staff have been rough with our loved one during bathing. The staff in the beginning were very nice and smiley towards the family. The care levels are based on assessments made by the community, and this will raise pricing of care for our loved one. The Director of Memory Care is not communicative with the family and will not return our calls. This is very frustrating to us. I feel the community is driven by money and this bothers me the most. The food is made for all the residents, even though the staff asked about my loved one's favorite items to eat or dietary restrictions. Now the food being made isn't designed for each individual resident. There have been times that the hot water hasn't worked in the community. The have the best activities in the world for the residents. However, it would be nice to see some of the memory care residents be taken on field trips or taken outside for fresh air.

When we toured at Cottonwood Creek, the facility was dirty. Trash cans not emptied. They had safety issues - for instance, electrical cords across the activity room that were a tripping hazard. It could have been an understaffing issue, but I didn’t go further into looking into them after what I saw.

I would recommend you keep looking at different communities. The management staff has all changed. My loved one isn't being moved properly and this has caused injuries, and my loved one has had to be hospitalized. The care isn't being done as it was promised. My loved one hasn't been bathed in several months. If I didn't live close, it would be worrisome to have my loved ones in the community. The positive about the community is that some of the staff seem like they care for the residents. It is a smaller community and they get to know the residents on an individualized basis.

I've lived here over a year. Constantly understaffed. Med techs and assistants are good. We have a new nurse for the facility as of a couple months ago. Previous nurse constantly was late with providing us with refills. Constantly late with refills leaving us residents to go without. I'm not aware if that's been resolved. With written instructions! They don't seem to mind if they ruin your clothing. Hallway and dining room are at 78 degrees or hotter. Dining room understaffed, service is slow. Half the time 30-45 minutes to take your order. Then 30 minutes to an hour to receive your food. Half the time it's cool. Heaven forbid if you wish to ask for anything in the meantime such as butter... After a surgery, no shower assistance. They'll just leave you hanging. Even though ask for additional service before, after the surgery, then it's a big No. It's the management that provides less than quality service. Though everyone is friendly. It's odd.

I would want others to know when looking into this facility that they did not let me know that my mom lost 50 pounds in 3 months I would not recommend Hacienda Del Rey to anybody. Look for someplace where they actually pay attention to your loved ones and let you know what is going on.

This is not the place I moved into. The food is not good, I throw out more than I eat!! Last night the beef dish was so bad I tossed it out in food waste!! Also the housekeeping staff is short-handed. I was told one person has from 11-14 apts in one day. My carpet has been cleaned once since I been here! Also last week at happy hr the tables in the patio I used 6 wet towels to clean. 2 tables I would not put anything on them, not very healthy!

Excellent job keeping us safe. I live in a cottage with no assistance. I am entitled to one meal per day. I am entitled to pick which meal, lunch or dinner. However, nearly without exception, the lunch item for the day is not available and I must take the dinner selection offered that day. The dinners are poorly planned and poorly prepared. Last week, the starch four nights in a row was a rice based dish. Often, the main course dish is not what was described. For example, two out of the last three times salmon was listed, the dinner delivered to me was a white fish. Often the mean course will be described as having a sauce. Nearly always there will be little or not sauce on the entrée. Often, the meat is so over cooked it is tough to cut and chew. Looking forward to dinner is a great disappointment. The main concern of the company/owner is cost per person, not quality of the experience provided. Luckily, the staff is kind and caring and I appreciate their the service they do provide. Unfortunately, there are few senior residential options in our community and I have no choice but to remain at Crystal Terrace.

Very bad for the money. I'm forced to stay here because of my health. Bad food, pills, bad deal.
